L’objectiu va ser determinar si la presència de anticossos antinuclears en pacients amb artritis reumatoide (AR) condiciona una pitjor desposta al tractament amb anti-TNF. Es tracta d’un estudi tipus cohorte retrospectiu. Es van incloure 66 malalts amb crtieris ACR per AR que anaven a iniciar tractament amb anti-TNF i tenien una determinación basal de ANA. D’aquests 37 eren ANA positius i 29 ANA negatius. Es va observar una tendencia als 6 mesos de tractament entre els ANA positius a no presentar resposta (19% en els ANA positius enfront a 11% dels ANA negatius), sense poder establir diferencies estadísticamente significatives (p=0,081).

L’objectiu de la recerca va ésser determinar si la preència de anticossos antinuclears (ANA) en els pacients amb artritis reumatoide (AR), condicionava una pitjor desposta al tractament amb anti-TNFα. Es va dissenyar un estudi observacional (cohorte retrospectiva) de 66 paciente que es disposaven a iniciar tractament amb anti-TNFα., i dels que es disposava d’una determinación basal de ANA. Es disposava en tots ells un index DAS-28 als 0, 3 i 6 mesos, amb el que es va avaluar la desposta segons els criteris EULAR: No s’observaren diferencies estaísticament significatives entre els pacients ANA-negatius i ANA-positius als 3 mesos de tractament. S’observà una tendència als 6 mesos de tractament entre els ANA-negatius a presentar millor resposta respecte als ANA-positius, sense poder establir-se diferències estadísticamente significatives

La malaltia de Crohn és una malaltia inflamatòria intestinal en la qual s´ha observat que la densitat i la diversitat bacteriana difereixen dels subjectes sans. En els últims anys s’estan emprant tractaments biològics com els anti-TNFα (Adalimumab) de manera alternativa a tractaments ja existents. Els resultats obtinguts després de l’anàlisi de la composició bacteriana de la mucosa intestinal de pacients amb Malaltia de Crohn tractats amb Adalimumab, són esperançadors ja que apunten a una recuperació del patró normal a través de la recuperació d’espècies pròpies de persones sanes i la desaparició d’aquelles més prevalents en malalts de Crohn.

El úso de anti-TNF-α se ha asociado a reactivación de infección tuberculosa latente (ITL). Los IGRAs son pruebas in vitro más sensibles y mas específicas que la prueba de la prueba de la tuberculina (PT). El objetivo de este trabajo es el de contrastar la correlación entre los resultados de las técnicas in vitro y la PT. Analizamos 103 pacientes con psoriasis en tratamiento sistémico. Los resultados de nuestra serie permiten sugerir una mayor sensibilidad y especificidad de los IGRAs en la detección de ITL respecto a la PT, de forma independiente al estado de inmunosupresión y al fármaco empleado, con alta concordancia entre los IGRAs.

Infliximab es un anticuerpo monoclonal efectivo en la psoriasis. Métodos: Estudio retrospectivo de 56 pacientes tratados con infliximab. Resultados: El 82% y 91,1% de los pacientes alcanzaron respuestas PASI 75 en la semana 12 y al año.La probabilidad de supervivencia global del tratamiento fue de 40% a los 4 años. Conclusiones: Infliximab ha demostrado ser efectivo y seguro a corto y largo plazo. La intensificación fue el predictor positivo más importante de aumento de supervivencia del tratamiento, seguido por el tratamiento combinado. El factor predictor negativo de supervivencia más importante parece ser el fracaso previo a otros anti-TNF.

Rats bearing the Yoshida AH-130 ascites hepatoma showed enhanced fractional rates of protein degradation in gastrocnemius muscle, heart, and liver, while fractional synthesis rates were similar to those in non-tumor bearing rats. This hypercatabolic pattern was associated with marked perturbations of the hormonal homeostasis and presence of tumor necrosis factor in the circulation. The daily administration of a goat anti-murine TNF IgG to tumor-bearing rats decreased protein degradation rates in skeletal muscle, heart, and liver as compared with tumor-bearing rats receiving a nonimmune goat IgG. The anti-TNF treatment was also effective in attenuating early perturbations in insulin and corticosterone homeostasis. Although these results suggest that tumor necrosis factor plays a significant role in mediating the changes in protein turnover and hormone levels elicited by tumor growth, the inability of such treatment to prevent a reduction in body weight implies that other mediators or tumor-related events were also involved.

Background In addition to its anticoagulant properties, heparin has anti-inflammatory effects, the molecular and mechanistic bases of which are incompletely defined. AIMS The current studies were designed to test the hypothesis that heparin abrogates the expression or function of leucocyte-endothelial adherence molecules which are fundamental to the acute inflammatory response. Methods The effects of heparin on tumour necrosis factor alpha (TNF-¿) induced leucocyte rolling, adhesion, and migration as well as vascular permeability were assessed in rat mesenteric venules using intravital microscopy. Expression of adhesion molecules was quantitated using a double radiolabelled monoclonal antibody (mAb) binding technique in vivo (P-selectin, intercellular cell adhesion molecule type 1 (ICAM-1), and vascular cell adhesion molecule 1 (VCAM-1)) or flow cytometry (CD11a, CD11b, and L-selectin). Ex vivo binding of heparin to neutrophils was assessed by flow cytometry. RESULTS TNF-alpha induced a significant increase in leucocyte rolling, adhesion, and migration, and vascular permeability, coincident with a significant increase in expression of P-selectin, ICAM-1, and VCAM-1. Ex vivo assessment of blood neutrophils showed significant upregulation of CD11a and CD11b and significant downregulation of L-selectin within five hours of TNF-¿ administration. Heparin pretreatment significantly attenuated leucocyte rolling, adhesion, and migration but did not affect expression of cell adhesion molecules or vascular permeability elicited by TNF-¿ administration. Binding of heparin was significantly increased on blood neutrophils obtained five hours after TNF-¿ administration. Preincubation with an anti-CD11b mAb but not with an anti-CD11a or anti-L-selectin antibody significantly diminished heparin binding ex vivo.

Background and aims: Increased pancreatitis associated protein (PAP) mRNA has been reported in active inflammatory bowel disease (IBD). The aims of the current study were to characterise PAP production in IBD and the effects of PAP on inflammation. Patients and methods: Serum PAP levels were determined in healthy controls (n¿=¿29), inflammatory controls (n¿=¿14), and IBD patients (n¿=¿171). Ex vivo PAP secretion in intestinal tissue was measured in 56 IBD patients and 13 healthy controls. Cellular origin of PAP was determined by immunohistochemistry. The effects of exogenous PAP on nuclear factor ¿B (NF¿B) activation, proinflammatory cytokine production, and endothelial adhesion molecule expression were also analysed ex vivo. Results: Patients with active IBD had increased serum PAP levels compared with controls, and these levels correlated with clinical and endoscopic disease severity. Ex vivo intestinal PAP synthesis was increased in active IBD and correlated with endoscopic and histological severity of inflammatory lesions. PAP localised to colonic Paneth cells. Incubation of mucosa from active Crohn¿s disease with PAP dose dependently reduced proinflammatory cytokines secretion. PAP prevented TNF-¿ induced NF¿B activation in monocytic, epithelial, and endothelial cells and reduced proinflammatory cytokine mRNA levels and adhesion molecule expression. Conclusions: PAP is synthesised by Paneth cells and is overexpressed in colonic tissue of active IBD. PAP inhibits NF¿B activation and downregulates cytokine production and adhesion molecule expression in inflamed tissue. It may represent an anti-inflammatory mechanism and new therapeutic strategy in IBD.

Countries specialised in tourism tend to face two problems with contradictory effects: the commons and the anti-commons, which lead to tourism over- and under-production, respectively. This paper develops a two-period model to analyse the joint effects of both problems on a small and remote tourism economy. Congestion and the complementariness between foreign transport and local tourism services are key features in this type of markets. As a result, direct selling and the presence of foreign tour-operators emerge as possible market arrangements with different implications in terms of welfare and public intervention. Four main results are obtained. First, in the direct selling situation the optimal policy depends on the relative importance of the problems. Second, the existence of tour-operators always leads to tourism over-production. Third, the presence of a single tour-operator does not solve the congestion problem. Lastly, the switch from several tour-operators to a single one is welfare reducing.

Presentem un estudi teòric comparatiu de les sensibilitats en volum dels sensors d’índex de refracció basats en polaritó de plasmó superficial (SPP) i en ressonància localitzada de plasmó superficial (LSPR). Els resultats demostren que el sensor SPP, àmpliament conegut i comercialitzat, ofereix una major sensibilitat en volum comparada amb la del sensor LSPR, estant aquest últim basat en l’ús de nanorods d’or. A més, la figura de mèrit del sensor LSPR, emprada per a analitzar la seva capacitat sensora, exhibeix una regió sensora òptima quan la longitud d’ona de ressonància es troba propera a 700 nm.

In a seminal paper [10], Weitz gave a deterministic fully polynomial approximation scheme for counting exponentially weighted independent sets (which is the same as approximating the partition function of the hard-core model from statistical physics) in graphs of degree at most d, up to the critical activity for the uniqueness of the Gibbs measure on the innite d-regular tree. ore recently Sly [8] (see also [1]) showed that this is optimal in the sense that if here is an FPRAS for the hard-core partition function on graphs of maximum egree d for activities larger than the critical activity on the innite d-regular ree then NP = RP. In this paper we extend Weitz's approach to derive a deterministic fully polynomial approximation scheme for the partition function of general two-state anti-ferromagnetic spin systems on graphs of maximum degree d, up to the corresponding critical point on the d-regular tree. The main ingredient of our result is a proof that for two-state anti-ferromagnetic spin systems on the d-regular tree, weak spatial mixing implies strong spatial mixing. his in turn uses a message-decay argument which extends a similar approach proposed recently for the hard-core model by Restrepo et al [7] to the case of general two-state anti-ferromagnetic spin systems.

El nostre objectiu és avaluar l’impacte de la utilització de fetges de donants anti-HBc positius en pacients trasplantats per hepatitis B, els quals van ser revisats de manera retrospectiva. La probabilitat acumulada d’hepatitis B post-trasplantament al 1er, 5é i 10é any va ésser del 3,7%, 8,3% i 8,3% en els receptors d’un empelt anti-HBc+ en comparació amb el 2,7%, 9,8% i 9,8% en els que varen rebre un empelt anti-HBc- (p=0,99). La probabilitat de supervivència al 1er, 5é i 10é any dels receptors d’un òrgan anti-HBc+ va ésser del 87%, 82% i 58% front al 87%, 82% i 74% en els que varen rebre un anti-HBc- (p=0,73). La utilització de donants anti-HBc positius no afecta ni a la supervivència ni al desenvolupament de hepatitis B en pacients trasplantats per VHB.
