Las infiltraciones intra-articulares de concorticoesteroides suelen indicarse como tratamiento coadyuvante en casos de presencia de dolor y limitación de la movilidad. La información existente sobre los efectos en el metabolismo hidrocarbonado son muy escasos y dispares. El objetivo de este estudio es definir el patrón y grado de la hiperglucemia después de la administración intra-articular de acetónido de triamcinolona en pacientes sin diabetes y con diabetes tipo 2. Se realiza un estudio observacional y prospectivo dónde se incluyen 14 pacientes a quienes se les ha indicado una infiltración en la rodilla o el hombro. Recogemos las glucemias preprandiales y 2 horas postprandiales, del día previo y los 6 días posteriores a la infiltración. En los pacientes sin diabetes, únicamente las glucemias medias el día de la infiltración y el siguiente fueron superiores a las del día previo a la infiltración. En los pacientes con diabetes no observamos diferencias estadísticamente significativas.

La diabetis mellitus és un trastorn metabòlic a causa del dèficit de secreció d'insulina i a un augment de la seva resistència perifèrica. En el present estudi es van determinar les causes específiques de mortalitat en pacients amb diabetis mellitus. Es va observar que la mortalitat cardiovascular segueix sent la principal causa de mort en pacients amb diabetis metllitus tipus 2, seguida de la mortalitat per neoplàsies. La mortalitat total i específica per patologia cardiovascular o per neoplàsies va ser significativament superior en els pacients homes.

Dels pacients amb DM1 diagnosticats entre 1985-1994 en el nostre centre, vàrem seleccionar 147 amb seguiment ininterromput des del debut i durant més de 10 anys. Vàrem revisar les dades registrades anualment relacionades amb el tractament, control glicèmic i risc cardiovascular, amb els objectius d'avaluar el Grau de control, l'existència de factors predictors, la prevalença de complicacions tardanes i la seva relació amb el control glucèmic. Vàrem objectivar que la insulinoteràpia intensiva permetia mantenir un control glucèmic adequat, i que el grau de control a l'any del diagnòstic predeia el control posterior. La prevalença de complicacions va ser inferior a la descrita prèviament.

El present estudi és de tipus observacional, descriptiu i analític, retrospectiu i longitudinal. Pretén analitzar-ne en dones del departament de Salut 6 de la Comunitat Valenciana diagnosticades de diabetis gestacional, la alteració en la tolerancia a la glucosa en una mitjana d'1,5 anys després del part. La mostra presa va ser de 128 pacients. El 22, 66% va mostrar alteració glucèmica postpart; les dones amb antecedents familiars de diabetis eren més jóvens (32,06 front a 34,93, amb p=0,04).Ninguna dona tractada amb dieta va desenvolupar diabetis; de les tractades amb insulina ho va fer el 8,16% (p=0,02) i tenien major glucèmia basal (p=0,048).

La diabetes mellitus (DM) está considerada como uno de los principales problemas de salud mundial con cifras que mantiene un crecimiento continuado año tras año. Por otro lado el creciente número de inmigrantes en nuestro país supone un reto para los profesionales de la salud debido a las implicaciones culturales, socio-económicas y linguísticas que conlleva. Cada vez más se hace patente la necesidad de adaptar los cuidados a unas necesidades cada vez menos homogéneas debido a la variedad de realidades culturales existentes hoy día. Una de estas realidades culturales más importantes en nuestro país la conforman los marroquíes. Presentan una mayor prevalencia de DM y un peor control de la enfermedad que la población española. Además de factores genéticos y soci- económicos, los aspectos culturales juegan un importante papel en el manejo de la DM. La vivencia y el conocimiento que tengan sobre la enfermedad facilitará o no una adherencia adecuada y continuada a las pautas de autocuidado que requiere la DM. El objetivo es conocer el significado que la población inmigrante de origen marroquí da a la DM y su experiencia respecto al hecho de vivir con la enfermedad. *Población: Pacientes marroquíes con DM residentes en Barcelona. El estudio se llevará a cabo durante el año 2012 en centros de Atención Primaria del distrito de Ciutat Vella, en Barcelona. Estos centros prestan asistencia a un alto porcentaje de población inmigrante. *Metodología: Se opta por un enfoque cualitativo de tipo etnográfico por considerarlo más adecuado a los objetivos del estudio. Recogida de Información: Observación participante y entrevistas semiestructuradas. *Análisis de Datos: Se realizará un análisis descriptivo de contenido temático siguiendo las etapas descritas por Huberman y Miles.

El trabajo estudia de forma prospectiva las características clínicas y la morbilidad perinatal de las gestantes de cinco grupos étnicos distintos con diabetes gestacional y tratadas en nuestro centro con el objetivo de valorar la influencia de la etnia en los resultados maternofetales e identificar los factores asociados de forma indepeniente al desarrollo de neonatos grandes para la edad gestacional.

El objetivo es comparar la prevalencia de fractura en casos incidentes de diabetes y en controles apareados. Estudio casos-control que incluye todos los pacientes diabéticos tipo 2 diagnosticados entre 2006-2011 y dos controles sin diabetes. Se identificaron fracturas y enfermedad cardiovascular prevalentes, calculando prevalencias de fracturas osteoporóticas, mayores y de cadera, y de enfermedad cardiovascular. Se identificaron 58931 diabéticos y 117862 controles. Los diabéticos presentaban mayor prevalencia de accidente cerebro-vascular y de cardiopatía isquémica. Las prevalencias de fracturas osteoporóticas, de cadera y mayores fue similar en ambos grupos. En conclusión el riesgo de fractura es similar al de los controles no diabéticos.

As the prevalence of smoking has decreased to below 20%, health practitioners interest has shifted towards theprevalence of obesity, and reducing it is one of the major health challenges in decades to come. In this paper westudy the impact that the final product of the anti-smoking campaign, that is, smokers quitting the habit, had onaverage weight in the population. To these ends, we use data from the Behavioral Risk Factors Surveillance System,a large series of independent representative cross-sectional surveys. We construct a synthetic panel that allows us tocontrol for unobserved heterogeneity and we exploit the exogenous changes in taxes and regulations to instrumentthe endogenous decision to give up the habit of smoking. Our estimates, are very close to estimates issued in the 90sby the US Department of Health, and indicate that a 10% decrease in the incidence of smoking leads to an averageweight increase of 2.2 to 3 pounds, depending on choice of specification. In addition, we find evidence that the effectovershoots in the short run, although a significant part remains even after two years. However, when we split thesample between men and women, we only find a significant effect for men. Finally, the implicit elasticity of quittingsmoking to the probability of becoming obese is calculated at 0.58. This implies that the net benefit from reducingthe incidence of smoking by 1% is positive even though the cost to society is $0.6 billions.

Background: To compare the characteristics and prognostic features of ischemic stroke in patients with diabetes and without diabetes, and to determine the independent predictors of in-hospital mortality in people with diabetes and ischemic stroke.Methods: Diabetes was diagnosed in 393 (21.3%) of 1,840 consecutive patients with cerebral infarction included in a prospective stroke registry over a 12-year period. Demographic characteristics, cardiovascular risk factors, clinical events, stroke subtypes, neuroimaging data, and outcome in ischemic stroke patients with and without diabetes were compared. Predictors of in-hospital mortality in diabetic patients with ischemic stroke were assessed by multivariate analysis. Results: People with diabetes compared to people without diabetes presented more frequently atherothrombotic stroke (41.2% vs 27%) and lacunar infarction (35.1% vs 23.9%) (P < 0.01). The in-hospital mortality in ischemic stroke patients with diabetes was 12.5% and 14.6% in those without (P = NS). Ischemic heart disease, hyperlipidemia, subacute onset, 85 years old or more, atherothrombotic and lacunar infarcts, and thalamic topography were independently associated with ischemic stroke in patients with diabetes, whereas predictors of in-hospital mortality included the patient's age, decreased consciousness, chronic nephropathy, congestive heart failure and atrial fibrillation. Conclusion: Ischemic stroke in people with diabetes showed a different clinical pattern from those without diabetes, with atherothrombotic stroke and lacunar infarcts being more frequent. Clinical factors indicative of the severity of ischemic stroke available at onset have a predominant influence upon in-hospital mortality and may help clinicians to assess prognosis more accurately.

Objective: To examine whether drawing is useful in the detection of problems of psychosocial adaptation in children and adolescents with type 1 diabetes (T1D) and in improving communication with health professionals. Methods: We performed an exploratory descriptive study in 199 children and adolescents with T1D aged 413 years. The participants were asked to render a drawing on a suggested topic. The variables analyzed were related to the drawing and to clinical and sociodemographic data. Results: Most participants showed evidence of having a well-balanced personality, but there were also signs of affective or psychosocial difficulties. Conclusion: Drawing is a useful technique by which to identify children"s and adolescents" feelings and possible problems in adapting to T1D, as well as to gain information directly from the children themselves. Future studies should delimit the possibilities of this technique in clinical practice in greater detail.

Methods: A descriptive, exploratory evaluation study using qualitative methodology was performed. Participants consisted of 40 adult patients diagnosed with DM and followed up in a public hospital in Barcelona, Spain. A semistructured interview and a focus group were used and a thematic content analysis was performed. Results: Patients described DM as a disease that is difficult to control and that provokes lifestyle changes requiring effort and sacrifice. Insulin treatment increased the perception of disease severity. The most frequent and dreaded complication was hypoglycemia. The main problems perceived by patients affecting the quality of care were related to a disease-centered medical approach, lack of information, limited participation in decision-making, and the administrative and bureaucratic problems of the health care system. Conclusion: The bureaucratic circuits of the health care system impair patients" quality of life and perceived quality of care. Health professionals should foster patient participation in decision-making. However, this requires not only training and appropriate attitudes, but also adequate staffing and materials.

Type 1 diabetic patients depend on external insulin delivery to keep their blood glucose within near-normal ranges. In this work, two robust closed-loop controllers for blood glucose regulation are developed to prevent the life-threatening hypoglycemia, as well as to avoid extended hyperglycemia. The proposed controllers are designed by using the sliding mode control technique in a Smith predictor structure. To improve meal disturbance rejection, a simple feedforward controller is added to inject meal-time insulin bolus. Simulations scenarios were used to test the controllers, and showed the controllers ability to maintain the glucose levels within the safe limits in the presence of errors in measurements, modeling and meal estimation
