Report for the scientific sojourn carried out at the Columbia University, United States, from 2010 to 2012. Expression of SoxB genes correlates with the commitment of cells to a neural fate; however, the relevance of SoxB proteins in early vertebrate neurogenesis has been difficult to prove genetically due to embryonic lethality and presumed redundant functions. The nematode C. Elegants has only 5 sox genes: sox-2 and sox-3 form the SoxB group while sem-2, sox-4 and egl-13 belong to other Sox groups. Our results show that sox-2 and sem-2 are the sox genes expressed earliest and in a broader manner during embryogenesis, being expressed in several neuronal progenitors. sox-3, sox-4 and egl-13 are expressed in few cells during late embryogenesis, when most neurons are already born. Both sox-2 and sem-2 null mutants are early larval lethal but do not show neuronal specification defects during embryonic development as indicated by quantification of a panneuronal reporter. Potential redundancy or compensatory mechanisms between different sox genes have been ruled out, strongly suggesting that sox genes are not required for specification of embryonically-derived neurons. However, at the first larval stage there are still several blast cells that will give rise to different postembryonic lineages, which generate several neurons amongst other cell types. nterestingly, sox-2 is expressed in many of these progenitor cells. Using mosaic analysis we have so far identified neurons derived from two different postembryonic lineages which fail to be generated in C. elegans sox-2 mutants. These results support the idea that postembryonic progenitor competence is compromised in the absence of sox-2.

S’ha elaborat una anàlisi del sistema agroecològic del municipi de Vila-seca (Tarragonès) per tal de realitzar en segon lloc, una proposta de guia per a la descoberta del medi per a infants d’entre 8 i 10 anys. A partir del treball de camp realitzat a la zona d’estudi s’han inventariat diferents elements del patrimoni socioecològic que es consideren de rellevada importància al terme. Entre ells destaquen oliveres, garrofers, avellaners, arbres singulars, masos, basses, marges de pedra seca i trones, els quals s’integren en el medi formant un veritable sistema agroecològic, fruit de la interacció entre vectors biofísics i socioeconòmics. D’altra banda, per tal de donar a conèixer aquest sistema s’ha realitzat una proposta de descoberta del medi agrícola a partir d’un itinerari d’educació ambiental per la zona estudiada, on s’engloben tots aquells elements d’especial interès en un recorregut de 2,9 quilòmetres. La guia disposa a més d’una sèrie d’activitats a realitzar a l’entorn per tal de motivar el sentiment de descoberta d’aquest sistema. D’aquesta manera, considerant les fortes pressions que aquest territori pateix per part de les infraestructures i les urbanitzacions és destriable la necessitat de conservació i l’addició dels elements inventariats al catàleg del Patrimoni del municipi.

El incidentaloma adrenal es aquella masa suprarrenal hallada casualmente al realizar una prueba de imagen por otro motivo. El estudio debe determinar su benignidad y funcionalidad. La controversia radica en el seguimiento. Se presenta un estudio de 67 pacientes con indicentaloma adrenal recogido en 5 años, estudio inicial y seguimiento a largo plazo. Se halló un 74,6% adenomas no funcionantes, 7,4% mielolipomas, 4,5% Síndrome de Cushing, 2,9% Hiperaldosteronismo primario, 1,5% feocromocitoma y metástasis. El seguimiento a largo plazo se realizó a los 6 y 18-24 meses, sin cambios. El estuido realizado apoya las guías actuales.

Introducció. La cirurgia de càncer de mama està associada a morbiditat de l’extremitat superior (ES) tributària de prevenció i de tractament rehabilitador. Objectius: Identificar la morbiditat de l’ES després de limfadenectomia axil·lar (LA) o biòpsia selectiva del gangli sentinella (BSGS); el moment del diagnòstic i quan és tributaria de tractament rehabilitador. Estudiar la relació d’aquesta morbiditat amb variables clíniques periquirúrgiques. Material i mètodes. Estudi longitudinal descriptiu amb 2 anys seguiment. Es recolliren les variables edat, pes, talla, tipus de tractament quirúrgic, complicacions peri-quirúrgiques, alteracions de l’espatlla, limfedema, dolor neuropàtic, lesions neurològiques i la indicació de tractament rehabilitador per aquestes alteracions. Les relacions es van estudiar amb t-test o ji quadrat, amb significació p& 0,05 Resultats: Es van incloure 312 pacients. Van requerir tractament rehabilitador 133 (42,6%) pacients, 81 (26,0%) per alteracions de l’espatlla, 63 (20,2%) per limfedema, 21 (6,7%) per dolor neuropàtic, i 9 (2.9%) per escàpula alada. Es van determinar relacions significatives de limfedema amb LA (p=0.000); les alteracions de l’espatlla amb les complicacions periquirúrgiques (p=0.04); dolor neuropàtic amb edat més jove (p=0.004); i l’escàpula alada amb hematoma (p=0.000). Més del 70% dels diagnòstics i tractaments de l’espatlla i limfedema es van realitzar durant el primer any de seguiment. Conclusions: Van requerir tractament rehabilitador 42.6% de pacients. Les alteracions de l’espatlla i el limfedema varen ser les més freqüentment tractades. La majoria de diagnòstics i tractament s’efectuaren durant el primer any de seguiment.

BACKGROUND--Oesophageal motor abnormalities have been reported in alcoholism. AIM--To investigate the effects of chronic alcoholism and its withdrawal on oesophageal disease. PATIENTS--23 chronic alcoholic patients (20 men and three women; mean age 43, range 23 to 54). METHODS--Endoscopy, manometry, and 24 hour pH monitoring 7-10 days and six months after ethanol withdrawal. Tests for autonomic and peripheral neuropathy were also performed. Motility and pH tracings were compared with those of age and sex matched control groups: healthy volunteers, nutcracker oesophagus, and gastro-oesophageal reflux disease. RESULTS--14 (61%) alcoholic patients had reflux symptoms, and endoscopy with biopsy showed oesophageal inflammation in 10 patients. One patient had an asymptomatic squamous cell carcinoma. Oesophageal motility studies in the alcoholic patients showed that peristaltic amplitude in the middle third was > 150 mm Hg (95th percentile (P95) of healthy controls) in 13 (57%), the ratio lower/ middle amplitude was < 0.9 in 15 (65%) (> 0.9 in all control groups), and the lower oesophageal sphincter was hypertensive (> 23.4 mm Hg, P95 of healthy controls) in 13 (57%). All three abnormalities were present in five (22%). Abnormal reflux (per cent reflux time > 2.9, P95 of healthy controls) was shown in 12 (52%) alcoholic patients, and was unrelated to peristaltic dysfunction. Subclinical neuropathy in 10 patients did not effect oesophageal abnormalities. Oesophageal motility abnormalities persisted at six months in six patients with ongoing alcoholism, whereas they reverted towards normal in 13 who remained abstinent; reflux, however, was unaffected. CONCLUSIONS--Oesophageal peristaltic dysfunction and reflux are frequent in alcoholism. High amplitude contractions in the middle third of the oesophagus seem to be a marker of excessive alcohol consumption, and tend to improve with abstinence.

Abstract Background: Effective promotion of exercise could result in substantial savings in healthcare cost expenses in terms of direct medical costs, such as the number of medical appointments. However, this is hampered by our limited knowledge of how to achieve sustained increases in physical activity. Objectives: To assess the effectiveness of a Primary Health Care (PHC) based physical activity program in reducing the total number of visits to the healthcare center among inactive patients, over a 15-month period. Research Design: Randomized controlled trial. Subjects: Three hundred and sixty-two (n = 362) inactive patients suffering from at least one chronic condition were included. One hundred and eighty-three patients (n = 183; mean (SD); 68.3 (8.8) years; 118 women) were randomly allocated to the physical activity program (IG). One hundred and seventy-nine patients (n = 179; 67.2 (9.1) years; 106 women) were allocated to the control group (CG). The IG went through a three-month standardized physical activity program led by physical activity specialists and linked to community resources. Measures: The total number of medical appointments to the PHC, during twelve months before and after the program, was registered. Self-reported health status (SF-12 version 2) was assessed at baseline (month 0), at the end of the intervention (month 3), and at 12 months follow-up after the end of the intervention (month 15). Results: The IG had a significantly reduced number of visits during the 12 months after the intervention: 14.8 (8.5). The CG remained about the same: 18.2 (11.1) (P = .002). Conclusions: Our findings indicate that a 3-month physical activity program linked to community resources is a shortduration, effective and sustainable intervention in inactive patients to decrease rates of PHC visits. The rate of leaf appearance of barley varies substantially with time of sowing. This variation has been related to both the length and the rate of change of photoperiod at the time of plant emergence. An outdoor pot experiment was conducted to test if rate of change of photoperiod directly affects phasic development and rate of leaf emergence of spring barley. Two photoperiod-sensitive cultivars (Bandulla and Galleon) were subjected to five photoperiod regimes: two constant photoperiods, of 14 and 15·5 h, and three different rates of change of photoperiod of c. 2, 9 and 13 min/day from seedling emergence to awn initiation. Photoperiod treatments significantly affected the duration from seedling emergence to awn initiation in both cultivars. Rate of change of photoperiod did not affect the rate of development towards awn initiation independently of the absolute daylength it produced. Although Bandulla had a longer duration than Galleon at any photoperiod regime, the cultivars did not vary in their sensitivity to photoperiod. When this phase was divided into the leaf initiation (LI) and spikelet initiation (SI) phases, it was evident that the sensitivity to photoperiod was not constant, being in general higher during the SI than during the LI phase. However, the magnitude of the change in sensitivity was cultivar-dependent, indicating that sensitivity to photoperiod during the different phases could be under independent genetic control. Final numbers of primordia (leaves together with maximum spikelet number) were negatively affected by increasing photoperiods, but once again, there was no evidence of any effect of the rate of change of photoperiod which was independent of the average photoperiod. Both cultivars showed similar sensitivities for final leaf number but maximum spikelet number was more sensitive to photoperiod in Galleon than in Bandulla. Highly significant linear relationships between leaf number and thermal time were found for all combinations of cultivars and photoperiod regimes (r2 > 0·98). The rate of leaf appearance (RLA) was similar for both cultivars (c. 0·0185 leaves/°Cd) and did not alter during plant development or in response to the change in photoperiod at awn initiation. The range in RLA was greater for Galleon (0·0170–0·0205 leaves/°Cd) than for Bandulla (0·0173–0·0186 leaves/°Cd). Neither of these cultivars exhibited a significant relationship between rate of leaf emergence and photoperiod or rate of change of photoperiod. The lack of significant relationships between RLA and length or rate of change of photoperiod is in contrast with previous reports using time of sowing as a main treatment.
