Amb l'augment de l’ús de la quimioradioteràpia concomitant (CCRT) al tractament del carcinoma avançat de cap i coll, la cirurgia ha perdut terreny com a primer tractament i s’ha reservat pel tractament de rescat en cas de recidiva loco-regional. L’objectiu de l’estudi es revisar la nostra experiència en pacients amb recidiva local o regional després del tractament amb CCRT i als que se’ls hi va realitzar cirurgia de rescat. El 83% del pacients amb cirurgia de rescat en el lloc del tumor primari van requerir algun tipus de reconstrucció amb penjalls regionals o lliures microanastomosats.

Objectiu: avaluar quines característiques del pacient i de la tècnica influeixen en la predicció de dificultat en la realització d’un bloqueig neuroaxial. Material i mètodes: estudi observacional prospectiu de 539 pacients sotmesos a un bloqueig regional neuroaxial. Registrem dades demogràfiques, anatòmiques, relacionades amb la tècnica (palpació d’estructures de referència, grau de flexió, morfologia de la curvatura lumbar), experiència anestesiòleg i número d’intents amb/sense redirigir l’agulla. Variable resposta: punció fàcil/difícil. Resultats i conclusions: variables predictives independents de dificultat: flexió dolenta/regular, palpació dolenta/regular d’estructures anatòmiques de referència i curvatura lumbar no convexa. Anatomia, edat i IMC van quedar excloses.

An examination of the impact in the US and EU markets of two major innovations in the provision of air services on thin routes - regional jet technology and the low-cost business model - reveals significant differences. In the US, regional airlines monopolize a high proportion of thin routes, whereas low-cost carriers are dominant on these routes in Europe. Our results have different implications for business and leisure travelers, given that regional services provide a higher frequency of flights (at the expense of higher fares), while low-cost services offer lower fares (at the expense of lower flight frequencies). Employment flexibility is commonly associated to greater labour mobility and thus faster cross-regional adjustments. The literature however offers very little hard evidence on this and quite limited theoretical guidance. This paper examines empirically the relationship between employment flexibility and cross-regional adjustment (migration) at the regional and local levels in the UK. Employment flexibility is associated to higher labour mobility (but only at a rather localised scale) and at the same time seems to reduce the responsiveness of migration to unemployment. This suggest that rising flexibility may be linked to higher persistence in spatial disparities, as intra-regional adjustments are strengthened while extraregional adjustments weakened. En el nostre projecte, considerem un escenari urbà o interurbà on persones amb dispositius mòbils (smartphones) o vehicles equipats amb interfícies de comunicació, estan interessats en compartir fitxers entre ells o descarregar-los al creuar Punts d’Accés (APs) propers a la carretera. Estudiem la possibilitat d’utilizar la cooperació en les trobades casuals entre nodes per augmentar la velocitat de descàrrega global. Amb aquest objectiu, plantejem algoritmes per a la selecció de quins paquets, per a quins destins i quins transportistes s’escullen en cada moment. Mitjançant extenses simulacions, mostrem com les cooperacions carry&forward dels nodes augmenten significativament la velocitat de descàrrega dels usuaris, i com aquest resultat es manté per a diversos patrons de mobilitat, col•locacions d'AP i càrregues de la xarxa. Per altra banda, aparells com els smartphones, on la targeta de WiFi està encesa contínuament, consumeixen l'energia de la bateria en poques hores. En molts escenaris, una targeta WiFi sempre activa és poc útil, perque sovint no hi ha necessitat de transmissió o recepció. Aquest fet es veu agreujat en les Delay Tolerant Networks (DTN), on els nodes intercanvien dades quan es creuen i en tenen l’oportunitat. Les tècniques de gestió de l’estalvi d’energia permeten extendre la duració de les bateries. El nostre projecte analitza els avantatges i inconvenients que apareixen quan els nodes apaguen períodicament la seva targeta wireless per a estalviar energia en escenaris DTN. Els nostres resultats mostren les condicions en que un node pot desconnectar la bateria sense afectar la probabilitat de contacte amb altres nodes, i les condicions en que aquesta disminueix. Per exemple, es demostra que la vida del node pot ser duplicada mantenint la probabilitat de contacte a 1. I que aquesta disminueix ràpidament en intentar augmentar més la vida útil.

El objetivo de este estudio es analizar el impacto, en emisiones de CO2, de la demanda final de Cataluña en relación a los vínculos comerciales interregionales con el resto de España y el resto del mundo. Este proceso implica el análisis del balance en CO2 incorporado para Cataluña, lo que permitirá evaluar la responsabilidad de la economía catalana respecto a estas emisiones. Para este propósito se construye, para esta determinada desagregación regional, un modelo Multi-Regional Input-Output (MRIO) extendido al medioambiente con sectores verticalmente integrados. La incorporación de la técnica de la integración vertical nos permite un enfoque alternativo para el Balance Neto y un análisis más detallado de los vínculos interregionales entre los diversos sectores productivos, centrado en la responsabilidad última de la demanda final de cada sector en cada región. Hasta el momento, los estudios previos sobre los impactos medioambientales incorporados al comercio español se han centrado principalmente en el ámbito nacional. No obstante, por un lado el comercio interregional con el resto de España en términos monetarios representa cerca de la mitad del comercio exterior catalán. Por otro lado, los distintos metabolismos energéticos de ambas economías tienen como consecuencia una importante diferencia en la intensidad de emisión en la producción de bienes y servicios. Esta situación genera para Cataluña un déficit en el Balance Neto estimado con el resto de España, aún teniendo un importante superávit monetario. De esto se desprende la importancia de integrar el nivel interregional en los estudios de los impactos medioambientales incorporados en el comercio y, en consecuencia, en la planificación y formalización de políticas económicas y ambientales a nivel nacional.

The 51st ERSA Conference held in Barcelona in 2011 was one of the largest ever. Here, by examining the characteristics of the conference, this paper identifies the main trends in Regional Science at a moment in which the discipline is renewing its efforts to provide responses in a complex, globalised world in which cities and regions are acquiring greater and greater importance. This paper follows in the tradition of a long list of studies that have examined the nature of the field of Regional Science and draws on a broad array of sources of information: the delegates’ demographic details, the conference program itself, a satisfaction survey conducted among delegates, a quality survey addressed to those chairing the sessions and, finally, a bibliometric database including each author signing a paper presented at the conference. With this information we describe the ERSA delegates: their relative youthfulness; the areas in which women are taking on a more important role; the countries and regions of the world that have the most dominant profile in Regional Science today; the thematic areas that are being driven by professionals as opposed to academics; the relevance of regional economic growth and innovation as trending topics in the field; the growing frequency of co-authorship and, consequently, of scientific collaboration; and, finally, and perhaps most importantly, the continuous enhancement of the quality of the work being undertaken in the discipline. Indeed, following on from this description, the results of the regression analysis conducted show that for ERSA delegates what matters most is quality, and this must be the direction that future conferences should move toward. Ultimately, therefore, ERSA conferences are comprehensive, all-embracing occasions, representing an ideal opportunity for regional scientists to present their work to each other and to network.

This note contains some addenda to a recent paper (de la Fuente, 2010) that analyzed the new Spanish system of regional financing and estimated its financial results for 2009. The note updates my previous estimates using the information contained in the recently published liquidation of the system for 2009. It also discusses some aspects of the new financing law that have to do with the future evolution of the system and collects some data that may be useful to project its results.

Esta memoria parte del estudio de la teoría de las Relaciones Internacionales, seguido de un análisis del regionalismo en el Este asiático, así como del multilateralismo y del minilateralismo como modus operandi en las relaciones internacionales y su desarrollo específico en el Noreste asiático, o Asia oriental. Todo ello se ha examinado con el fin de poder aprehender la dinámica internacional de la segunda crisis nuclear con Corea del Norte, así como proveer con un mayor entendimiento sobre las conversaciones a seis bandas (6PT), como ejemplo de foro de negociación minilateral creado ad hoc con el fin de buscar una solución al problema norcoreano. Hemos procedido al examen del pensamiento estratégico de los diferentes países que conforman las 6PT, así como al estudio de los intereses, acuerdos y desacuerdos de cada uno de ellos en la mesa de negociaciones con el objetivo de intentar comprender el porqué de su aparente fracaso a la hora de solucionar dicha crisis nuclear. Finalmente, hemos procedido a un estudio de la comunidad del Noreste asiático y del marco de seguridad existente en la región, así como de las perspectivas para la institucionalización de las conversaciones a seis bandas para poder comprender si, desarrollado como organización multilateral, este foro podría favorecer la estabilidad en la región, la cooperación regional y con ello una mayor integración.

Our purpose in this article is to define a network structure which is based on two egos instead of the egocentered (one ego) or the complete network (n egos). We describe the characteristics and properties for this kind of network which we call “nosduocentered network”, comparing it with complete and egocentered networks. The key point for this kind of network is that relations exist between the two main egos and all alters, but relations among others are not observed. After that, we use new social network measures adapted to the nosduocentered network, some of which are based on measures for complete networks such as degree, betweenness, closeness centrality or density, while some others are tailormade for nosduocentered networks. The paper analyses the regional flows of domestic tourism that took place in Spain in year 2000, contributing to the state of knowledge on tourism required by authorities and private firms when faced with decision making, for example, for regional infrastructure planning. Although tourism is one of the main income-generating economic activities in Spain, domestic tourism has received little attention in the literature compared to inbound tourism. The paper uses among others, gravitational model tools and concentration indices, to analyse regional concentration of both domestic demand and supply; tourism flows among regions, and the causes that may explain the observed flows and attractiveness between regions. Among the most remarkable results are the high regional concentration of demand and supply, and the role of population and regional income as explanatory variables. Also remarkable are the attractiveness of own region and neighbour ones, and that domestic tourism may be acting as a regional income redistributing activity

This paper presents a new charging scheme for cost distribution along a point-to-multipoint connection when destination nodes are responsible for the cost. The scheme focus on QoS considerations and a complete range of choices is presented. These choices go from a safe scheme for the network operator to a fair scheme to the customer. The in-between cases are also covered. Specific and general problems, like the incidence of users disconnecting dynamically is also discussed. The aim of this scheme is to encourage the users to disperse the resource demand instead of having a large number of direct connections to the source of the data, which would result in a higher than necessary bandwidth use from the source. This would benefit the overall performance of the network. The implementation of this task must balance between the necessity to offer a competitive service and the risk of not recovering such service cost for the network operator. Throughout this paper reference to multicast charging is made without making any reference to any specific category of service. In this paper, we define a new scheme to develop and evaluate protection strategies for building reliable GMPLS networks. This is based on what we have called the network protection degree (NPD). The NPD consists of an a priori evaluation, the failure sensibility degree (FSD), which provides the failure probability, and an a posteriori evaluation, the failure impact degree (FID), which determines the impact on the network in case of failure, in terms of packet loss and recovery time. Having mathematical formulated these components, experimental results demonstrate the benefits of the utilization of the NPD, when used to enhance some current QoS routing algorithms in order to offer a certain degree of protection

In this paper a novel methodology aimed at minimizing the probability of network failure and the failure impact (in terms of QoS degradation) while optimizing the resource consumption is introduced. A detailed study of MPLS recovery techniques and their GMPLS extensions are also presented. In this scenario, some features for reducing the failure impact and offering minimum failure probabilities at the same time are also analyzed. Novel two-step routing algorithms using this methodology are proposed. Results show that these methods offer high protection levels with optimal resource consumption

Quantitatively assessing the importance or criticality of each link in a network is of practical value to operators, as that can help them to increase the network's resilience, provide more efficient services, or improve some other aspect of the service. Betweenness is a graph-theoretical measure of centrality that can be applied to communication networks to evaluate link importance. However, as we illustrate in this paper, the basic definition of betweenness centrality produces inaccurate estimations as it does not take into account some aspects relevant to networking, such as the heterogeneity in link capacity or the difference between node-pairs in their contribution to the total traffic. A new algorithm for discovering link centrality in transport networks is proposed in this paper. It requires only static or semi-static network and topology attributes, and yet produces estimations of good accuracy, as verified through extensive simulations. Its potential value is demonstrated by an example application.
