Changes in the informal caregiver life styles, relations and alterations of mental health. Justification.The increase of the population aged 65 years and older in developed countries is generating a proportional increasing of the dependency caused by chronic pathologies. The care of these persons is being carried out by relatives, informal carers, leading to some alterations in diverse areas, among them mental alterations, as a consequence of taking care. Objectives. To develop a profile of the informal carers and to correlate the caregiver burden with the carers disorders. To detect risk factors to allow carrying out premature nursing interventions to diminish the disorders in the informal carers. Design. An observational and cross-sectional study is made in primary care health services area of the city of Girona. Methodology. Random sample of carer of dependent people at their home , assigned to the program of domiciliary attention (ATDOM) of the educational basic areas of health (ABS). A sample size of 269 persons, with a level of precision of 9% was needed. Questionnaire ICUB 97, the test of Barthel, the Philadelphia Geriatric Center scale, and a questionnaire are used as instruments that includes demographic and social variables, and mental and physical carers disorders.Results. A total of 80 participants were interviewed. The most common problems as consequence of taking care were the backache, to feel more tired, to feel impotent and to have less free time. Correlation between the level of dependence of the patient with the number of problems perceived by the caretaker does not exist. Since they are informal carers they eat frequently outside the habitual schedule, took place an increase or loss of weight, experienced more disorders in sexual life and an increasing lack of interest in surroundings. A 33% of the informal carers are being treated for problems related to the mental health.Conclusion. The fact of being informal carer carries important consequences in their health, the lifestyles and the social relations. These can be attenuated if the nursing interventions are able to consider to the well-taken care of subject, and its surroundings in an holistic way. The professionals of infirmary of mental health must contribute to their knowledge and abilities contributing to make an assessment, a diagnosis and an intervention paying special attention to psycho-social aspects mentioned

Aim. To evaluate the usefulness of COOP/WONCA charts as a screening tool for mental disorders in primary care in the immigrant healthcare users in Salt. To measure self-rated health of Salt immigration population using the COOP / WONCA charts and to assess its associated factorsDesign. Descriptive and transversal studyParticipants. 370 non-EU immigrants seniors selected by consecutive sampling stratified by sexMain measures. Personal information will be collected (age, sex, country of origin, years of residency in Spain, number of people living in the household and associated comorbidities). Each participant will complete the COOP/WONCA charts. An analysis of the validity of the diagnostic test will be done: sensibility, specificity, positive predictive value, negative predictive value, ROC curve and area under the curve (AUC). All variables will be subjected to descriptive analysis. Bivariate and multivariate analysis between the variables collected (sex, years of residency in Spain... ) and the results of COOP / WONCA charts will be performedResults. Preliminary results are available on a pilot test with 30 patients. The mental disorder prevalence is around 30%. Sensibility (0,89), specificity (0,89), VPP (0,80), VPN (0,94) cutoff score (3.5) and AUC (0,941). Women, people with 10 or more years of residency in Spain and unemployed people have worse self-rated healthConclusions. Based on the preliminary results, is possible to conclude that COOP/WONCA charts could be an useful, valid and applicable screening test for mental disorders in primary care with immigrant population

En este artículo se pretende enfatizar el concepto de promoción de la salud mental; un espacio reservado para las intervenciones dirigidas a reforzar, mantener, incrementar y, en global, potenciar el bienestar psicológico de las personas 'mentalmente sanas'. Para su comprensión se introduce el constructo 'salud mental positiva', cuyo significado está relacionado tanto con estados de bienestar y felicidad como con situaciones de sufrimiento y conflicto emocional. Se enfatiza la necesidad de discriminar la normalidad/anormalidad de esos estados psicológicos, teniendo en cuenta la complejidad de la estructura mental del ser humano y las dificultades que existen para determinar lo que es salud y lo que es enfermedad mental. Asimismo, se vincula la promoción de la salud mental al concepto de autocuidado, relacionando la capacidad de cuidarse uno mismo con la capacidad para cuidar a los demás. Esta relación resulta especialmente significativa para la enfermería, como profesión centrada en el cuidado. El artículo finaliza con una serie de sugerencias para la promoción de la salud mental positiva.

Background: A holistic perspective on health implies giving careful consideration to the relationship between physical and mental health. In this regard the present study sought to determine the level of Positive Mental Health (PMH) among people with chronic physical health problems, and to examine the relationship between the observed levels of PMH and both physical health status and socio-demographic variables. Methods: The study was based on the Multifactor Model of Positive Mental Health (Lluch, 1999), which comprises six factors: Personal Satisfaction (F1), Prosocial Attitude (F2), Self-control (F3), Autonomy (F4), Problem-solving and Self-actualization (F5), and Interpersonal Relationship Skills (F6). The sample comprised 259 adults with chronic physical health problems who were recruited through a primary care center in the province of Barcelona (Spain). Positive mental health was assessed by means of the Positive Mental Health Questionnaire (Lluch, 1999). Results: Levels of PMH differed, either on the global scale or on specific factors, in relation to the following variables: age: global PMH scores decreased with age (r=-0.129; p=0.038); b) gender: men scored higher on F1 (t=2.203; p=0.028) and F4 (t=3.182; p=0.002), while women scored higher on F2 (t -3.086; p=0.002) and F6 (t=-2.744; p=0.007); c) number of health conditions: the fewer the number of health problems the higher the PMH score on F5 (r=-0.146; p=0.019); d) daily medication: polymedication patients had lower PMH scores, both globally and on various factors; e) use of analgesics: occasional use of painkillers was associated with higher PMH scores on F1 (t=-2.811; p=0.006). There were no significant differences in global PMH scores according to the type of chronic health condition. The only significant difference in the analysis by factors was that patients with hypertension obtained lower PMH scores on the factor Autonomy (t=2.165; p=0.032). Conclusions: Most people with chronic physical health problems have medium or high levels of PMH. The variables that adversely affect PMH are old age, polypharmacy and frequent consumption of analgesics. The type of health problem does not influence the levels of PMH. Much more extensive studies with samples without chronic pathology are now required in order to be able to draw more robust conclusions.

Objetivo: evaluar la eficacia de la intervención de musicoterapia, según la taxonomía NIC, en población general sana, a partir de la utilidad, la finalidad, la continuidad y la satisfacción percibida. Metodología: estudio descriptivo y transversal. Los sujetos de estudio fueron los familiares de los estudiantes de la Diplomatura de Enfermería de la Universidad de Barcelona que realizaron la asignatura optativa de 'Intervenciones psicosociales Nursing lntervention (NIC)' durante el curso 2008-2009. La intervención de musicoterapia (NIC 4400) desarrollada, implementada y evaluada por los estudiantes, se componía de las actividades seleccionadas para el tipo de población y el contexto comunitario en el que se llevó a cabo. Resultados: la intervención se realizó en 131 familiares o allegados de los estudiantes, 92,4% mujeres y el 71,8% de 18-24 años. Para un 84% de los casos, la relajación fue el propósito de la intervención, considerada satisfactoria por el 91,6% y útil por el 88,6%; un 74% dijeron que continuarían realizándola. El tipo de música fue seleccionado individualmente en función del propósito y las preferencias personales; fue escuchada dos veces al día por un 91,2% de los casos, entre 15 y 30 minutos/día. El 97% realizó la intervención en su domicilio. Conclusiones: la musicoterapia (NIC 4400) usada en población sana con el propósito de obtener relajación es percibida como satisfactoria y útil para ese fin. La mayoría de los que la practicaron expresaron su intención de continuar utilizándola. Las actividades de la intervención NIC 4400 están bien planteadas, puesto que permiten una buena aplicación y adaptación según el contexto y aprueban una atención individualizada
