Factors affecting hospital admission and recovery stay duration of in-patient motor victims in Spain

Hospital expenses are a major cost driver of healthcare systems in Europe, with motor injuries being the leading mechanism of hospitalizations. This paper investigates the injury characteristics which explain the hospitalization of victims of traffic accidents that took place in Spain. Using a motor insurance database with 16.081 observations a generalized Tobit regression model is applied to analyse the factors that influence both the likelihood of being admitted to hospital after a motor collision and the length of hospital stay in the event of admission. The consistency of Tobit estimates relies on the normality of perturbation terms. Here a semi-parametric regression model was fitted to test the consistency of estimates, concluding that a normal distribution of errors cannot be rejected. Among other results, it was found that older men with fractures and injuries located in the head and lower torso are more likely to be hospitalized after the collision, and that they also have a longer expected length of hospital recovery stay.

En la última dècada, els esports de muntanya, en qualsevol de les seves modalitats, s’han vist incrementats en nombre de practicants. Aquest fet ha comportat un important augment d’incidència de la hipotèrmia accidental. La hipotèrmia accidental és defineix com un estat patològic caracteritzat per la disminució de la temperatura corporal central per sota dels 35ºC, com a resultat de la incapacitat de l’organisme per generar el calor necessari per garantir el manteniment de les funcions fisiològiques. A través d’aquest estudi es vol demostrar que l’actuació in situ, a través de certes mesures de reescalfament, de la víctima que pateix hipotèrmia, milloren la supervivència i les complicacions secundàries d’aquesta patologia, a més a més d’avaluar l’eficàcia d’aquestes tècniques. És a dir, volem demostrar l’efectivitat de l’aplicació terapèutica extrahospitalaria de les tècniques de reescalfament. Realitzarem un estudi quantitatiu observacional analític de caràcter prospectiu a la zona dels Alps francesos d’una mostra de 237 casos d’hipotèrmia en persones practicants d’esports de muntanya que reuneixin uns criteris d’inclusió i exclusió. Els professionals de rescat aplicaran les mesures de reescalfament prèviament pautades, per les quals hauran estat formats, i enregistraran en el full de recollida de dades tota la informació sobre les variables d’estudi. És un estudi on la intervenció principal es porta a terma al medi natural i que per tant les condicions climatològiques i medioambientals poden actuar com a gran limitador de l’estudi, cal afegir també com a limitació el fet que el participant no vulgui formar part de l’estudi o que el professional de rescat que ha de portar a terme el pla terapèutic no ho faci adequadament.

El càncer produeix un impacte a la família en totes les seves esferes, la família ha de modificar les seves rutines familiars i adoptar nous rols. En el moment del diagnòstic del càncer, a cada família hi ha una persona que assumeix el rol de cuidador principal. Aquest cuidador és el que s’ocupa del malalt a casa i fa d’enllaç entre la família i l’equip sanitari. En front del càncer, la família i el cuidador tenen un conjunt de dubtes, d’inquietuds, de preocupacions que s’acaben traduint en necessitats. L’equip sanitari i Infermeria especialment ha d’atendre aquestes necessitats i ajudar a satisfer-les. Objectiu: L’objectiu principal d’aquest treball es descriure quines són les necessitats que presenten la família i el cuidador dels pacients oncològics. Metodologia: Es va dur a terme una revisió de la literatura entre setembre del 2013 i maig del 2014. Es va utilitzar la base de dades Pubmed, a més de pàgines web i llibres. Es van obtenir un total de 75 fonts, 56 en angles, 18 en espanyol i 1 en català. Resultats: La bibliografia agrupa les necessitats de les famílies i els cuidadors en 7 grans categories: físiques, psicològiques i emocionals, socials, de informació i formació, econòmiques, de transport i espirituals. Pel que fa a necessitats més especifiques, algunes de les més freqüents són tenir informació sobre el pronòstic del malalt, assegurar-se de que l’equip sanitari està donant la millor atenció possible al malalt. Sembla haver-hi una relació entre el grau de no satisfacció de les necessitats i el grau d’ansietat i depressió en la família i el cuidador. Els autors consultats no coincideixen en afirmar si la majoria del les necessitats es veuen satisfetes o no. Conclusions: La família i el cuidador d’un pacient amb càncer també han de rebre atenció per part dels professionals. En moltes ocasions la família treu importància a les seves necessitats per tal de que l’equip sanitari es centri només en el malalt. Infermeria té un paper fonamental en detectar les necessitats dels familiars i cuidadors i en ajudar a satisfer-les, de manera que la família estigui en les millors condicions possibles per cuidar del malalt. L'octubre de 2010 es va iniciar un estudi sobre els cartells de la Festa Majar de Vilafranca que esconserven al Centre de Documentació del Museu de les Cultures del Vi de Catalunya (CDV i VINSEUM)de Vilafranca del Penedes. Aquests cartells són part de la documentació grafica que genera aquestafesta i cronológicament se situen entre el 1907 i el 2007.El treball es va dur a terme mitjanc;ant conven i entre el Museu i la Universitat de Barcelona per ala real ització del Practicum del Máster en Direcció de Projectes de Conservació-Restauració ambI'objectiu d'estudiar I'estat de conservació de la Col·lecció de Cartells del CDV i propasar un seguit derecomanacions practiques per a la seva preservació futura.L'interes que va generar aquest primer estudi va donar pas a la necessitat d'aprofundir en lescaracterístiques tecniques deis Cartells de la Col ·lecció. A més, el fet de poder estudiar una col·leccióde cartells cronológicament correlatius i fets en un mateix Iloc geografic, Catalunya, permetria coneixerI'evo!ució tant de les tecniques d'impressió com deis materials utilitzats en la producció de cartells.En I'estudi es plantejaven dos obj ectius principals: en primer lIoc, recollir el maxim d'informaciósobre els mat erials i les tecniques deis cartells; i en segon, plantej ar la possibi litat de relacionarels diferents materials deis cartells amb el seu estat de conservació, intent ant posar de manifest sirealment els elements constitutius del paper I'havien afectat i si ho feien de la mateixa manera endiferents exemplars.

Long-term relative sea-level cycles (0 5 to 6 Myr) have yet to be fully understood for the Cretaceous. During the Aptian, in the northern Maestrat Basin (Eastern Iberian Peninsula), fault-controlled subsidence created depositional space, but eustasy governed changes in depositional trends. Relative sea-level history was reconstructed by sequence stratigraphic analysis. Two forced regressive stages of relative sea-level were recognized within three depositional sequences. The first stage is late Early Aptian age (intra Dufrenoyia furcata Zone) and is characterized by foreshore to upper shoreface sedimentary wedges, which occur detached from a highstand carbonate platform, and were deposited above basin marls. The amplitude of relative sea-level drop was in the order of tens of metres, with a duration of <1 Myr. The second stage of relative sea-level fall occurred within the Late Aptian and is recorded by an incised valley that, when restored to its pre-contractional attitude, was >2 km wide and cut 115 m down into the underlying Aptian succession. With the subsequent transgression, the incision was back-filled with peritidal to shallow subtidal deposits. The changes in depositional trends, lithofacies evolution and geometric relation of the stratigraphic units characterized are similar to those observed in coeval rocks within the Maestrat Basin, as well as in other correlative basins elsewhere. The pace and magnitude of the two relative sea-level drops identified fall within the glacio-eustatic domain. In the Maestrat Basin, terrestrial palynological studies provide evidence that the late Early and Late Aptian climate was cooler than the earliest part of the Early Aptian and the Albian Stage, which were characterized by warmer environmental conditions. The outcrops documented here are significant because they preserve the results of Aptian long-term sea-level trends that are often only recognizable on larger scales (i.e. seismic) such as for the Arabian Plate.

[cat] En aquest article es documenta Pere Compte com a mestre major de la Seu de Tortosa(1490), fet que es relaciona amb la promoció del protonotari apostòlic Joan Girona i es compara amb la direcció de les obres de Pere Moragues, que tingué lloc un segle abans. D'altra banda, es proposa la hipòtesi que sigui Pere Compte el responsable del disseny de la capella del Roser de l'esmentat edifici. També s'apunta la possibilitat que fos Antoni Queralt el mestre encarregat de dirigir el dia a dia de l'obra mentre Compte n'era absent i es posa de manifest la importancia que va poder tenir aquest mestre, especialment a Tortosa i a Lleida, a l'entorn del 1500. Finalment, es dóna notícia de la presencia d'altres mestres importants a la Tortosa del canvi de segles paral·lelament a la de Compte i Queralt al front de les obres de la Seu.

With the aim of better understanding avalanche risk in the Catalan Pyrenees, the present work focuses on the analysis of major (or destructive) avalanches. For such purpose major avalanche cartography was made by an exhaustive photointerpretation of several flights, winter and summer field surveys and inquiries to local population. Major avalanche events were used to quantify the magnitude of the episodes during which they occurred, and a Major Avalanche Activity Magnitude Index (MAAMI) was developed. This index is based on the number of major avalanches registered and its estimated frequency in a given time period, hence it quantifies the magnitude of a major avalanche episode or winter. Furthermore, it permits a comparison of the magnitude between major avalanche episodes in a given mountain range, or between mountain ranges, and for a long enough period, it should allow analysis of temporal trends. Major episodes from winter 1995/96 to 2013/14 were reconstructed. Their magnitude, frequency and extent were also assessed. During the last 19 winters, the episodes of January 22-23 and February 6-8 in 1996 were those with highest MAAMI values,followed by January 30-31, 2003, January 29, 2006, and January 24-25, 2014. To analyze the whole twentieth century, a simplified MAAMI was defined in order to attain the same purpose with a less complete dataset. With less accuracy, the same parameters were obtained at winter time resolution throughout the twentieth century. Again, 1995/96 winter had the highest MAAMI value followed by 1971/72, 1974/75 and 1937/38 winter seasons. The analysis of the spatial extent of the different episodes allowed refining the demarcation of nivological regions, and improving our knowledge about the atmospheric patterns that cause major episodes and their climatic interpretation. In some cases, the importance of considering a major avalanche episode as the result of a previous preparatory period, followed by a triggering one was revealed.

Humanity system of life is highly supported by maritime transport when circa 8 thousand million people require about 8.800 million tons of merchandises by sea, going in some 105.000 merchant ships of over 100 GT, sailing every thinkable dangerous waters 365 days year 24 hours day. All that Enormous activity plus others different factors produce accidents, as is shown in an ascendant 1.7 rate related to ships lost with big number in life, cargoes losses, and pollution. That is why this study pretend to detect causes factors of maritime accidents, to try to reduce them, and with that targetin mind it was tested the new theory of Induced Maritime Accidents, crossing itsproposals with relevant sinister of different times and circumstances, as Andrea Doria, Torrey Canyon, Costa Concordia, among others. Those cases were re evaluated to establish the key points of such theory, as they are the Production Pressure, the Risk Homeostasis, technological advances and the rupture of safety margin. Cases studies gave as result the existence of referred key points, in a manner combined that the chain of events derived to the fatality, and more than that highlights the possibility that been suppressed to acceptable limits the production pressure or the risk homeostasis, a permissible safety margin were been maintained, avoiding catastrophe.

BACKGROUND: This study examined potential predictors of remission among patients treated for major depressive disorder (MDD) in a naturalistic clinical setting, mostly in the Middle East, East Asia, and Mexico. METHODS: Data for this post hoc analysis were taken from a 6-month prospective, noninterventional, observational study that involved 1,549 MDD patients without sexual dysfunction at baseline in 12 countries worldwide. Depression severity was measured using the Clinical Global Impression of Severity and the 16-item Quick Inventory of Depressive Symptomatology Self-Report (QIDS-SR16). Depression-related pain was measured using the pain-related items of the Somatic Symptom Inventory. Remission was defined as a QIDS-SR16 score ≤5. Generalized estimating equation regression models were used to examine baseline factors associated with remission during follow-up. RESULTS: Being from East Asia (odds ratio [OR] 0.48 versus Mexico; P<0.001), a higher level of depression severity at baseline (OR 0.77, P=0.003, for Clinical Global Impression of Severity; OR 0.92, P<0.001, for QIDS-SR16), more previous MDD episodes (OR 0.92, P=0.007), previous treatments/therapies for depression (OR 0.78, P=0.030), and having any significant psychiatric and medical comorbidity at baseline (OR 0.60, P<0.001) were negatively associated with remission, whereas being male (OR 1.29, P=0.026) and treatment with duloxetine (OR 2.38 versus selective serotonin reuptake inhibitors, P<0.001) were positively associated with remission. However, the association between Somatic Symptom Inventory pain scores and remission no longer appeared to be significant in this multiple regression (P=0.580), (P=0.008 in descriptive statistics), although it remained significant in a subgroup of patients treated with selective serotonin reuptake inhibitors (OR 0.97, P=0.023), but not in those treated with duloxetine (P=0.182). CONCLUSION: These findings are largely consistent with previous reports from the USA and Europe. They also highlight the potential mediating role of treatment with duloxetine on the negative relationship between depression-related pain and outcomes of depression.
