The diagnosis of synovial amyloidosis is based upon synovial biopsy. Synovial fluid (SF) in seven patients with amyloid arthropathy associated with chronic renal failure undergoing haemodialysis were studied. The SF and synovial samples of 10 consecutive patients with seronegative mono- or oligoarthritis served as controls. Six of the seven patients with amyloid positive synovial biopsy specimens showed amyloid in their SF. No amyloid was found in the synovial tissue or fluid of the 10 patients in the control group, the sensitivity being 87.7%. The finding of amyloid in SF was highly reproducible, showing its presence in the same joint on several occasions. The deposits were Congophilia resistant to potassium permanganate pretreatment, and the immunohistochemical analysis proved that they contained beta 2 microglobulin. Seven patients (five male and two female) with chronic renal failure (CRF) treated by periodical haemodialysis presented with swelling and effusion of more than three months' duration in knees (four bilateral), shoulders (two, one of them bilateral), elbow (one), and ankle (one). Four had a carpal tunnel syndrome both clinically and electromyographically (three bilateral). All patients had hyperparathyroidism secondary to their CRF, which was not due to amyloidosis in any of them. The dialysis duration period varied from five to 14 years, with an average of 8.6 years. Amyloid deposits (Congo red positive areas with green birefringence under polarising microscopy) were shown in six of the seven synovial biopsy specimens of the knee, in five of the sediments of the synovial fluids, and in specimens removed during carpal tunnel syndrome surgery. No amyloid was found in the biopsy specimen of abdominal fat of six of the patients. The finding of amyloid only in the synovial membrane and fluid, and carpal tunnel, its absence in abdominal fat, and the lack of other manifestations of generalised amyloidosis (cardiomyopathy, malabsorption syndrome, macroglossia, etc.) and of Bence Jones myeloma (protein immunoelectrophoresis normal) raises the possibility that this is a form of amyloidosis which is peculiar to CRF treated by periodical haemodialysis.

Erythropoietin (rHuEPO) has proven to be effective in the treatment of anemia of chronic renal failure (CRF). Despite improving the quality of life, peak oxygen uptake after rHuEPO therapy is not improved as much as the increase in hemoglobin concentration ([Hb)] would predict. We hypothesized that this discrepancy is due to failure of O2 transport rates to rise in a manner proportional to [Hb]. To test this, eight patients with CRF undergoing regular hemodialysis were studied pre- and post-rHuEPO ([Hb] = 7.5 +/- 1.0 vs. 12.5 +/- 1.0 g x dl-1) using a standard incremental cycle exercise protocol. A group of 12 healthy sedentary subjects of similar age and anthropometric characteristics served as controls. Arterial and femoral venous blood gas data were obtained and coupled with simultaneous measurements of femoral venous blood flow (Qleg) by thermodilution to obtain O2 delivery and oxygen uptake (VO2). Despite a 68% increase in [Hb], peak VO2 increased by only 33%. This could be explained largely by reduced peak leg blood flow, limiting the gain in O2 delivery to 37%. At peak VO2, after rHuEPO, O2 supply limitation of maximal VO2 was found to occur, permitting the calculation of a value for muscle O2 conductance from capillary to mitochondria (DO2). While DO2 was slightly improved after rHuEPO, it was only 67% of that of sedentary control subjects. This kept maximal oxygen extraction at only 70%. Two important conclusions can be reached from this study. First, the increase in [Hb] produced by rHuEPO is accompanied by a significant reduction in peak blood flow to exercising muscle, which limits the gain in oxygen transport. Second, even after restoration of [Hb], O2 conductance from the muscle capillary to the mitochondria remains considerably below normal.

Background: Drug dosing errors are common in renal-impaired patients. Appropriate dosing adjustment and drug selection is important to ensure patients" safety and to avoid adverse drug effects and poor outcomes. There are few studies on this issue in community pharmacies. The aims of this study were, firstly, to determine the prevalence of dosing inadequacy as a consequence of renal impairment in patients over 65 taking 3 or more drug products who were being attended in community pharmacies and, secondly, to evaluate the effectiveness of the community pharmacist"s intervention in improving dosing inadequacy in these patients when compared with usual care. Methods: The study was carried out in 40 Spanish community pharmacies. The study had two phases: the first, with an observational, multicentre, cross sectional design, served to determine the dosing inadequacy, the drug-related problems per patient and to obtain the control group. The second phase, with a controlled study with historical control group, was the intervention phase. When dosing adjustments were needed, the pharmacists made recommendations to the physicians. A comparison was made between the control and the intervention group regarding the prevalence of drug dosing inadequacy and the mean number of drug-related problems per patient. Results: The mean of the prevalence of drug dosing inadequacy was 17.5% [95% CI 14.6-21.5] in phase 1 and 15.5% [95% CI 14.5-16.6] in phase 2. The mean number of drug-related problems per patient was 0.7 [95% CI 0.5-0.8] in phase 1 and 0.50 [95% CI 0.4-0.6] in phase 2. The difference in the prevalence of dosing inadequacy between the control and intervention group before the pharmacists" intervention was 0.73% [95% CI (−6.0) - 7.5] and after the pharmacists" intervention it was 13.5% [95% CI 8.0 - 19.5] (p < 0.001) while the difference in the mean of drug-related problems per patient before the pharmacists" intervention was 0.05 [95% CI( -0.2) - 0.3] and following the intervention it was 0.5 [95% CI 0.3 - 0.7] (p < 0.001). Conclusion: A drug dosing adjustment service for elderly patients with renal impairment in community pharmacies can increase the proportion of adequate drug dosing, and improve the drug-related problems per patient. Collaborative practice with physicians can improve these results.

Forty-three patients with cirrhosis and ascites, 21 with normal renal function, 10 with a progressive functional renal failure (FRF), and 12 with a steady FRF, were investigated for the presence of endotoxaemia by the Limulus lysate test. Endotoxaemia was found in nine patients with FRF and in none of the 21 with normal renal function (P less than 0-01). A positive Limulus test was almost exclusively associated with a progressive FRF (eight of 10 patients) and all but one of them died. Renal function improved as endotoxaemia disappeared in the survivor. Endotoxaemia was also associated with haemorrhage due to acute erosions of the gastric mucosa, being present in six of the seven patients who had this complication. Intravascular coagulation was not found in any patient. The Limulus test was positive in the ascitic fluid in 18 of 21 patients tested, although only two of them had peritonitis. These results suggest that endotoxaemia may play a critical role in the development of progressive renal failure and haemorrhagic gastritis in cirrhosis, and emphasise the potential risk of procedures involving reinfusion of ascitic fluid.

Los riñones son los responsables de funciones muy importantes dentro del organismo como la regulación del volumen -líquido- y del equilibrio acidobásico del plasma, la excreción de compuestos nitrogenados y la síntesis de eritropoyetina, hidroxicolecalciferol y renina. Las manifestaciones generales de la insuficiencia Renal aparecen en los sistemas cardiovascular, gastrointestinal, neuromuscular, esquelético, hematológico y dermatológico. Por este motivo, es muy importante conocer sus principales características para prevenir y tratar correctamente los problemas que puedan surgir durante el tratamiento dental. Este artículo describe la fisiopatología, complicaciones y manifestaciones bucales asociadas a esta condición así como las consideraciones y el tratamiento odontológico del paciente con una Insuficiencia Renal.

Descrito en 1856 por Wunderlich, el hematoma perirrenal espontáneo es una patología infrecuente, en la que a pesar de llegar al diagnóstico de una manera relativamente sencilla con estudios imagen, el poder determinar la etiología representa un reto y es motivo de múltiples estudios y procedimientos invasivos, algunas veces sin poder llegar a esclarecer la causa1. Introducció: El fracàs renal agut és una complicació que afecta entre el 6 i el 25% dels pacients crítics. Un 72% d’aquests malalts necessitaran de tècniques de depuració extrarenal contínues (TCDER) o intermitents en algun moment de l’ingrés. Actualment la dosi d’ultrafiltrat a cada situació clínica és objecte d’estudi i publicacions. Objectius: Analitzar les diferències entre les dosis pautades i administrades en les TCDER. Comparar les diferències entre la dosi en funció del tipus de TCDER: hemofiltració venovenosa contínua (HVVC) i hemodiafiltració venovenosa contínua. Descriure la tassa de recuperació de la funcionalitat renal o bé la necessitat d’hemodiàlisis convencional a l’alta d’UCI en els malalts que s’han sotmès a TCDER. Determinar la mortalitat Material i mètodes: Estudi observacional de 40 pacients crítics sotmesos a TCDER, 20 pacients en HVVC i 20 pacients en HDFVVC. S’han analitzat les dades mitjançant proves no paramètriques U de Mann Withney, test exacte de Fischer i Chi quadrat. Resultats: L’edat mitjana de la mostra fou 63 ± 13 anys, amb un 57% d’homes. La incidència d’hipertensió arterial fou del 60%, de diabetis mellitus del 30%, de dislipèmia del 25% i de malaltia renal crònica del 27%. La mediana de puntuació de SOFA fou de 10, i d’AKIN de 3. Un 85% dels pacients van precisar ventilació mecànica i/o amines durant l’ingrés. Vam registrar la dosi d’ultrafiltrat pautada pel facultatiu, essent la mitjana de la mostra de 34,34 mL/kg/h (34,73 en HVVC i 33,95 en HDFVVC), mentre que la dosi administrada fou de 27,93 mL/kg/h (26,85 en HVVC i 29 en HDFVVC), essent la diferència entre ambdues estadísticament significativa. L’estada mitjana a UCI fou de 23,7 dies, i la mortalitat de la sèrie del 65%. Conclusió: Les dosis d’ultrafiltrat rebudes en les TCDER són menors a les dosis pautades independentment del tipus de tècnica. La mortalitat de la nostra sèrie fou similar a les sèries publicades.

El fracàs renal agut constitueix una de les complicacions més greus en els pacients de les unitats de cures intensives, amb una mortalitat segons publicacions del 42%. Un nombre important d’aquests pacients requereixen tècniques continues de depuració extrarrenal (TCDE). Hem realitzat un estudi en el que s’ha inclós 29 pacients ingressats durant l’any 2008 a la UCI del Hospital Germans Trias i Pujol, que foren sotmesos a TCDE. Analitzem les possibles variables associades a la mortalitat. Les variables relacionades amb la mortalitat són el tipus de patologia motiu d'ingrés en UCI, l'edat i els dies de tractament amb TCDE.

Estudi observacional de 60 pacients ingressats a la UCI per patologia extraabdominal, que demostra alta incidència de HIA (90%) i que el valor de PIA màxima és marcador pronòstic del SCA però no prediu mortalitat. Els graus més elevats d’HIA s’associen amb VMI, shock, SDRA, insuficiència renal e ili. La PIA màxima del pacient crític no neurològic (sèptic, traumatològic i respiratori) és més elevada que la del no neurològic, 20 i 16 mmHg respectivament. El valor de PIA màxima que millor prediu insuficiència renal al pacient no neurològic és 18,5mmHg. La PIA màxima es correlaciona amb l’estada a UCI i hospitalària.
