This Phd research report aims to obtain a better understanding of the recent changes in mortality of the elderly population in developed countries. It’s essence will be a critical thinking, without performing empirical work, which means that the analysis and concepts of the actual demographic literature will form the main body of this work. There are two central questions: Which age and cause specific mortality trends caused these recent changes in the life expectancy?Did the increase in life expectancy of the elderly population accelerate, continue or decline in recent years?”

Aquest informe mira de documentar el projecte GeriaTic, que desenvolupa l'Ajuntament de Barcelona al districte 22@. L'informe fa una anàlisi de la situació de la gent gran en relació amb les tecnologies de la informació i la comunicació en el sector de la salut, el control de l'entorn i el lleure. L'anàlisi de la documentació trobada i la informació extreta d'un grup de discussió amb persones grans usuàries de les TIC permeten identificar el mercat existent, els productes i serveis, les necessitats no cobertes i les bones pràctiques relacionades amb els tres sectors estudiats. Les conclusions de l'estudi assenyalen tres aspectes clau: la necessitat que la tecnologia s'incorpori a la vida quotidiana de les persones, perquè aparegui com a quelcom necessari i útil; la cura en cercar un disseny modular, escalable i integrat de la tecnologia que acompanyi les característiques específiques de la persona i els canvis al llarg del seu cicle vital, i, finalment, la consideració que la incorporació de les TIC en l'àmbit de la gent gran no tan sols involucra la persona, sinó també el conjunt de professionals i la societat en general. Aquests tres aspectes permeten plantejar l'exigència que la incorporació de la tecnologia en la vida de la gent gran abandoni el model assistencial i es presenti a la persona i a la societat com una font de noves possibilitats i una oportunitat per al canvi.

Background: Spain has recently become an inward migration country. Little is known about the occupational health of immigrant workers. This study aimed to explore the perceptions that immigrant workers in Spain had of their working conditions.Methods: Qualitative, exploratory, descriptive study. Criterion sampling. Data collected between September 2006 and May 2007 through semi-structured focus groups and individual interviews, with a topic guide. One hundred and fifty-eight immigrant workers (90 men/68 women) from Colombia (n = 21), Morocco (n = 39), sub-Saharan Africa (n = 29), Romania (n = 44) and Ecuador (n = 25), who were authorised (documented) or unauthorised (undocumented) residents in five medium to large cities in Spain.Results: Participants described poor working conditions, low pay and health hazards. Perception of hazards appeared to be related to gender and job sector. Informants were highly segregated into jobs by sex, however, so this issue will need further exploration. Undocumented workers described poorer conditions than documented workers, which they attributed to their documentation status. Documented participants also felt vulnerable because of their immigrant status. Informants believed that deficient language skills, non-transferability of their education and training and, most of all, their immigrant status and economic need left them with little choice but to work under poor conditions.Conclusions: The occupational health needs of immigrant workers must be addressed at the job level, while improving the enforcement of existing health and safety regulations. The roles that documentation status and economic need played in these informants' work experiences should be considered and how these may influence health outcomes.

Background: Data provided by the social sciences as well as genetic research suggest that the 8-10 million Roma (Gypsies) who live in Europe today are best described as a conglomerate of genetically isolated founder populations. The relationship between the traditional social structure observed by the Roma, where the Group is the primary unit, and the boundaries, demographic history and biological relatedness of the diverse founder populations appears complex and has not been addressed by population genetic studies. Results: Recent medical genetic research has identified a number of novel, or previously known but rare conditions, caused by private founder mutations. A summary of the findings, provided in this review, should assist diagnosis and counselling in affected families, and promote future collaborative research. The available incomplete epidemiological data suggest a non-random distribution of disease-causing mutations among Romani groups.Conclusion: Although far from systematic, the published information indicates that medical genetics has an important role to play in improving the health of this underprivileged and forgotten people of Europe. Reported carrier rates for some Mendelian disorders are in the range of 5 -15%, sufficient to justify newborn screening and early treatment, or community-based education and carrier testing programs for disorders where no therapy is currently available. To be most productive, future studies of the epidemiology of single gene disorders should take social organisation and cultural anthropology into consideration, thus allowing the targeting of public health programs and contributing to the understanding of population structure and demographic history of the Roma.

Background: Analyzing social differences in the health of adolescents is a challenge. The accuracy of adolescent's report on familial socio-economic position is unknown. The aims of the study were to examine the validity of measuring occupational social class and family level of education reported by adolescents aged 12 to 18, and the relationship between social position and self-reported health.Methods: A sample of 1453 Spanish adolescents 12 to 18 years old from urban and rural areas completed a self-administered questionnaire including the Child Health and Illness Profile-Adolescent Edition (CHIP-AE), and data on parental occupational social class (OSC) and level of education (LE). The responsible person for a sub-sample of teenagers (n = 91) were interviewed by phone. Kappa coefficients were estimated to analyze agreement between adolescents and proxy-respondents, and logistic regression models were adjusted to analyze factors associated with missing answers and disagreements. Effect size (ES) was calculated to analyze the relationship between OSC, LE and the CHIP-AE domain scores.Results: Missing answers were higher for father's (24.2%) and mother's (45.7%) occupational status than for parental education (8.4%, and 8.1% respectively), and belonging to a non-standard family was associated with more incomplete reporting of social position (OR = 4,98; 95%CI = 1,3–18,8) as was agreement between a parent and the adolescent. There were significant social class gradients, most notably for aspects of health related to resilience to threats to illness.ConclusionAdolescents can acceptably self-report on family occupation and level of education. Social class gradients are present in important aspects of health in adolescents.

We study the effect of the business cycle on the health of newborn babies using 30 years of birth certificate data for Spain. Exploiting regional variation over time, we find that babies are born healthier when the local unemployment rate is high. Although fertility is lower during recessions, the effect on health is not the result of selection (healthier mothers being more likely to conceive when unemployment is high). We match multiple births to the same parents and find that the main result survives the inclusion of parents fixed-effects. We then explore a range of maternal behaviors as potential channels. Fertility-age women do not appear to engage in significantly healthier behaviors during recessions (in terms of exercise, nutrition, smoking and drinking). However, they are more likely to be out of work. Maternal employment during pregnancy is in turn negatively correlated with babies' health. We conclude that maternal employment is a plausible mediating channel.

El treball aquí exposat té com a objectiu conèixer i comprendre l’experiència dels excuidadors i excuidadores principals informals que han tingut cura d’un familiar vell amb demència després de la seva mort. Es plantegen les hipòtesis i els objectius generals i específics. S’hi exposa també, la justificació del tema, l’interès, la rellevància a nivell social i professional, i la seva aportació en el món de la salut. En l’apartat de l’estat de la qüestió i la fonamentació conceptual, després d’una revisió bibliogràfica sobre el tema, es descriuen els diferents apartats que formen part del marc teòric: perfil de la persona gran dement que rep cures informals , el perfil dels cuidadors/es, les conseqüències de tenir cura d’una persona dement, l’estat dels excuidadors/es després de la mort del familiar vell amb demència i el procés de dol. El segon apartat inclou la metodologia, i el mètode d’investigació. A més, inclou les matrius d’anàlisi del contingut i del discurs, i es plantegen els resultats en funció dels objectius marcats. En la discussió, a partir de les dades obtingudes, es realitza la seva anàlisi, s’interpreta el resultat i es contraposa amb altres estudis consultats. Finalment s’exposen les conclusions per resumir la informació obtinguda i la metodologia utilitzada. Es realitza una reflexió sobre els aprenentatges adquirits i les aportacions del treball i s’apunten noves vies de recerca. En les referències bibliogràfiques s’especifiquen les fonts consultades i en els annexos hi consten els models d’entrevista i de matriu per a l’anàlisi de dades, el full de consentiment informat i també el buidatge i l’anàlisi de les entrevistes.

Historical exploitation of the Mediterranean Sea and the absence of rigorous baselines makes it difficult to evaluate the current health of the marine ecosystems and the efficacy of conservation actions at the ecosystem level. Here we establish the first current baseline and gradient of ecosystem structure of nearshore rocky reefs at the Mediterranean scale. We conducted underwater surveys in 14 marine protected areas and 18 open access sites across the Mediterranean, and across a 31-fold range of fish biomass (from 3.8 to 118 g m22). Our data showed remarkable variation in the structure of rocky reef ecosystems. Multivariate analysis showed three alternative community states: (1) large fish biomass and reefs dominated by non-canopy algae, (2) lower fish biomass but abundant native algal canopies and suspension feeders, and (3) low fish biomass and extensive barrens, with areas covered by turf algae. Our results suggest that the healthiest shallow rocky reef ecosystems in the Mediterranean have both large fish and algal biomass. Protection level and primary production were the only variables significantly correlated to community biomass structure. Fish biomass was significantly larger in well-enforced no-take marine reserves, but there were no significant differences between multi-use marine protected areas (which allow some fishing) and open access areas at the regional scale. The gradients reported here represent a trajectory of degradation that can be used to assess the health of any similar habitat in the Mediterranean, and to evaluate the efficacy of marine protected areas.

The focus of physical activity promotion is moving from methods for increasing health enhancing physical activity on the individual level to higher level strategies including environmental and policy approaches. Scientific inquiry, traditionally related to individual-based strategies, requires adaptation and refinement when environmental and policy changes become more relevant. The objective of this study is to investigate the significance for behaviour and health of community-based environments that encourage physical activity. DESIGN AND SETTING The article presents data and results from a cross sectional comparative survey of the general population in six European countries (Belgium, Finland, Germany (East and West), Netherlands, Spain, Switzerland). Specifically, the relation between perceived community-based opportunities for physical activity, self reported physical activity, and self rated health status is investigated. PARTICIPANTS Representative samples of general populations (adults 18 years or older). Overall response rate: 53.5%. Sample sizes realised: Belgium: n=389; Finland: n=400; Germany (East): n = 913; Germany (West): n=489; Netherlands: n=366; Spain: n=380; Switzerland: n=406. MAIN RESULTS Analyses show that best opportunities are reported by people who are lightly to moderately physically active. People's self rated health is moderately, but significantly associated with both perceived opportunities, and physical activity itself. These predictors interact in that especially for women, the health impact of physical activity is more pronounced in case of good opportunities. CONCLUSIONS The paper shows the potential of opportunities within residential and community environments with regard to physical activity, both for behaviour and health. Opportunities may enable the population, especially women, to develop an active lifestyle, and thus improve their health. Future studies with objective indicators for physical activity related environments should test the findings that are based on perceptions.

Introducció: La preocupació per la qualitat assistencial, i concretament per la qualitat de les cures infermeres, la satisfacció percebuda pel pacient com element fonamental com a indicador de la qualitat dels serveis sanitaris i clau en la millora de la qualitat assistencial, i el fet que a l’estat espanyol hi ha pocs estudis de satisfacció específics de la població anciana hospitalitzada, ens va portar a plantejar un estudi amb l’objectiu de valorar el grau de satisfacció de les persones grans hospitalitzades en relació a l’atenció rebuda dels professionals d’infermeria. Metodologia: S’ha realitzat un estudi descriptiu i transversal en un col.lectiu de 42 persones hospitalitzades d’edat ≥ 60 anys. El grau de satisfacció es va determinar amb “La Monica Oberst Patient Satisfaction Scale 12” (LOPSS 12), versió reduïda en castellà. La personalització de les cures es va valorar amb un qüestionari que valora la relació infermera-usuari extret de l’Índex de Personalització de les Cures. L'anàlisi estadística es va dur a terme amb el programa informàtic SPSS® vs 21.0. Resultats: Es va evidenciar un elevat grau de satisfacció global amb les cures d’infermeria (8,8  1,5 / 10), però el nivell global de personalització d’aquestes cures no va ser molt satisfactori (3,4  2,1 / 7). Els homes van valorar millor a les infermeres respecte a que consideraven les seves opinions i preferències (p=0,03) i contestaven amb rapidesa les seves trucades (p=0,009). La personalització de les cures millorava conforme incrementava el grau de satisfacció global (r = 0,582; p = 0,000) i amb el temps d’hospitalització (r = 0,344; p = 0,026). Conclusions: El grau de satisfacció global respecte la satisfacció en l’atenció rebuda per part d’infermeria, va ser elevat però amb baix grau de personalització, mostrant major satisfacció els homes respecte les dones. Conèixer l’opinió dels usuaris representa una oportunitat pels professionals de reflexionar sobre la pràctica clínica i ser crítics per avançar en el desenvolupament de la professió, identificant aspectes susceptibles de millora 5 amb la finalitat d’augmentar la qualitat de les cures que es presten, així com la qualitat de vida de les persones.

Preliminary results are available on a pilot test with 30 patients. The mental disorder prevalence is around 30%. Sensibility (0,89), specificity (0,89), VPP (0,80), VPN (0,94) cutoff score (3.5) and AUC (0,941). Women, people with 10 or more years of residency in Spain and unemployed people have worse self-rated healthConclusions. Based on the preliminary results, is possible to conclude that COOP/WONCA charts could be an useful, valid and applicable screening test for mental disorders in primary care with immigrant population

El present estudi s’emmarca en una investigació centrada en les persones grans de 65 a 84 anys del municipi de Canovelles. L’objectiu d’aquesta recerca és conèixer les principals motivacions i barreres d’aquest col·lectiu a l’hora d’anar al Complex Esportiu Municipal – Thalassa. Lligat amb els objectius marcats, la finalitat és relacionar els resultats amb possibles propostes de millora, per tal de minimitzar les barreres i potenciar les motivacions. Per obtenir els resultats s’ha passat un qüestionari a 40 persones demanant les raons per les quals van o no al complex. Un cop analitzats podem extreure, a tall de conclusions, que la principal motivació va relacionada amb temes de salut i la principal barrera amb temes econòmic, el cost mensual de la instal·lació per un abonat.

This paper provides a spatial and temporal multi-scale approach of European submarine canyons. We fi rst present the long-term geologic view of European margins as related to controls on submarine canyon development. Then we discuss the extent to which submarine canyon systems resemble river systems because both essentially form drainage networks. Finally, we deal with the hortest-term, highestresolution scale to get a fl avor of the current functioning and health of modern submarine canyons in the northwestern Mediterranean Sea. Submarine canyons are unique features of the seafl oor whose existence was known by European fi shermen centuries ago, especially for those canyons that have their heads at short distance from shoreline. Popular names given to specifi c canyons in the different languages spoken in European coastal communities refer to the concepts of a"deep" or"trench." In the old times it was also common thinking that submarine canyons where so deep that nobody could measure their depth or even that they had no bottom. Submarine canyons are just one of the seven different types of seafl oor valleys identifi ed by Shepard (1973) in his pioneering morphogenetic classifi cation. Shepard (1973) defined submarine canyons as"steep-walled, sinuous valleys, with V-shaped cross sections, and relief comparable even to the largest of land canyons; tributaries are found in most of the canyons and rock outcrops abound on their walls." Canyons are features typical of continental slopes with their upper reaches and heads cut into the continental shelf.

Introduction: in the present study several collectives with knowledge of the reality of family care of the elderly assessed several socia and healthcare resources and suggested several possibilities for improvement. Method: four discussion groups were used as a data collection technique. The groups were composed of caregivers, representatives-users of associations for the elderly, experts in geriatrics, and social services professionals. Results: the various discussion groups positively evaluated the Home Help Service, the Support Teams of the Home Service Programs, Interdisciplinary Community Health Workers Units, Daytime Care Centres, and the Family Rest Programme but suggested some changes to all of them. The discussion groups also indicated the need to improve the material, economic and emotional assistance given to caregivers and asked for training, institutional coordination, anddissemination of information about available resources and assistance. Conclusions: some changes are required to improve the current social and health resources available to families caring for the elderly within the family unit. Among the suggestions for improvement proposed by the participants, many are useful and could easily be applied, whereas others provide an interesting starting point for debate and reflection. Knowledge and understanding of the situation of caregiving families, based on their own experiences and those of the people who know them, is in itself sufficient to initiate and implement changes to provide resources appropriate to their needs
