Raimon Pannikar deia, ja fa uns anys, que «no és possible un jo i un tu sense una tercera persona, i aquesta tercera persona existent en tot diàleg és sempre la matèria, l'espai, el temps, el cosmos, la terra». I Jo penso que és molt intel·ligent parlar d'una «tercera persona», d'un tercer element que en realitat és i no és una persona. En tot el que seguira intentaré explicar aixà que die, perquè em sembla quelcom interessant per a l'educació ambiental.

Population ageing brings new challenges to long-term household economic decisions. In the event of old-age dependency, housing assets become a key self-insurance device. However, little empirical evidence has been reported regarding an individual"s expectations of having to use their housing wealth for such a purpose. This paper draws upon two complementary data sources to empirically examine: (1) the influence of housing assets on an individual"s willingness-to-sell (WTS) their dwelling for care purposes, and (2) the willingness to take out a reverse mortgage contract loan in the event of old-age dependency. The paper"s findings suggest that homeowners" WTS in old age is unaffected by their income or housing assets and is, rather, determined by socio-environmental housing characteristics and the individual"s health and personal needs. Conversely, the study finds that the uptake of home reversion loans is largely dependent on income or education, but not on a household"s housing assets.

We describe a community intervention program performed in three rural villages in the Alt Empordà region (Girona province). The study provides data on the services available to the elderly in a specific context, based on an initiative proposed by the villages, the local councils, and the elderly themselves. The program is a research-action plan which applies the participative and qualitative methodology characteristic of strategic planning. The study analyses how the initiative arose, the description of the process, and the different parts of the intervention program

The aim of this study is to examine the lifestyle characteristics and health status of the elderly in order to identify aspects that might help promote active ageing. Participants were 26 elderly citizens aged 75 or over who were the recipients of home care services in Vilafranca del Penedès (Barcelona, Spain). They were recruited by means of convenience sampling, and a case study approach was adopted. Fieldwork was conducted in April 2007, with a specially designed questionnaire administered in the context of an in-depth interview. Quantitative data were analyzed with SPSS 15, while qualitative data were analysed and grouped by category. The results indicated that 53.8% of the elderly interviewees perceived their health status as being regular, most did not have adequate nutrition and hydration, while 42.3% had chewing problems and 65.3% presented slight dependence. Most of them received visits at home, but did not go on leisure outings. The only exercise they took was walking. These findings highlight the need to promote and strengthen activities for healthy ageing, and to ensure that people over 75 receive adequate care. Programmes should be implemented to improve those aspects that are currently under-addressed.

El siguiente estudio de investigación se plantea en los CAP de la ciudad de Lleida. Una vez cumplimentadas las encuestas realizadas expresamente para el estudio a 450 personas mayores de 65 años con estreñimiento y no institucionalizados, se crearán los grupos control y de intervención con una muestra de (N=168) participantes. El protocolo será: a) Grupo de control: grupo cegado que continuará con el tratamiento convencional, se reunirá una vez al mes durante los 4 meses para realizar un seguimiento controlado. b) Grupo de intervención: se le educará con un programa de masaje abdominal, sesión una vez por semana durante los 4 meses que dure la intervención. En este trabajo se pretende determinar la efectividad de la educación sanitaria del masaje abdominal en atención primaria en pacientes ancianos no institucionalizados. La metodología que se ha utilizado ha sido a través de una revisión sistemática en la base de datos Medline, PEDro, Cochrane, ScienceDirect, PubMed, Dialnet y Wiley Online Library de estudios publicados entre 1999 y 2014 de lengua inglesa o española, realizados sobre sujetos diagnosticados de estreñimiento.

Hipòtesi: El treball de l’equilibri estàtic que ens permet la pràctica regular del Tai-txi, pot ser un mètode eficaç per a reduir les caigudes en gent d’edat avançada. Objectius: Determinar si la pràctica del Tai-txi ofereix avantatges en quant a la prevenció de caigudes en la població d’edat avançada en comparació amb un tractament de fisioteràpia convencional. Metodologia: Es durà a terme un estudi aleatori controlat (RCT) amb 45 voluntaris provinents de Lleida i rodalies, i de diferents geriàtrics i hospitals de Lleida i que hagin sofert almenys un episodi de caiguda en el darrer any, que tinguin 65 anys o més i que no necessitin ajuda per caminar. Com a criteris d’exclusió es consideraran aquells subjectes que presentessin malalties degeneratives, problemes vestibulars, problemes cardiovasculars o alteracions neurològiques. Aquests participants estaran distribuïts de manera aleatòria en dos grups; l’experimental que realitzarà les sessions de Tai-txi, i el control que seguirà un programa de fisioteràpia convencional. La intervenció durarà un semestre, amb una freqüència de 3 sessions/setmana d’una hora de durada. Les sessions d’ambdós grups seguiran el mateix esquema: escalfament, cos de la sessió (formes de Tai-txi o exercicis) i tornada a la calma. Els participants de l’estudi passaran revisió mensualment i se’ls revaluarà amb diferents testos i escales: Test de Tinetti tant en l’apartat d’equilibri dinàmic com estàtic, recompte del número de caigudes anotades en una llibreta, The Falls Efficacy Scale International (FES-I) per a obtenir informació sobre la por a caure, el qüestionari SERVQUAL per a la satisfacció, i lectura dels folis d’auto-registre. Després s’avaluaran els resultats i es presentaran les conclusions extretes i les línies de futur.

Ensuring the accuracy of dietary assessment instruments is paramount for interpreting diet-disease relationships. The present study assessed the relative and construct validity of the 14-point Mediterranean Diet Adherence Screener (MEDAS) used in the Prevencio´n con Dieta Mediterra´nea (PREDIMED) study, a primary prevention nutrition-intervention trial. A validated FFQ and the MEDAS were administered to 7146 participants of the PREDIMED study. The MEDASderived PREDIMED score correlated significantly with the corresponding FFQ PREDIMED score (r = 0.52; intraclass correlation coefficient = 0.51) and in the anticipated directions with the dietary intakes reported on the FFQ. Using Bland Altman"s analysis, the average MEDAS Mediterranean diet score estimate was 105% of the FFQ PREDIMED score estimate. Limits of agreement ranged between 57 and 153%. Multiple linear regression analyses revealed that a higher PREDIMED score related directly (P , 0.001) to HDL-cholesterol (HDL-C) and inversely (P , 0.038) to BMI, waist circumference, TG, the TG:HDL-C ratio, fasting glucose, and the cholesterol:HDL-C ratio. The 10-y estimated coronary artery disease risk decreased as the PREDIMED score increased (P , 0.001). The MEDAS is a valid instrument for rapid estimation of adherence to the Mediterranean diet and may be useful in clinical practice.

Objective: Independently of total caloric intake, a better quality of the diet (for example, conformity to the Mediterranean diet) is associated with lower obesity risk. It is unclear whether a brief dietary assessment tool, instead of full-length comprehensive methods, can also capture this association. In addition to reduced costs, a brief tool has the interesting advantage of allowing immediate feedback to participants in interventional studies. Another relevant question is which individual items of such a brief tool are responsible for this association. We examined these associations using a 14-item tool of adherence to the Mediterranean diet as exposure and body mass index, waist circumference and waist-to-height ratio (WHtR) as outcomes. Design: Cross-sectional assessment of all participants in the"PREvención con DIeta MEDiterránea" (PREDIMED) trial. Subjects: 7,447 participants (55-80 years, 57% women) free of cardiovascular disease, but with either type 2 diabetes or $3 cardiovascular risk factors. Trained dietitians used both a validated 14-item questionnaire and a full-length validated 137-item food frequency questionnaire to assess dietary habits. Trained nurses measured weight, height and waist circumference. Results: Strong inverse linear associations between the 14-item tool and all adiposity indexes were found. For a two-point increment in the 14-item score, the multivariable-adjusted differences in WHtR were 20.0066 (95% confidence interval,- 0.0088 to 20.0049) for women and-0.0059 (-0.0079 to-0.0038) for men. The multivariable-adjusted odds ratio for a WHtR.0.6 in participants scoring $10 points versus #7 points was 0.68 (0.57 to 0.80) for women and 0.66 (0.54 to 0.80) for men. High consumption of nuts and low consumption of sweetened/carbonated beverages presented the strongest inverse associations with abdominal obesity. Conclusions: A brief 14-item tool was able to capture a strong monotonic inverse association between adherence to a good quality dietary pattern (Mediterranean diet) and obesity indexes in a population of adults at high cardiovascular risk.

High blood pressure (BP) has been ranked as the most important risk factor worldwide regarding attributable deaths. Dietary habits are major determinants of BP. Among them, frequent intake of low-fat dairy products may protect against hypertension. Our aim was to assess the relationship between low-fat dairy product intake and BP levels and their changes after 12 month follow-up in a cohort of asymptomatic older persons at high cardiovascular risk recruited into a large-scale trial assessing the effects of Mediterranean diets on cardiovascular outcomes. Data from 2290 participants, including 1845 with hypertension, were available for analyses. Dairy products were not a specific part of the intervention; thus, data were analysed as an observational cohort. Dietary information was collected with validated semi-quantitative FFQ and trained personnel measured BP. To assess BP changes, we undertook cross-sectional analyses at baseline and at the end of follow-up and longitudinal analyses. A statistically significant inverse association between low-fat dairy product intake and systolic BP was observed for the 12-month longitudinal analysis. In the longitudinal analysis, the adjusted systolic and diastolic BP were significantly lower in the highest quintile of low-fat dairy product intake ( 2 4·2 (95% CI 2 6·9, 2 1·4) and 2 1·8 (95% CI 2 3·2, 2 0·4) mmHg respectively), whereas the point estimates for the difference in diastolic BP indicated a modest non-significant inverse association. Intake of low-fat dairy products was inversely associated with BP in an older population at high cardiovascular risk, suggesting a possible protective effect against hypertension.

Background: Epidemiologic studies have suggested that flavonoid intake plays a critical role in the prevention of coronary heart disease. Because atherosclerosis is considered a low-grade inflammatory disease, some feeding trials have analyzed the effects of cocoa (an important source of flavonoids) on inflammatory biomarkers, but the results have been controversial. Objective: The objective was to evaluate the effects of chronic cocoa consumption on cellular and serum biomarkers related to atherosclerosis in high-risk patients. Design: Forty-two high-risk volunteers (19 men and 23 women; mean 6 SD age: 69.7 6 11.5 y) were included in a randomized crossover feeding trial. All subjects received 40 g cocoa powder with 500 mL skim milk/d (C+M) or only 500 mL skim milk/d (M) for 4 wk. Before and after each intervention period, cellular and serum inflammatory biomarkers related to atherosclerosis were evaluated. Results: Adherence to the dietary protocol was excellent. No significant changes in the expression of adhesion molecules on T lymphocyte surfaces were found between the C+M and M groups. However, in monocytes, the expression of VLA-4, CD40, and CD36 was significantly lower (P = 0.005, 0.028, and 0.001, respectively) after C+M intake than after M intake. In addition, serum concentrations of the soluble endothelium-derived adhesion molecules P-selectin and intercellular adhesion molecule-1 were significantly lower (both P = 0.007) after C+M intake than after M intake. Conclusions: These results suggest that the intake of cocoa polyphenols may modulate inflammatory mediators in patients at high risk of cardiovascular disease. These antiinflammatory effects may contribute to the overall benefits of cocoa consumption against atherosclerosis. Currently, simultaneous pancreas-kidney transplantation (SPK Tx) is the treatment of choice in selected patients with type 1 diabetes mellitus (DM1) and terminal kidney failure (TRF). A functioning SPK transplant allows dialysis and insulin therapy to be discontinued and stabilizes or improves the complications of DM1. Nevertheless, to a greater or lesser degree, these complications (physical and psychological alterations, secondary effects of immunosuppressive therapy and the need for lifelong medication and medical follow-up) can persist after SPK Tx. Health professionals have mainly investigated the clinical features of transplant recipients. However, in the last few years, interest in analyzing perceived health and health-related quality of life (QoL) has increased. This latter concept includes the features of QoL most closely associated with a particular disease, its treatment and follow-up and therefore those elements most susceptible to modification by the health system. The general aim of this study was to measure health-related QoL in our population with SPK Tx and to determine whether there are significant differences between these patients and those with DM1 and TRF who continue to receive renal replacement therapy (RRT) and insulin therapy. More specific aims were to evaluate whether there are significant differences between the study groups and the means of the Spanish reference population in the distinct dimensions of a QoL questionnaire and whether other variables such as age, sex, years" duration of DM1, length of dialysis, and time since SPK Tx significantly affect health-related QoL.

El objetivo del presente estudio es examinar el tip0 y sentido de 10s cambios que las personas mayores perciben como asociados al envejecimiento. Para el10 se entrevist6 a 200 personas mayores de 65 años y se les preguntó qué cambios definíun ese proceso. ~uesirorse sultados muestran Que la percepción del enveiecimiento, pese a estar dominada por cambios de tipi bioiógico, incluye' también cimbios psicológicos y sociales. La gran mayoría de 10s cambios mencionados, independientemente de la dimensión a la,que pertenecían, hicieron referencia a pérdidas, encontrándose pocas referencias a ganancias. Esto parece indicar que el envejecimiento es percibido por las personas mayores de nuestra muestra en términos de declive.
