L’envelliment de la població demana cada cop més centres especialitzats en geriatria en els països desenvolupats. Aquests mateixos països pateixen recentment una falta de personal qualificat en infermeria. Aquests dos factors es combinen i posen en evidència la necessitat de desenvolupar eines informàtiques que modernitzin les residències geriàtriques. El projecte que s’ha desenvolupat consisteix en una aplicació de gestió d’una residència geriàtrica amb una base de dades centralitzada en un PC des de la qual s’hi pot accedir a través d’una aplicació feta per PDA. La seva funció principal és poder agilitzar tota la repartició de medicació dins la residència geriàtrica, gestionant des de les pautes de medicació corresponents a cada resident, fins a un control d’stock. Consta, per tant, d’una aplicació per PC, programada amb Visual Basic i utilitzant com a base de dades Microsoft Access, i d’una aplicació per PDA, programada amb Embedded Visual Basic i utilitzant la mateixa base de dades creada per l’aplicació per PC. L’aplicació de PC proporcionarà als usuaris una gestió ràpida i còmode i, alhora, rapidesa i estalvi de treball si es vol fer un determinat manteniment a través de l’aplicació de PDA. En el manteniment de l’aplicació, però, no s’hi inclou el manteniment dels usuaris tot i haver un control d’aquests. En aquest projecte s’estudia com es desenvolupa una aplicació per PDA i com comparteix una base de dades amb una altra aplicació a través del sistema de sincronització PC↔PDA anomenat ActiveSync.

En los últimos años, el número de pacientes que presenta osteonecrosis maxilar asociada al uso de bifosfonatos (BIONJ) se ha incrementado. Esto es debido al aumento en el consumo de bifosfonatos, los cuales se asocian al tratamiento de carcinomas con metástasis óseas, mieloma múltiple, osteoporosis, osteopenia y enfermedades metabólicas como la enfermedad de Paget. Objetivo: Analizar el número de casos de pacientes que han desarrollado osteonecrosis de maxilares asociado al uso de bifosfonatos, publicados desde Junio del año 2006 hasta Abril 2010. Método: En esta revisión, se consultaron las bases de datos Pubmed-Medline, Scielo e Índice Médico Español, incluyendo límites en la búsqueda, para recopilar los casos de BIONJ que se hayan publicado desde el año 2006 hasta la actualidad. Los artículos seleccionados presentaban casos, en los cuales los pacientes que recibían bifosfonatos desarrollaban BIONJ. Resultados: Se encontraron 491 casos de BIONJ en total, de ellos 49,3% eran mujeres, 32% hombres y 18,7% no se establecía el género. La mayoría de estos casos se presentaron en la mandíbula y asociados particularment al ácido zolendrónico. Conclusiones: En los próximos años se espera que el número de pacientes que desarrollen esta complicación vaya en aumento, en particular en mujeres, a las cuales se les indica cada vez más esta medicación para el tratamiento de la osteoporosis.

Objetivo: Se comparó in vitro la acción antimicrobiana de diversas medicaciones intraconducto frente a Enterococcus faecalis y Actinomyces israelii. Material y métodos: para evaluar las zonas de inhibición microbiana se utilizó el test de difusión en agar frente a diversas pastas, incluyendo una pasta con base de metronidazol (Grinazole ®), una pasta con base de dexametasona, tiretrocina, polimixana y neomicina (Septomixine forte ®) y otra de hidróxido de calciio (Calcipulpe ®) y paroclorofenol alcanforado (KRI-3 ®), frente a E. faecalis y A. israelii. Resultados: La acción inhibitoria frente a E. faecalis fue significativamente mayor (p< 0,05) con Septomixine forte ® (30,7 mm) seguido de KRI- 3 ® (22,5 mm), Calcipulpe ® (16,6 mm) y Grinazole ® (11,4 mm). La inhibición microbiana frente a E. israelii fue significativamente mejor con Septomixine forte ® (p< 0,05) (40,1 mm) seguido de KRI-3 ® (36,6 mm), Grinazole ® (7,6 mm) y Calcipulpe ® (6 mm). Conclusión: Septomixine forte ® fue la pasta antibiótica más efectiva frente a E. faecalis y A. israelii comunmente hallados en las infecciones endodóncicas.

Un dels objectius centrals en biomedicina és la cerca de noves teràpies. Hi ha moltes aproximacions possibles, depenent de la malaltia i la tecnologia disponible, des de les més expeditives, com la cirurgia, fins a les més sofisticades, basades en l'enginyeria genètica, passant pels medicaments convencionals,que aprofiten els efectes de determinats productes sobre el nostre cos. En qualsevol cas, cada cop que s'utilitza una nova estratègia terapèutica o es comercialitza un nou fàrmac hi ha la possibilitat que es produeixin efectes secundaris inesperats [...].

Purpose: The aim of the study was to evaluate the opinions and concerns of hospital doctors about adverse drug reactions (ADRs) and pharmacovigilance. Methods: A qualitative study was undertaken using focus groups in sessions on pharmacovigilance activities conducted in thirteen clinical services of a tertiary university hospital. A total of 296 physicians participated in these sessions by giving their opinions or expressing their doubts about ADR and pharmacovigilance activities which were recorded by different observers and subsequently analysed. Results: Doctors remarked on: a) the importance, concern, frequency and specific types of ADRs that were observed in clinical practice; b) problems of clinical decision making related to the suspected ADRs; c) methods for improving detection and reporting ADRs; d) monitoring of specific ADRs or ADRs caused by specific drugs; e) and measures to prevent and minimize the risk of ADRs. Physicians expressed doubts related to: a) the basic concepts of ADRs; b) the methods of ADR identification and evaluation; c) the objectives and procedures of pharmacovigilance programmes; d) and the impact of pharmacovigilance activities. Conclusions: Hospital doctors believe that ADRs are a matter for concern in their daily clinical practice, and monitoring ADRs as well as measures for preventing the risk of ADRs are needed. Nevertheless, doctors have doubts about what an ADR is, the accuracy of diagnostic methods, the development of pharmacovigilance activities and their impact on clinical practice. Pharmacovigilance should be better explained through a continuous feedback and close relationship with hospital doctors.

Background: Scientific evidence on treatments of chronic diseases in patients 85 years old or older is very limited, as is available information on inappropriate prescription (IP) and its associated factors. The study aimed to describe medicine prescription, potentially inappropriate medicines (PIM) and potentially prescribing omissions (PPO) and their associated factors on this population. Methods: In the context of an observational, prospective and multicentric study carried out in elderly patients admitted to seven Spanish hospitals for a year, a sub-analysis of those aged 85 years and over was performed. To assess PIMs, the Beers and STOPP criteria were used, and to assess PPOs, the START and the ACOVE-3 criteria were used. To assess factors associated with IP, a multivariate logistic regression analysis was performed. Patients were selected randomly every week on consecutive days from the hospitalization lists. Results: A total of 336 patients were included in the sub-analysis with a median (Q1-Q3) age of 88 (8690) years. The median medicines taken during the month prior to admission was 10 (713). Forty-seven point two per cent of patients had at least one Beers-listed PIM, 63.3% at least one STOPP-listed PIM, 53.6% at least one START-listed PPO, and 59.4% at least one ACOVE-3-listed PPO. Use of benzodiazepines in patients who are prone to falls (18.3%) and omission of calcium and vitamin D supplements in patients with osteoporosis (13.3%) were the most common PIM and PPO, respectively. The main factor associated with the Beers-listed and the STOPP-listed PIM was consumption of 10 or more medicines (OR = 5.7, 95% CI 1.8-17.9 and OR = 13.4, 95% CI 4.0-44.0, respectively). The main factors associated with the START-listed PPO was a non-community dwelling origin (OR 2.3, 95% CI 1.0-5.0), and multimorbidity (OR1.8, 95% CI 1.0-3.1). Conclusions: Prescribed medicines and PIM and PPO prevalence were high among patients 85 years and over. Benzodiazepine use in those who are prone to falls and omission of calcium and vitamin D in those with osteoporosis were the most frequent PIM and PPO, respectively. Factors associated with PIM and PPO differed with polypharmacy being the most important factor associated with PIM.

A major goal in antimicrobial drug discovery is the generation of new chemicals capable of killing pathogens with high selectivity, particularly multi-drug-resistant ones. Here we report the design, preparation and activity of new compounds based on a tunable, chemically accessible and upscalable lipopeptide scaffold amenable to suitable hit-to-lead development. Such compounds could become therapeutic candidates and future antibiotics available on the market. The compounds are cyclic, contain two D-amino acids for in vivo stability and their structures are reminiscent of other cyclic disulfide-containing peptides available on the market. The optimized compounds prove to be highly active against clinically relevant Gram-negative and Gram-positive bacteria. In vitro and in vivo tests show the low toxicity of the compounds. Their antimicrobial activity against resistant and multidrug-resistant bacteria is at the membrane level, although other targets may also be involved depending on the bacterial strain.
