Exenteration of the orbit is a disfiguring and destructive procedure; it is generally performed for orbital malignancies and often provides a significant reconstructive challenge. Our purpose was to evaluate the clinical indications for orbital exenteration in a tertiary referral center and to assess the reconstructive options employed. A retrospective nonrandomized analysis was performed, selecting all patients undergoing orbital exenteration over a 5-year period, between January 2005 and January 2010. Patient demographics, tumor characteristics, and reconstructive techniques used were evaluated. Twenty patients with a mean age of 76.5 years underwent total orbital exenteration. Basal cell carcinoma was the main operative indication (45%), followed by squamous cell carcinoma (15%). Reconstructive techniques included cover of the raw orbital cavity with a temporal muscular flap in all cases followed with split skin grafting (25%), bilaterally pedicle V-Y advancement flap (10%) and a fasciocutaneous island flap of the retroauricular region (65%). Twenty percent of patients had local complications and all were treated in a satisfactory fashion. Eyelid skin tumors remain an important cause of orbital exenteration. Temporal muscle flap is a reliable and stable reconstructive solution after orbital exenteration and additional aid is supplied with skin grafts or local flaps. This technique ensures a good aesthetic outcome and better situation for later complementary treatments and minimal associated donor site morbidity.

Introduction & Objectives: Several factors may influence the decision to pursue nonsurgical modalities for the treatment of non-melanoma skin cancer. Topical photodynamic therapy (PDT) is a non-invasive alternative treatment reported to have a high efficacy when using standardized protocols in Bowen’s disease (BD), superficial basal cell carcinoma (BCC) and in thin nodular BCC. However, long-term recurrence studies are lacking. The aim of this study was to evaluate the long-term efficacy of PDT with topical methylaminolevulinate (MAL) for the treatment of BD and BCC in a dermato-oncology department. Materials & Methods: All patients with the diagnosis of BD or BCC, treated with MAL-PDT from the years 2004 to 2008, were enrolled. Treatment protocol included two MAL-PDT sessions one week apart repeated at three months when incomplete response, using a red light dose of 37-40 J/cm2 and an exposure time of 8’20’’. Clinical records were retrospectively reviewed, and data regarding age, sex, tumour location, size, treatment outcomes and recurrence were registered. Descriptive analysis was performed using chi square tests, followed by survival analysis with the Kaplan-Meier and Cox regression models. Results: Sixty-eight patients (median age 71.0 years, P25;P75=30;92) with a total of 78 tumours (31 BD, 45 superficial BCC, 2 nodular BCC) and a median tumour size of 5 cm2 were treated. Overall, the median follow-up period was 43.5 months (P25;P75=0;100), and a total recurrence rate of 33.8% was observed (24.4 % for BCC vs. 45.2% for BD). Estimated recurrence rates for BCC and BD were 5.0% vs. 7.4% at 6 months, 23.4% vs. 27.9% at 12 months, and 30.0% vs. 72.4% at 60 months. Both age and diagnosis were independent prognostic factors for recurrence, with significantly higher estimated recurrence rates in patients with BD (p=0.0036) or younger than 58 years old (p=0.039). The risk of recurrence (hazard ratio) was 2.4 times higher in patients with BD compared to superficial BCC (95% CI:1.1-5.3; p=0.033), and 2.8 times higher in patients younger than 58 years old (95% CI:1.2-6.5; p=0.02). Conclusions: In the studied population, estimated recurrence rates are higher than those expected from available literature, possibly due to a longer follow-up period. To the authors’ knowledge there is only one other study with a similar follow-up period, regarding BCC solely. BD, as an in situ squamous cell carcinoma, has a higher tendency to recur than superficial BCC. Despite greater cosmesis, PDT might no be the best treatment option for young patients considering their higher risk of recurrence.

Merkel cell carcinoma (MCC) is a rare malignant and primary neuroendocrine carcinoma with several known risk factors. Early diagnosis and aggressive treatment are critical. We report the case of an 82-year old woman with a Merkel cell carcinoma on the face. Clinical and histopathological features are presented. In addition, dermoscopic features and the differential diagnosis of this rare tumor are discussed. Although nodules with atypical dermoscopic vascular pattern and milky-red areas will end up being excised, this report adds more clues to the rarely described dermoscopic morphologic presentation of MCC.

BACKGROUND: Most available studies on the efficacy of topical photodynamic therapy focus on short-to medium-term results. Long-term data are scarce. OBJECTIVE: To evaluate the long-term efficacy of photodynamic therapy with topical methylaminolevulinate to treat Bowen's disease and basal cell carcinoma in the clinical practice setting of a dermato-oncology department. METHODS: The study included patients diagnosed with Bowen's disease or basal cell carcinoma, and who received photodynamic therapy from 2004 to 2008. Treatment protocol and clinical follow-up were standardized. The primary endpoint was clinically observed recurrence in a previous photodynamic therapy-treated area. Descriptive and survival analyses were performed. RESULTS: A total of 31 Bowen's disease lesions and 44 superficial basal cell carcinoma were treated, with a median follow-up of 43.5 months. Recurrence was observed in 14 Bowen's disease lesions (53.8%) and in 11 superficial basal cell carcinoma (33.3%). Significantly higher estimates for recurrence rates were found in patients with Bowen's disease (p=0.0036) or those aged under 58 years (p=0.039). The risk of recurrence was higher in patients with Bowen's disease than in those with superficial basal cell carcinoma and younger patients. CONCLUSIONS: Recurrence should be considered when choosing to treat non-melanoma skin cancer with photodynamic therapy. Younger age and Bowen's disease were independent predictors for long-term recurrence, suggesting the need to establish an extended period of follow-up for this subset of patients.

Apresenta-se o caso de um homem de 62 anos de idade com um episódio de dispneia súbita que motivou estudo ecocardiográfico, o qual revelou uma massa na aurícula direita. A avaliação posterior demonstrou tratar-se de extensão de neoplasia do rim direito. A massa tumoral foi removida através duma abordagem multidisciplinar, com cirurgia torácica e abdominal. O estudo anatomopatológico mostrou tratar-se de um carcinoma de células renais.

Skin testing remains an essential diagnostic tool in modern allergy practice. A signifi cant variability has been reported regarding technical procedures, interpretation of results and documentation. This review has the aim of consolidating methodological recommendations through a critical analysis on past and recent data. This will allow a better understanding on skin prick test (SPT) history; technique; (contra-) indications; interpretation of results; diagnostic pitfalls; adverse reactions; and variability factors.

Nos doentes com carcinoma hepatocelular a incidência de metástases cardíacas é de 0.67-3%. Mesmo na ressecção com intuito curativo, o prognóstico é reservado, sendo a sobrevida aos 5 anos de 12-39%. Descrevemos um caso clínico, pouco habitual, de um indivíduo do sexo masculino, de 51 anos de idade, que apresentava uma massa localizada à aurícula direita, diagnosticada por exame histopatológico, como sendo metástase de um carcinoma hepatocelular, tendo sido necessária a ressecção cirúrgica urgente, devido a instabilidade cardiovascular.

Nos países europeus, o hemoperitoneu por ruptura de carcinoma hepatocelular (HCC) é muito raro. Apresentam-se dois casos clínicos de hemoperitoneu secundário a ruptura espontânea de carcinoma hepatocelular, em que a Tomografia Computorizada (TC) abdominal foi de primordial importância para o diagnóstico. A propósito faz-se revisão da literatura sobre os aspectos mais relevantes desta patologia.

Os autores apresentam um caso clínico de derrame pleural direito, num doente do sexo feminino com 68 anos de idade, surgindo fora do contexto de patologia cardiovascular ou infecciosa previamente conhecida. O diagnóstico final foi de neoplasia pleural secundária a carcinoma adenoquístico, com localização primitiva no palato duro, diagnosticado e tratado 8 anos antes, e que nunca apresentara recidiva local. Salienta-se a raridade deste diagnóstico e o facto de que, estando descrita a metastização pulmonar com possível envolvimento secundário da pleura, é muito raro o aparecimento de metástase pleural isolada.

The distinction between primary melanoma and melanoma metastatic to the skin has major prognostic implications. We report a case of a 67-year-old male with a diagnosis of a superficial spreading melanoma (stage IB) rendered 6 years earlier who presented clinically with an atypical nevus on his left thigh. Histopathological examination showed an intraepidermal melanocytic proliferation that was interpreted as melanoma in situ. Subsequently, 45 additional pigmented macules appeared in crops over a 9-month period. Clinically and dermoscopically, these lesions were extremely polymorphic. Histopathological findings were compatible with melanoma in situ, as each lesion consisted of a wholly intraepidermal proliferation of markedly atypical melanocytes arranged singly and in nests. A complete gastrointestinal study showed multiple pigmented metastatic lesions throughout the stomach and small bowel, which supported a diagnosis of metastatic melanoma with gastrointestinal and epidermotropic skin involvement. Monosomy of chromosome 9 and a BRAF V600E mutation were detected in the primary tumor sample and in macro-dissected secondary lesions. No CDKN2A or CDK4 germline mutations were found. Intraepidermal epidermotropic metastases of melanoma have been rarely described in literature. In this case, histopathology alone was insufficient to distinguish metastatic melanoma from multiple in situ melanomas. The recognition of epidermotropic metastases should be based on the correlation between clinical, dermoscopic, histopathological and molecular findings.

Based on a case of gastric antral vascular ectasia (watermelon stomach) that was associated with hemorrhagic pericarditis, small cell lung carcinoma with mediastinal lymph node metastases and a synchronous squamous cell carcinoma of the base of the tongue, the authors made a review of the clinical, endoscopic and histopathological aspects of this type of gastropathy, and its association with other diseases, and of the results of its endoscopic therapy. The causes of hemorrhagic pericarditis are considered, emphasizing the necessity to know if the effusion has a malignant etiology. To the best of our knowledge the association of watermelon stomach to small cell lung carcinoma and squamous cell carcinoma of the base of the tongue has not yet been described. Extensive metastases to mediastal lymph nodes are common to small cell lung carcinoma.

Estão raramente descritos na literatura casos de metástases uterinas de cancros extra genitais, sendo o aparelho genital feminino normalmente invadido por contiguidade. Mais frequentes são contudo, as metástases ováricas de neoplasias extragenitais sobretudo do aparelho gastrointestinal e mama. Descrevemos o caso clínico de uma doente de 71 anos que recorreu ao serviço de urgência por metrorragias pós menopausa (sem THS), tendo sido diagnosticado pólipo do endocolo e efectuada polipectomia. No exame anatomopatológico foi identificada, no eixo do pólipo, uma infiltração neoplásica compatível com metástase de carcinoma lobular da mama. A doente encontra-se desde há um ano a efectuar Quimioterapia com boa resposta.

Introdução: A rotura de carcinoma hepatocelular é um evento que ocorre em 3 a 26% dos casos de carcinoma hepatocelular e está associado a uma elevada taxa de mortalidade. A apresentação habitual é a de um quadro de hemoperitoneu. A abordagem destes doentes ainda não é consensual e encontra-se dependente da reserva hepática funcional e estadiamento tumoral, mas também dos meios tecnológicos disponíveis na instituição hospitalar e experiência da equipa cirúrgica. Caso clínico: Doente de 57 anos admitida no serviço de urgência por um quadro clínico de abdómen agudo associado a sinais de hipovolémia. Durante a observação registou-se um agravamento clínico e instabilidade hemodinâmica. Dos exames complementares destacava-se a existência de acidose metabólica, anemia aguda, e após medidas iniciais de ressuscitação com estabilização hemodinâmica, tomografia computorizada que revelou sinais de hemoperitoneu associados à existência de um tumor hepático em rotura no segmento V/VI. A doente foi abordada cirurgicamente, encontrando-se tumor hepático solitário em hemorragia activa. Realizado controlo hemostático e ressecção hepática. Pós-operatório sem intercorrências. Na histologia da peça operatória foi identificado carcinoma hepatocelular. Conclusão: O presente caso clínico pretende chamar a atenção para uma causa não frequente de hemoperitoneu espontâneo, mas associada a elevada taxa de mortalidade. O diagnóstico e abordagem precoces são essenciais para evitar um evento fatal.

O Mesotelioma peritoneal maligno é um tumor maligno relacionado frequentemente com exposição prolongada a fibras de amianto, de mau prognóstico, de diagnóstico geralmente tardio, face à pouca expressão clínica na fase inicial da doença. Como o mesotelioma evolui geralmente só na cavidade peritoneal, doentes seleccionados poderão ter maior sobrevivência se for possível a peritonectomia extensa e quimioterapia hipertérmica intraperitoneal intraoperatória. Os autores referem a sincronicidade ainda não descrita, de mesotelioma peritoneal maligno primário e carcinoma de Grawitz. São revistos concisamente: a clínica destes tumores, síndromes paraneoplásicos (disfunção bioquímica hepática, emagrecimento extremo); etiopatogenia da acção cancerígena das fibras de amianto; mecanismos de disseminação intraperitoneal; avaliação tomodensitométrica; importância da imunohistoquímica no diagnóstico histopatológico; estadiamento; importância do tratamento multidisciplinar destes tumores.
