INTRODUCTION: Adults with repaired tetralogy of Fallot (TOF) may be at risk for progressive right ventricular (RV) dilatation and dysfunction, which is commonly associated with arrhythmic events. In frequently volume-overloaded patients with congenital heart disease, tissue Doppler imaging (TDI) is particularly useful for assessing RV function. However, it is not known whether RV TDI can predict outcome in this population. OBJECTIVE: To evaluate whether RV TDI parameters are associated with supraventricular arrhythmic events in adults with repaired TOF. METHODS: We studied 40 consecutive patients with repaired TOF (mean age 35 +/- 11 years, 62% male) referred for routine echocardiographic exam between 2007 and 2008. The following echocardiographic measurements were obtained: left ventricular (LV) ejection fraction, LV end-systolic volume, LV end-diastolic volume, RV fractional area change, RV end-systolic area, RV end-diastolic area, left and right atrial volumes, mitral E and A velocities, RV myocardial performance index (Tei index), tricuspid annular plane systolic excursion (TAPSE), myocardial isovolumic acceleration (IVA), pulmonary regurgitation color flow area, TDI basal lateral, septal and RV lateral peak diastolic and systolic annular velocities (E' 1, A' 1, S' 1, E' s, A' s, S' s, E' rv, A' rv, S' rv), strain, strain rate and tissue tracking of the same segments. QRS duration on resting ECG, total duration of Bruce treadmill exercise stress test and presence of exercise-induced arrhythmias were also analyzed. The patients were subsequently divided into two groups: Group 1--12 patients with previous documented supraventricular arrhythmias (atrial tachycardia, fibrillation or flutter) and Group 2 (control group)--28 patients with no previous arrhythmic events. Univariate and multivariate analysis was used to assess the statistical association between the studied parameters and arrhythmic events. RESULTS: Patients with previous events were older (41 +/- 14 vs. 31 +/- 6 years, p = 0.005), had wider QRS (173 +/- 20 vs. 140 +/- 32 ms, p = 0.01) and lower maximum heart rate on treadmill stress testing (69 +/- 35 vs. 92 +/- 9%, p = 0.03). All patients were in NYHA class I or II. Clinical characteristics including age at corrective surgery, previous palliative surgery and residual defects did not differ significantly between the two groups. Left and right cardiac chamber dimensions and ventricular and valvular function as evaluated by conventional Doppler parameters were also not significantly different. Right ventricular strain and strain rate were similar between the groups. However, right ventricular myocardial TDI systolic (Sa: 5.4+2 vs. 8.5 +/- 3, p = 0.004) and diastolic indices and velocities (Ea, Aa, septal E/Ea, and RV free wall tissue tracking) were significantly reduced in patients with arrhythmias compared to the control group. Multivariate linear regression analysis identified RV early diastolic velocity as the sole variable independently associated with arrhythmic history (RV Ea: 4.5 +/- 1 vs. 6.7 +/- 2 cm/s, p = 0.01). A cut-off for RV Ea of < 6.1 cm/s identified patients in the arrhythmic group with 86% sensitivity and 59% specificity (AUC = 0.8). CONCLUSIONS: Our results suggest that TDI may detect RV dysfunction in patients with apparently normal function as assessed by conventional echocardiographic parameters. Reduction in RV early diastolic velocity appears to be an early abnormality and is associated with occurrence of arrhythmic events. TDI may be useful in risk stratification of patients with repaired tetralogy of Fallot.

INTRODUCTION: Peripheral embolism is frequently related to a cardiac source of embolism. Transesophageal echocardiography (TEE) is a useful tool for identifying such sources. OBJECTIVES: Our laboratory has gained wide experience in TEE, with a large number of exams performed to search for a cardiac source of embolism. We therefore thought it would be useful to present our experience in the last 12 years following the introduction of the technique. METHODS: This was a retrospective study of 1110 consecutive patients undergoing TEE to search for a cardiac source of embolism, after an embolic event and a transthoracic echocardiogram. RESULTS: The patients' mean age was 53 +/- 14 years, 52% male. There was peripheral embolism in 5% of cases and cerebral embolism in the remainder. The exam identified a potential embolic source in 35.6% of cases, the most frequent diagnoses being intracardiac shunt at the atrial level (9.5%), atrial septal aneurysm (ASA) (6.6%), intracardiac thrombi (6.4%) and atherosclerotic plaques in the thoracic aorta (9.6%). The presence of ASA was frequently associated with patent foramen ovale (27%), which was more frequent in younger patients. Overall, we identified a cardiac source of embolism more often in elderly patients, with a predominance of atherosclerotic plaques in the aorta. ETE was more frequently diagnostic in patients with peripheral embolism, but there were no differences in terms of etiology. CONCLUSIONS: TEE is very useful to search for cardiac sources of embolism, especially in younger patients, in whom causes potentially treatable surgically or percutaneously can be identified. In elderly patients, therapeutic strategy will probably not be changed by the findings (mostly thrombi and atherosclerotic plaques). The presence of ASA and embolic events makes it essential to perform a thorough search by TEE for intracardiac shunts, which are frequently associated.

É apresentada uma revisão teórica sobre Hérnia Diafragmática Congénita (HDC). Os autores realizam um estudo retrospectivo de 12 casos de HDC referenciados ou diagnosticados na Unidade de Ecografia da Maternidade Dr. Alfredo da Costa (MAC) entre os anos de 1996 a 1999. Após avaliação o aconselhamento foi efectuado, caso a caso, sendo proposta Interrupção Médica da Gravidez ou cirurgia pós-natal.

Objectivos: Estudar a prevalência, factores de risco, evolução clínica e abordagem terapêutica da sífilis congénita em recém-nascidos (RN) de risco, nascidos numa maternidade de referência com apoio perinatal diferenciado. Método: Realizou-se um estudo transversal para cálculo de prevalência à nascença de sífilis congénita, entre Janeiro de 1993 e Dezembro de 2004, através de recolha de dados registados nos processos clínicos das mães e respectivos RN. De acordo com os critérios definidos pelo Centers for Disease Control and Prevention (CDC) em 1989, os RN filhos de mãe com VDRL e/ou TPHA positivo foram divididos em três grupos de risco. Resultados: Foram identificados 467 recém-nascidos, verificando-se que a prevalência de risco de sífilis congénita à nascença se tem mantido ao longo dos anos (5,6‰). A maioria dos recém-nascidos (65%) enquadra-se no grupo de maior risco. Dezanove RN (4%) apresentaram sífilis congénita sintomática ao nascimento, a maioria pertencente ao grupo de maior risco. Outros factores de risco encontrados foram a gravidez não-vigiada, em 30% das mães, toxicodependência em 9%, coinfecção por vírus da hepatite B em 5%, por vírus da hepatite C em 4,7% e por vírus de imunodeficiência humana em 3,4% dos casos. Em alguns casos existia mais do que um factor de risco associado. Conclusões: Verificou-se que a prevalência de risco de sífilis congénita não sofreu grandes variações ao longo dos doze anos, pelo que a sífilis continua a constituir um problema de Saúde Pública em Portugal, com custos económicos e sociais.

Our purposes are to determine the impact of histological factors observed in zero-time biopsies on early post transplant kidney allograft function. We specifically want to compare the semi-quantitative Banff Classification of zero time biopsies with quantification of % cortical area fibrosis. Sixty three zero-time deceased donor allograft biopsies were retrospectively semiquantitatively scored using Banff classification. By adding the individual chronic parameters a Banff Chronic Sum (BCS) Score was generated. Percentage of cortical area Picro Sirius Red (%PSR) staining was assessed and calculated with a computer program. A negative linear regression between %PSR/ GFR at 3 year post-transplantation was established (Y=62.08 +-4.6412X; p=0.022). A significant negative correlation between arteriolar hyalinosis (rho=-0.375; p=0.005), chronic interstitial (rho=0.296; p=0.02) , chronic tubular ( rho=0.276; p=0.04) , chronic vascular (rho= -0.360;P=0.007), BCS (rho=-0.413; p=0.002) and GFR at 3 years were found. However, no correlation was found between % PSR, Ci, Ct or BCS. In multivariate linear regression the negative predictive factors of 3 years GFR were: BCS in histological model; donor kidney age, recipient age and black race in clinical model. The BCS seems a good and easy to perform tool, available to every pathologist, with significant predictive short-term value. The %PSR predicts short term kidney function in univariate study and involves extra-routine and expensive-time work. We think that %PSR must be regarded as a research instrument.

PURPOSE: To evaluate whether prostatic arterial embolization (PAE) might be a feasible procedure to treat lower urinary tract symptoms associated with benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: Fifteen patients (age range, 62-82 years; mean age, 74.1 y) with symptomatic BPH after failure of medical treatment were selected for PAE with nonspherical 200-μm polyvinyl alcohol particles. The procedure was performed by a single femoral approach. Technical success was considered when selective prostatic arterial catheterization and embolization was achieved on at least one pelvic side. RESULTS: PAE was technically successful in 14 of the 15 patients (93.3%). There was a mean follow-up of 7.9 months (range, 3-12 months). International Prostate Symptom Score decreased a mean of 6.5 points (P = .005), quality of life improved 1.14 points (P = .065), International Index of Erectile Function increased 1.7 points (P = .063), and peak urinary flow increased 3.85 mL/sec (P = .015). There was a mean prostate-specific antigen reduction of 2.27 ng/mL (P = .072) and a mean prostate volume decrease of 26.5 mL (P = .0001) by ultrasound and 28.9 mL (P = .008) by magnetic resonance imaging. There was one major complication (a 1.5-cm(2) ischemic area of the bladder wall) and four clinical failures (28.6%). CONCLUSIONS: In this small group of patients, PAE was a feasible procedure, with preliminary results and short-term follow-up suggesting good symptom control without sexual dysfunction in suitable candidates, associated with a reduction in prostate volume.

Os AA avaliaram dois grupos de crianças oriundas de diferentes estratos socio-familiares, procurando variantes do normal, que confirmem a importância do jardim de infância como factor atenuante de ambiente familiar menos estimulante. Material Métodos: Foram seleccionados aleatoriamente pelo Serviço Social do Hospital, dois jardins de infância, em Lisboa. A avaliação incidiu num grupo de crianças dos dois aos quatro anos, sem patologia. Na caracterização social e familiar foi utilizada a escala de Graffar. A avaliação de desenvolvimento psicomotor foi efectuada por observado único; o teste utilizado foi o "Schedule of Growing Skills in Practice" e a análise estatística foi efectuada pelo teste The Student (significância p=<0,05). Resultados: A população estudad foi constituída por 34 crianças, 14 das quais frequentava Jardim de Infância particular (JIP) e as restantes 20, Insitutição Particular de Solidariedade Social (IPSS), com idades compreendidas entre os 2 e os 4 anos. Na subescala da Locomoção, a pontuação obtida foi de 12,8 e 12,9 respectivamente no IPSS e JIP (=0,824) e na Manipulação foi de 19,3 (IPSS) e 20,7 (JIP) (p=0,006). Os resultados obtidos na área da Visão foram de 16,1 e 17,3 respectivamente na IPSS e JIP (p=0,005). A avaliação da Audição/Linguagem revelou resultados de 13,1 (IPSS) e 15,5 (JIP) (p=0,002) e na subescala da Fala/Linguagem, foram obtidos resultados de 14,5 (IPSS)e 17,3 (JIP) (p=0,008). As áreas de interacção social e autonomia, revelaram ambas pontuações de 18,3 (IPSS) e 19,8 (JIP), (respectivamente p=0,001 e p=0,017). Conclusões: Na avaliação efectuada, não encontrámos diferenças estatísticamente significativas nas subescalas de Locomoção e da Manipulação. Nas áreas da Autonomia, Audição/Linguagem e Fala/linguagem, os resultados foram estatísticamente superioes no grupo de crianças que frequentavam o JIP (oriundas de classes socio-familiares mais favorecidas e de famílias menos numerosas), comparativamente às que frequentavam a IPSS.

Os autores analisam os processos clínicos dos doentes com carcinoma do pulmão, internados no Serviço 1 de Medicina do Hospital de Santo António dos Capuchos, num período de 12 anos (1978-1989). Dos 112 doentes considerados, 50% tinham confirmação histológica, os restantes tinham forte evidência clínico-radiológica, endoscópica e/ou necrópsica ou pesquisa positiva de células neoplásicas na expectoração, secreções brônquicas ou no líquido pleural. Apresentam-se os aspectos clínicos, meios complementares de diagnóstico utilizados, e realça-se o estado geralmente avançado da doença, a dificuldade do seu diagnóstico histológico e a elevada mortalidade (55,4%).

Objectivo — Conhecer a epidemiologia da meningite bacteriana em recém-nascidos admitidos na Unidade de Cuidados Intensivos Neonatais do Hospital de Dona Estefânia. Doentes e métodos — Foi feita a revisão dos processos de recém-nascidos admitidos na Unidade de Cuidados Intensivos Neonatais do Hospital de Dona Estefânia de Janeiro de 1985 a Dezembro de 1996 — 12 anos, provenientes da maternidade do Hospital ou do exterior. Foram excluídas as infecções congénitas e as crianças com idade superior a 28 dias. Definiu-se como precoce a infecção com início nas primeiras 72 horas de vida. Resultados — Houve 36 casos de meningite bacteriana correspondendo a 1,1% das admissões. A incidência de meningite bacteriana precoce na Maternidade do Hospital foi 0,13 por mil nados-vivos. Vinte e quatro crianças eram do sexo masculino (66,7%), 7 eram pré-termo, 4 de baixo peso e 1 de muito baixo peso. Dez recém-nascidos tiveram meningite precoce (27,8%) e 26 (72,2%) meningite tardia. Houve isolamento do agente bacteriano no líquido cefalorraquidiano em 27 crianças (77,1%): E. coli (n=7); Streptococcus do grupo B(SGB) (n=6); Klebsiella pneumoniae (n=3); Proteus mirabilis (n=2), Listeria monocytogenes (n=1), Streptococcus bovis (n=1), Staphylococcus aureus (n=1), Neisseria meningitidis (n=2) e Salmonella tiphy (n=1). Houve ainda o isolamento de 3 Gram negativos não identificados. A hemocultura foi positiva em 19 de 32 colheitas (59,4%). Na ausência de terapêutica antibiótica, em 6 casos a cultura do líquor foi positiva e a hemocultura negativa e noutros 2 ambas as culturas foram negativas. Durante o internamento faleceram 9 recém-nascidos — mortalidade de 25% e em 11 foram detectadas sequelas. Conclusão — Houve um predomínio de casos de meningite tardia, em recém-nascidos de termo e do sexo masculino. Os agentes mais frequentemente encontrados foram a E. coli e a Streptococcus do grupo B.

Objective:We aimed to identify the cut-off for risk of pre-eclampsia (PE) in Portuguese population by applying the first trimester prediction model from Fetal Medicine Foundation (FMF) in a prospective enrolled cohort of low risk pregnant women. Population and methods: A prospective cohort of low risk singleton pregnancies underwent routine first-trimester scree - ning from 2011 through 2013. Maternal characteristics, blood pressure, uterine artery Doppler, levels of pregnancy-associated plasma protein-A (PAPP-A) and free b-human chorionic gonadotropin were evaluated. The prediction of PE in first trimester was calculated through software Astraia, the outcome obtained from medical records and the cutoff value was subse quently calculated. Results:Of the 273 enrolled patients, 7 (2.6%) developed PE. In first trimester women who developed PE presented higher uterine arteries resistance, represented by higher values of lowest and mean uterine pulsatility index, p <0.005. There was no statistical significance among the remaining maternal characteristics, body mass index, blood pressure and PAPP-A. Using the FMF first trimester PE algorithm, an ideal cut-off of 0.045 (1/22) would correctly detect 71% women who developed PE for a 12% false positive rate and a likelihood ratio of 12.98 (area under the curve: 0.69; confidence interval 95%: 0.39-0.99). By applying the reported cutoff to our cohort, we would obtain 71.4% true positives, 88.3% true negatives, 11.4% false positives and 28.6% false negatives. Conclusion: By applying a first trimester PE prediction model to low risk pregnancies derived from a Portuguese population, a significant proportion of patients would have been predicted as high risk. New larger studies are required to confirm the present findings.
