Pretende-se realizar uma revisão sobre as estruturas neuroanatómicas e os processos de neurotransmissão e de modulação neuronal envolvidos na memória. A memória é vista como um processo activo, baseado na reunião de variados aspectos dos padrões de activação neuronal, num processo dependente da experiência. Descreve-se a importância das regiões límbicas e sua inter-relação no processamento da memória, desde a codificação até à consolidação que implica uma reestruturação neuronal. Procede-se à caracterização dos tipos de memória (implícita e explícita) e suas características,a relação com as diferentes fases do desenvolvimento, tocando alguns aspectos mais descritivos de diferentes componentes da memória. Numa perspectiva contextual são também focadas as áreas da amnésia infantil, a lembrança, o esquecimento e o trauma. O esquecimento é parte essencial da memória explícita. Vários estudos apontam para um efeito de curva em U invertida, em relação com o impacto emocional. Eventos com intensidade moderada a elevada parecem ser etiquetados como importantes (envolvimento de estruturas do sistema límbico como a amígdala e o córtex orbito-frontal) e são mais facilmente recordadas no futuro. Se os eventos são muitos intensos o processamento no hipocampo da codificação memoria explícita e subsequentemente a recordação são inibidos,há também um bloqueio da consolidação cortical da experiência, porém mantém-se a memória implícita, que pode levar a vivência de imagens intrusivas não elaboradas. No que concerne ao trauma, levanta-se a questão da precisão da memória e do seu impacto, tanto ao nível das implicações neurofisiológicas como das consequências psicopatológicas. As alterações ao nível das neurotransmissões induzidas pelo stress vão condicionar alterações na mielinização, sinaptogénese e neurogénese, que podem levar a alterações a longo prazo em várias regiões do cérebro entre as quais a amígdala. Nestas condições, o processo explícito e a aprendizagem podem ser cronicamente impedidas, para além do risco importante para o desenvolvimento de psicopatologia, nomeadamente: Perturbação da Personalidade, Perturbações de Humor (depressão); Perturbações de Ansiedade; Perturbações Dissociativas e maior risco de Psicose.

Our purposes are to determine the impact of histological factors observed in zero-time biopsies on early post transplant kidney allograft function. We specifically want to compare the semi-quantitative Banff Classification of zero time biopsies with quantification of % cortical area fibrosis. Sixty three zero-time deceased donor allograft biopsies were retrospectively semiquantitatively scored using Banff classification. By adding the individual chronic parameters a Banff Chronic Sum (BCS) Score was generated. Percentage of cortical area Picro Sirius Red (%PSR) staining was assessed and calculated with a computer program. A negative linear regression between %PSR/ GFR at 3 year post-transplantation was established (Y=62.08 +-4.6412X; p=0.022). A significant negative correlation between arteriolar hyalinosis (rho=-0.375; p=0.005), chronic interstitial (rho=0.296; p=0.02) , chronic tubular ( rho=0.276; p=0.04) , chronic vascular (rho= -0.360;P=0.007), BCS (rho=-0.413; p=0.002) and GFR at 3 years were found. However, no correlation was found between % PSR, Ci, Ct or BCS. In multivariate linear regression the negative predictive factors of 3 years GFR were: BCS in histological model; donor kidney age, recipient age and black race in clinical model. The BCS seems a good and easy to perform tool, available to every pathologist, with significant predictive short-term value. The %PSR predicts short term kidney function in univariate study and involves extra-routine and expensive-time work. We think that %PSR must be regarded as a research instrument.

Introdução: A amiloidose é uma doença sistémica, cujo diagnóstico cabe frequentemente ao nefrologista. O tipo de amiloidose varia de acordo com o grau de desenvolvimento do país, com maior prevalência de amiloidose AL nos países ricos. Material e métodos: Revisão retrospectiva de todas as biopsias de rim nativo avaliadas no serviço entre 1981 e 2008. Caracterização clínica dos doentes à data da biópsia. Avaliação morfológica qualitativa do tipo de substância amiloíde por imunofluorescência e imunoperoxidase. Avaliação semi-quantitativa do grau de depósitos de acordo com a sua localização; grau de glomeruloesclerose e fibrose tubulo -interstical. Resultados: Neste período de 28 anos, observámos 202 biópsias positivas para substância amiloíde (3,5% de 5797) num total de 197 doentes (54,4% homens vs 45,5 mulheres), com idade mediana de 59,5 ± 15,6 anos. A maioria (68%) dos doentes foi biopsada por síndrome nefrótico. A insuficiência renal e as alterações assintomáticas urinárias foram os outros principais motivos de biopsia em 15 % e 7% dos casos, respectivamente. Os doentes na altura da biopsia apresentavam proteinúria mediana de 5 g/dia ± 5,4 (n=144) e creatinina mediana de 1,3 ± 1,7 mg/dl (n=150). As amiloidoses foram classificadas como AA em 51% dos casos, AL em 31,6% (25,5% lambda e 5,9% kappa) e Polineuropatia Amiloidótica Familiar em 3,5%. Não foi possível a caracterização do tipo de amilóide, por dificuldade técnica, em 12,8% das biópsias. A amiloidose revelou-se a terceira causa de síndrome nefrótico nos doentes com mais de 65 anos. Os doentes com amiloidose primária são significativamente mais velhos do que aqueles com amiloidose secundária ou PAF (65,2 vs 53,7 vs 52,7 respectivamente, p <0,05).Verificámos uma diminuição da incidência das amiloidoses AA com aumento das AL, com inversão do predomínio das AA em relação as AL a partir de 1995. Em termos morfológicos, a maioria das biópsias caracteriza-se por deposição marcada de amilóide no glomérulo (30% com +++) e nos vasos (40% com +++), com escassa deposição a nível intersticial cortical (60% sem depósitos) e medular (50% sem depósitos). Estudámos as possíveis relações entre manifestações clínicas e morfologia renal. Verificámos uma correlação positiva entre creatinina e grau de fibrose e/ou grau de deposição intersticial. Não encontrámos relação entre proteinúria e grau/local de deposição de amilóide. Conclusões: Actualmente, em Portugal, predomina a amiloidose AL, que surge em doentes mais idosos e se manifesta mais frequentemente por sindrome nefrótico. A função renal a data da biópsia correlaciona-se com o grau de fibrose tubulo-interstical renal.

Clinical history - A 4-year-old boy, born prematurely at 29 weeks (twin pregnancy), with periventricular leukomalacia and epilepsy underwent brain MRI. Neurological examination showed severe developmental retardation with axial hypotonia, spastic tetraparesis and convergent strabismus. Imaging findings - Cranial MRI revealed typical aspects of partial rhombencephalosynapsis with vermian hypoplasia, midline fusion of the cerebellar hemispheres and transversely oriented folia and fissures. There was also mild dilatation and dysmorphism of the ventricular system, the septum pellucidum was absent, the hippocampi were malrotated and had vertical orientation and additional finding of associated periventricular cystic leukomalacia. Discussion - Rhombencephalosynapsis (RS) is a rare congenital defect of the cerebellum classically characterised by vermian agenesis or hypogenesis, fusion of the hemispheres, and closely apposed or fused dentate nuclei. It is now considered to result from an absence of division of the cerebellar hemispheres, following an insult between the 28th and 44th day of gestation (i.e., before the formation of the vermis). Other features have also been described such as fusion of the thalami and cerebral peduncles, malrotated hippocampi, corpus callosum agenesis, hypoplastic chiasm, absence of the septum pellucidum, ventriculomegaly, agenesis of the posterior lobe of the pituitary and cortical malformations. Musculoskeletal, cardiovascular, urinary tract, and respiratory abnormalities have been reported. Typical symptoms consist of swallowing difficulties, delayed motor acquisitions, muscular hypotonia, spastic quadriparesis, cerebellar signs including dysarthria, gait ataxia, abnormal eye movements, and seizures and hydrocephalus. The major MRI signs consist of fused cerebellar hemispheres, with absent or hypoplastic vermis, narrow diamond-shaped fourth ventricle and fused dentate nuclei. In a minority of cases, partial RS has been identified by MRI, demonstrating the presence of the nodulus and the anterior vermis and absence of part of the posterior vermis with only partial fusion of the hemispheres in the inferior part. Other cerebellar malformations involving vermian agenesis or hypoplasia include the Dandy–Walker continuum, Joubert syndrome, tectocerebellar dysraphy or pontocerebellar hypoplasias, and are now easily distinguished from RS by both brain MRI and morphology.

Antiphospholipid syndrome nephropathy and lupus nephritis have similar clinical and laboratory manifestations and achieving the accuracy of diagnosis required for correct treatment frequently necessitates a kidney biopsy. We report the case of a 29-year-old woman referred to the nephrology service for de novo hypertension, decline of renal function and proteinuria. She had had systemic lupus erythematosus and antiphospholipid syndrome since the age of 21 and was taking oral anticoagulation. Two weeks later, after treatment of hypertension and achievement of adequate coagulation parameters, a percutaneous renal biopsy was performed. The biopsy revealed chronic lesions of focal cortical atrophy, arterial fibrous intimal hyperplasia and arterial thromboses, which are typical features of antiphospholipid syndrome nephropathy. We describe the clinical manifestations and histopathology of antiphospholipid syndrome nephropathy and review the literature on renal biopsy in patients receiving anticoagulation.

Objective: Localizing epileptic foci in posterior brain epilepsy remains a difficult exercise in surgery for epilepsy evaluation. Neither clinical manifestations, neurological, EEG nor neuropsychological evaluations provide strong information about the area of onset, and fast spread of paroxysms often produces mixed features of occipital, temporal and parietal symptoms. We investigated the usefulness of the N170 event-related potential to map epileptic activity in these patients. Methods: A group of seven patients with symptomatic posterior cortex epilepsy were submitted to a high-resolution EEG (78 electrodes), with recordings of interictal spikes and face-evoked N170. Generators of spikes and N170 were localized by source analysis. Range of normal N170 asymmetry was determined in 30 healthy volunteers. Results: In 3 out of 7 patients the N170 inter-hemispheric asymmetry was outside control values. Those were the patients whose spike sources were nearest (within 3 cm) to the fusiform gyrus, while foci further away did not affect the N170 ratio. Conclusions: N170 event-related potential provides useful information about focal cortical dysfunction produced by epileptic foci located in the close neighborhood of the fusiform gyrus, but are unaffected by foci further away. Significance: The N170 evoked by faces can improve the epileptic foci localization in posterior brain epilepsy.

Nas últimas décadas, houve um declínio marcado da incidência global da insuficiência renal aguda (IRA) associada a causas obstétricas nos países desenvolvidos, devendo-se à melhoria significativa dos cuidados perinatais e à diminuição do número de abortos sépticos. Não obstante, a IRA ainda ocorre, tendo uma mortalidade elevada durante o período agudo, não sendo rara a progressão para insuficiência renal terminal. A IRA durante a gravidez pode ter várias causas, as quais são diferentes na primeira e na segunda metades da gravidez. Numa fase inicial da gravidez os problemas mais comuns são a doença prérenal devido à hiperémese gravídica e a necrose tubular aguda resultante do aborto séptico. Nos estadios mais tardios, pode ser causada por necrose tubular aguda ou necrose cortical renal, as quais podem ser originadas no contexto de várias situações, tais como a pré-eclampsia e/ou eclampsia, o descolamento prematuro da placenta, a placenta prévia, a hemorragia pós parto, o fígado gordo da gravidez e a embolia do líquido amniótico. Pode ainda ser causada pelo síndrome hemolítico-urémico pós-parto e a obstrução do aparelho urinário, entre outras. Na abordagem terapêutica da IRA na gravidez, é preconizada a colaboração estreita entre obstetra e nefrologista, devendo o tratamento ser individualizado.

Baseados num caso de associação de abcessos renal, perirrenal e de quisto renal infectado, ocorrendo após biopsia prostática, os autores fazem revisão bibliográfica dos abcessos renais, classificados segundo características anatómicas perinéfrico, cortical e cortico- -medular (nefrite bacteriana aguda focal/multifocal, pielonefrite enfisematosa e pielonefrite xantogranulomatosa). Os abcessos renais são mais frequentes em diabéticos, menos vezes em alcoólicos, desnutridos e imunodeprimidos, estando associados geralmente a pielonefrites recorrentes, refluxo vesicoureteral, litíase urinária, ou resultam de êmbolos sépticos hematogéneos originados em focos extra-renais de infecção. A Escherichia coli é responsável pela maioria dos casos, e menos vezes a Klebsiella Proteus, Enterobacter, Pseudomonas, e Estafilococo aureus. Descrevem-se a expressão clínica, complicações evolutivas e diagnóstico radiológico, enfatizando-se as infecções acompanhadas de formação de gás. Referem-se opções terapêuticas médico-cirúrgicas, cujas indicações dependem do diagnóstico precoce, complicações e factores predisponentes.

Objective: The epilepsy associated with the hypothalamic hamartomas constitutes a syndrome with peculiar seizures, usually refractory to medical therapy, mild cognitive delay, behavioural problems and multifocal spike activity in the scalp electroencephalogram (EEG). The cortical origin of spikes has been widely assumed but not specifically demonstrated. Methods: We present results of a source analysis of interictal spikes from 4 patients (age 2–25 years) with epilepsy and hypothalamic hamartoma, using EEG scalp recordings (32 electrodes) and realistic boundary element models constructed from volumetric magnetic resonance imaging (MRIs). Multifocal spike activity was the most common finding, distributed mainly over the frontal and temporal lobes. A spike classification based on scalp topography was done and averaging within each class performed to improve the signal to noise ratio. Single moving dipole models were used, as well as the Rap-MUSIC algorithm. Results: All spikes with good signal to noise ratio were best explained by initial deep sources in the neighbourhood of the hamartoma, with late sources located in the cortex. Not a single patient could have his spike activity explained by a combination of cortical sources. Conclusions: Overall, the results demonstrate a consistent origin of spike activity in the subcortical region in the neighbourhood of the hamartoma, with late spread to cortical areas.

Objective: Gelastic seizures are a frequent and well established manifestation of the epilepsy associated with hypothalamic hamartomas. The scalp EEG recordings very seldom demonstrate clear spike activity and the information about the ictal epilepsy dynamics is limited. In this work, we try to isolate epileptic rhythms in gelastic seizures and study their generators. Methods: We extracted rhythmic activity from EEG scalp recordings of gelastic seizures using decomposition in independent components (ICA) in three patients, two with hypothalamic hamartomas and one with no hypothalamic lesion. Time analysis of these rhythms and inverse source analysis was done to recover their foci of origin and temporal dynamics. Results: In the two patients with hypothalamic hamartomas consistent ictal delta (2–3 Hz) rhythms were present, with subcortical generators in both and a superficial one in a single patient. The latter pattern was observed in the patient with no hypothalamic hamartoma visible in MRI. The deep generators activated earlier than the superficial ones, suggesting a consistent sub-cortical origin of the rhythmical activity. Conclusions: Our data is compatible with early and brief epileptic generators in deep sub-cortical regions and more superficial ones activating later. Significance: Gelastic seizures express rhythms on scalp EEG compatible with epileptic activity originating in sub-cortical generators and secondarily involving cortical ones.

Objective: The Panayiotopoulos type of idiopathic occipital epilepsy has peculiar and easily recognizable ictal symptoms, which are associated with complex and variable spike activity over the posterior scalp areas. These characteristics of spikes have prevented localization of the particular brain regions originating clinical manifestations. We studied spike activity in this epilepsy to determine their brain generators. Methods: The EEG of 5 patients (ages 7–9) was recorded, spikes were submitted to blind decomposition in independent components (ICs) and those to source analysis (sLORETA), revealing the spike generators. Coherence analysis evaluated the dynamics of the components. Results: Several ICs were recovered for posterior spikes in contrast to central spikes which originated a single one. Coherence analysis supports a model with epileptic activity originating near lateral occipital area and spreading to cortical temporal or parietal areas. Conclusions: Posterior spikes demonstrate rapid spread of epileptic activity to nearby lobes, starting in the lateral occipital area. In contrast, central spikes remain localized in the rolandic fissure. Significance: Rapid spread of posterior epileptic activity in the Panayitopoulos type of occipital lobe epilepsy is responsible for the variable and poorly localized spike EEG. The lateral occipital cortex is the primary generator of the epileptic activity.

Objective: The epilepsies associated with the tuberous sclerosis complex (TSC) are very often refractory to medical therapy. Surgery for epilepsy is an effective alternative when the critical link between the localization of seizure onset in the scalp and a particular cortical tuber can be established. In this study we perform analysis of ictal and interictal EEG to improve such link. Methods: The ictal and interictal recordings of four patients with TSC undergoing surgery for epilepsy were submitted to independent component analysis (ICA), followed by source analysis, using the sLORETA algorithm. The localizations obtained for the ictal EEG and for the average interictal spikes were compared. Results: The ICA of ictal EEG produced consistent results in different events, and there was good agreement with the tubers that were successfully removed in three of the four patients (one patient refused surgery). In some patients there was a large discrepancy between the localization of ictal and interictal sources. The interictal activity produced more widespread source localizations. Conclusions: The use of ICA of ictal EEG followed by the use of source analysis methods in four cases of epilepsy and TSC was able to localize the epileptic generators very near the lesions successfully removed in surgery for epilepsy. Significance: The ICA of ictal EEG events may be a useful add-on to the tools used to establish the connection between epileptic scalp activity and the cortical tubers originating it, in patients with TSC considered for surgery of epilepsy.

Clinically childhood occipital lobe epilepsy (OLE) manifests itself with distinct syndromes. The traditional EEG recordings have not been able to overcome the difficulty in correlating the ictal clinical symptoms to the onset in particular areas of the occipital lobes. To understand these syndromes it is important to map with more precision the epileptogenic cortical regions in OLE. Experimentally, we studied three idiopathic childhood OLE patients with EEG source analysis and with the simultaneous acquisition of EEG and fMRI, to map the BOLD effect associated with EEG spikes. The spatial overlap between the EEG and BOLD results was not very good, but the fMRI suggested localizations more consistent with the ictal clinical manifestations of each type of epileptic syndrome. Since our first results show that by associating the BOLD effect with interictal spikes the epileptogenic areas are mapped to localizations different from those calculated from EEG sources and that by using different EEG/fMRI processing methods our results differ to some extent, it is very important to compare the different methods of processing the localization of activation and develop a good methodology for obtaining co-registration maps of high resolution EEG with BOLD localizations.

In the context of focal epilepsy, the simultaneous combination of electroencephalography (EEG) and functional magnetic resonance imaging (fMRI) holds a great promise as a technique by which the hemodynamic correlates of interictal spikes detected on scalp EEG can be identified. The fact that traditional EEG recordings have not been able to overcome the difficulty in correlating the ictal clinical symptoms to the onset in particular areas of the lobes, brings the need of mapping with more precision the epileptogenic cortical regions. On the other hand, fMRI suggested localizations more consistent with the ictal clinical manifestations detected. This study was developed in order to improve the knowledge about the way parameters involved in the physical and mathematical data, produced by the EEG/fMRI technique processing, would influence the final results. The evaluation of the accuracy was made by comparing the BOLD results with: the high resolution EEG maps; the malformative lesions detected in the T1 weighted MR images; and the anatomical localizations of the diagnosed symptomatology of each studied patient. The optimization of the set of parameters used, will provide an important contribution to the diagnosis of epileptogenic focuses, in patients included on an epilepsy surgery evaluation program. The results obtained allowed us to conclude that: by associating the BOLD effect with interictal spikes, the epileptogenic areas are mapped to localizations different from those obtained by the EEG maps representing the electrical potential distribution across the scalp (EEG); there is an important and solid bond between the variation of particular parameters (manipulated during the fMRI data processing) and the optimization of the final results, from which smoothing, deleted volumes, HRF (used to convolve with the activation design), and the shape of the Gamma function can be certainly emphasized.

A 34-year-old woman with no known medical history was evaluated for multiple painful brown nodules and papules on the anterior aspect of the trunk. She mentioned a history of similar cutaneous findings on her mother. Biopsies of three lesions revealed piloleiomyomata. Renal and adrenal ultrasound revealed an isolated simple cortical cyst, and pelvic and endovaginal ultrasound revealed two uterine myomata. The clinical diagnosis of hereditary leiomyomatosis and renal cell cancer was corroborated by the identification of a heterozygous variant on exon 5 of the fumarate hydratase gene (c.578C>T p.T193I). Identification of the tumor piloleiomyoma should alert the dermatologist to this rare genodermatosis, which is associated with an increased risk of renal cell tumors, demanding multidisciplinary follow-up, and personal and family counseling.
