Homem de 75 anos, com história de perda ponderal (>10% peso inicial) nos sete meses anteriores, presença de massa abdominal no flanco esquerdo com dor tipo moinha, associadas a queixas de anorexia não selectiva e enfartamento pós-prandial, com três meses de evolução. Antecedentes de adenocarcinoma do estômago e adenocarcinoma do cólon. Submetido a intervenção cirúrgica, tendo-se ressecado peça volumosa, constituída por células malignas, com componente quístico e componente sólido (tecido fibroso, cartilagem e osso maduro). O exame da anatomia patológica concluiu tratar-se de um Osteossarcoma Extraesquelético Retroperitoneal Telangiectásico. Clínica e laboratorialmente sem alterações na reavaliação aos doze meses de pós-operatório. Os exames complementares de diagnóstico, nesta data, não revelaram recidiva local ou metastização.

INTRODUCTION: Pancreatic involvement by plasma cell neoplasms is an extremely rare event, with only 50 cases described in the literature. They can present as a primary solitary extramedullary plasmacytoma or plasmacytoma secondary to a plasma cell myeloma. Clinical manifestations are due to the presence of a pancreatic mass usually in the pancreas head, which causes extra-biliary obstruction and abdominal pain. METHODS: Abdominal imaging including CT scan or endoscopic ultrasound with fine-needle aspiration tissue sampling is essential for the initial diagnostic procedure. However, immunohistochemical analysis of the biopsy specimen or flow cytometry of the aspirated material is crucial to prove the monoclonality and the final diagnosis of a plasma cell neoplasm. DISCUSSION: Management of these situations include radiotherapy, chemotherapy, surgery or combined therapy. Novel medications including the immunomodulatory drugs or the proteasome inhibitors followed by consolidation with intensive chemotherapy and haematopoietic stem cell transplantation are nowadays used as upfront treatment in the cases associated to a plasma cell myeloma. CONCLUSION: Despite the rarity, plasma cell neoplasms should be considered in the differential diagnosis of obstructive jaundice and pancreatic neoplasms since they are potentially treatable situations.

Apresentam-se seis casos de síndrome da veia cava superior causado por neoplasia do pulmão e que foram tratados por endopróteses colocadas por via percutânea. Descreve-se a técnica e avaliam-se os resultados. Num caso ocorreu trombose aguda da endoprótese que foi tratada por uroquinase. Não se registaram outras complicações. Um doente faleceu 1 mês depois por progressão da doença. Os restantes mantinham-se sem queixas de síndrome da veia cava superior durante mais do que 6 meses. Conclui-se que a colocação de endopróteses é uma técnica pouco invasiva, eficaz e com melhoria imediata no síndrome da veia cava superior.

GOALS OF WORK: Recent literature has indicated the need for rapid evaluation of psychosocial issues secondary to cancer. Because of the problems of routine use of psychometric instruments, short instruments such as visual analogue scales or one-item 0-10 scales have been developed as valid assessment alternatives. PATIENTS AND METHODS: A study was conducted to examine the role of two 0-10 scales in measuring emotional stress (distress thermometer, DT) and depressed mood (mood thermometer, MT), respectively, in a multicenter study carried out in southern European countries (Italy, Portugal, Spain, and Switzerland). A convenience sample of 312 cancer outpatients completed the DT and MT and the Hospital Anxiety Depression Scale (HADS). MAIN RESULTS: DT was more significantly associated HADS anxiety than HADS depression while MT was related both to HADS anxiety and depression. The correlation of MT with HADS was higher than DT. A cutoff point >4 on the DT maximized sensitivity (65%) and specificity (79%) for general psychosocial morbidity while a cutoff >5 identified more severe "caseness" (sensitivity=70%; specificity=73%). On the MT, sensitivity and specificity for general psychosocial morbidity were 85% and 72% by using the cutoff score >3. A score >4 on the MT was associated with a sensitivity of 78% and a specificity of 77% in detecting more severe caseness. CONCLUSIONS: Two simple instruments, the DT and the MT, were found to have acceptable levels of sensitivity and specificity in detecting psychosocial morbidity. Compared to the HADS, however, the mood MT performed better than the DT.

OBJECTIVE: In the last decade, some attention has been given to spirituality and faith and their role in cancer patients' coping. Few data are available about spirituality among cancer patients in Southern European countries, which have a big tradition of spirituality, namely, the Catholic religion. As part of a more general investigation (Southern European Psycho-Oncology Study--SEPOS), the aim of this study was to examine the effect of spirituality in molding psychosocial implications in Southern European cancer patients. METHOD: A convenience sample of 323 outpatients with a diagnosis of cancer between 6 to 18 months, a good performance status (Karnofsky Performance Status > 80), and no cognitive deficits or central nervous system (CNS) involvement by disease were approached in university and affiliated cancer centers in Italy, Spain, Portugal, and Switzerland (Italian speaking area). Each patient was evaluated for spirituality (Visual Analog Scale 0-10), psychological morbidity (Hospital Anxiety and Depression Scale--HADS), coping strategies (Mini-Mental Adjustment to Cancer--Mini-MAC) and concerns about illness (Cancer Worries Inventory--CWI). RESULTS. The majority of patients (79.3%) referred to being supported by their spirituality/faith throughout their illness. Significant differences were found between the spirituality and non-spirituality groups (p ≤ 0.01) in terms of education, coping styles, and psychological morbidity. Spirituality was significantly correlated with fighting spirit (r = -0.27), fatalism (r = 0.50), and avoidance (r = 0.23) coping styles and negatively correlated with education (r = -0.25), depression (r = -0.22) and HAD total (r = -0.17). SIGNIFICANCE OF RESULTS: Spirituality is frequent among Southern European cancer patients with lower education and seems to play some protective role towards psychological morbidity, specifically depression. Further studies should examine this trend in Southern European cancer patients.

INTRODUCTION: Carotid body tumours (CBT) are neoplasms that develop from paragangionic cells of this structure. They are rare, with an estimated incidence of 1:30000 and can be associated with other neuro-endocrine neoplasia. The authors report their experience in the management of the disease, in the last 10 years. MATERIAL AND METHODS: Eight patients (with eight tumours) were treated, all submitted to tumour resection. 75% were female and the mean age was 56 years. We report a 12,5% incidence of neurological sequelae from surgery, and no mortality. In the follow-up (which varied between 1 and 10 years), no local or contralateral recurrence or metastasis were registered. Also, we did not found family cases of this disease. CONCLUSIONS: The authors noticed an unusually high proportion of female patients. The tumour resection was curative in all patients, with a rate of neurological complications inferior to that reported in other published series. These neurological sequelae were reported in patients with large tumours, thus reinforcing the outmost importance of an early diagnosis. Pre-operative selective embolization of these tumours can be helpful in the resection of large tumours, allowing a potential reduction in neurological complications.

Based on a case of gastric antral vascular ectasia (watermelon stomach) that was associated with hemorrhagic pericarditis, small cell lung carcinoma with mediastinal lymph node metastases and a synchronous squamous cell carcinoma of the base of the tongue, the authors made a review of the clinical, endoscopic and histopathological aspects of this type of gastropathy, and its association with other diseases, and of the results of its endoscopic therapy. The causes of hemorrhagic pericarditis are considered, emphasizing the necessity to know if the effusion has a malignant etiology. To the best of our knowledge the association of watermelon stomach to small cell lung carcinoma and squamous cell carcinoma of the base of the tongue has not yet been described. Extensive metastases to mediastal lymph nodes are common to small cell lung carcinoma.

The simultaneous presence of infectious organisms within cutaneous lesions of Kaposi sarcoma in persons with AIDS has been demonstrated. We describe a patient with concurrent leprosy and Kaposi sarcoma presenting as an immune reconstitution inflammatory syndrome in the setting of AIDS.

Objectivos: Numerosas séries têm sido publicadas documentando a experiência em nefrectomias por via transperitoneal e retroperitoneal. Este artigo pretende documentar as primeiras 10 nefrectomias e nefroureterectomias laparoscópicas via transperitoneal realizadas no Hospital D. Estefânia. Material e Métodos: Revisão retrospectiva de cinco nefrectomias e cinco nefroureterectomias laparoscópicas realizadas no período de Fevereiro a Setembro de 2010 em crianças com idades compreendidas entre os 2 e os 18 anos. Todos os doentes foram referenciados por complicações de rins não funcionantes. Os procedimentos, via transperitoneal, foram unilaterais, com recurso a 3 portas. Foi igualmente efectuada a análise da duração da cirurgia, taxa de conversão, tempo de internamento e complicações associadas à técnica. Resultados: Todos os rins foram removidos com sucesso por via laparoscópica, sem necessidade de conversão em nenhum dos casos. O tempo operatório médio foi de 103 minutos. Cinco doentes tiveram alta ao 2º dia de pós-operatório, três doentes ao 3º dia de pós-operatório e dois doentes, pela complexidade médico-cirúrgica inerente tiveram internamentos mais prolongados. Não se verificaram complicaçõesno per- ou pós-operatório, com retoma precoce da actividade normal em todos os doentes. Conclusão: A nefrectomia laparoscópica na criança parece ser uma alternativa viável e segura à cirurgia aberta. Consideramos necessário a aquisição de mais experiência para reduzir os tempos operatórios e definir a sua verdadeira eficácia. A experiência adquirida com outros procedimentos laparoscópicos transperitoneais foi determinante na opção por esta via (versus retroperitoneal).

BACKGROUND: Although hopelessness has been studied in cancer, no data are available in non-English-speaking countries. OBJECTIVE: The authors sought to amass data from Southern European countries (Italy, Portugal, Spain, and Switzerland) in order to fill this void. METHOD: A group of 312 cancer patients completed the Mini-MAC Hopelessness subscale, the Hospital Anxiety and Depression Scale (HADS), the Cancer Worry Inventory (CWI), and a six-item Visual Analog scale (VAS) to measure intensity of physical symptoms, general well-being, difficulty in coping with cancer, intensity of social support from close relationships, leisure activity, and support from religious beliefs. RESULTS: Regression analysis indicated that HADS-Depression, VAS Maladaptive Coping and Well-Being, and the CWI explained 42% of the variance. CONCLUSION: Hopelessness in cancer patients seems not exclusively to correspond to depression, but is related to various other psychosocial factors, such as maladaptive coping, as well.

BACKGROUND: The detection of psychosocial distress is a significant communication problem in Southern Europe and other countries. Work in this area is hampered by a lack of data. Because not much is known about training aimed at improving the recognition of psychosocial disorders in cancer patients, we developed a basic course model for medical oncology professionals. METHODS: A specific educational and experiential model (12 hours divided into 2 modules) involving formal teaching (ie, journal articles, large-group presentations), practice in small groups (ie, small-group exercises and role playing), and discussion in large groups was developed with the aim of improving the ability of oncologists to detect emotional disturbances in cancer patients (ie, depression, anxiety, and adjustment disorders). RESULTS: A total of 30 oncologists from 3 Southern European countries (Italy, Portugal, and Spain) participated in the workshop. The training course was well accepted by most participants who expressed general satisfaction and a positive subjective perception of the utility of the course for clinical practice. Of the total participants, 28 physicians (93.3%) thought that had they been exposed to this material sooner, they would have incorporated the techniques received in the workshop into their practices; 2 participants stated they would likely have done so. Half of the doctors (n = 15) believed that their clinical communication techniques were improved by participating in the workshop, and the remaining half thought that their abilities to communicate with cancer patients had improved. CONCLUSIONS: This model is a feasible approach for oncologists and is easily applicable to various oncology settings. Further studies will demonstrate the effectiveness of this method for improving oncologists skills in recognizing emotional disorders in their patients with cancer.

A Neurofibromatose tipo a (NF1) é uma doença multissistémica de carácter progressivo. Tem uma expressão fenotípica muito variada, sem factores preditivos de gravidade. Objectivos: caracterizar a população pediátrica com NF! seguida na consulta de Neurologia Pediátrica no Hospital de Dona Estefânia (HDE) e os principais motivos de referência; definir um protocolo de seguimento; definir critérios de referenciação para o neuropediatra. Material e Métodos: realizou-se um estudo retrospectivo e longitudinal através da consulta dos processos clínicos. Incluiu todas as crianças com critérios de diagnóstico de NF1 de acordo com National Institutes of Health Consensus Development Conference criteria de 1988. Os dados foram comparados com outras séries recentes. Resultados: São 28 as crianças com critérios de diagnóstico de NF1, 15 (53%) do sexo feminino e 17 (60,7%) com anetecendentes familiares de NF1. A média da idade do diagnóstico é de 3,6 +-3,3 anos (3m-14a) e a idade actual é de 8 +-7,05 anos (11m-15a). O principal motivo de referenciação à consulta de Neurologia foi um pedido de orientação face à presença de manchas "café cpm leite", que estiveram presentes em 100% dos casos. a epilepsia ocorreu em 4 (14,2%)crianças, com crises parciais complexas. Surgiram complicações graves em 7 crianças (25%): tumor do SNC (3); linfoma (1); pseudoartrose da tíbia (1); epilepsia refractária (1); neurofibroma retroperitoneal (1).

Adnexal skin tumours are rare conditions, and often clinically indistinguishable from other cutaneous neoplasms. Porocarcinoma, a sweat gland malignant tumour, is more commonly found on extremities. Few reports in other anatomic locations can be found in the literature, and those arising on the scalp are even scarcer. The authors report the case of an 84-year-old diabetic man, with a tumour on the left parietal region for 1 year, which histopathological features were consistent with porocarcinoma. The importance of histopathologic diagnosis is hereby emphasized by the more aggressive behaviour of this tumour, therefore requiring clinical actuation accordingly.
