Background: Childhood asthma represents an increasing health problem and is the leading cause of hospital admission and absenteeism in children with chronic disease. It also compromises quality of life, eventually contributing to disturbances in self-concept. Self-concept is a recent and global perspective of “the self” and relates to skills, self-image and self-esteem. Little information is available on this topic and there are no data from Portuguese countryside towns. Objective: The aim of this study was to determine the prevalence of asthma among all school children in the 5th and 6th grades in a Portuguese countryside town and to establish its possible correlation with absenteeism and self-concept. Methods: In April 2002, two questionnaires were administered in the presence of the researcher to a group of 950 children attending different schools. The children completed the internationally renowned questionnaires: ISAAC and the Self-Concept Scale by Susan Harter. Results: Our sample (n = 818) had a mean age of 11 years (10-15 years) and a male-to-female ratio of 1/1. The cumulative prevalence of asthma was11.9% and that of active asthma was 8.8 %; 63.9% of asthmatics were male and 36.1 % were female. The mean age of asthmatics was 11.34 years and 74 % had active symptoms. Comparison of this group of 97 asthmatic children with the remaining children revealed a statistically significant correlation between the presence of asthma and school absenteeism (global: p = 0.04; gymnastics: 0.05). Regarding the Self-Concept Scale a statistically significant association was found between the presence of asthma and school achievement (p = 0.027), physical appearance (p = 0.015), behavior (p < 0.000) and self-esteem (p < 0.000). No statistically significant correlations were found in social acceptance (p = 0.289) or athletic competence (p = 0.085). Asthmatic boys had higher self-concept scores than girls, except in the domain of behavior. Conclusions: Twelve percent of the population studied was asthmatic. In asthmatic children, absenteeism was higher and self-concept was lower for almost all domains, except social acceptance and athletic achievement, probably due to overprotection.

Objectivo e desenho: estudo prospectivo de avaliação da possibilidade de aplicação e utilidade clínica da ecocardiografia transtorácica (ETT) na avaliação da hpotensão numa Unidade de Cuidados Intensivos Polivalente (UCIP). Local: UCIP de 16 camas. Material e métodos: Incluídos doentes com hipotensão (pressão arterial sistólica 90 mmHg ou média (PAM) < 60 mmHg, que não respondeu à administração de soros no espaço de 30 minutos). Os objectivos do ETT foram: excluir cardiopatia estrutural grave, avaliação de outras alterações cardíacas (alterações das dimensões das cavidades e função ventricular esquerda), análise da veia cava inferior (VCI) e determinação do índex cardíaco (IC). Resultados: de um total de 208 doentes foram incluídos 198 (4,5% de exames impossíveis), com média etária de 63,4 +/- 16,2 anos, 129 do sexo masculino, APACHE II 30,1 +/- 9,9, SAPS II 68,8 +/- 20,5, SOFA 11,6 +/- 3,8 MODS 10,9 +/- 3,9. Observou-se uma mortalidade de 51% (n=101), e 168 (85,2%) doentes estavam ventilados. Oitenta e oito (44,4%) doentes apresentaram alterações cardíacas, dos quais 28 (14%) classificadas como graves: três valvulopatias aórticas graves, quatro endocardites, nove miocardiopatias dilatadas, dois tamponamentos (18 doentes com alterações graves insuspeitas, 9%), seis enfartes agudos do miocárdio, quatro alterações da cinética segmentar. Estes doentes apresentaram uma mortalidade e índices de gravidade mais elevados (p <0,001). Em relação ao IC, 157 doentes apresentaram um valor normal ou elevado, os quais apresentaram todos um valor de resistências ventriculares periféricas baixo. Por análise de regressão logística, verificou-se uma relação entre o índex da VCI e os dias de internamento (p = 0,05) e entre o IC, índex da VCI e a mortalidade (p =0,008 e 0,041 respectivamente). Conclusões: Observou-se uma elevada prevalência de patologia cardíaca entre os doentes admitidos numa UCIP com hipotensão (n =88, 44,4%), dos quais 14% consideradas graves. Estes doentes tiveram maior mortalidade e índices de gravidade mais elevados. A análise conjunta do IC e da VCI pode ser útil na definição do padrão hemodinâmico do doente hipotenso e certos parâmetros ecocardiográficos, em especial o índex da VCI, podem ser úteis no prognóstico destes doentes.

Purpose: To evaluate the safety and efficiency of sclerotherapy in ovarian vein varicocele. Study population: During 6 years, 36 women with the clinical diagnosis of ovarian vein varicocele were evaluated. Methods: The diagnosis was confirmed by flebography of the ovarian vein in 35 of the patients. In these patients sclerotherapy of the ovarian vein was performed with success by selective retrograde catheterization of the ovarian vein by femoral approach in 31 patients and by brachial approach in 4 patients. Four to eight ml of polidocanol l3% was used in each vein. Results: There was clinical improvement with complete resolution of all symptoms in 29 patients(82.9%) and partial symptomatic relief in 6 (17.1%). Long term results, evaluated between 1 and 6 years (mean 37.3 months), showed complete resolution of symptoms in 27 (77.1%) and recurrence in 8 (22.9%). Four patients with recurrence, improved following repeated sclerotherapy. Thus, there was long term improvement in 31 patients (88.6%). Conclusion: Sclerotherapy of ovarian vein appears to be a safe and efficient treatment of ovarian vein varicocele.

O bloqueio contínuo de nervos periféricos (BCNP), descrito inicialmente em 1946 (1), consiste na colocação e utilização de um cateter percutâneo adjacente a um ou vários nervos periféricos para administração de anestésico local. A utilização de cateteres perineurais permite prologar a analgesia associada a técnica single shot, que dura, habitualmente, 24 horas. Apesar de bem estudado para analgesia no pos-operatório, o BCNP tem actualmente várias indicações descritas como tratamento de soluços(2), de vasospasmo associado a Doença de Raynaud (3), simpaticectomia e vasodilatação após cirurgia vascular (4), reimplantação de membros amputados(5), analgesia em trauma(6) e tratamento de dor cronica (7). Apesar de estar provado que no tratamento da dor pós-operatória o uso de cateteres perineurais permite uma analgesia mais eficaz comparativamente a analgesia com opióides sistémicos (reduzindo assim a incidência de complicações associadas ao uso destes) (8) e que melhora, a curto prazo, outcomes funcionais após cirurgia da extremidade, não esta evidenciado o beneficio desta técnica regional a longo prazo (9,10,11). Considerando a eficácia da técnica loco-regional para o tratamento da dor neuropática associada a doença arterial periférica (12,13) e a segurança associada ao bloqueio de nervo periférico (8,9,10,11), relata-se um caso de utilização desta técnica num doente para tratamento da agudização de dor crónica secundária a um agravamento da isquémia do membro inferior.

OBJECTIVE: A familial predisposition to abdominal aortic aneurysms (AAAs) is present in approximately one-fifth of patients. Nevertheless, the clinical implications of a positive family history are not known. We investigated the risk of aneurysm-related complications after endovascular aneurysm repair (EVAR) for patients with and without a positive family history of AAA. METHODS: Patients treated with EVAR for intact AAAs in the Erasmus University Medical Center between 2000 and 2012 were included in the study. Family history was obtained by written questionnaire. Familial AAA (fAAA) was defined as patients having at least one first-degree relative affected with aortic aneurysm. The remaining patients were considered sporadic AAA. Cardiovascular risk factors, aneurysm morphology (aneurysm neck, aneurysm sac, and iliac measurements), and follow-up were obtained prospectively. The primary end point was complications after EVAR, a composite of endoleaks, need for secondary interventions, aneurysm sac growth, acute limb ischemia, and postimplantation rupture. Secondary end points were specific components of the primary end point (presence of endoleak, need for secondary intervention, and aneurysm sac growth), aneurysm neck growth, and overall survival. Kaplan-Meier estimates for the primary end point were calculated and compared using log-rank (Mantel-Cox) test of equality. A Cox-regression model was used to calculate the independent risk of complications associated with fAAA. RESULTS: A total of 255 patients were included in the study (88.6% men; age 72 ± 7 years, median follow-up 3.3 years; interquartile range, 2.2-6.1). A total of 51 patients (20.0%) were classified as fAAA. Patients with fAAA were younger (69 vs 72 years; P = .015) and were less likely to have ever smoked (58.8% vs 73.5%; P = .039). Preoperative aneurysm morphology was similar in both groups. Patients with fAAA had significantly more complications after EVAR (35.3% vs 19.1%; P = .013), with a twofold increased risk (adjusted hazard ratio, 2.1; 95% confidence interval, 1.2-3.7). Secondary interventions (39.2% vs 20.1%; P = .004) and aneurysm sac growth (20.8% vs 9.5%; P = .030) were the most important elements accounting for the difference. Furthermore, a trend toward more type I endoleaks during follow-up was observed (15.6% vs 7.4%; P = .063) and no difference in overall survival. CONCLUSIONS: The current study shows that patients with a familial form of AAA develop more aneurysm-related complications after EVAR, despite similar AAA morphology at baseline. These findings suggest that patients with fAAA form a specific subpopulation and create awareness for a possible increase in the risk of complications after EVAR.

AIM: The morbidity associated with osteoporosis and fractures in children and adolescents with spina bifida highlights the importance of osteoporosis prevention and treatment in these patients. The aim of this study was to examine the occurrence and pattern of bone fractures in paediatric patients with spina bifida. METHOD: We reviewed the data of all paediatric patients with spina bifida who were treated in our centre between 1999 and 2008. RESULTS: One hundred and thirteen patients were included in the study (63 females, 50 males; mean age 10y 8mo, SD 4y 10mo, range 6mo-18y). The motor levels were thoracic in six, upper lumbar in 22, lower lumbar in 42, and sacral in 43 patients. Of the 113 patients, 58 (51.3%) had shunted hydrocephalus. Thirty-six (31.8%) were non-ambulatory (wheelchair-dependent [unable to self-propel wheelchair] n=3, wheelchair-independent [able to self-propel wheelchair] n=33), 13 were partial ambulators, 61 were full ambulators, and three were below the age of walking. Forty-five fractures were reported in 25 patients. The distal femur was the most common fracture site. Statistical analyses showed that patients with higher levels of involvement and in wheelchairs had a significantly increased risk of having a [corrected] fracture (p<0.001). Spontaneous fractures were the principal mechanism of injury, and an association was identified between fracture mechanism, type of ambulation, and lesion level: the fractures of patients with higher levels of motor functioning and those in wheelchairs were mainly pathological (p=0.01). We identified an association between risk of a second fracture, higher motor level lesion, and non-ambulation. There was an increased risk of having a second fracture after a previous spontaneous fracture (p=0.004). INTERPRETATION: Data in this study indicate a high prevalence of fractures in patients with spina bifida.
