Introduction: Antibiotics are one of the most common prescribed drugs in the NICU; despite this, studies on its use are scarce. Aim: To assess antibiotics utilization ratio in a medical surgical NICU. Methods: Prospective, observational study. Daily registry of antibiotics given to newborn infants; two periods of two months, 2010; data collected every day after the second medical round. Variables: treated patients, days on antibiotics, treatment/patient days, number of courses, number of antibiotics. Antibiotics utilization ratio – ratio days on antibiotics/days at the NICU. Results: Patients enrolled - 113; admission days – 1722; length of stay - 15.2 days; 85 newborn infants were given antibiotics; days on antibiotics - 771; antibiotics utilization ratio – 44.8; 292 antibiotics were prescribed; 61.8% of patients were given more than two antibiotics and 15.3% had more than one course. The most frequents were gentamicin, cefotaxime, ampicillin, vancomycin and metronidazole. Conclusion: Antibiotics utilization ratio should be subject of audits and a quality criteria on NICUs evaluation.

Introduction. Fabry disease is a rare metabolic disorder caused by the genetic deficiency of the lysosomal hydrolase alpha-galactosidase A, located on chromosome X. Females with the defective gene are more than carriers and can develop a wide range of symptoms. Nevertheless, disease symptoms generally occur later and are less severe in women than in men. The enzyme deficiency manifests as a glycosphingolipidosis with progressive accumulation of glycosphingolipids and deposit of inclusion bodies in lysosomes giving a myelinlike appearance. Patients and Methods. Records of renal biopsies performed on adults from 1st January 2008 to 31st August 2011, were retrospectively examined at the Renal Pathology Laboratory. We retrieved biopsies diagnosed with Fabry disease and reviewed clinical and laboratory data and pathology findings. Results. Four female patients with a mean age of 49.3±4.5 (44-55) years were identified. The mean proteinuria was 0.75±0.3 g/24h (0.4-1.2) and estimated glomerular filtration rate (CKD EPI equation) was 71±15.7 ml/min/1.73m2 (48-83). Three patients experienced extra-renal organ involvement (cerebrovascular, cardiac, dermatologic, ophthalmologic and thyroid) with distinct severity degrees. Leukocyte α-GAL A activity was below normal range in the four cases but plasma and urinary enzymatic activity was normal. Light microscopy showed predominant vacuolisation of the podocyte cytoplasm and darkly staining granular inclusions on paraffin and plastic-embedded semi-thin sections. Electron microscopy showed in three patients the characteristic myelin-like inclusions in the podocyte cytoplasm and also focal podocyte foot process effacement. In one case the inclusions were also present in parietal glomerular cells, endothelial cells of peritubular capillary and arterioles. Conclusion. Clinical signs and symptoms are varied and can be severe among heterozygous females with Fabry disease. Intracellular accumulation of glycosphingolipids is a characteristic histologic finding of Fabry nephropathy. Since this disease is a potentially treatable condition, its early identification is imperative. We should consider it in the differential diagnosis of any patient presenting with proteinuria and/or chronic kidney disease, especially if there is a family history of kidney disease.

As infecções associadas à prestação de cuidados de saúde são ocorrências temíveis em unidades de cuidados intensivos neonatais (UCIN). A vigilância prospectiva dá aos neonatologistas um conhecimento da sua frequência, dos agentes mais comuns isolados na sua unidade e respectiva sensibilidade, influenciando de modo positivo as tentativas de controlar a sua frequência. Objectivo: Divulgar os resultados da vigilância epidemiológica da infecção de origem hospitalar numa UCIN. População e Métodos: Todos os recém-nascidos (RN) admitidos na unidade são registados, independentemente da idade gestacional, do peso ao nascer e da idade na admissão. Todos os episódios de infecção sistémica ocorridos desde a admissão até à alta são registados: sépsis, septicémia, pneumonia e meningite. É classificada como infecção de origem hospitalar a que surge 72h após a admissão. Resultados: No período de seis anos decorrido entre 2002 e 2007, foram tratados 1648 recém-nascidos a que corresponderam 27 862 dias de tratamento, 4 395 dias de ventilação e 10 537 dias de cateterismo venoso central (CVC); 261 RN eram de muito baixo peso (MBP) e 369 foram submetidos a grande cirurgia. Houve 242 episódios de infecção em 229 doentes. As taxas de infecção foram as seguintes (mediana e limites): RN infectados em % de tratados 13% (10,4%-17,4%); septicémia/100 doentes tratados-6,5 (3,8-11,3); septicémia/1000 dias doente-3,7 (3-6,2); septicémia/1000dias de CVC-4.1 (0,9-5,8); infecção por Staphylococcus coagulase negativa (SCN) em RN MBP - 9,8% (4%-12,5%); pneumonia relacionada com tubo traqueal/1000 dias de tubo traqueal – 3,2 (0-9,1). O agente mais frequentemente isolado foi o SCN. Conclusão: A vigilância epidemiológica é um instrumento muito útil para conhecer as taxas de infecção de uma unidade de saúde e a sua tendência, assim como os microorganismos e respectiva sensibilidade. Esse conhecimento constitui a base a partir da qual se pode tentar reduzir a sua prevalência.
