Objectivo: Avaliar a incidência de complicações relacionadas com o cateterismo venoso central e a existência de eventuais factores de risco para a sua ocorrência. Material e métodos: Estudo retrospectivo de 305 catéteres venosos centrais colocados pelos médicos da Unidade de Cuidados Intensivos Pediátricos (UCIP) do Hospital Dona Estefânia, durante 5 anos. As variáveis estudadas foram: idade, peso, instituição de ventilação mecânica, alteração da coagulação, local de inserção, número de lúmens e tempo de utilização do catéter, número e tipo de complicações. Na análise estatística foram utilizados o teste do Quiquadrado, o teste exacto de Fisher e o t-teste de Student, considerando-se haver diferenças estatisticamente significativas para valores de p< 0,05. Resultados: Foram submetidos a cateterismo 296 crianças, com idades compreendidas entre 0.08 e 16.00 anos (média=3,6 anos) e pesos entre 2 e 85 Kg (média=16,2 Kg). Os locais de inserção foram, por ordem decrescente de utilização, a veia subclávia (63,3%), a veia femural (29,8%) e veia jugular (6,9%). Os catéteres de duplo lúmen foram os mais utilizados (61,9%), seguidos dos de mono (32,5%) e de triplo lúmen (5,6%). Ocorreram 46 (15,1%) complicações, mas não se verificaram óbitos directamente relacionados com o cateterismo. A remoção do catéter foi electiva em 98 (32,1%) casos, por óbito em 97 (31,8%), por complicações em 39 (12,8%) e por outras causas em 71 (23,2%). O tempo de utilização dos catéteres foi em média de 7,6 dias. Os catéteres com e sem sépsis tiveram uma duração média de utilização de 6,9 dias e 17,0 dias, respectivamente. Conclusões: Este estudo revelou um padrão de complicações do cateterismo venoso central semelhante ao descrito por outros autores, sendo que nenhuma das variáveis analisadas se revelou, por si só, como factor de risco de complicações, excepto o tempo de utilização do catéter na ocorrência de sépsis.

INTRODUCTION: Coarctation of the aorta (CoA) is a stenosis usually located in the descending aorta. Treatment consists of surgical or percutaneous removal of the obstruction and presents excellent immediate results but significant residual problems often persist. OBJECTIVES: To describe the presentation, treatment and long-term evolution of a population of 100 unselected consecutive patients with isolated CoA in a single pediatric cardiology center. METHODS: This was a retrospective study of all patients with isolated CoA treated during4 the last 21 years (1987-2008). RESULTS: The patients (n=100, 68.3% male) were diagnosed at a median age of 94 days (1 day to 16 years). The clinical presentation differed between patients aged less or more than one year, the former presenting with heart failure and the latter being asymptomatic with evidence of hypertension (88 and 63%, respectively; p < 0.01). Treatment, a median of 8 days after diagnosis, was surgical in 79 cases (20 end-to-end anastomosis, 31 subclavian flap, 28 patch) and percutaneous in the remaining 21 (15 balloon angioplasty, 6 with stenting). The mean age of surgical patients was younger than in those treated percutaneously (3.4 vs. 7.5 years; p < 0.01). Immediate mortality was 2% and occurred in the surgical group. There was no late mortality, in a mean follow-up of 7.2 +/- 5.4 years. Recoarctation occurred in 8 patients (6 surgical, 2 percutaneous). There are 46 patients who currently have hypertension (19 at rest, 27 with effort), their median age at diagnosis being older than the others (23 vs. 995 days; p < 0.01). CONCLUSIONS: Isolated CoA has an excellent short-term prognosis but a significant incidence of long-term complications, and should thus no longer be seen as a simple obstruction in the descending aorta, but rather as a complex pathology that requires careful follow-up after treatment. Its potentially insidious presentation requires a high level of clinical suspicion, femoral pulse palpation during physical examination of newborns and older children being particularly important. Delay in treatment has an impact on late morbidity and mortality. Taking into account the data currently available on late and immediate results, the final choice of therapeutic technique depends on the patient's age, associated lesions and the experience of the medical-surgical team. Hypertension should be closely monitored in the follow-up of these patients, as well as its risk factors and complications.

GOALS OF WORK: Recent literature has indicated the need for rapid evaluation of psychosocial issues secondary to cancer. Because of the problems of routine use of psychometric instruments, short instruments such as visual analogue scales or one-item 0-10 scales have been developed as valid assessment alternatives. PATIENTS AND METHODS: A study was conducted to examine the role of two 0-10 scales in measuring emotional stress (distress thermometer, DT) and depressed mood (mood thermometer, MT), respectively, in a multicenter study carried out in southern European countries (Italy, Portugal, Spain, and Switzerland). A convenience sample of 312 cancer outpatients completed the DT and MT and the Hospital Anxiety Depression Scale (HADS). MAIN RESULTS: DT was more significantly associated HADS anxiety than HADS depression while MT was related both to HADS anxiety and depression. The correlation of MT with HADS was higher than DT. A cutoff point >4 on the DT maximized sensitivity (65%) and specificity (79%) for general psychosocial morbidity while a cutoff >5 identified more severe "caseness" (sensitivity=70%; specificity=73%). On the MT, sensitivity and specificity for general psychosocial morbidity were 85% and 72% by using the cutoff score >3. A score >4 on the MT was associated with a sensitivity of 78% and a specificity of 77% in detecting more severe caseness. CONCLUSIONS: Two simple instruments, the DT and the MT, were found to have acceptable levels of sensitivity and specificity in detecting psychosocial morbidity. Compared to the HADS, however, the mood MT performed better than the DT.

Fungal infections are an important cause of morbidity and mortality in patients with acute leukemia (AL). Candidemia, once rare, is now a common nosocomial infection because of the intensity of chemotherapy, prolonged neutropenia, administration of broad-spectrum antibiotics and use of central venous catheters (CVC). We retrospectively identified patients treated for AL from 6/86 to 6/95 who also had candidemia. We describe 28 patients (incidence 6.3%) with a median age of 39 years, 24 of whom were on remission induction and 4 on postremission chemotherapy. All patients had CVC and empiric antimicrobial therapy, 4 had been given prophylactic antifungal drugs, and 2 had parenteral nutrition. Neutropenia was profound (median leukocyte nadir 200/microliters, median duration 19 days). Candida was isolated in blood cultures 10 days (median) after the start of neutropenia. The clinical presentation included fever (100%), respiratory symptoms (71.4%), skin lesions (39.2%) and septic shock (17.8%). Amphotericin B was given to 17 patients and liposomal amphotericin to 5 patients. Infection resolved in 18 patients (64.2%). 10 of whom were in complete remission. Mortality from candidemia was 17.8% (5/28). In conclusion, fungal infections are responsible for death in a significant number of patients. In our series treatment success was related to its rapid onset and to the recovery of neutropenia.

INTRODUCTION: Obesity is a chronic disease and a serious health problem that leads to increased prevalence of diabetes, hypertension, dyslipidemia and gallbladder disease. OBJECTIVE: To evaluate the efficacy of orlistat for weight loss and improved lipid profile compared to placebo in obese patients with hypercholesterolemia, treated over a period of 6 months. METHODOLOGY: In a 6-month, multicenter (10 centers in Portugal), double-blind, parallel, placebo-controlled study, 166 patients, aged 18-65 years, body mass index (BMI) > or = 27 kg/m2, LDL cholesterol > 155 mg/dl, were randomized to a reduced calorie diet (600 kcal/day deficit) plus orlistat three times a day or placebo. Exclusion criteria included triglycerides > 400 mg/dl, severe cardiovascular disease, uncontrolled hypertension, type 1 or 2 diabetes under pharmacological treatment, and gastrointestinal or pancreatic disease. RESULTS: The mean difference in weight from baseline was 5.9% (5.6 kg) in the orlistat group vs. 2.3% (2.2 kg) in the placebo group. In the orlistat group 49% of patients achieved 5-10% weight loss and 8.8% achieved > 10%. The orlistat group showed a significant reduction in total and LDL cholesterol, with similar changes for HDL in both treatment groups. The frequency of gastrointestinal adverse events was slightly higher in the orlistat group than in the placebo group, leading to discontinuation in 7 patients. CONCLUSION: Treatment with orlistat plus a reduced calorie diet for 6 months achieved significant reductions in weight, BMI and lipid parameters.

INTRODUCTION: Conventional risk stratification after acute myocardial infarction is usually based on the extent of myocardial damage and its clinical consequences. However, nowadays, more aggressive therapeutic strategies are used, both pharmacological and invasive, with the aim of changing the course of the disease. OBJECTIVES: To evaluate whether the number of drugs administered can influence survival of these patients, based on recent clinical trials that demonstrated the benefit of each drug for survival after acute coronary events. METHODS: This was a retrospective analysis of 368 consecutive patients admitted to our ICU during 2002 for acute coronary syndrome. A score from 1 to 4 was attributed to each patient according to the number of secondary prevention drugs administered--antiplatelets, beta blockers, angiotensin-converting enzyme inhibitors and statins--independently of the type of association. We evaluated mortality at 30-day follow-up. RESULTS: Mean age was 65 +/- 13 years, 68% were male, and 43% had ST-segment elevation acute myocardial infarction. Thirty-day mortality for score 1 to 4 was 36.8%, 15.6%, 7.8% and 2.5% respectively (p < 0.001). The use of only one or two drugs resulted in a significant increase in the risk of death at 30 days (OR 4.10, 95% CI 1.69-9.93, p = 0.002), when corrected for other variables. There was a 77% risk reduction associated with the use of three or four vs. one or two drugs. The other independent predictors of death were diabetes, Killip class on admission and renal insufficiency. CONCLUSIONS: The use of a greater number of secondary prevention drugs in patients with acute coronary syndromes was associated with improved survival. A score of 4 was a powerful predictor of mortality at 30-day follow-up

INTRODUCTION: Adults with repaired tetralogy of Fallot (TOF) may be at risk for progressive right ventricular (RV) dilatation and dysfunction, which is commonly associated with arrhythmic events. In frequently volume-overloaded patients with congenital heart disease, tissue Doppler imaging (TDI) is particularly useful for assessing RV function. However, it is not known whether RV TDI can predict outcome in this population. OBJECTIVE: To evaluate whether RV TDI parameters are associated with supraventricular arrhythmic events in adults with repaired TOF. METHODS: We studied 40 consecutive patients with repaired TOF (mean age 35 +/- 11 years, 62% male) referred for routine echocardiographic exam between 2007 and 2008. The following echocardiographic measurements were obtained: left ventricular (LV) ejection fraction, LV end-systolic volume, LV end-diastolic volume, RV fractional area change, RV end-systolic area, RV end-diastolic area, left and right atrial volumes, mitral E and A velocities, RV myocardial performance index (Tei index), tricuspid annular plane systolic excursion (TAPSE), myocardial isovolumic acceleration (IVA), pulmonary regurgitation color flow area, TDI basal lateral, septal and RV lateral peak diastolic and systolic annular velocities (E' 1, A' 1, S' 1, E' s, A' s, S' s, E' rv, A' rv, S' rv), strain, strain rate and tissue tracking of the same segments. QRS duration on resting ECG, total duration of Bruce treadmill exercise stress test and presence of exercise-induced arrhythmias were also analyzed. The patients were subsequently divided into two groups: Group 1--12 patients with previous documented supraventricular arrhythmias (atrial tachycardia, fibrillation or flutter) and Group 2 (control group)--28 patients with no previous arrhythmic events. Univariate and multivariate analysis was used to assess the statistical association between the studied parameters and arrhythmic events. RESULTS: Patients with previous events were older (41 +/- 14 vs. 31 +/- 6 years, p = 0.005), had wider QRS (173 +/- 20 vs. 140 +/- 32 ms, p = 0.01) and lower maximum heart rate on treadmill stress testing (69 +/- 35 vs. 92 +/- 9%, p = 0.03). All patients were in NYHA class I or II. Clinical characteristics including age at corrective surgery, previous palliative surgery and residual defects did not differ significantly between the two groups. Left and right cardiac chamber dimensions and ventricular and valvular function as evaluated by conventional Doppler parameters were also not significantly different. Right ventricular strain and strain rate were similar between the groups. However, right ventricular myocardial TDI systolic (Sa: 5.4+2 vs. 8.5 +/- 3, p = 0.004) and diastolic indices and velocities (Ea, Aa, septal E/Ea, and RV free wall tissue tracking) were significantly reduced in patients with arrhythmias compared to the control group. Multivariate linear regression analysis identified RV early diastolic velocity as the sole variable independently associated with arrhythmic history (RV Ea: 4.5 +/- 1 vs. 6.7 +/- 2 cm/s, p = 0.01). A cut-off for RV Ea of < 6.1 cm/s identified patients in the arrhythmic group with 86% sensitivity and 59% specificity (AUC = 0.8). CONCLUSIONS: Our results suggest that TDI may detect RV dysfunction in patients with apparently normal function as assessed by conventional echocardiographic parameters. Reduction in RV early diastolic velocity appears to be an early abnormality and is associated with occurrence of arrhythmic events. TDI may be useful in risk stratification of patients with repaired tetralogy of Fallot.

INTRODUCTION: Recent clinical trials have studied parameters that could predict response to cardiac resynchronization therapy (CRT) in patients with advanced heart failure. Left ventricular end-diastolic dimension (LVEDD) is regarded as a possible predictor of response to CRT. OBJECTIVE: To study the response to CRT in patients with very dilated cardiomyopathy, i.e. those at a more advanced stage of the pathology, analyzing both the responder rate and reverse remodeling in two groups of patients classified according to LVEDD. METHODS: We performed a retrospective analysis of 71 patients who underwent CRT (aged 62 +/- 11 years; 65% male; 93% in NYHA functional class > or = III; 31% with ischemic cardiomyopathy; left ventricular ejection fraction [LVEF] 25.6 +/- 6.8%; 32% in atrial fibrillation; QRS 176 +/- 31 ms). Twenty-two (31%) patients with LVEDD > or = 45 mm/m2 (49.2 +/- 3.5 mm/m2) were considered to have very dilated cardiomyopathy (Group A) and 49 patients had LVEDD > 37 mm/m2 and < 45 mm/m2 (39.4 +/- 3.8 mm/m2) (Group B). All patients were assessed by two-dimensional echocardiography at baseline and six months after CRT. The following parameters were analyzed: NYHA functional class, LVEF and LVEDD. Responders were defined clinically (improvement of > or = 1 NYHA class) and by echocardiography, with a minimum 15% increase over baseline LVEF combined with a reduction in LVEDD (reverse remodeling). RESULTS: There were no significant differences in baseline demographic characteristics between the two groups. At six-month followup, we observed an improvement in LVEF (delta 8.5 +/- 11.8%) and a reduction in LVEDD (delta 3.7 +/- 6.8 mm/m2), with fifty-seven (79%) patients being classified as clinical responders. The percentage of patients with reverse remodeling was similar in both groups (64% vs. 73%, p = NS), as were percentages of improved LVEF (delta 6.3 +/- 11% vs. delta 9.6 +/- 12%; p = NS) and decreased LVEDD (delta 3.7 +/- 5.5 mm/m2 vs. delta 3.7 +/- 7.4 mm/m2; p = NS). We found a higher percentage of clinical responders in patients with very dilated cardiomyopathy (96% vs. 72%, p < 0.05). CONCLUSION: In this study, a significant number of responders showed reverse remodeling after CRT. Although a higher percentage of patients with very dilated cardiomyopathy showed improvement in functional class, the extent of reverse remodeling was similar in both groups.

Introdução: Em doentes com enfarte agudo do miocárdio (EAM), a angioplastia percutânea (ACTP) é de forma crescente aceite como a terapêutica de reperfusão mais eficaz. No sexo feminino estão descritos factores com implicação prognóstica na evolução do EAM. Existem diferenças detectáveis de morbilidade e mortalidade, após ACTP directa, no sexo feminino? Objectivos: Avaliar o perfil clínico e os resultados, em termos de morbilidade e mortalidade intrahospitalar, em mulheres submetidas a angioplastia directa por síndrome coronário agudo (SCA) com supradesnivelamento do segmento ST. Metodologia: Análise retrospectiva de 245 doentes consecutivos, 72 dos quais do sexo feminino (29,4 %), admitidos entre Janeiro de 2000 a Dezembro de 2001 por SCA com supradesnivelamento do segmento ST e submetidos a ACTP directa. Avaliámos a prevalência dos vários factores de risco cardiovascular – hipertensão arterial (HTA), tabagismo, dislipidémia, diabetes mellitus, história familiar –; antecedentes pessoais de angina, EAM e angioplastia prévios; horas de evolução do enfarte; características angiográficas e evolução clínica.Resultados: Do estudo descritivo dos dados, verificou-se que os doentes do sexo feminino eram mais idosos (67,9 ± 11,6 vs 59,6 ± 13; p < 0,001), com maior prevalência de HTA (65,3% vs 47,4 %; p < 0,05) e angina (29,0% vs 16,0 %; p < 0,05) e menor prevalência de tabagismo (27,8% vs 54,3 %; p < 0,001). A demora média (horas) entre o início da sintomatologia e a ACTP foi significativamente maior nas mulheres (6,8 ± 4,1 vs 5,4 ± 3,7; p < 0,05). Angiograficamente, o número de lesões existentes e abordadas foi semelhante nos dois grupos. Utilizaram-se inibidores da glicoproteína IIb/IIIa em 84,7% das mulheres e 90,8% dos homens. Dos eventos considerados, verificou-se maior ocorrência de complicações hemorrágicas minor (5,6% vs 5,2 %), arritmias (15,3% vs 10,4 %) e mortalidade intrahospitalar (9,7% vs 6,4 %) no sexo feminino, sem no entanto apresentar significado estatístico. A demora média intra- -hospitalar foi semelhante nos dois grupos. Conclusão: Apesar da crescente preocupação com a homogeneidade de abordagens diagnósticas e terapêuticas nos dois sexos, particularmente no acesso a terapêutica de reperfusão, a morbilidade e mortalidade são tendencialmente superiores na nossa população, sem contudo atingir significado estatístico. A idade mais avançada e a maior demora até à realização da angioplastia, podem explicar esta discrepância.

Objectivos: Estudar a prevalência, factores de risco, evolução clínica e abordagem terapêutica da sífilis congénita em recém-nascidos (RN) de risco, nascidos numa maternidade de referência com apoio perinatal diferenciado. Método: Realizou-se um estudo transversal para cálculo de prevalência à nascença de sífilis congénita, entre Janeiro de 1993 e Dezembro de 2004, através de recolha de dados registados nos processos clínicos das mães e respectivos RN. De acordo com os critérios definidos pelo Centers for Disease Control and Prevention (CDC) em 1989, os RN filhos de mãe com VDRL e/ou TPHA positivo foram divididos em três grupos de risco. Resultados: Foram identificados 467 recém-nascidos, verificando-se que a prevalência de risco de sífilis congénita à nascença se tem mantido ao longo dos anos (5,6‰). A maioria dos recém-nascidos (65%) enquadra-se no grupo de maior risco. Dezanove RN (4%) apresentaram sífilis congénita sintomática ao nascimento, a maioria pertencente ao grupo de maior risco. Outros factores de risco encontrados foram a gravidez não-vigiada, em 30% das mães, toxicodependência em 9%, coinfecção por vírus da hepatite B em 5%, por vírus da hepatite C em 4,7% e por vírus de imunodeficiência humana em 3,4% dos casos. Em alguns casos existia mais do que um factor de risco associado. Conclusões: Verificou-se que a prevalência de risco de sífilis congénita não sofreu grandes variações ao longo dos doze anos, pelo que a sífilis continua a constituir um problema de Saúde Pública em Portugal, com custos económicos e sociais.

Objectivo: Identificar os factores psicossociais que influenciam a percepção da dor pós-operatória em doentes submetidos a cirurgia de revascularização do miocárdio (CRM). Material e Métodos: Estudo exploratório correlacional de 91 doentes (71 homens e 20 mulheres) submetidos a CRM (pontagem aortocoronária) por esternotomia. A idade média era de 63,8 ± 9,6 anos (entre 39 e 84). Foram utilizados os seguintes instrumentos: Escala Analógica Visual às 24, 48 e 96 horas do pós-operatório; Questionário de Caracterização Demográfica; Mental Health Inventory de 5 itens; Percepção de Saúde Geral (SF-36); Escala de Expectativas de Dor; Escala de Percepção de Apoio; Escala de Expectativas de Auto-eficácia; Satisfação com o tratamento, médicos e enfermeiros (American Pain Society Questionnaire) aplicados às 96 horas após a cirurgia. Resultados: Os doentes que apresentaram expectativas elevadas de dor, percepcionaram maior apoio, apresentaram níveis elevados de auto-eficácia para lidar com a dor ou, se pertenciam ao sexo masculino, sentiram menos dor. De igual modo, os doentes que apresentaram melhor saúde mental, percepcionaram a sua saúde como boa e os doentes que expressaram maior satisfação com o tratamento sentiram menos dor. A dor não foi influenciada pela idade, grau de escolaridade ou pela satisfação com a conduta de médicos e enfermeiros. Conclusão: Após as primeiras 48 horas do pós-operatório, a experiência de dor é influenciada por factores psicossociais, em particular pela expectativa de dor, expectativa de auto-eficácia, apoio percebido, percepção da saúde geral, percepção de saúde mental e satisfação com o tratamento para a dor. Perante os resultados, evidencia-se a necessidade de conjugar conhecimentos no sentido de dar respostas mais alargadas e de carácter multidisciplinar no tratamento da dor pós-operatória em CRM devendo, a par de outros aspectos, focar-se na gestão das expectativas dos doentes.

Background: Children with spina bifida represent the major risk group for latex sensitization. Purpose: To determine the prevalence of latex sensitization in these children and to identify risk factors. Material and methods: We studied 57 patients with spina bifida. The mean age was 5.6 years and the male/female ratio was 0.8/1. In all patients a questionnaire, skin prick test (SPT) with latex (UCBStallergènes, Lofarma and ALK-Abelló), common aeroallergens and fruits (UCB-Stallergènes) and serum determination of total IgE (AlaSTAT) were performed. Results: The prevalence of latex sensitization was 30 %; only two sensitized children (12 %) had symptoms after exposure. Risk factors for latex sensitization were age 5 years (p = 0.008; OR = 6.0; 95% CI = 1.7-22.1), having at least four previous surgical interventions (p < 0.0001; OR = 18.5; 95% CI = 3.6-94.8), having undergone surgery in the first 3 months of life (p = 0.008; OR = 5.4; 95% CI = 0.7-29.2) and total serum IgE 44 IU/ml (p = 0.03; OR = 3.8; 95 %CI = 1.1-13.1). Multiple logistic regression analysis showed that only a history of four or more surgical interventions (p < 0.0001; OR = 26.3; 95 %CI = 2.9-234.2) and total serum IgE 44 IU/ml (p = 0.02; OR = 8.6; 95% CI = 1.4-53.4) were independently associated with latex sensitization. Sex, family and personal allergic history, hydrocephalus with ventriculoperitoneal shunt, cystourethrograms, intermittent bladder catheterization and atopy were not related to latex sensitization. Conclusions: In children with spina bifida, significant and independent risk factors identified for latex sensitization were multiple interventions and higher levels of total serum IgE. A prospective study will clarify the clinical evolution of assymptomatic children sensitized to latex.

Acute renal failure (ARF) is common after orthotopic liver transplantation (OLT). The aim of this study was to evaluate the prognostic value of RIFLE classification in the development of CKD, hemodialysis requirement, and mortality. Patients were categorized as risk (R), injury (I) or failure (F) according to renal function at day 1, 7 and 21. Final renal function was classified according to K/DIGO guidelines. We studied 708 OLT recipients, transplanted between September 1992 and March 2007; mean age 44 +/- 12.6 yr, mean follow-up 3.6 yr (28.8% > or = 5 yr). Renal dysfunction before OLT was known in 21.6%. According to the RIFLE classification, ARF occurred in 33.2%: 16.8% were R class, 8.5% I class and 7.9% F class. CKD developed in 45.6%, with stages 4 or 5d in 11.3%. Mortality for R, I and F classes were, respectively, 10.9%, 13.3% and 39.3%. Severity of ARF correlated with development of CKD: stage 3 was associated with all classes of ARF, stages 4 and 5d only with severe ARF. Hemodialysis requirement (23%) and mortality were only correlated with the most severe form of ARF (F class). In conclusion, RIFLE classification is a useful tool to stratify the severity of early ARF providing a prognostic indicator for the risk of CKD occurrence and death.

Introdução: A encefalopatia hipoxico-isquémica (EHI) é uma causa importante de mortalidade e morbilidade a longo prazo. Estima-se que anualmente ocorram 1 a 8 casos /1000 nascimentos. A asfixia perinatal mantida é a causa mais frequente de EHI. Objectivo: Avaliar a evolução a curto e longo prazo de crianças com EHI devido a asfixia perinatal. Metodologia: Estudo prospectivo histórico. População: Recém-nascidos (RN) consecutivos, nascidos na maternidade do HDE, com idade gestacional ≥ 37 semanas, admitidos na UCIN com os diagnósticos de EHI e asfixia perinatal, sem anomalias congénitas major. Período: 04/04/2001 – 31/12/2010. Variáveis: morbilidade, sobrevivência e sequelas. Covariáveis: peso ao nascer, idade gestacional, índice de Apgar, eventos peri-parto e tipo de parto. Resultados: Dos 19365 nados-vivos (NV), foram incluídos 28 RN (incidência 1,4/1000). Características da amostra: 68 % do sexo masculino; peso médio ao nascer 3180 g, mediana da idade gestacional 39,4 semanas. Índice de Apgar: moda ao 1º, 5º e 10º minuto, respectivamente 2, 5 e 7. Eventos peri-parto mais frequentes: líquido amniótico meconial (46,4%), alterações cardiotocográficas (50%) e circular cervical apertada (10,7%). Em 67,8% dos casos, foi realizada cesariana de emergência. Todos os RN necessitaram de manobras de reanimação. Desenvolveram EHI: ligeira 12 (42,9%), moderada 7 (25%) e grave 9 (32,1%). Ocorreram convulsões em 66% dos RN e disfunção multiorgânica em 12 RN (42,9%). Dos exames de imagem, a ecografia transfontanelar (ECOTF) foi realizada em 89,3% dos casos, o electroencefalograma (EEG) em 67,9% e a RMN em 32,1%. A ECOTF revelou alterações sugestivas de EHI em todos os casos; o EEG evidenciou anomalia da actividade eléctrica em 73,7% e a RMN revelou achados compatíveis com EHI em 100%. Houve 3 óbitos. Dos sobreviventes, 75% foram seguidos em consulta hospitalar, apresentando como sequelas graves paralisia cerebral (22,2%), atraso global do desenvolvimento (16,6%), epilepsia (11,1%), surdez neurossensorial (5,6%) e hemiparésia (5,6%). Conclusões: A incidência de EHI por asfixia foi 1,4/1000 NV. Verificou-se EHI ligeira em 42,8% dos casos. A mortalidade foi de 14,3%. Nos 18 casos seguidos, 38,9% não apresentavam sequelas ou tinham sequelas ligeiras. Apenas um caso foi submetido a hipotermia, sendo esta uma terapêutica promissora.

clinical presentation is self limited. It is classified into five groups (genogroups I through V). There are numerous reports of neurologic complications, namely afebrile seizures, but only two reports of associated encephalopathy. Case Report: A 12 month old girl with previous history of a pneumonia treated with amoxicillin-clavulanic acid and clarythromycin, presented in our emergency department with strabismus, ataxia for 3 days, later associated with vomiting and diarrhea. On admission she had ataxia and an episode of strabismus, but her later neurologic exam was normal. Laboratory data revealed: 10,9 g/dL hemoglobin, 11.200/μL leukocytes, 29,1% neutrophils and 65,2% lymphocytes, 488.000/μL platelets and negative CRP. The brain MRI showed middle ear, maxillary sinus and ethmoidal opacification, with no other abnormalities. During the first day of admission she had a tonic (?) seizure for 20 minutes. CSF analysis showed 5,6 cells/μL, 100% lymphocytes, 80 mg/dL glucose and 154,1 mg/dL protein. The EEG revealed short duration paroxystic activity located to the vertex. She was treated with acyclovir, ciprofloxacin, cefthriaxone and phenytoin. Her symptoms resolved by the third day of admission. Blood samples were tested for numerous pathogens, including serology for Borrelia, which was positive for IgG but negative for IgM. Fecal sample analysis revealed positive PCR for norovirus, although it was negative in CSF samples. IL-6 was measured in the CSF and was negative (5,8 pg/mL). She had a history of recurrent otitis media and pernieal candidiasis, which led to a detailed immune function study, which showed Immunology tests revealed diminished IgA (< 0,244 g/L) and absent antibody response to vaccinations. Since she was only 13 months old when she was tested, only follow up will determine the relevance of these values. Follow up at two years of age showed no delays and a normal development. Conclusion: Norovirus encephalitis is a rare entity, although gastrointestinal infection with this agent is relatively common. Here we present a case of a probable norovirus associated encephalopathy, although PCR for norovirus was negative in CSF samples and there was no CSF cytokine increase. It was not associated with adverse neurologic outcome and so far her development is normal, unlike the evolution described in previous case reports.
