The objective was to validate Regulatory Sensory Processing Disorders’ criteria (DC:0-3R, 2005) using empirical data on the presence and severity of sensory modulation deficits and specific psychiatric symptoms in clinical samples. Sixty toddlers who attended a child mental health unit were diagnosed by a clinical team. The following two groups were created: toddlers with RSPD(N = 14) and those with ‘‘other diagnoses in Axis I/II of the DC:0-3R00(OD3R) (N = 46). Independently of the clinical process, parents completed the Infant Toddler Sensory Profile (as a checklist for sensory symptoms) and the Achenbach Behavior Checklist for ages 1/2–5 (CBCL 1/2–5). The scores from the two groups were compared. The results showed the following for the RSPD group: a higher number of affected sensory areas and patterns than in the OD3R group; a higher percentage of sensory deficits in specific sensory categories; and a higher severity of behavioral symptoms such as withdrawal, inattention, other externalizing problems and pervasive developmental problems in CBCL 1/2–5. The results confirmed our hypotheses by indicating a higher severity of sensory symptoms and identifying specific behavioral problems in children with RSPD. The results revealed convergent validity between the instruments and the diagnostic criteria for RSPD and supported the validity of RSPD as a unique diagnosis. The findings also suggested the importance of identifying sensory modulation deficits in order to develop an early intervention to enhance the sensory capacities of children who do not fully satisfy the criteria for some DSM-IV-TR disorders.

A História do tratamento cirúrgico do cancro da mama é bem demonstrativa de que a verdade em Medicina é circunstancial, e que o caminho a percorrer no sentido de a alcançar é árduo. Mostra, também, como ao longo dos séculos o empirismo vai dando lugar ao método científico, e como os estudos prospectivos, controlados e randomizados determinam a alteração dos conceitos e a consequente modificação das técnicas cirúrgicas. Estas vão desde os métodos bárbaros, à luz dos conceitos actuais, que, da Grécia antiga se estendem até à descoberta da anestesia – em que se destacam nomes como Henri de Mondeville, Guy de Chaulliac e Lanfranco na Idade Média, von Hilden e Ambroise Paré no Renascimento, ou H.F. Le Dran, J.L. Petit e Benjamim Bell, no tempo do Iluminismo – até às técnicas cada vez mais meticulosas e racionais, já na Idade Contemporânea, sucessivamente devidas a James Paget, Joseph Pancoast, Charles Moore, William Stewart Halsted, William e Richard Handley, Geoffrey Keynes, Robert McWhirter, George Crile Jr., Cushman Haagensen, Dahl-Iversen, Jerome Urban, David Patey, Bernard Fisher e Umberto Veronesi, entre outros que souberam criar novos paradigmas na História do Tratamento Cirúrgico do Cancro da Mama, para se chegar à prática actual de cirurgia conservadora (mamária e axilar)com reconstrução.

The aims of this retrospective review were to determine the frequency of malignant endometrial polyps diagnosed with ambulatory hysteroscopy in the Obstetrics/Gynaecology Department of HDE, Lisbon, between January 2001 and December 2005 and to characterize these cases according to risk factors, sonographic and endoscopic findings tumoral histology, and tumor stage.We found seven cases of malignant endometrial polyps in a total of 1333 polyps initially diagnosed: an incidence rate of 0. 53%. These seven patients had a mean age of 68 years (55–82 years), and all were postmenopausal, with five having one risk factor each for endometrial cancer. Metrorrhagia was present in six of the seven patients(85.7%). Ultrasonography was abnormal in all seven patients, with a mean endometrial thickness of 26 mm(range: 12–44 mm). The hysteroscopy images suggested malignancy in all cases. All except one patient had a single polyp. The polyps had volumes between 1.5 and 3 cm; two were removed completely and five were biopsed. The histological subtype was: mixed endometrioid/serous papillary or clear cell (2), adenocarcinoma with squamous differentiation (2), carcinosarcoma (2), and clear cell carcinoma (1). Malignancy inside polyps is rare, but diagnostic hysteroscopy with visual guided biopsies can identify these cases in the earlier stages. The risk factors are not different from those of other endometrial carcinomas, but the histological subtype seems to point to more aggressive cancers.
