INTRODUCTION: Invasive fungal infections (IFIs) are a life-threatening complication in patients with hematologic malignancies, mainly in acute leukemia patients, following chemotherapy. IFI incidence is increasing, and associated mortality remains high due to unreliable diagnosis. Antifungal drugs are often limited by inadequate antimicrobial spectrum and side effects. Thus, the detection of circulating fungal DNA has been advocated as a rapid, more sensitive diagnostic tool. PATIENTS AND METHODS: Between June 01 and January 03, weekly blood samples (1,311) were screened from 193 patients undergoing intensive myelosuppressive or immunosuppressive therapy. IFI cases were classified according to European Organization for Research and Treatment of Cancer/Mycoses Study Group criteria. Fungal DNA was extracted from whole blood and amplified using polymerase chain reaction (PCR) published primers that bind to the conserved regions of the fungal 18S rRNA gene sequence. In our study, two or more consecutive positive samples were always associated with fungal disease. RESULTS: PCR screening predicted the development of IFI to be 17 days (median). This test had a specificity of 91.1% and a sensitivity of 75%. IFI incidence was 7.8%. DISCUSSION: Therefore, our results confirm the potential usefulness of PCR serial screening and the clinical applicability in everyday routine. PCR screening offers a noninvasive repeatable aid to the diagnosis of IFI.

In this study the authors evaluated the efficacy of prophylaxis with liposomal amphotericin B (L-AmB) in the incidence of fungal infections (FI) during the first 3 months after liver transplant (LT). The study was retrospective and accessed a 4-year period from 2008 to 2011. All patients who died in the first 48 hours after LT were excluded. Patients were divided by the risk groups for FI: Group 1, high-risk (at least 1 of the following conditions: urgent LT; serum creatinine >2 mg/dL; early acute kidney injury [AKI] after LT; retransplantation; surgical exploration early post-LT; transfused cellular blood components [>40 U]); and Group 2, low-risk patients. Group 1 patients were further separated into those who received antifungal prophylaxis with L-AmB and those who did not. Prophylaxis with L-AmB consisted of intravenous administration of L-AmB, 100 mg daily for 14 days. Four hundred ninety-two patients underwent LT; 31 died in the first 48 hours after LT. From the remaining 461 patients, 104 presented with high-risk factors for FI (Group 1); of these, 66 patients received antifungal prophylaxis and 38 did not. In this group 8 FI were observed, 5 in patients without antifungal prophylaxis (P = .011). Three more FI were identified in Group 2. By logistic regression analysis, the categorical variable high-risk group was independently related to the occurrence of invasive FI (P = .006). We conclude that prophylaxis with L-AmB after LT was effective in reducing the incidence of FI. No influence on mortality was detected.

Apresenta-se um caso de endocardite fúngica numa localização pouco habitual, em doente com hospitalização prolongada por queimaduras extensas. As vegetações localizam-se na veia cava superior (provavelmente aderentes a um trombo), em localização prévia de cateter venoso central. A doente foi tratada apenas com terapêutica médica (voriconazol) e após 5 meses de terapêutica, a doente permanece sem febre, tendo o ecocardiograma transesofágico demonstrado o desaparecimento do trombo e da vegetação.

Fungal infections are an important cause of morbidity and mortality in patients with acute leukemia (AL). Candidemia, once rare, is now a common nosocomial infection because of the intensity of chemotherapy, prolonged neutropenia, administration of broad-spectrum antibiotics and use of central venous catheters (CVC). We retrospectively identified patients treated for AL from 6/86 to 6/95 who also had candidemia. We describe 28 patients (incidence 6.3%) with a median age of 39 years, 24 of whom were on remission induction and 4 on postremission chemotherapy. All patients had CVC and empiric antimicrobial therapy, 4 had been given prophylactic antifungal drugs, and 2 had parenteral nutrition. Neutropenia was profound (median leukocyte nadir 200/microliters, median duration 19 days). Candida was isolated in blood cultures 10 days (median) after the start of neutropenia. The clinical presentation included fever (100%), respiratory symptoms (71.4%), skin lesions (39.2%) and septic shock (17.8%). Amphotericin B was given to 17 patients and liposomal amphotericin to 5 patients. Infection resolved in 18 patients (64.2%). 10 of whom were in complete remission. Mortality from candidemia was 17.8% (5/28). In conclusion, fungal infections are responsible for death in a significant number of patients. In our series treatment success was related to its rapid onset and to the recovery of neutropenia.

Introdução. A sépsis por fungos causa elevada morbilidade e~mortalidade em Unidades de Cuidados Intensivos Neonatais (UCIN). Objectivo. Determinar a incidência de casos de sépsis por fungos no recém-nascido de muito baixo peso à nascença (RNMBP), analisar a presença de factores de risco e a efectividade terapêutica. População e métodos. Estudaram-se RNMBP admitidos numa UCIN de um hospital de apoio perinatal diferenciado entre Janeiro de 1990 e Dezembro de 2002 (nascidos ou transferidos por prematuridade). Reviram-se processos clínicos de RNMBP com suspeita clínica de sépsis e uma hemocultura, cultura de liquor ou exame anatomo-patológico positivo para fungos. Dados referentes a variáveis contínuas apresentam-se como mediana (mínimo-máximo). Resultados. Registaram-se 20 casos de sépsis por fungos, correspondendo a uma incidência de 1,05% (população total de RNMBP =1901). Isolou-se Candida albicans em dezanove hemoculturas e dois exames anatomo-patológicos. A mediana da idade gestacional foi 26 semanas (24-32) e do peso de nascimento 835g (700-1450). Na altura do diagnóstico, a maioria dos recém-nascidos tinha catéteres centrais (100%) e tinha sido sujeita a ventilação mecânica (95%), alimentação parentérica total prolongada (100%) e antibioterapia de largo espectro (100%). A mediana da idade no diagnóstico foi 21 dias (10-80). Verificou-se atingimento de órgão em seis casos. A terapêutica efectuada foi predominantemente a anfotericina B (95%) com efectividade de 83% e taxa de toxicidade hepática de 20%. A taxa de letalidade foi 15%. Conclusões. O principal grupo afectado foi o recém-nascido com menos de 1000g de peso à nascença e menos de 28 semanas de idade gestacional. A presença de factores de risco foi frequente. A anfotericina B foi o fármaco de eleição com boa efectividade e toxicidade moderada. O exame anatomo-patológico post mortem foi decisivo no diagnóstico etiológico de sépsis sem agente isolado, de atingimento de órgão e da causa de morte.

Dermatoscopy can be used to evaluate the nail apparatus (ie, onychoscopy), and it is helpful for the diagnosis of numerous nail diseases and tumors. This article reviews the information that can be obtained in cases of nail dyschromia and especially in cases of melanonychia, in which the distinction between benign melanocytic activation or proliferation and malignancy is crucial. Dermatoscopic changes that accompany specific nail diseases are also reviewed, such as those observed with subungual hemorrhage, bacterial and fungal nail infections, psoriasis of the nail, lichen planus of the nail, and vascular abnormalities of the nail fold.

A rinossinusite fúngica alérgica (RSFA) é uma forma de sinusite fúngica reconhecida desde há cerca de vinte anos. À medida que se tem acumulado evidência clínica sobre esta doença, surgiu a controvérsia quanto à sua etiologia, patogénese, história natural e terapêutica adequada. O mecanismo fisiopatológico responsável pela RSFA permanece desconhecido, embora se pense tratar-se de um processo imunológico, não infeccioso, desencadeado por fungos. A RSFA e o papel desempenhado pelos fungos na patogénese da rinossinusite crónica são assuntos que suscitam grande interesse e controvérsia em Rinologia. Muito está ainda por descobrir sobre rinossinusite crónica, sendo numerosos os factores predisponentes. Recentemente existe grande interesse nas investigações que apontam os fungos como importante factor etiológico. Os autores fazem uma revisão de dados e teorias recentes sobre a fisiopatologia da RSFA, bem como o papel da terapêutica médica e cirúrgica nesta doença.

The genus Alternaria is one of the most common black moulds and appears to be increasing as a causative agent of subcutaneous phaeohyphomycosis, particularly among immunosuppressed patients. A 53-year-old patient who had received a kidney transplant presented with multiple verrucous lesions on the distal extremities. Positive histopathology and cultures, in addition to rDNA ITS region sequencing, identified the fungal isolate as Alternaria infectoria. Oral itraconazole was administered for 10 months. A follow-up at 15 months demonstrated no signs of infection. Clinical manifestations of cutaneous alternariosis vary significantly and only a few cases have been described in the literature. Although optimal treatment options remain controversial, this case of phaeohyphomycosis was successfully treated with itraconazole monotherapy.

Nails have a limited number of reactive patterns to disease. Accordingly, toenail changes of different etiologies may mimic onychomycosis. OBJECTIVE To determine the prevalence of toenail onychomycosis among patients with leg ulcer and toenail abnormalities attending a dermatology clinic. METHODS A cross-sectional study was conducted through the analysis of clinical records and results of mycological examination. RESULTS A total of 81 patients were included, with a median age of 76.0 years. Most ulcers were of venous etiology, followed by those of mixed and arterial pathogenesis. The mycological evaluation confirmed the diagnosis of onychomycosis in 27.2% of the patients. The etiologic agent was a dermatophyte in 59.1% of isolates in nail samples, while Trichophyton interdigitale was the most frequent fungal species (40.9%). CONCLUSIONS Most toenail abnormalities in patients with chronic leg ulcer were not onychomycosis. This study highlights the importance of systematic mycological examination in these patients, in order to avoid overtreatment with systemic antifungals, unnecessary costs and side effects.

Fungal invasive infections are rare in general population but are an emergent cause of infection in the immunocompromized population, especially in the solid organ transplant recipients. Herein the authors report a clinical case of a liver transplanted patient suffering a cutaneous co-existent infection with A. alternata as well as A. infectoria. To our knowledge this is the first case of cutaneous concomitant infection due to those two species reported not only in Portugal but also worldwide. The patient was treated with surgical excision of the lesions and oral itraconazol without relapse.
