A pigmentação macular eruptiva idiopática é uma patologia rara, que se caracteriza pelo aparecimento de múltiplas máculas de cor acastanhada, assintomáticas, que envolvem mais frequentemente a região cervical, o tronco e a região proximal dos membros. A incidência parece ser maior durante a infância e adolescência. Trata-se de uma entidade distinta do eritema discrómico perstans, de natureza benigna e, observando-se habitualmente remissão espontânea ao fim de alguns meses a anos, sem necessidade de tratamento ou investigação clínica adicional.

Diabetic macular oedema (DMO) is a leading cause of vision loss in the working-age population worldwide. Corticosteroid drugs have been demonstrated to inhibit the expression of both the vascular endothelial growth factor (VEGF) gene and other anti-inflammatory mediators, such as prostaglandins. Triamcinolone, fluocinolone and dexamethasone are the main steroids that have been studied for the treatment of macular oedema. Over the last few years, several studies have suggested an important role for dexamethasone in the management of DMO. The dexamethasone intravitreal implant (DEX implant) (Ozurdex®; Allergan, Inc., Irvine, CA) is a novel approach approved by the US Food and Drug Administration (FDA) and by the EU for the intravitreal treatment of macular oedema after branch or central retinal vein occlusion, and for the treatment of non-infectious uveitis affecting the posterior segment of the eye. We reviewed manuscripts that had investigated the pharmacokinetics, efficacy and safety of the DEX implant regarding DMO treatment.

The aim of this study is to report a clinical case of asymptomatic female Caucasian children with torpedo maculopathy. A 5-year-old girl was referred to our clinic for routine evaluation. The ophthalmic examination revealed best-corrected visual acuity of 20/20 in both eyes, without any changes in the biomicroscopy. Fundus examination showed normal findings in one eye, whereas in the contralateral eye it disclosed, in the temporal sector of the macular region, a whitish, atrophic, oval chorioretinal lesion with clearly defined margins. Posterior evaluations documented the stability of the lesion. Torpedo maculopathy diagnosis is based on its characteristic shape and peculiar location. The differential diagnosis has to be established versus choroidal lesions (melanoma and nevus), congenital or iatrogenic hyperplasia of the retinal pigment epithelium (RPE) and particularly versus the congenital pigmented lesions associated with Gardner's syndrome.

Multifocal intraocular lenses (MF IOLs) have concentric optical zones with different dioptric power, enabling patients to have good visual acuity at multiple focal points. However, several optical limitations have been attributed to this particular design. The purpose of this study is to access the effect of MF IOLs design on the accuracy of retinal optical coherence tomography (OCT). Cross-sectional study conducted at the Refractive Surgery Department of Central Lisbon Hospital Center. Twenty-three eyes of 15 patients with a diffractive MF IOL and 27 eyes of 15 patients with an aspheric monofocal IOL were included in this study. All patients underwent OCT macular scans using Heidelberg Spectralis®. Macular thickness and volume values and image quality (Q factor) were compared between the two groups. There were no statistically significant differences between both groups regarding macular thickness or volume measurements. Retinal OCT image quality was significantly lower in the MF IOL group (p < 0.01). MF IOLs are associated with a significant decrease in OCT image quality. However, this fact does not seem to compromise the accuracy of spectral domain OCT retinal measurements.

Objectivo: Avaliar a espessura da coróide na área macular em doentes com oclusão de ramo venoso da retina (ORVR) unilateral através de tomografia de coerência óptica de domínio espectral (SD-OCT) em modo enhanced depth imaging (EDI). Material e Métodos: Estudo retrospectivo não randomizado que incluiu 34 olhos de 17 doentes com ORVR unilateral (média de idade 68,6 ± 11,2 anos). Foi realizada análise estatística para comparar a espessura da coróide através de 3 medições (subfoveal e 750 µm temporal e nasal à fóvea) em cada uma de 7 linhas nos 15⁰x5⁰ centrais à fóvea para cada um dos olhos afectados e adelfos (21 medições em cada). Foi ainda realizada comparação entre a espessura macular central da retina e a espessura da coróide, para além da relação entre esta última e o tempo de evolução. Relacionou-se ainda a idade com a espessura da coróide no grupo controlo. Resultados: A média da espessura da coróide nos 17 olhos com ORVR foi de 211,8 ±55,97µm, o que foi superior à média verificada nos 17 olhos adelfos (185,7±46,1µm), sendo a diferença estatisticamente significativa (p⁼0,019). A coróide foi mais espessa a nível subfoveal (197,5±40,3µm) e mais delgada a nível nasal (176,9±54,9µm) no grupo controlo. Não se demonstrou haver relação entre o tempo de evolução e a espessura da coróide nos olhos com ORVR. Por outro lado houve relação entre a espessura da coróide e a espessura macular central (r⁼0,6; r²⁼0,36). Verificou-se uma correlação negativa, embora fraca, entre a idade e a espessura da coróide no grupo controlo (r⁼⁻0,022). Conclusões: A espessura da coróide pode ser avaliada através do EDI SD-OCT. Segundo alguns relatos, a mesma parece diminuir com a idade, tendência essa que se revelou também neste estudo. Tal como verificado na única publicação sobre a espessura da coróide na OVCR, demonstrou-se haver alteração na espessura da coróide na área macular em olhos com ORVR. Contudo são necessários mais estudos, com amostras maiores, que confirmem a alteração desta camada em olhos com ORVR e investiguem a sua influência na fisiopatologia da doença, no prognóstico visual e na resposta ao tratamento.

PURPOSE: The aim of the this study was to determine the effect of intravitreal antivascular endothelial growth factor injections on intraocular pressure (IOP) and identify possible risk factors for the development of increased IOP. MATERIALS AND METHODS: This prospective study included a total of 106 eyes receiving intravitreal injection of bevacizumab as treatment for macular edema or active choroidal neovascularization. IOP was measured by Goldmann applanation tonometry immediately before the intravitreal injection and 5 min, 1 h and 15 days after the procedure. The records of the study patients were reviewed for age, gender, history of glaucoma, diabetes mellitus, phakic status, systemic and topical medication and number of previous injections. Subconjunctival reflux was registered. IOP elevation was defined as IOP ≥21 mm Hg and/or a change from baseline of ≥5 mm Hg recorded at least on two or more measurements on the same visit. RESULTS: Mean preoperative IOP was 15.31 ± 3.90 mm Hg and postoperative IOP values were 27.27 ± 11.87 mm Hg (after 5 min), 17.59 ± 6.24 mm Hg (after 1 h) and 16.86 ± 3.62 mm Hg (after 15 days). The IOP variation was statistically significant between pre- and postoperative measurements (p < 0.05). Subconjunctival reflux was recorded in 11.3%, and in this subgroup the IOP at 5 min and at 1 h was lower than preoperative IOP (p < 0.05). CONCLUSIONS: More than one third of the eyes achieved IOPs >30 mm Hg 5 min after injection. Subconjunctival reflux contributed to a lower mean postoperative IOP (p < 0.05). Considerations for the management include prophylactic IOP lowering with medical therapy and/or preinjection ocular decompression for patients with a history of glaucoma or ocular hypertension and switching to an as-needed injection protocol in patients suffering a marked IOP rise in previous injections. © 2015 S. Objective: To report a case of retinal toxicity associated with efavirenz in an adult. Methods: We describe a case of gradual-onset blurry vision in both eyes in a 37-year-old HIV Caucasian woman, on antiretroviral therapy (ART), including efavirenz. Results: The patient presented with a best corrected visual acuity of 20/100 for the right eye (RE) and 20/125 for the left eye (LE). Fundoscopy revealed mottled atrophic changes of the macular retinal pigment epithelium (RPE) in both eyes. Fluorescein angiography revealed an annular pattern of RPE atrophy in both eyes. Full-field electretinography (ERG) was normal. Conclusions: Based on our patient’s history and on previous reports, efavirenz seems to be the culprit in this case. Our report provides evidence in support of routine ophthalmological evaluation of patients on efavirenz.
