Introduction: Anatomical variations of the extensor tendons to the fingers are of great clinical interest, due to the relatively high frequency of tendon injury in clinical practice. Material and methods: During routine dissection of the right upper limb of a 67-year-old female preserved corpse, the extensor indicis proprius (EIP) muscle belly originated 3 independent tendons, each with a separate fascial sheath, forming a triple EIP tendon. There was a larger tendon, which occupied a central position, that represented the usual single EIP tendon. In addition, there were two thinner radial and ulnar accessory EIP tendons. The radial-EIP tendon crossed deep to the extensor digitorum communis (EDC) tendon to the index finger in the distal half of the dorsum of the hand to reach the radial side of the extensor expansion hood of the index finger. Discussion: According to the literature, the frequency of a triple EIP tendon ranges from 0%, to as high as 7%, although most authors do not acknowledge the presence of this variant in their series. This variant of the EIP tendon, in which the radial-EIP terminated laterally to the termination of the tendon of the EDC to the index finger, may be a source of confusion intraoperatively, as the EIP tendon has traditionally been identified on the basis of its ulnar location with respect to the EDC tendon. Conclusion: The possibility of a triple EIP tendon should certainly be born in mind by all surgeons when performing tendon repairs, tenoplasties or tendon transfers.

INTRODUCTION: Synovial sarcoma is a high-grade, soft-tissue sarcoma that most frequently is located in the vicinity of joints, tendons or bursae, although it can also be found in extra-articular locations. Most patients with synovial sarcoma of the hand are young and have a poor prognosis, as these tumors are locally aggressive and are associated with a relatively high metastasis rate. According to the literature, local recurrence and/or metastatic disease is found in nearly 80% of patients. Current therapy comprises surgery, systemic and limb perfusion chemotherapy, and radiotherapy. However, the 5-year survival rate is estimated to be only around 27% to 55%. Moreover, most authors agree that synovial sarcoma is one of the most commonly misdiagnosed malignancies of soft tissues because of their slow growing pattern, benign radiographic appearance, ability to change size, and the fact that they may elicit pain similar to that caused by common trauma. CASE PRESENTATION: We describe an unusual case of a large synovial sarcoma of the hand in a 63-year-old Caucasian woman followed for 12 years by a multidisciplinary team. In addition, a literature review of the most pertinent aspects of the epidemiology, diagnosis, treatment and prognosis of these patients is presented. CONCLUSION: Awareness of this rare tumor by anyone dealing with hand pathology can hasten diagnosis, and this, in turn, can potentially increase survival. Therefore, a high index of suspicion for this disease should be kept in mind, particularly when evaluating young people, as they are the most commonly affected group.

Objectivos: Descrever a experiência sexual feminina no terceiro trimestre de gravidez. Tipo de estudo: Estudo descritivo transversal. Local: Consulta de Referência da Maternidade Dr. Alfredo da Costa – Lisboa, Portugal. População: Amostra de conveniência de cem mulheres com gravidez de baixo risco, que não tinham restrições médicas na sua actividade sexual. Métodos: As participantes completaram voluntariamente e em anonimato um questionário elaborado pelos autores respeitante a dados demográficos, história obstétrica, relacionamento sexual e esclarecimento/mitos sobre a sexualidade durante o último mês de gravidez. Resultados: A maioria das grávidas refere diminuição da actividade sexual após engravidar, embora mantenha relações sexuais uma ou mais vezes por semana e coito vaginal no terceiro trimestre, sem mudanças significativas para outros tipos de comportamento. Os factores que influenciam a redução da actividade sexual no terceiro trimestre são a diminuição do desejo, dispareunia e cansaço físico. Os mitos, crenças religiosas, nível de educação e dificuldade na informação sentida também parecem contribuir para a redução da actividade sexual. Frequentemente as grávidas indicam que o parceiro tem medo de prejudicar o bebé. Poucas mulheres obtiveram informação através dos profissionais de saúde sobre este tópico e consideram que gostariam de ter sido mais bem esclarecidas pelos mesmos. Conclusões: Os autores acreditam que na maioria dos casos a gravidez não provoca uma ruptura na sexualidade de um casal se esta era previamente satisfatória. No entanto, consideram importante a abordagem deste tema com a grávida por parte dos profissionais de saúde.
