BACKGROUND: Prospective testing for posttransplant circulating anti-HLA antibodies seems to be a critical noninvasive tool, but confirmatory data are lacking. MATERIALS AND METHODS: Over the last 3 years, peritubular capillary (PTC) C4d deposition was prospectively sought by an immunofluorescence technique applied to frozen tissue in biopsies obtained for allograft dysfunction. Screening for circulating anti-HLA class I/II alloantibodies (AlloAb) by the flow cytometric test was performed simultaneously. RESULTS: We evaluated 132 sets of biopsies and simultaneous serum samples. PTC C4d deposition was demonstrated in 15.9% (21/132) of biopsies. Circulating anti-HLA I/II AlloAb were detected in 25% (33/132) of serum samples. Employing receiver-operator characteristic (ROC) curves for all C4d-positive biopsies, screening for AlloAb showed a global specificity of 82% and sensitivity of 61.9%. When this analysis was restricted to biopsies obtained in the first month posttransplantation, the sensitivity increased to 81.8%, but the specificity decreased to 76.9%. After the first month posttransplantation, we observed sensitivity of 40.0% and a specificity of 86.4%. In the first month posttransplantation, all patients with a diagnosis of acute antibody-mediated rejection displayed circulating anti-HLA class I/II, but not always at the same time as the C4d-positive biopsy. CONCLUSIONS: In the first month posttransplantation, prospective monitoring of anti-HLA antibodies may be useful. The high sensitivity allows the identification of patients at risk, affording an earlier diagnosis of antibody-mediated rejection. After the first month, the test can be used to evaluate allograft dysfunction episodes, since positivity is highly suggestive of an antibody-mediated process.

Aim: To characterise clinically the patients with C4d in peritubular capillaries deposits (C4dPTCD) and/or circulating anti-HLA class I/II alloantibodies. To determine the correlation between positive C4dPTCD and circulating anti-HLA class I/II alloantibodies during episodes of graft dysfunction. Subjects and Methods: C4d staining was performed in biopsies with available frozen tissue obtained between January 2004 and December 2006. The study was prospective from March 2005, when a serum sample was obtained at the time of biopsy to detect circulating anti-HLA class I/II alloantibodies. Results: We studied 109 biopsies in 86 cadaver renal transplant patients. Sixteen of these (14.7%) presented diffuse positive C4dPTCD. There was a 13.5% rate of +C4dPTCD incidence within the first six months of transplantation and 16% after six months (p>0.05). Half of the +C4dPTCD in the first six months was associated with acute humoral rejection. After six months, the majority of +C4dPTCD (n=7/8) was present in biopsies with evidence of interstitial fibrosis/tubular atrophy and/or transplant glomerulopathy. The C4dPTCD was more frequent in patients with positive anti-HCV antibodies(p<0.0001), a previous renal transplant (p=0.007), and with a panel reactivity antibody (PRA) ≥ 50%(p=0.0098). The anti-HCV+ patients had longer time on dialysis (p=0.0019) and higher PRA(p=0.005). Circulating anti-HLA I/II alloantibodies were screened in 46 serum samples. They were positive in 10.9% of samples, all obtained after six months post transplant. Circulating alloantibodies were absent in 92.5% of the C4d negative biopsies. Conclusion: We found an association between the presence of C4dPTCD and 2nd transplant recipients,higher PRA and the presence of anti-HCV antibodies. The presence of HCV antibodies is not a risk factor for C4dPTCD per se, but appears to reflect longer time on dialysis and presensitisation. In renal dysfunction a negative alloantibody screening is associated with a reduced risk of C4dPTCD (<10%).

Embora com características genéticas diferentes, o embrião não é rejeitado pelo sistema imunitário materno, com o qual está em contacto através do trofoblasto. O HLA-G é uma molécula de HLA-I não clássica que é quase exclusivamente expresso pelo citotrofoblasto e é responsável pela inibição das células natural killer (NK) presentes na decídua. São descritos outros mecanismos envolvidos na tolerância materno-fatal, nomeadamente a não expressão de antigénios de classe II do MHC pelo citotrofoblasto, a produção de IL-10, e o HLA-G solúvel. A alteração destes mecanismos está associada a abortos de repetição e à preeclampsia.

Allelic differences in gene promoter or codifying regions have been described to affect regulation of gene expression, consequently increasing or decreasing cytokine production and signal transduction responses to a given stimulus. This observation has been reported for interleukin (IL)-10 (-1082 A/G; -819/-592 CT/CA), transforming growth factor (TGF)-beta (codon 10 C/T, codon 25 G/C), tumor necrosis factor (TNF)-alpha (-308 G/A), TNF-beta (+252 A/G), interferon (IFN)-gamma (+874 T/A), IL-6 (-174 G/C), and IL-4R alpha (+1902 G/A). To evaluate the influence of these cytokine genotypes on the development of acute or chronic rejection, we correlated the genotypes of both kidney graft recipients and cadaver donors with the clinical outcome. Kidney recipients had 5 years follow-up, at least 2 HLA-DRB compatibilities, and a maximum of 25% anti-HLA pretransplantation sensitization. The clinical outcomes were grouped as follows: stable functioning graft (NR, n = 35); acute rejection episodes (AR, n = 31); and chronic rejection (CR, n = 31). The cytokine genotype polymorphisms were defined using PCR-SSP typing. A statistical analysis showed a significant prevalence of recipient IL-10 -819/-592 genotype among CR individuals; whereas among donors, the TGF-beta codon 10 CT genotype was significantly associated with the AR cohort and the IL-6 -174 CC genotype with CR. Other albeit not significant observations included a strong predisposition of recipient TGF-beta codon 10 CT genotype with CR, and TNF-beta 252 AA with AR. A low frequency of TNF-alpha -308 AA genotype also was observed among recipients and donors who showed poor allograft outcomes.

Introdução: Actualmente, a maioria dos casos pediátricos de infecção por VIH é devida a transmissão materna do vírus. Na ausência de medidas de profilaxia, verificam se taxas de transmissão vertical do VIH-1 entre 15-25% na Europa Ocidental e Estados Unidos, 65% dos casos no peri-parto, 23% in útero e 12% no período pós-natal durante a amamentação. Caso clínico: Criança de 9 anos, sexo feminino, que recorre à urgência por febre, anorexia e tumefacção cervical com 2 dias de evolução. Dos antecedentes pessoais há a destacar: gravidez não vigiada, parto eutócico de termo, aleitamento materno até aos 3 anos e atraso do desenvolvimento estaturo-ponderal. Antecedentes patológicos de parotidite bilateral aos 5 anos e múltiplas cáries dentárias. À observação apresentava-se febril, emagrecida (peso < P5 e estatura no P5), com tumefacção cervical e retroauricular direitas, e aumento de volume das glândulas parótidas. Sem hepatoesplenomegalia e sem adenopatias palpáveis nas restantes cadeias ganglionares periféricas. Analíticamente, VS de 90 mm/h, sem outras alterações relevantes. Ecografia cervical mostrou adenofleimão e alterações compatíveis com parotidite. Internada com a hipótese diagnóstica de adenofleimão cervical e medicada com penicilina e clindamicina endovenosas (ev). Realizou serologias para VIH, com positividade para VIH tipo 1, confirmado por Western Blot. Contagem de linfócitos T CD4+ de 240 células/mm3. Carga viral de 3,82 x 103 cópias de RNA/mL. Genótipo HLA-B*5701 negativo. Confirmada infecção VIH 1 materna por Western Blot. Diagnóstico prévio de infecção VIH no período neonatal ocultado pela mãe. Restantes serologias negativas, assim como a pesquisa de BK no suco gástrico e o estudo do lavado bronco-alveolar. Ao 17º dia de internamento foi realizada punção do adenofleimão e alterada a antibioticoterapia para flucloxacilina ev (7 dias de terapêutica). Pesquisa de micobactérias e fungos no pús drenado negativa. Durante o internamento manteve-se clinicamente estável, iniciando profilaxia para Pneumocystis jirovecii com cotrimoxazol, e terapêutica anti-retroviral (Lamivudina, Abacavir, Lopinavir/Ritonavir), com melhoria clínica, virulógica e imunológica. Conclusões: Este caso ilustra um exemplo de transmissão vertical do VIH-1 caracterizado por uma evolução crónica, cuja apresentação cursou com parotidite, um dos sinais indicadores de infecção VIH.

Introdução: A narcolepsia é uma doença do sono REM com desregulação do ciclo de sono-vigília, consequente sonolência diurna e eventual associação a alucinações hipnagógicas, paralisia do sono e cataplexia. A sua prevalência é de 0,05 a 0,02% no adulto mas desconhecida na idade pediátrica. Caso clínico: Criança de seis anos, previamente saudável com sonolência excessiva até 18 horas/dia e discinésia oromandibular, desequilíbrio na marcha e movimentos coreiformes dos membros superiores. Duas semanas antes realizara vacinação para a gripe pandémica. Registou-se ainda hiperfagia diurna e nocturna durante cinco dias com resolução espontânea, episódios de cataplexia perante riso e alterações emocionais e tremor da cabeça e dos membros superiores com melhoria clínica progressiva após oito dias. Realizou RMN-CE e EEG sem alterações. O exame líquido céfalo-raquidiano e PCR para painel de vírus herpes, Mycoplasma pneumoniae e enterovírus negativas. Nesta fase realizou polissonografia com teste de latências múltiplas do sono (TLMS) sem alterações. Exame cultural do exsudado faríngeo, TASO e anticorpo AntiDnase B negativos. Da exaustiva investigação que realizou apresentava serologias ELISA e WB compatíveis com infecção por Borrelia burdorferi, pelo que cumpriu ceftriaxone 14 dias. Serologias para influenza A mostraram IgM 39 UA/mL com IgG 194 UA/mL com segunda amostra com IgM 43 UA/mL e IgG 162 UA/mL (VR IgM<20;IgG<20). O estudo da autoimunidade revelou ANA 1/320, anticorpos anticardiolipina e antinucleares extraíveis negativos. Restantes autoanticorpos e doseamento de complemento normal. Anticorpos Anti-NMDA e VKCG negativos. Doseamento de hipocretina muito diminuído com HLA DR2 e DQB1*0602 presentes. A polissonografia com TLMS, sete meses após a primeira, confirmou sonolência excessiva com quatro inícios do sono REM sugestivos de narcolepsia. Faz terapêutica com metilfenidato, a sonolência diurna diminuiu e cumpre o seu horário escolar sem limitações. Comentários: O diagnóstico de narcolepsia foi sugerido pela clínica e confirmado pelo teste de latências múltiplas. O valor de hipocretina diminuído pode sugerir uma etiologia autoimune. Uma infecção como a borreliose ou a vacinação prévia para H1N1, responsabilizada por outros casos de narcolepsia podem ter sido desencadeantes de uma alteração imunitária responsável pela doença, nesta criança com a susceptibilidade HLA DR2 e DBQ1*0602.

Consideram se dois tipos de aloantigénios plaquetários: o tipo 1 que corresponde a aloantigénios compartilhados pelas plaquetas e outras células e o tipo II, designando os aloantigénios específicos das plaquetas. O sistema de Aloantigénios Plaquetários Humanos (HPA) é constituído por cinco aloantigénios bialélicos e onze antigénios de baixa frequência. As glicoproteínas (GP) da membrana plaquetária associadas a uma maior frequência de anticorpos são as GPIIb-IIIa, GPIb-IX e GPIa-IIa, de acordo com estudos serológicos de fixação de antigénios, imunoquímicos e do âmbito da Biologia Molecular. A detecção de anticorpos antiplaquetários é possível através de diversos métodos laboratoriais, sendo apresentada a nossa experiência no serviço de Imuno-Hemoterapia do Hospital Curry Cabral, relativamente à pesquisa em Fase Sólida. A utilização da técnica indirecta para a detecção de aloanticorpos e da técnica directa para os autoanticorpos, bem como a realização do procedimento com solução de cloroquina para o esclarecimento da presença de anticorpos anti-HLA, representam os aspectos fundamentais descritos. Comentamos os resultados obtidos em cem amostras no período de onze meses e procedemos a uma breve revisão de aspectos clínicos relacionados com o diagnóstico da trombocitopénia autoimune, aloimune, a fármacos e por outras causas frequentes. São aborda dos conceitos da prática transfusional, aplicados a situações de emergência, de estratégia para a refractariedade plaquetária e de enquadramento multidisciplinar das medidas terapêuticas.

Transplant glomerulopathy is a sign of chronic kidney allograft damage. It has a distinct morphology and is associated with poor allograft survival. We aimed to assess the prevalence and clinic-pathologic features of transplant glomerulopathy, as well as determine the functional and histological implications of its severity. We performed a single-centre retrospective observational study during an eight-year period. Kidney allograft biopsies were diagnosed and scored according to the Banff classification, coupled with immunofluorescence studies. The epidemiology, clinical presentation, outcomes (patient and graft survival) and anti-HLA alloantibodies were evaluated. Transplant glomerulopathy was diagnosed in 60 kidney transplant biopsies performed for clinical reasons in 49 patients with ABO compatible renal transplant and a negative T-cell complement dependent cytotoxicity crossmatch at transplantation. The estimated prevalence of transplant glomerulopathy was 7.4% and its cumulative prevalence increased over time. C4d staining in peritubular capillaries (27.6%) was lower than the frequency of anti-HLA antibodies (72.5%), the majority against both classes I and II. Transplant glomerulopathy was associated with both acute (mainly glomerulitis and peritubular capillaritis) and chronic histologic abnormalities. At diagnosis, 30% had mild, 23.3% moderate and 46.7% severe transplant glomerulopathy. The severity of transplant glomerulopathy was associated with the severity of interstitial fibrosis. Other histological features, as well as clinical manifestations and graft survival, were unrelated to transplant glomerulopathy severity.

Moyamoya disease is an idiopathic progressive steno-occlusive disorder of the intracranial arteries located at the base of the brain. It is associated with the development of compensatory extensive network of fine collaterals. Moyamoya disease is considered syndromic when certain genetic or acquired disorders such as polycystic kidney disease, neurofibromatosis, or meningitis are also present. Although the genetic contribution in moyamoya is indisputable, its cause and pathogenesis remain under discussion. Herein, we report a rare occurrence of moyamoya syndrome in two European Caucasian siblings in association with unusual multisystemic malformations (polycystic kidney disease in one, and intestinal duplication cyst in the other). The karyotype was normal. No mutation in the RFN213 gene was found, and none of the HLA types linked to moyamoya disease or described in similar familial cases were identified. By describing these multisystemic associations, polycystic kidney disease for the second time, and intestinal malformation for the first time in the literature, our report expands the phenotypic variability of moyamoya syndrome. The coexistence of disparate malformations among close relatives suggests an underlying common genetic background predisposing to structural or physiological abnormalities in different tissues and organs.

Background: Economic evaluations help health authorities facing budget constraints. This study compares the health-related quality of life (HRQOL) and costs in patient subgroups on haemodialysis (HD) and renal transplantation (KT). Methods: In a prospective study with follow-up of 1-3 years, we performed a costutility analysis of KT vs. HD, adopting a lifetime horizon. A societal perspective was taken. Costs for organ procurement, KT eligibility, transplant surgery and follow-up of living donors were included. Key clinical events were recorded. HRQOL was assessed using the EuroQol instrument. Results: The HRQOL remained stable on HD patients. After KT, mean utility score improved at 3 months while mean EQ-VAS scores showed a sustained improvement. Mean annual cost for HD was 32,567.57€. Mean annual costs for KT in the year-1 and in subsequent years were, 60,210.09€ and 12,956.77€ respectively. Cost for initial hospitalization averaged 18,740.74€. HLA-mismatches increased costs by 75% for initial hospitalization (p < 0.001) and 41% in the year-1 (p < 0.05), and duplicate the risk of readmission in the year-1 (p < 0.05). The incremental costutility ratio was 5,534.46€/QALY, increasing 35% when costs for organ procurement were added. KT costs were 41,541.63€ more but provided additional 7.51 QALY. Conclusions: The KT is cost-effective compared with HD. Public funding should reflect the value created by the intervention and adapt to the organ demand.

Descrevem-se nove casos de processos neoplásicos de células B, com baixa densidade de Ig da membrana. Em todos estes casos se observa uma alta percentagem de MRFC e a presença de antigénios da classe II do MHC. Dos nove doentes apresentados, oito têm um quadro clínico e laboratorial típico de LLC e um tem um quadro compatível com uma LLC na sua fase inicial. A identificação dos isotipos da Ig de membrana sugere que a transformação neoplásica na LLC pode ocorrer em diferentes estádios de diferenciação do linfócito B. A expressão simultânea de três marcadores de superfície (baixa densidade de 1g. MRFC e Ia) e a sua estreita correlação com a LLC — B, evidencia a sua utilidade no diagnóstico da doença.

INTRODUCTION: The aim of this preliminary work is to analyze the clinical features of 52 patients with a functional transplanted kidney for >25 years (all first transplant and all deceased donor recipients) and to compare with a similar though more complete study from Hôpital Necker-Paris 2012. METHODS: The mean graft survival at 25 years is 12.7% and at 30 years is 10%. The actual mean serum creatinine concentration is 1.3 mg/L. We analyzed recipient age (mean, 35.9 years) and gender (29 men and 23 women). Donor age was 26.7 ± 10.3 years. Seven patients (13.4%) were transplanted with 1 HLA mismatch, 42.3% with 2 mismatches, and 44.2% with 3 mismatches. Mean cold ischemia time was 15.45 ± 7.7 hours. Of the recipients, 76% had immediate graft function; 38% experienced 1 acute rejection episode and 4 patients had 2 rejection crises. The initial immunosuppressive regimen was azathioprine (AZA) + prednisolone (Pred) in 14 patients, cyclosporin (CSA) + Pred in 13 patients, and CSA + AZA + Pred in 25 patients. Of these patients, 19% maintained their initial regimen, and 54% (28 patients) were very stable on a mixed CSA regimen for >25 years. RESULTS: We present the major complications (diabetes, neoplasia, and hepatitis C virus positivity). CONCLUSION: Our results in deceased donor kidney recipients for >25 years are similar to the mixed population (deceased donors and living donors) presented by the Necker group, although 54% of our patients remain on CSA immunosuppression, contradicting the idea that its use is not compatible with good long-term kidney function in transplant recipients.
