Está demonstrado o papel do teste de inclinação na avaliação de doentes com síncope de causa não esclarecida, sendo utilizado como a técnica gold standard para o diagnóstico da síncope neurocardiogénica, em particular com o recurso a agentes provocativos farmacológicos de modo a melhorar a acuidade diagnóstica. A estimulação com nitroglicerina sub-lingual é, regra geral, bem tolerada e permite aumentar a sensibilidade do teste reduzindo a sua duração. Neste sentido, tem-se verificado também o alargamento da sua aplicação em doentes idosos. Objectivos: Avaliar, numa população referenciada por síncope de etiologia desconhecida, a utilidade do teste de inclinação com recurso a nitroglicerina sub-lingual e comparar o tipo de respostas obtidas nos doentes idosos com o dos restantes doentes. Métodos: Estudámos 158 doentes submetidos a teste de inclinação com utilização de nitroglicerina como agente provocativo. Foram considerados doentes com idade <65 anos (Grupo A, n=74) e ≥65 anos (Grupo B, n=84). O teste de inclinação foi efectuado segundo o protocolo “italiano”, sob monitorização contínua do electrocardiograma e da tensão arterial (Task Force Monitor; CNSystems). Foram incluídos somente doentes assintomáticos na fase passiva do teste. O teste foi considerado positivo para resposta neurocardiogénica quando houve reprodução da sintomatologia acompanhada de bradicardia e/ou hipotensão arterial e as respostas foram definidas como cardio-inibitória, vasodepressora ou mista. Uma descida gradual e paralela da tensão arterial após administração de nitroglicerina seguida de síncope foi considerada como resposta exagerada aos nitratos. Resultados: Não se verificam diferenças na distribuição por sexos entre os grupos. O teste de inclinação foi positivo em 57% do Grupo A e 51% do Grupo B (p=NS), tendo ocorrido resposta exagerada aos nitratos em 11% e 16%, respectivamente (p=NS). Relativamente ao tipo de respostas neurocardiogénicas, as vasodepressoras foram mais frequentes no Grupo B (53% versus 24%; p=0,001) e as mistas tendencialmente mais frequentes no Grupo A (59% versus 40%; p=0,07), sem diferenças significativas no que se refere à resposta do tipo cardio-inibitória (17% nos Grupo A versus 7% no Grupo B; p=NS). Conclusões: Numa população com síncope de etiologia não conhecida, o teste de inclinação potenciado pela nitroglicerina: a) contribui de modo significativo para o esclarecimento diagnóstico com utilidade idêntica em idosos e nos restantes doentes. B) associa-se a maior incidência de resposta neurocardiogénica vasodepressora nos idosos embora com uma taxa semelhante de respostas exageradas aos nitratos.

OBJECTIVE: In the last decade, some attention has been given to spirituality and faith and their role in cancer patients' coping. Few data are available about spirituality among cancer patients in Southern European countries, which have a big tradition of spirituality, namely, the Catholic religion. As part of a more general investigation (Southern European Psycho-Oncology Study--SEPOS), the aim of this study was to examine the effect of spirituality in molding psychosocial implications in Southern European cancer patients. METHOD: A convenience sample of 323 outpatients with a diagnosis of cancer between 6 to 18 months, a good performance status (Karnofsky Performance Status > 80), and no cognitive deficits or central nervous system (CNS) involvement by disease were approached in university and affiliated cancer centers in Italy, Spain, Portugal, and Switzerland (Italian speaking area). Each patient was evaluated for spirituality (Visual Analog Scale 0-10), psychological morbidity (Hospital Anxiety and Depression Scale--HADS), coping strategies (Mini-Mental Adjustment to Cancer--Mini-MAC) and concerns about illness (Cancer Worries Inventory--CWI). RESULTS. The majority of patients (79.3%) referred to being supported by their spirituality/faith throughout their illness. Significant differences were found between the spirituality and non-spirituality groups (p ≤ 0.01) in terms of education, coping styles, and psychological morbidity. Spirituality was significantly correlated with fighting spirit (r = -0.27), fatalism (r = 0.50), and avoidance (r = 0.23) coping styles and negatively correlated with education (r = -0.25), depression (r = -0.22) and HAD total (r = -0.17). SIGNIFICANCE OF RESULTS: Spirituality is frequent among Southern European cancer patients with lower education and seems to play some protective role towards psychological morbidity, specifically depression. Further studies should examine this trend in Southern European cancer patients.

AIMS: Protocols using sublingual nitrates have been increasingly used to improve diagnostic accuracy of head-up tilt testing (HUT). Nevertheless, exaggerated responses to nitrates have been frequently described, particularly in elderly patients. The aim of this article is to evaluate, in an elderly population with unexplained syncope, whether the impact of sublingual nitroglycerin (NTG) used as a provocative agent is dose-dependent. METHODS AND RESULTS: One hundred and twenty consecutive elderly patients submitted to HUT using NTG after an asymptomatic drug-free phase were studied. Patients were divided into three groups according to the NTG dosage: 500, 375 and 250 microg. The test was considered positive when there was reproduction of symptoms with bradycardia and/or arterial hypotension. A gradual decrease in the blood pressure after NTG was considered an exaggerated response to nitrates. There were no differences in the clinical characteristics of the different subgroups. A positive test was obtained in 50% of the patients in each group. The rate of exaggerated responses was identical in all groups and ranged between 15 and 17%. CONCLUSION: In an elderly population with syncope of unknown origin submitted to HUT, the response to NTG is not dose-dependent, and no difference was found in the rate of exaggerated responses to nitrates with different NTG dosages.

Every month we see to be published dozens of scientific papers about etiology and physiopathology of CP, imaging, treatment, survival, quality of life of patients and of mothers (just a few) and so on. Papers dealing with the feelings and the problems of siblings of children and adults with CP in the most important scientific journals are extremely rare. However in internet we can find the sites of the most important Cerebral Palsy Societies, like the British, the Australian and the American ones already devoting a special attention to the issue of siblings; we also can see several interesting blogs of parents sharing their experiences not only with the handicapped child but also with the siblings, even counseling some books written for children giving practical advices how to deal and live with a handicapped sibling. What was a surprise to me were the several sites of adults having a disabled sibling, frequently with CP, in a new situation: without parents to care them.

The purpose of our study was to evaluate the accuracy of dynamic incremental bolus-enhanced conventional CT (DICT) with intravenous contrast administration, early phase, in the diagnosis of malignancy of focal liver lesions. A total of 122 lesions were selected in 74 patients considering the following criteria: lesion diameter 10 mm or more, number of lesions less than six per study, except in multiple angiomatosis and the existence of a valid criteria of definitive diagnosis. Lesions were categorized into seven levels of diagnostic confidence of malignancy compared with the definitive diagnosis for acquisition of a receiver-operator-characteristic (ROC) curve analysis and to determine the sensitivity and specificity of the technique. Forty-six and 70 lesions were correctly diagnosed as malignant and benign, respectively; there were 2 false-positive and 4 false-negative diagnoses of malignancy and the sensitivity and specificity obtained were 92 and 97%. The DICT early phase was confirmed as a highly accurate method in the characterization and diagnosis of malignancy of focal liver lesions, requiring an optimal technical performance and judicious analysis of existing semiological data.

Introdução: A artroplastia unicompartimental evoluiu nos últimos 40 anos, sendo hoje em dia considerada uma estratégia cirúrgica apropriada para a osteoartrose do compartimento interno da articulação do joelho. Desenvolvimentos nos instrumentos cirúrgicos, desenho do implante, abordagem cirúrgica e selecção dos doentes levaram a uma grande melhoria dos resultados pós-operatórios e aumento da longevidade das próteses unicompartimentais do joelho. Comparada com a prótese total, tem como vantagens a preservação óssea, menos complicações pós‐operatórias (perdas sanguíneas, dor pós‐operatória, taxa de infecção, trombose venosa profunda (TVP) e tromboembolismo pulmonar (TEP)), manutenção da normal cinemática do joelho, alta precoce e reabilitação mais rápida. A prótese unicompartimental Oxford phase 3 foi introduzida em 1998 e é uma prótese cimentada com menisco móvel de polietileno. Material e Métodos: Foi realizado um estudo retrospectivo das artroplastias unicompartimentais do joelho Oxford phase 3 realizadas no nosso serviço. Desde 2006 realizaram-se 37 artroplastias unicompartimentais (num total de 34 doentes). Sete dos quais não compareceram à avaliação pós-operatória e por isso foram excluídos do estudo. Todos os doentes incluídos no estudo foram avaliados clínica e radiograficamente. Foram revistos os processos de consulta e do internamento. Registou-se a idade,sexo, classificação ASA (American Society of Anesthesiologists), grau de satisfação, flexão‐extensão actual, Oxford knee score pré e pós‐operatório e alterações radiográficas a salientar. Resultados: O follow‐up médio foi de 47 meses (10 ‐ 83 meses). A idade média dos doentes é de 64 anos, com predomínio do sexo feminino. O ASA médio foi de 2,4. Um dos doentes foi submetido a conversão para artroplastia total do joelho por falência do componente tibial. Há 2 doentes não satisfeitos com a cirurgia (que corresponde aos doentes em que o Oxford knee score piorou). Há 1 doente pouco satisfeito e 23 satisfeitos ou muito satisfeitos. Todos os doentes conseguem fazer extensão completa e a média de flexão é 111º. A média do Oxford knee score pré‐operatório é de 17,4 (5 ‐ 30) e pós‐operatório é 36,6 (11 ‐ 48). Radiologicamente, há uma média de desvio em varo de 1,68º (varo 8º ‐ valgo 5º). Ocorreu artrose femoro‐tibial externa em três casos (dois dos quais também com artrose femoro‐patelar),um caso com slope tibial exagerado (19º), um caso com componente femoral em varo (15º), um caso com componente tibial demasiado grande com protusão interna, um caso de extrusão do menisco de polietileno, um caso com o componente tibial em valgo e um caso com falência deste (descelamento?) com provável necessidade de conversão para artroplastia total. Dos doentes não avaliados não há registo de conversão para artroplastia total do joelho ou outras complicações. Discussão: A larga maioria dos doentes encontram‐se satisfeitos ou muito satisfeitos, havendo uma melhoria do Oxford knee score para mais do dobro. Não se registaram complicações pós‐operatórias imediatas. Das artropastias unicompartimentas realizadas só uma foi convertida para artroplastia total e outra provavelmente a necessitar de conversão, com uma longevidade de 94,6% aos 47 meses (em média). Conclusão: A artroplastia unicompartimental do joelho demonstrou‐se uma excelente opção para doentes com osteoartrose não-inflamatória do compartimento interno do joelho. Para se obterem bons resultados os doentes devem ser criteriosamente seleccionados. Considerando a curva de aprendizagem necessária para o sucesso da cirurgia, a pouca experiência da maioria dos cirurgiões que colocaram as próteses não teve influência nos resultados finais, estando de acordo com a literatura existente, provando que a artroplastia unicompartimental do joelho tem bons resultados clínicos e funcionais. Um maior tempo de follow-up será necessário para se avaliar a longevidade das próteses unicompartimentais.

The Ponseti method is reportedly effective for treating clubfoot in children up to 9 years of age. However, whether age at the beginning of treatment influences the rate of successful correction and the rate of relapse is unknown. We therefore retrospectively reviewed 68 consecutive children with 102 idiopathic clubfeet treated by the Ponseti technique in four Portuguese hospitals. We followed patients a minimum of 30 months (mean, 41.4 months; range, 30–61 months). The patients were divided into two groups according to their age at the beginning of treatment; Group I was younger than 6 months and Group II was older than 6 months. All feet(100%) were initially corrected and no feet required extensive surgery regardless of age at the beginning of treatment. There were no differences between Groups I and II in the number of casts, tenotomies, success in terms of rate of initial correction, rate of recurrence, and rate of tibialis anterior transference. The rate of the Ponseti method in avoiding extensive surgery was 100% in Groups I and II; relapses occurred in 8% of the feet in younger and older children. Poor ventilation at day care centres (DCCs) was already reported, although its effects on attending children are not clear. This study aimed to evaluate the association between wheezing in children and indoor CO2 (a ventilation surrogate marker) in DCC and to identify behaviours and building characteristics potentially related to CO2. In phase I, 45 DCCs from Lisbon and Oporto (Portugal) were selected through a proportional stratified random sampling. In phase II, 3 months later, 19 DCCs were further reassessed after cluster analysis for the greatest difference comparison. In both phases, children’s respiratory health was assessed by ISAAC-derived questionnaires. Indoor CO2 concentrations and building characteristics of the DCC were evaluated in both phases, using complementary methods. Mixed effect models were used to analyze the data. In phase I, which included 3,186 children (mean age 3.1±1.5 years), indoor CO2 concentration in the DCC rooms was associated with reported wheezing in the past 12months (27.5 %) (adjusted odds ratio (OR) for each increase of 200 ppm 1.04, 95 % CI 1:01 to 1:07). In phase II, the association in the subsample of 1,196 children seen in 19 out of the initial 45 DCCs was not significant (adjusted OR 1.02, 95 % CI 0.96 to 1.08). Indoor CO2 concentration was inversely associated with the practices of opening Windows and internal doors and with higher wind velocity. A positive trend was observed between CO2 and prevalence of reported asthma (4.7 %). Conclusion: Improved ventilation is needed to achieve a healthier indoor environment in DCC.

BACKGROUND: Few randomised studies have compared antiandrogen intermittent hormonal therapy (IHT) with continuous maximal androgen blockade (MAB) therapy for advanced prostate cancer (PCa). OBJECTIVE: To determine whether overall survival (OS) on IHT (cyproterone acetate; CPA) is noninferior to OS on continuous MAB. DESIGN, SETTING, AND PARTICIPANTS: This phase 3 randomised trial compared IHT and continuous MAB in patients with locally advanced or metastatic PCa. INTERVENTION: During induction, patients received CPA 200 mg/d for 2 wk and then monthly depot injections of a luteinising hormone-releasing hormone (LHRH; triptoreline 11.25 mg) analogue plus CPA 200 mg/d. Patients whose prostate-specific antigen (PSA) was <4 ng/ml after 3 mo of induction treatment were randomised to the IHT arm (stopped treatment and restarted on CPA 300 mg/d monotherapy if PSA rose to ≥20 ng/ml or they were symptomatic) or the continuous arm (CPA 200 mg/d plus monthly LHRH analogue). OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: Primary outcome measurement was OS. Secondary outcomes included cause-specific survival, time to subjective or objective progression, and quality of life. Time off therapy in the intermittent arm was recorded. RESULTS AND LIMITATIONS: We recruited 1045 patients, of which 918 responded to induction therapy and were randomised (462 to IHT and 456 to continuous MAB). OS was similar between groups (p=0.25), and noninferiority of IHT was demonstrated (hazard ratio [HR]: 0.90; 95% confidence interval [CI], 0.76-1.07). There was a trend for an interaction between PSA and treatment (p=0.05), favouring IHT over continuous therapy in patients with PSA ≤1 ng/ml (HR: 0.79; 95% CI, 0.61-1.02). Men treated with IHT reported better sexual function. Among the 462 patients on IHT, 50% and 28% of patients were off therapy for ≥2.5 yr or >5 yr, respectively, after randomisation. The main limitation is that the length of time for the trial to mature means that other therapies are now available. A second limitation is that T3 patients may now profit from watchful waiting instead of androgen-deprivation therapy. CONCLUSIONS: Noninferiority of IHT in terms of survival and its association with better sexual activity than continuous therapy suggest that IHT should be considered for use in routine clinical practice.
