A mucinose cutânea focal é um padrão de reação histológica descrita em várias doenças no qual existe uma deposição focal anormal de mucinas na derme. Os autores apresentam um caso de um doente de 62 anos de idade, com placas infiltradas assintomáticas no couro cabeludo frontal e occipital, com três meses de evolução. Biópsias seriadas demostraram um infiltrado linfocitário moderado e deposição homogénea de mucinas na derme, sem alterações da epiderme, unidade pilossebácea, epidermotropismo ou granulomas. Investigações posteriores revelaram um adenocarcinoma primário do pulmão. A mucinose cutânea focal pode ter um amplo espectro de apresentações clínicas. Relatamos este caso para ilustrar uma apresentação incomum de mucinose cutânea focal no couro cabeludo e realçar a importância de excluir uma doença secundária associada. No melhor conhecimento dos autores este é o primeiro caso relatado de uma possível associação entre o adenocarcinoma do pulmão e mucinose cutânea.

Angiolymphoid hyperplasia with eosinophilia is a rare vascular proliferation characterized by single or multiple purplish, brownish papules and subcutaneous nodules, sometimes associated with pain or pruritus. This rare benign process occurs with a female predominance. Approximately 85% of the lesions occur in the skin of the head and neck; most of them are around the ear or on the forehead or scalp. Whether angiolymphoid hyperplasia with eosinophilia represents a benign neoplasm or an unusual reaction to varied stimuli, including trauma, the etiology remains unclear. Histopathologically, the lesions consist of a proliferation of blood vessels of variable size lined by large epithelioid endothelial cells and a variable inflammatory infiltrate of lymphocytes and eosinophils, sometimes with lymphoid follicle formation. The lesion is benign but may be persistent and is difficult to eradicate. We report on a case of a 58-year-old Caucasian man who presented a purplish pink dome-shaped tumor of size up to 8 cm in diameter located on the chest. We emphasize this case considering the unusual dimensions of the lesion (8 cm diameter) and the atypical location on the chest.

We describe the case of a 22-year-old black female with type 1 diabetes mellitus diagnosed when she was 12 years old. She first presented (March 1994) with pustules and ulcerations on the upper and lower limbs, trunk and scalp at the age 17. The diagnosis of pyoderma gangrenosum was made. Since presentation, changes in liver function were detected and subsequent study led to the diagnosis of sclerosing cholangitis. The diagnosis of ulcerative colitis was made after colonoscopy. Partial response was obtained with minocycline and clofazimine, but treatment with 5-aminosalicylic acid achieved no improvement of the ulcerations. Liver transplantation, followed by immunosuppressive therapy led to complete regression of the cutaneous lesions.
