Introdução: A angioplastia primária é a terapêutica de reperfusão com melhores resultados no enfarte agudo do miocárdio (EAM). Este facto assume particular relevância na população acima dos 75 anos, em que o risco hemorrágico relacionado com a trombólise se associa ao acréscimo de mortalidade condicionado pela idade. Objectivos: Avaliar em doentes com síndrome coronário agudo (SCA) com supradesnivelamento do segmento ST e submetidos a angioplastia directa, a influência da idade nos resultados, incluindo evolução clínica, morbilidade e mortalidade intrahospitalar. População e métodos: Análise retrospectiva de 245 doentes consecutivos admitidos entre Janeiro de 2000 e Dezembro de 2001 por SCA com supradesnivelamento ST, com idades entre os 31 e 90 anos (63+-13), submetidos a angioplastia primária. Destes, 46 doentes (18,8%) com idade superior a 75 anos foram comparados com os restantes. Avaliaram-se os seguintes parâmetros: factores de risco cardiovascular – hipertensão arterial (HTA), diabetes mellitus (DM), tabagismo, dislipidémia, história familiar -, antecedentes de angina ou EAM e angioplastia prévia; extensão de doença coronária, resultados, evolução clínica e mortalidade intrahospitalar. Resultados: Nos doentes com idade superior a 75 anos, o sexo feminino foi predominante (26 doentes, 56,5%), assim como a existência de HTA, angina e DM tipo2, embora sem significado estatístico. O tabagismo foi mais prevalente em idades inferiores (54,3% vs 13%;p<0,001), assim como EAM antigo (p<0,05). A taxa de sucesso primário global foi de 96,7% (93,5% no grupo acima de 75 anos). A doença de 3 vasos foi significativamente mais frequente na população idosa (41,3% vs 26,2%; p<0,05); a utilização de inibidores da glicoproteína IIb/IIIa foi menor (80,4% vs 91%;p<0,05). A evolução em classe clínica Killip (KK) foi mais favorável no grupo com menos de 75 anos (KKI: 88,4% vs 69,8%;p<0,001). Dos eventos estudados, observámos na população idosa maior ocorrência de complicações hemorrágicas minor (3,5% vs 13,0%; p<0,05) e de morte intrahospitalar (4,5% vs 19,6%; p<0,001). Conclusão: A angioplastia primária mantém nesta população uma taxa de sucesso primário semelhante. A maior incidência de complicações hemorrágicas implica um maior cuidado na selecção da terapêutica antitrombótica adjuvante. A mortalidade significativamente mais alta neste grupo etário mantém-se, mesmo quando a angioplastia é o método de reperfusão escolhido.

A consulta de hipocoagulação de um hospital central contempla uma população bastante diversificada, pelo que uma melhor compreensão das características da mesma, poderá levar à melhoria da prestação de cuidados de saúde e à diminuição do número de complicações tromboembólicas (resultantes da patologia base) e hemorrágicas [resultantes da própria terapêutica anticoagulante (ACO)]. Objectivos: Avaliar as características da população que frequenta a consulta de hipocoagulação e analisar quais podem predizer um maior risco de complicações. Métodos: Utilizaram-se os dados colhidos por um médico através de um questionário colocado a doentes durante a consulta de hipocoagulação. Foram efectuados 101 questionários e avaliaram-se as características demográficas (sexo, idade, escolaridade, grau de analfabetismo), os factores de risco clássicos para doença coronária, o diagnóstico que levou ao início da ACO, a duração da ACO, a periodicidade da determinação e valores mínimos, máximos e à data do questionário de INR e as complicações desta terapêutica. Consideraram-se como complicações o aparecimento de fenómenos hemorrágicos e/ou tromboembólicos, no decurso da terapêutica hipocoagulante. Resultados: Foram estudados 101 doentes, 74 do sexo feminino (73,3%), com idade média de 6410 anos (21-85). A população analisada tinha 4,5 ± 3,5 anos de escolaridade, com 15% de analfabetismo. A maioria dos doentes iniciou ACO após colocação de prótese valvular mecânica (56,4%). Em cada doente existia em média 1 factor de risco para doença coronária. O número de meses de ACO era de 99,489 (1-360). Sessenta e seis doentes (65,3%) conheciam o motivo pelo qual iniciaram esta terapêutica. Cada doente tinha efectuado 1,20,6 determinações de INR por mês e tinha, em média, um tempo máximo sem verificação do mesmo de 6,210,4 semanas. Quarenta e cinco doentes sofreram alguma complicação tromboembólica e/ou hemorrágica no decurso da terapêutica ACO. Ocorreram 50 complicações hemorrágicas, em 41 doentes, das quais 7 motivaram internamento. Detectaram-se 7 episódios de tromboembolismo central ou periférico, em 7 doentes. Posteriormente, dividiu-se a população em dois grupos: grupo I – com complicações (GI) e grupo II – sem complicações (GII). GI – 45 doentes, idade média 63,59,1 anos (39-80) e GII – 56 doentes, idade média 64,711,3 anos (21-85). Nos doentes que iniciaram ACO por prótese mitral detectou-se um maior número de complicações (60,6% no GI e 39,4% no GII, p=0,024). Também nos doentes com INR máximo recomendado > 3 (55,2% no GI e 44,8% no GII, p=0,013) e nos que tinham sido sujeitos a terapêutica estomatológica (68,3% no GI e 31,7% no GII, p<0,001) se verificou um maior número de complicações. A duração da ACO foi o factor mais significativo para o aparecimento de complicações (GI – 138,196,5 meses, GII – 67,868,2 meses, p <0,00005). Na análise multivariada apenas a duração da ACO se manteve como factor preditivo independente. Conclusões: Na população existe uma percentagem importante de doentes com baixa escolaridade, que se poderá repercutir sobre a compreensão desta terapêutica específica, não tendo contudo, neste estudo, revelado influência significativa na taxa de complicações. O aparecimento de complicações durante a terapêutica anticoagulante é dependente da duração desta, do valor do INR máximo recomendado e da realização ou não de procedimentos estomatológicos, sendo o primeiro factor o mais significativo.

OBJECTIVE: We set out to evaluate whether changes in N-terminal pro-brain natriuretic peptide (proBNP) can predict changes in functional capacity, as determined by cardiopulmonary exercise testing (CPET), in patients with chronic heart failure (CHF) due to dilated cardiomyopathy (DCM). METHODS: We studied 37 patients with CHF due to DCM, 81% non-ischemic, 28 male, who performed symptom-limited treadmill CPET, with the modified Bruce protocol, in two consecutive evaluations, with determination of proBNP after 10 minutes rest prior to CPET. The time between evaluations was 9.6+/-5.5 months, and age at first evaluation was 41.1+/-13.9 years (21 to 67). RESULTS IN THE FIRST AND SECOND EVALUATIONS RESPECTIVELY WERE: NYHA functional class >II 51% and 16% (p<0.001), sinus rhythm 89% and 86.5% (NS), left ventricular ejection fraction 24.9+/-8.9% and 26.6+/-8.6% (NS), creatinine 1.03+/-0.25 and 1.09+/-0.42 mg/dl (NS), taking ACE inhibitors or ARBs 94.5% and 100% (NS), beta-blockers 73% and 97.3% (p<0.001), and spironolactone 89% and 89% (NS). We analyzed the absolute and percentage variation (AV and PV) in peak oxygen uptake (pVO2--ml/kg/min) and proBNP (pg/ml) between the two evaluations. RESULTS: (1) pVO2 AV: -17.4 to 15.2 (1.9+/-5.7); pVO2 PV: -56.1 to 84% (11.0+/-25.2); proBNP AV: -12850 to 5983 (-778.4+/-3332.5); proBNP PV: -99.0 to 379.5% (-8.8+/-86.3); (2) The correlations obtained--r value and p value [r (p)]--are shown in the table below; (3) We considered that a coefficient of variation of pVO2 PV of >10% represented a significant change in functional capacity. On ROC curve analysis, a proBNP PV value of 28% showed 80% sensitivity and 79% specificity for pVO2 PV of >10% (AUC=0.876, p=0.01, 95% CI 0.75 to 0.99). CONCLUSIONS: In patients with CHF due to DCM, changes in proBNP values correlate with variations in pVO2, as assessed by CPET. However, our results suggest that only a proBNP PV of >28% predicts a significant change in functional capacity.

Objectivos: Avaliar a possibilidade de realizar angiografia de alta resolução com reconstrução tridimensional, com recurso a equipamento de ressonância magnética de 1 Tesla, em doentes com patologia aórtica. Métodos: Efectuaram-se trinta e um exames de ressonâncias magnética em dezassete doentes, após administração, sob a forma de bolus, de gadolinium. Foram adquiridas imagens em spin echo rápido, gradiente echo e angiografia com time-of-flight. Resultados: Todas as aquisições foram bem toleradas e conseguiu-se a opacificação do lumen aórtico, no momento apropriado, em todos os doentes. As sequências angiográficas foram obtidas com respiração suspensa que requereu cerca de 25 segundos por imagem e seis a oito minutos para conseguir uma reconstrução tridimensional. Conclusão: A angiografia aórtica por reconstrução tridimensional é possível com um equipamento de 1 Tesla, com imagens de elevada qualidade e resolução, obtidas em pouco minutos. É possível obter toda a informação facultada pela aquisição em spin echo, convencional ou acelerado, a partir da reconstrução das imagens tridimensionais, afastando assim a necessidade de aquisição, tão incómoda, das imagens spin echo, melhorar a rapidez e diminuir, portanto, o tempo de aquisição, o que se torna relevante em doentes com síndromes aórticos agudos.

A terapêutica antidislipidémica redutora do colesterol é um paradigma da melhor aplicação da evidência científica na prática clínica. Face às opções disponíveis, é fundamental que as estatinas tenham um perfil claro de segurança e tolerabilidade e uma relação de benefícios e risco favorável. A terapêutica intensiva do colesterol não está associada a consequências deletérias, dependentes da maior eficácia, ou a efeitos laterais graves. As estatinas não são hepatotóxicas. A flutuação enzimática é um fenómeno corrente na dislipidemia. O risco de aumento das transamínases está directa e intimamente relacionada com a dose (e com o tipo de estatina usada). Alterações pouco importantes do perfil hepático basal não são uma contra-indicação ao seu uso (doentes com um risco cardiovascular justificável). A monitorização iterada do perfil hepático não está justificada. A causa da miotoxicidade com as estatinas não está devidamente esclarecida. O risco de miopatia ou rabdomiólise não tem relação com a redução percentual ou absoluta do LDL-C (nem com o valor de LDL-C alcançado). Os efeitos adversos das estatinas podem depender das características físico-químicas da molécula e das suas características farmacocinéticas. Numa estratégia de farmacovigilância, o doente tem também uma palavra. A partilha de propósitos obriga o doente a responsabilidades partilhadas com a sua Equipa de Saúde, motivadoras de tratamentos mais seguros e de melhor prevenção cardiovascular. Aperfeiçoar o ingresso em programas de Saúde de qualidade e aprimorar o tratamento e os objectivos alcançados são as razões que devem fundamentar a terapêutica e a redução efectiva intensiva das dislipidemias.

There is a body of evidence that supports the important role of the renin-angiotensin system (RAS) in atherosclerotic disease and in the cardiovascular disease continuum: from endothelial dysfunction to vascular occlusion. In the earlier stages of vascular disease, the RAS promotes functional changes, of which endothelial dysfunction is the best example. The deposition of atherogenic lipoproteins in the intima, their oxidative modification and the onset and amplification of the inflammatory response strengthens the atherogenic role of the RAS. Inflammatory cells are one of the main sources of angiotensin-converting enzyme (ACE) and angiotensin II (Ang II) in the vascular wall, in a process that leads to structural changes in the artery and progression of atherosclerotic disease. Ang II promotes the migration of vascular smooth muscle cells and their phenotypic differentiation in synthesis that accelerates vascular disease. By modulating the inflammatory response and, in general, all the elements of the plaque, Ang II plays a part in its instability, in the onset of acute events and in the promotion of the local prothrombotic state that leads to infarction.

INTRODUCTION: Transthoracic echocardiography is the method of choice for the diagnosis of cardiac myxomas, but the transesophageal approach provides a better definition of the location and characteristics of the tumor. The authors review their thirteen years' experience on the echocardiographic diagnosis of this pathology. METHODS: From 1994 to 2007, 41 cardiac tumors were diagnosed in our echocardiographic laboratory, of which 27 (65.85%) were cardiac myxomas. The exams and the patients' clinical files were retrospectively reviewed. RESULTS: Of the 27 patients, 22 (81.5%) were female, with a mean age of 62.1 +/- 13.6 years (25-84 years). The predominant clinical features were due to the obstruction caused by the tumor in more than two thirds of the patients, followed by constitutional symptoms in one third and embolic events in 30%. In the lab results, anemia was found in three patients and elevated sedimentation rate and CRP in two. In two patients the myxoma was found by chance. All the cases were of the sporadic type, although we found a prevalence of thyroid disease of 14% (4 patients). All patients underwent urgent surgical resection except one, in whom surgery was refused due to advanced age and comorbidities. The myxomas followed a typical distribution with 24 (88.8%) located in the left atrium, 18 of them attached to the atrial septum (AS) and two to the mitral valve. In one patient, the tumor involved both atria. The other two cases originated in the right atrium at the AS. Embolic phenomena were more frequent in small tumors (p = 0.027) and in those with a villous appearance (p = 0.032). Obstructive manifestations were associated with larger tumors (p = 0.046) and larger left atria (p = 0.048). In our series, there were no deaths during hospitalization or in the follow-up period of 5.2 +/- 3.7 years in 19 patients. There were two recurrences, both patients being successfully reoperated. CONCLUSION: Myxoma is the most common cardiac tumor. Transesophageal echocardiography provides excellent morphologic definition, aiding in diagnosis and follow-up. Most clinical manifestations are obstructive and are associated with larger tumors. Small tumors with a friable appearance have a higher chance of embolization. Surgical resection is usually curative and the long-term prognosis is excellent.

Several studies have shown that patients with congestive heart failure (CHF) have a compromised health-related quality of life (HRQL), and this, in recent years, has become a primary endpoint when considering the impact of treatment of chronic conditions such as CHF. OBJECTIVES: To evaluate the psychometric properties of the Portuguese version of a new specific instrument to measure HRQL in patients hospitalized for CHF: the Kansas City Cardiomyopathy Questionnaire (KCCQ). METHODS: The KCCQ was applied to a sample of 193 consecutive patients hospitalized for CHF. Of these, 105 repeated the assessment 3 months after admission, with no events during this period. Mean age was 64.4 +/- 12.4 years (21-88), and 72.5% were 72.5% male. CHF was of ischemic etiology in 4% of cases. RESULTS: This version of the KCCQ was subjected to statistical validation, with assessment of reliability and validity, similar to the American version. Reliability was assessed by the internal consistency of the domains and summary scores, which showed similar values of Cronbach alpha (0.50-0.94). Validity was assessed by convergence, sensitivity to differences between groups and sensitivity to changes in clinical condition. We evaluated the convergent validity of all domains related to functionality, through the relationship between them and a measure of functionality, the New York Heart Association (NYHA) classification. Significant correlations were found (p < 0.01) for this measure of functionality i patients with CHF. Analysis of variance between the physical limitation domain, the summary scores and NYHA class was performed and statistically significant differences were found (F = 23.4; F = 36.4; F = 37.4, p = 0.0001) in the ability to discriminate severity of clinical condition. A second evaluation was performed on 105 patients at the 3-month follow-up outpatient appointment, and significant changes were observed in the mean scores of the domains assessed between hospital admission and the clinic appointment (differences from 14.9 to 30.6 on a scale of 0-100), indicating that the domains assessed are sensitive to changes in clinical condition. The correlation between dimensions of quality of life in the KCCQ is moderate, suggesting that the dimensions are independent, supporting the multifactorial nature of HRQL and the suitability of this measure for its evaluation. CONCLUSION: The KCCQ is a valid instrument, sensitive to change and a specific measure of HRQL in a population with dilated cardiomyopathy and CHF.

INTRODUCTION: Chylothorax is a rare but serious postoperative condition in children with congenital heart disease. Conventional medical treatment consists of specific long-term dietary modification, and surgical reintervention, such as lymphatic duct ligation, may be indicated in refractory cases. In recent years, an additional conservative treatment, octreotide, a synthetic analog of somatostatin, has been used in management of congenital and postoperative chylothorax. METHODS: The objective of this work was to analyze the efficacy and safety of this treatment for chylothorax after congenital heart surgery. We reviewed the records of sixteen patients with chylothorax after surgery for congenital heart disease between January 1999 and December 2007, and collected the following data: demographic information; type of surgical procedure; onset, duration and management of chylothorax and treatment; and duration of hospital stay. To analyze efficacy we compared these parameters in children receiving conventional treatment only with those receiving octreotide. To analyze safety we compared the adverse effects of both treatments. Octreotide was administered at a dose of 4 to 10 microg/kg/hour, with monitoring of side effects. RESULTS: The incidence of chylothorax in our population was 1.6%. It occurred more often after Glenn and Fontan procedures (8 patients). Octreotide was begun three days after diagnosis of chylothorax and continued for a median of seventeen days (ranging from 4 to 26 days), until complete resolution. Side effects were frequent (in 3 of the 8 patients) but of no clinical relevance. All patients responded to the therapy and there was no indication for further surgical intervention. DISCUSSION AND CONCLUSIONS: Octreotide is safe and effective in the treatment of postoperative chylothorax in children with congenital heart disease. It is a useful adjunctive therapy to the conventional treatment of this complication.

Doente do sexo feminino, 78 anos de idade, internada previamente por EAM combinado. A evolução clínica subsequente decorreu sem complicações, tendo alta ao 10.º dia. Cerca de 12 horas depois foi readmitida no Serviço de Urgência, por dor retrosternal prolongada, hipotensa e prostrada. O ECG (Fig. 1) mostrou re-elevação do segmento ST nas derivações anteriores e inferiores, sendo admitida na Unidade de Cuidados Intensivos com o diagnóstico de provável re-enfarte. O Ecocardiograma realizado na admissão revelou função sistólica global comprometida, sendo possível definir um volumoso espaço anecogénico no ápex do ventrículo esquerdo (VE), com solução de continuidade na transição do terço médio/apical da parede inferior (Fig. 2) e uma segunda solução de continuidade na transição do terço médio/apical da parede lateral do VE (Fig. 3). Em ambos os orifícios foi confirmada a presença de fluxo bidireccional por Doppler Pulsado e Cor (Fig. 2), sugerindo a presença de um pseudoaneurisma do VE com duplo orifício de entrada. A doente foi transferida para um centro cirúrgico, onde, após confirmação intra-operatória do pseudoaneurisma com duplo orifício, foi submetida a encerramento do colo com patch de teflon. Teve alta estável, sendo seguida em consulta de Cardiologia desde há cerca de 16 meses, sem eventos.

Caseous calcification of the mitral annulus is a rare form of mitral annular calcification, whose etiology is not completely understood and which can lead to an erroneous diagnosis of intracardiac tumor. The authors describe the cases of six patients, five of them female, mean age 74.8 +/- 6.4 years (65-81). Four patients presented with heart failure, two with atrial fibrillation and five with hypertension. Round, echogenic images, 18-26 mm in their largest diameter with a central echolucent area, were identified by transthoracic echocardiography on the lateral and posterior segments of the mitral annulus. Severe mitral regurgitation was also found in four patients. Only three patients with severe mitral regurgitation and heart failure were operated on, and one patient refused surgical treatment. A caseous mass, similar to toothpaste, was obtained from the mitral annulus zone during surgery.

INTRODUCTION: Atrial septal defects (ASD) are among the most common congenital anomalies and account for 10% of congenital heart disease in the pediatric age-group and 30% in adults. Closure is indicated when there is evidence of hemodynamic significance or after a paradoxical embolic event. Ten years ago, percutaneous closure became the treatment of choice in our center for all patients with a clear indication and favorable anatomy. In this paper we report the experience of this first decade. OBJECTIVE: To assess the short- and long-term results of our ten-year experience with percutaneous closure of atrial septal defects. METHODS: We studied retrospectively all patients with ASD treated with a percutaneous approach between November 1998 and December 2008. The pediatric age-group consisted of patients younger than 19 years old. Demographic data, clinical indications, minor and major complication rates, success rate and long-term outcome were assessed. RESULTS: In the first ten years of experience 510 patients, of whom 166 were in the pediatric group, were treated in our center by a team of adult and pediatric cardiologists. The overall success rate of the procedure was 98% (97.5% in ASD and 99.5% in patent foramen ovale (PFO). The minor complication rate was 3% (3.4% in ASD and 2% in PFO). The most frequent complication was supraventricular tachycardia. The major complication rate was 1.2% (0.6% in ASD and 2% in PFO). Two patients developed cardiac tamponade due to hemopericardium that was resolved by pericardiocentesis, without need for surgery. One patient had an arterial pseudoaneurysm corrected by vascular surgery. There was no device embolization and no need for urgent surgery in this population. During follow-up two patients had recurrence of ischemic stroke, one had a transient ischemic attack and another had a hemorrhagic stroke. Mortality was 0.6% (0.6% in ASD and 0.5% in PFO). There were no in-hospital deaths. During follow-up there were two deaths, both in the adult group. DISCUSSION AND CONCLUSION: In this population the success rate was high and most of the complications were minor. The results of this collaboration between adult and pediatric cardiologists in the first ten years of activity confirm the safety and efficacy of percutaneous closure of septal defects, when there is careful patient selection and a standardized technique.

INTRODUCTION: Coarctation of the aorta (CoA) is a stenosis usually located in the descending aorta. Treatment consists of surgical or percutaneous removal of the obstruction and presents excellent immediate results but significant residual problems often persist. OBJECTIVES: To describe the presentation, treatment and long-term evolution of a population of 100 unselected consecutive patients with isolated CoA in a single pediatric cardiology center. METHODS: This was a retrospective study of all patients with isolated CoA treated during4 the last 21 years (1987-2008). RESULTS: The patients (n=100, 68.3% male) were diagnosed at a median age of 94 days (1 day to 16 years). The clinical presentation differed between patients aged less or more than one year, the former presenting with heart failure and the latter being asymptomatic with evidence of hypertension (88 and 63%, respectively; p < 0.01). Treatment, a median of 8 days after diagnosis, was surgical in 79 cases (20 end-to-end anastomosis, 31 subclavian flap, 28 patch) and percutaneous in the remaining 21 (15 balloon angioplasty, 6 with stenting). The mean age of surgical patients was younger than in those treated percutaneously (3.4 vs. 7.5 years; p < 0.01). Immediate mortality was 2% and occurred in the surgical group. There was no late mortality, in a mean follow-up of 7.2 +/- 5.4 years. Recoarctation occurred in 8 patients (6 surgical, 2 percutaneous). There are 46 patients who currently have hypertension (19 at rest, 27 with effort), their median age at diagnosis being older than the others (23 vs. 995 days; p < 0.01). CONCLUSIONS: Isolated CoA has an excellent short-term prognosis but a significant incidence of long-term complications, and should thus no longer be seen as a simple obstruction in the descending aorta, but rather as a complex pathology that requires careful follow-up after treatment. Its potentially insidious presentation requires a high level of clinical suspicion, femoral pulse palpation during physical examination of newborns and older children being particularly important. Delay in treatment has an impact on late morbidity and mortality. Taking into account the data currently available on late and immediate results, the final choice of therapeutic technique depends on the patient's age, associated lesions and the experience of the medical-surgical team. Hypertension should be closely monitored in the follow-up of these patients, as well as its risk factors and complications.

OBJECTIVES: To assess the feasibility of performing pulmonary angiography using MRI with contrast enhancement in patients with pulmonary vascular disease. METHODS: We present our experience in ten individuals, two controls and eight patients who underwent the exam after injection of a gadolinium-based contrast agent on a 1 Tesla MR scanner using a time-of-flight sequence and breath-holding during injection of contrast. RESULTS: Pathology in the main pulmonary artery and its major branches was detected easily while resolution at the segmental and subsegmental levels was inadequate. CONCLUSION: Contrast-enhanced magnetic resonance pulmonary angiography is feasible on a 1 Tesla MR scanner for the study of pathology of the main pulmonary artery and its major branches, like massive pulmonary embolism. However its ability to detect and define distal vessel pathology as found in chronic thromboembolic pulmonary hypertension and small pulmonary emboli is limited.

BACKGROUND: Metabolic syndrome (MS) is associated with increased incidence of diabetes and atherosclerotic complications. The new definition of the International Diabetes Federation (IDF) increases the population with this entity, compared to the NCEP ATP III definition. OBJECTIVES: To study the prevalence of coronary artery disease (CAD) and carotid intima-media thickness (IMT) in patients with and without MS, according to the NCEP ATP III and IDF definitions, and the predictive ability of carotid IMT for CAD. METHODS: We studied 270 consecutive patients admitted for elective coronary angiography due to suspicion of CAD. All patients underwent ultrasound study of the carotid arteries to measure IMT (the highest value between the right and left common carotid arteries was used in the analysis). Coronary stenosis of > or =70% (or 50% for the left main coronary artery) was considered significant. RESULTS: By the ATP III definition, 14% of the patients had MS, and these patients had a higher prevalence of CAD (87% vs. 63%, p = 0.004), but no significant difference was found for carotid IMT (1.03 +/- 0.36 mm vs. 0.95 +/- 0.35 mm, p=NS). With the IDF definition, 61% of the patients had MS; this group was slightly older and included more women. There were no differences in terms of CAD (68% vs. 63%) or carotid IMT (0.97 +/- 0.34 vs. 0.96 +/- 0.39 mm). On multivariate analysis, the ATP III definition of MS predicts CAD (OR 4.76, 95% CI 1.71-13.25, p = 0.003), but the IDF definition does not (OR 1.29, 95% CI 0.74-2.27, p = 0.37). On ROC curve analysis, an IMT of > or = 0.95 mm predicts CAD (AUC 0.66, p < 0.001), with a sensitivity of 52% and specificity of 75%. CONCLUSIONS: The new IDF definition increases the population with MS, decreasing the capacity to predict the presence of CAD. In our population, neither the ATP III nor the IDF definition showed differences in terms of carotid IMT. Carotid IMT can predict CAD, but with only modest sensitivity.
