Neurotransmitter diseases are a group of inherited disorders attributable to a disturbance of neurotransmitter metabolism. Biogenic amines are neurotransmitters with multiple roles including psychomotor function, hormone secretion, cardiovascular, respiratory and gastrointestinal control, sleep mechanisms, body temperature and pain. Given the multiple functions of monoamines, disorders of their metabolism comprise a wide spectrum of manifestations, with motor dysfunction being the most prominent clinical feature. Methods: Case review of 12 patients from 4 families, with primary disorders of biogenic amine metabolism. Results: Aromatic L-amino acid decarboxylase deficiency (4 patients from 2 families), and GTP-cyclohydrolase (8 patients from 2 families) were the two diseases identified. Age at first symptoms varied between 2 months and 6 years. Developmental delay was present in all cases except 2 patients with GTP cyclohydrolase deficiency. The combination of axial hypotonia and limb dystonia was also frequent. Children with aromatic L-amino acid decarboxylase deficiency exhibited temperature instability, oculogyric crisis and disturbances of sleep. The index case of one family with GTP cyclohydrolase deficiency presented with Parkinsonism (bradykinesia, rigidity and hypomimia). Analysis of neurotransmitters and their metabolites in CSF was crucial for the identification of index cases. Response to therapy was variable but in general unsatisfactory except in a family with GTP cyclohydrolase deficiency. Conclusions: These disorders should be considered in the differential diagnosis of paediatric neurodegenerative diseases, in order to allow an adequate therapeutic trial that can favor prognosis.

clinical presentation is self limited. It is classified into five groups (genogroups I through V). There are numerous reports of neurologic complications, namely afebrile seizures, but only two reports of associated encephalopathy. Case Report: A 12 month old girl with previous history of a pneumonia treated with amoxicillin-clavulanic acid and clarythromycin, presented in our emergency department with strabismus, ataxia for 3 days, later associated with vomiting and diarrhea. On admission she had ataxia and an episode of strabismus, but her later neurologic exam was normal. Laboratory data revealed: 10,9 g/dL hemoglobin, 11.200/μL leukocytes, 29,1% neutrophils and 65,2% lymphocytes, 488.000/μL platelets and negative CRP. The brain MRI showed middle ear, maxillary sinus and ethmoidal opacification, with no other abnormalities. During the first day of admission she had a tonic (?) seizure for 20 minutes. CSF analysis showed 5,6 cells/μL, 100% lymphocytes, 80 mg/dL glucose and 154,1 mg/dL protein. The EEG revealed short duration paroxystic activity located to the vertex. She was treated with acyclovir, ciprofloxacin, cefthriaxone and phenytoin. Her symptoms resolved by the third day of admission. Blood samples were tested for numerous pathogens, including serology for Borrelia, which was positive for IgG but negative for IgM. Fecal sample analysis revealed positive PCR for norovirus, although it was negative in CSF samples. IL-6 was measured in the CSF and was negative (5,8 pg/mL). She had a history of recurrent otitis media and pernieal candidiasis, which led to a detailed immune function study, which showed Immunology tests revealed diminished IgA (< 0,244 g/L) and absent antibody response to vaccinations. Since she was only 13 months old when she was tested, only follow up will determine the relevance of these values. Follow up at two years of age showed no delays and a normal development. Conclusion: Norovirus encephalitis is a rare entity, although gastrointestinal infection with this agent is relatively common. Here we present a case of a probable norovirus associated encephalopathy, although PCR for norovirus was negative in CSF samples and there was no CSF cytokine increase. It was not associated with adverse neurologic outcome and so far her development is normal, unlike the evolution described in previous case reports.

Estão raramente descritos na literatura casos de metástases uterinas de cancros extra genitais, sendo o aparelho genital feminino normalmente invadido por contiguidade. Mais frequentes são contudo, as metástases ováricas de neoplasias extragenitais sobretudo do aparelho gastrointestinal e mama. Descrevemos o caso clínico de uma doente de 71 anos que recorreu ao serviço de urgência por metrorragias pós menopausa (sem THS), tendo sido diagnosticado pólipo do endocolo e efectuada polipectomia. No exame anatomopatológico foi identificada, no eixo do pólipo, uma infiltração neoplásica compatível com metástase de carcinoma lobular da mama. A doente encontra-se desde há um ano a efectuar Quimioterapia com boa resposta.

Os doentes com cardiopatias submetidos a intervenção cirúrgica com implantação de condutos entre o ventrículo direito e a artéria pulmonar (VD-AP) podem desenvolver estenose e/ou insuficiência pulmonares ao longo do tempo. Nestas situações, associadas a arritmias cardíacas, disfunção ventricular direita e morte súbita, a re-intervenção cirúrgica permanece um desafio pela complexidade, mortalidade e morbilidade significativas. A implantação percutânea de válvulas pulmonares (IPVP), recentemente desenvolvida, representa uma abordagem alternativa para estes doentes. Objectivo: Reportar a experiência inicial com a IPVP, analisando o seu impacto no manejo destes doentes. Material e Métodos: Avaliação prospectiva dos dados clínicos, ecocardiográficos, da ressonância magnética, hemodinâmicos e angiográficos dos doentes submetidos a IPVP. Resultados: Seis doentes em classe funcional igual ou superior a II com disfunção do conduto VD-AP foram submetidos a IPVP. Tinham todos evidência de insuficiência e 5/6 de estenose. O procedimento consistiu na implantação de stents não cobertos no conduto para reforçar a sua rigidez e prevenir fracturas dos stents valvulados pulmonares Melody ® (Medtronic), que foram em seguida implantados com sucesso em todos. Obteve-se uma redução da pressão ventricular direita (94±27 para 44±7mmHg), da relação entre a pressão ventricular direita e esquerda (94±27 para 44±7%), do gradiente do conduto (65±28 para 11±4mmHg) e ausência de regurgitação pulmonar. O procedimento teve uma duração mediana de 180 minutos e decorreu sem complicações major. Os doentes tiveram alta em mediana dois dias após o procedimento. Na última avaliação, em mediana 7,8 meses após o procedimento, os doentes encontramse em classe funcional I (5) ou II (1), sem evidência não invasiva de disfunção do conduto. Conclusões: Os nossos resultados acompanham a experiência actual com esta modalidade terapêutica, com excelentes resultados nestes grupo de doentes complexos. A implementação da IPVP em doentes com cardiopatia congénita que requerem re-intervenção no tracto de saída do ventrículo direito tem-se revelado uma técnica promissora, embora complexa, alternativa a mais uma intervenção cirúrgica.

O enfisema lobar congénito é umamalformação rara do tracto respiratório inferior detectada habitualmente nos primeiros meses de vida. Apresenta-se o caso de um lactente, do sexo masculino, de dois meses de idade, internado por suspeita de pneumonia, no qual a segunda radiografia de tórax revelou hipertransparência arredondada no lobo inferior esquerdo, sugestiva de enfisema lobar congénito. A tomografia computorizada torácica mostrou uma imagem quística,multilobar, e a broncoscopia excluiu obstrução brônquica. Foi programada cirurgia para os seis meses de idade, mas aos três meses foi submetido a intervenção cirúrgica urgente por pneumotórax hipertensivo. O exame anatomo-patológico da peça operatória permitiu o diagnóstico de enfisema lobar congénito. Destaca-se a apresentação multilobar e a complicação por pneumotórax.

A Síndrome de Bartter é uma tubulopatia hereditária perdedora de sal, rara (cerca de 1,2 novos casos por 100 000 nados vivos por ano1), caracterizada por alcalose metabólica, hipocaliémia, hiperreninémia e hiperaldosterolémia de gravidade variável. A indometacina e elevadas doses de potássio oral têm sido até hoje as estratégias terapêuticas usadas, com elevado risco de lesão gastrointestinal. Desde Abril de 2009, o aliscireno – inibidor da renina – tem sido utilizado em casos pontuais de Síndrome de Bartter no adulto, desconhecendo-se a sua utilização em Pediatria. Os autores apresentam o caso clínico de uma criança de oito anos com Síndrome de Bartter, medicada com cloreto de potássio e indometacina oral, a quem foi diagnosticada uma úlcera gástrica gigante. De modo a permitir a redução da quantidade de potássio administrado, optou-se por iniciar o aliscireno.

BACKGROUND: Wireless capsule endoscopy has been introduced as an innovative, non-invasive diagnostic technique for evaluation of the gastrointestinal tract, reaching places where conventional endoscopy is unable to. However, the output of this technique is an 8 hours video, whose analysis by the expert physician is very time consuming. Thus, a computer assisted diagnosis tool to help the physicians to evaluate CE exams faster and more accurately is an important technical challenge and an excellent economical opportunity. METHOD: The set of features proposed in this paper to code textural information is based on statistical modeling of second order textural measures extracted from co-occurrence matrices. To cope with both joint and marginal non-Gaussianity of second order textural measures, higher order moments are used. These statistical moments are taken from the two-dimensional color-scale feature space, where two different scales are considered. Second and higher order moments of textural measures are computed from the co-occurrence matrices computed from images synthesized by the inverse wavelet transform of the wavelet transform containing only the selected scales for the three color channels. The dimensionality of the data is reduced by using Principal Component Analysis. RESULTS: The proposed textural features are then used as the input of a classifier based on artificial neural networks. Classification performances of 93.1% specificity and 93.9% sensitivity are achieved on real data. These promising results open the path towards a deeper study regarding the applicability of this algorithm in computer aided diagnosis systems to assist physicians in their clinical practice.

BACKGROUND: Upper gastrointestinal bleeding is the severe complication of stress-related mucosal disease in hospitalized patients. In intensive care units (ICU), risk factors are well defined and only mechanical ventilation and coagulopathy proved to be relevant for significant bleeding. On the contrary, in non-ICU settings there is no consensus about this issue. Nevertheless, omeprazole is still widely used in prophylaxis of bleeding. The objective of our study was to evaluate the relevance of stress-related mucosal disease bleeding in patients admitted to an internal medicine ward, and the role of omeprazole in its prophylaxis. METHODS: We conducted a retrospective study in which we analysed consecutive patients who were admitted to our ward over a year. We recorded demographic characteristics of the patients, potential risk factors for stress-related mucosal disease (clinical data, laboratory, and medication), administration of prophylactic omeprazole, and total cost of this prophylaxis. Patients with active gastrointestinal bleeding on the admission were excluded. We recorded every upper gastrointestinal bleeding event with clinical relevance. RESULTS: Five hundred and thirty-five patients, mean age 70 years, mean length of stay 9.6+/-7.7 days; 140 (26.2%) patients were treated with 40 mg of omeprazole intravenously, 193 (36.1%) with 20mg of omeprazole orally, and 202 (37.8%) patients had no prophylaxis. There was only one episode (0.2%) of clinically relevant bleeding. CONCLUSION: In patients admitted to an internal medicine ward, incidence of upper gastrointestinal bleeding as a complication of stress-related mucosal disease is low. We found that there is no advantage in prophylaxis with omeprazole.

Background and aims: Dysphagic patients who underwent endoscopic gastrostomy (PEG) usually present protein-energy malnutrition, but little is known about micronutrient malnutrition. The aim of the present study was the evaluation of serum zinc in patients who underwent endoscopic gastrostomy and its relationship with serum proteins, whole blood zinc, and the nature of underlying disorder. Methods: From patients that underwent gastrostomy a blood sample was obtained minutes before the procedure. Serum and whole blood zinc was evaluated using Wavelength Dispersive X-ray Fluorescence Spectroscopy. Serum albumin and transferrin were evaluated. Patients were studied as a whole and divided into two groups: head and neck cancer (HNC) and neurological dysphagia (ND). Results: The study involved 32 patients (22 males), aged 43-88 years: HNC = 15, ND = 17. Most (30/32) had low serum zinc, 17/32 presented normal values of whole blood zinc. Only two, with traumatic brain injury, presented normal serum zinc. Serum zinc levels showed no differences between HNC and ND patients. There was no association between serum zinc and serum albumin or transferrin. There was no association between serum and whole blood zinc. Conclusions: Patients had low serum zinc when gastrostomy was performed, similar in HNC and ND, being related with prolonged fasting and unrelated with the underlying disease. Decrease serum zinc was unrelated with low serum proteins. Serum zinc was more sensitive than whole blood zinc for identifying reduced zinc intake. Teams taking care of PEG-patients should include zinc evaluation as part of the nutritional assessment, or include systematic dietary zinc supply.

Em 1989 foi criada na Maternidade Dr. Alfredo da Costa uma consulta de diabetes e gravidez. Desde essa altura, 300 grávidas diabéticas foram vigiadas nesta consulta: 210 com diabetes gestacional e 90 com diabetes pré-gestacional. Os problemas obstétricos mais frequentemente encontrados foram a doença hipertensiva e as infecções do tracto génito-urinário. A prematuridade, a macrossomia e as anomalias congénitas foram as complicações mais importantes no grupo dos RN filhos de mãe com diabetes pré-gestacional e a macrossomia o principal problema nos filhos de mãe com diabetes gestacional.

Using 49 prostatectomized patients as experimental subjects, we studied the effects of Inclometnacin and acetylsalicylic acid — accredited prostaglandin synthetase inhibitors — from a urodynamic and clinical standpoint. Relevant urodynamic data was gathered 1 hr 30 mi after the patients had taken the drugs and placebo. Clinical results were further scrutinized after 8 days of treatment, at which time a new urodynamic workup was again performed on some patients. Results were again studied shortly after the end of treatment. The effect of the drugs on bladder and urethral structures was borne out by clear-ct!t clinical and urodynamic changes. After statistically analyzing such changes, we concluded that prostaglandin synthesis inhibition resulting in the inhibition of prostaglandin action had, at least in part, led to the changes noted. In the present report we shall discuss the role played by the highly complex mechanisms at work.

INTRODUCTION: Primary small bowel malignancy is unusual and accounts for 1-3% of all gastrointestinal tract neoplasms. Adenocarcinoma is one of the most common histologic types, but its frequency decreases with more distal locations. Its clinical presentation is nonspecific and is usually associated with advanced disease, which contributes to delayed diagnosis. PRESENTATION OF CASE: A 66-year-old woman was admitted to the hospital with a 6-day history of progressively worsening abdominal pain localized in the right lower quadrant, nausea, and vomiting. Investigation revealed an inflammatory appendiceal tumor. The patient underwent surgery and an unexpected tumor involving the distal ileal segment and ileocecal appendix was found. Right radical hemicolectomy with en bloc resection of the distal ileum was performed. Histopathological examination revealed adenocarcinoma of the ileum. DISCUSSION: This rare entity is associated with a nonspecific clinical presentation that contributes to delayed diagnosis and treatment, and consequently to a worse prognosis. Approximately half of the cases are only diagnosed at surgery. Primary treatment consists of wide resection with locoregional lymphadenectomy. The role of adjuvant chemotherapy has yet to be determined. CONCLUSION: This case demonstrates an unusual condition characterized by late and challenging diagnosis. We highlight the importance of an earlier diagnosis and optimal treatment for improved patient outcomes.

A enterocolite necrosante (ECN) constitui o problema gastrointestinal mais grave e mais frequente no recém-nascido (RN) de baixo peso. A melhoria na taxa de sobrevivência tem sido atribuída ao diagnóstico mais precoce e à experiência adquirida no tratamento do recém-nascido pré-termo em estado crítico. Desde 1977 que a drenagem peritoneal como actuação prioritária nos quadros de ECN tem sido preconizada nos recém- -nascidos de peso inferior a 1500 g com perfuração intestinal, e nos de peso superior a 1500 g com instabilidade hemodinâmica. Neste artigo relata-se o caso de um recém-nascido, com 1473 g de peso e 30 semanas de idade gestacional, ECN, sinais de perfuração intestinal e de instabilidade hemodinâmica, o qual foi submetido a drenagem peritoneal com evolução favorável e sem sequelas. Na discussão faz-se referência especial, de acordo com dados de literatura, aos mecanismos que explicam os bons resultados do procedimento em cerca de 2/3 dos casos de ECN com perfuração, os quais estão relacionados com as características particulares da cicatrização nos tecidos imaturos. Em conclusão, admite-se que a drenagem peritoneal deverá constituir a forma de actuação prioritária nos casos de ECN com perfuração e instabilidade hemodinâmica em RN pré-termo de muito baixo peso.

Intramural dissecting hematoma is an unusual esophageal condition with a threatening presentation but excellent prognosis when managed conservatively.We report the case of an 88-year-old woman who developed an intramural hematoma of the esophagus after intravenous thrombolysis for an acute ischemic stroke. Before thrombolysis, nasogastric intubation was attempted unsuccessfully. She was kept on nil by mouth, intravenous hydration, proton pump inhibitor, antiemetics,and an antibiotic initiated 2 days before for periodontal disease. The esophageal hematoma regressed, and she resumed oral diet asymptomatically.To our knowledge, this is the first report of this type of lesion after thrombolysis for an ischemic stroke. A brief discussion and literature review are presented.
