Os autores apresentam seis casos clínicos de doentes com urticária crónica idiopática (UCI) que não respondiam à terapêutica anti-histamínica convencional, necessitando de corticoterapia sistémica frequente para controlar as queixas. Após várias tentativas com outras terapêuticas, foram medicados com sulfassalazina. Efectuou-se uma análise deste grupo de doentes, procurando estabelecer uma avaliação do risco-benefício para a utilização da sulfassalazina na UCI, concluindo-se favoravelmente da sua utilização. Apresenta-se uma breve revisão dos mecanismos de acção, efeitos adversos e cuidados na utilização da sulfassalazina.

A anafilaxia idiopática, descrita pela primeira vez em 1978, é um diagnóstico de exclusão, sendo definida como uma forma de anafilaxia na qual, após investigação exaustiva, não é possível encontrar uma causa específica. Após o seu reconhecimento, foram descritos novos casos, publicadas várias séries de doentes, sendo actualmente uma entidade bem caracterizada, com um sistema de classificação e com um esquema de tratamento bem definido e eficaz. Apesar de ser potencialmente fatal, está associada a um bom prognóstico na maioria dos doentes. Um diagnóstico atempado, bem como a aplicação dos protocolos terapêuticos recomendados, resulta numa diminuição do número de episódios, do número de hospitalizações e dos custos associados.

Introduction. Pulmonary arterial hypertension (PAH) is a rare disease that must be managed in specialized centers; therefore, the availability of epidemiological national data is critical. Methods. We conducted a prospective, observational, and multicenter registry with a joint collaboration from five centers from Portugal and included adult incident patients with PAH or chronic thromboembolic pulmonary hypertension (CTEPH). Results. Of the 79 patients enrolled in this study, 46 (58.2%) were classified as PAH and 33 patients (41.8%) as CTEPH. PAH patients had a mean age of 43.4 ± 16.4 years. Idiopathic PAH was the most common etiology (37%). At presentation, PAH patients had elevated right atrial pressure (RAP) (7.7 ± 5.9mmHg) and mean pulmonary vascular resistance (11.4 ± 6.5 Wood units), with a low cardiac index (2.7 ± 1.1 L⋅min−1 m−2); no patient was under selective pulmonary vasodilators; however, at follow-up, most patients were on single (50%), double (28%), or triple (9%) combination vasodilator therapy. One-year survival was 93.5%, similar to CTEPH patients (93.9%), that were older (60.0 ± 12.5 years) and had higher RAP (11.0 ± 5.2mmHg,

Moyamoya disease is an idiopathic progressive steno-occlusive disorder of the intracranial arteries located at the base of the brain. It is associated with the development of compensatory extensive network of fine collaterals. Moyamoya disease is considered syndromic when certain genetic or acquired disorders such as polycystic kidney disease, neurofibromatosis, or meningitis are also present. Although the genetic contribution in moyamoya is indisputable, its cause and pathogenesis remain under discussion. Herein, we report a rare occurrence of moyamoya syndrome in two European Caucasian siblings in association with unusual multisystemic malformations (polycystic kidney disease in one, and intestinal duplication cyst in the other). The karyotype was normal. No mutation in the RFN213 gene was found, and none of the HLA types linked to moyamoya disease or described in similar familial cases were identified. By describing these multisystemic associations, polycystic kidney disease for the second time, and intestinal malformation for the first time in the literature, our report expands the phenotypic variability of moyamoya syndrome. The coexistence of disparate malformations among close relatives suggests an underlying common genetic background predisposing to structural or physiological abnormalities in different tissues and organs.

Objective: The Panayiotopoulos type of idiopathic occipital epilepsy has peculiar and easily recognizable ictal symptoms, which are associated with complex and variable spike activity over the posterior scalp areas. These characteristics of spikes have prevented localization of the particular brain regions originating clinical manifestations. We studied spike activity in this epilepsy to determine their brain generators. Methods: The EEG of 5 patients (ages 7–9) was recorded, spikes were submitted to blind decomposition in independent components (ICs) and those to source analysis (sLORETA), revealing the spike generators. Coherence analysis evaluated the dynamics of the components. Results: Several ICs were recovered for posterior spikes in contrast to central spikes which originated a single one. Coherence analysis supports a model with epileptic activity originating near lateral occipital area and spreading to cortical temporal or parietal areas. Conclusions: Posterior spikes demonstrate rapid spread of epileptic activity to nearby lobes, starting in the lateral occipital area. In contrast, central spikes remain localized in the rolandic fissure. Significance: Rapid spread of posterior epileptic activity in the Panayitopoulos type of occipital lobe epilepsy is responsible for the variable and poorly localized spike EEG. The lateral occipital cortex is the primary generator of the epileptic activity.

The Ponseti method is reportedly effective for treating clubfoot in children up to 9 years of age. However, whether age at the beginning of treatment influences the rate of successful correction and the rate of relapse is unknown. We therefore retrospectively reviewed 68 consecutive children with 102 idiopathic clubfeet treated by the Ponseti technique in four Portuguese hospitals. We followed patients a minimum of 30 months (mean, 41.4 months; range, 30–61 months). The patients were divided into two groups according to their age at the beginning of treatment; Group I was younger than 6 months and Group II was older than 6 months. All feet(100%) were initially corrected and no feet required extensive surgery regardless of age at the beginning of treatment. There were no differences between Groups I and II in the number of casts, tenotomies, success in terms of rate of initial correction, rate of recurrence, and rate of tibialis anterior transference. The rate of the Ponseti method in avoiding extensive surgery was 100% in Groups I and II; relapses occurred in 8% of the feet in younger and older children. Clinically childhood occipital lobe epilepsy (OLE) manifests itself with distinct syndromes. The traditional EEG recordings have not been able to overcome the difficulty in correlating the ictal clinical symptoms to the onset in particular areas of the occipital lobes. To understand these syndromes it is important to map with more precision the epileptogenic cortical regions in OLE. Experimentally, we studied three idiopathic childhood OLE patients with EEG source analysis and with the simultaneous acquisition of EEG and fMRI, to map the BOLD effect associated with EEG spikes. The spatial overlap between the EEG and BOLD results was not very good, but the fMRI suggested localizations more consistent with the ictal clinical manifestations of each type of epileptic syndrome. Since our first results show that by associating the BOLD effect with interictal spikes the epileptogenic areas are mapped to localizations different from those calculated from EEG sources and that by using different EEG/fMRI processing methods our results differ to some extent, it is very important to compare the different methods of processing the localization of activation and develop a good methodology for obtaining co-registration maps of high resolution EEG with BOLD localizations.

A Hematúria monossintomática idiopática é definida como a expressão clínica de uma doença glomerular que se reveste de uma grande variedade de alterações estruturais. São revistos os critérios de diagnóstico, os mecanismos patogénicos envolvidos, assim como os padrões histológicos encontrados. Ressalta-se a baixa agressividade da doença que poderá resultar de características próprias do sistema imunitário e acentua-se a dúvida quanto à sua benignidade sob o ponto de vista do prognóstico. A Biópsia Renal não está sistematicamente indicada devendo ser reservada para os casos cujos critérios selectivos são definidos. Em conclusão, sugere-se uma atitude de prudente expectativa no follow-up da doença, quer esperando o aparecimento de novos sinais que incriminem uma etiologia, quer observando a evolução da função renal e a sua eventual deterioração que exijam uma reavaliação da estratégia terapêutica.

Aseptic meningitis can be an adverse drug reaction to intravenous immunoglobulin. We describe a previously healthy 4-yearold boy, admitted for idiopathic thrombocytopaenic purpura. He received two infusions of intravenous immunoglobulin. Four hours after the last administration the patient developed a meningeal syndrome. Analysis of cerebrospinal fluid revealed 500 cells/μl (predominantly neutrophils) and normal biochemistry. Bacteriological and virological tests were negative. After 48h he was asymptomatic. Given the absence of other aetiological factors and the temporal relationship between the administration of immunoglobulin and the development of symptoms, we believe the patient had an aseptic meningitis related to intravenous immunoglobulin. This therapy may cause headache, fever and vomiting; however, lumbar puncture is not usually performed, so this complication may be underdiagnosed.

Introdução: O efeito dos fármacos inibidores da aromatase (IA) na estatura de jovens do sexo masculino com baixa estatura idiopática (BEI) tem vindo a ser estudado desde que foi evidenciado o papel dos estrogénios na paragem do crescimento linear. Os ensaios clínicos aleatorizados, duplamente cegos, de caso-controlo com letrozol indicam impacto positivo na previsão de estatura final em jovens do sexo masculino com BEI, com ou sem atraso constitucional do crescimento e puberdade. Por persistirem aspetos de segurança a requerer melhor estudo, a sua utilização terapêutica continua a ser off label. Objetivos: Tendo em vista a implementação de um ensaio clínico sobre a terapêutica com IA em jovens do sexo masculino com BEI procedeu-se a uma revisão sistemática da literatura, na qual se fundamenta a proposta de protocolo apresentada. Métodos: Pesquisa na base de dados eletrónica Medline de revisões sistemáticas, ensaios clínicos alea-torizados controlados e referências bibliográficas dos artigos seleccionados, publicados entre Janeiro de 2001 e Dezembro de 2012. Conclusões: A terapêutica com um IA de terceira geração (letrozol) poderá ser considerada em jovens do sexo masculino com BEI, altura inferior a pelo menos −2,0 SDS para a idade ou previsão de altura final pelo menos 2,0 SDS abaixo da estatura média parental, desde que já tenha sido iniciada a puberdade e a idade óssea seja inferior a 14 anos. Os principais aspectos de segurança ainda sob discussão na literatura referem-se a potenciais efeitos a nível ósseo. A utilização de medicamentos off-label deve obedecer a critérios estritos de prescrição e seguimento das crianças, de forma a minimizar os riscos e obter resultados fiáveis e comparáveis. Apresenta-se proposta de sistematização de monitorização clínica, imagiológica e laboratorial, bem como critérios de término ou suspensão da mesma.

Short-lasting headaches have been studied infrequently in children and it is not known if the main categories of primary headaches of this type in adults are applicable to children. We report our experience with a group of 20 children with a brief headache. Two patients had a secondary headache. One patient had a headache with some clinical characteristics of paroxysmal hemicrania. The remaining 17 had a very brief headache. They were in many aspects comparable to others from previous studies on idiopathic stabbing headache in children: no associated symptoms, no other associated headache, frequent family history of migraine. They differed, however, in the younger age of the patients and themore frequent extratrigeminal location of the pain. Extratrigeminal ice-pick pain may be a variant of idiopathic stabbing headache, more prevalent in young children.

OBJECTIVE: Mutations of the PROP1 gene lead to combined pituitary hormone deficiency (CPHD), which is characterized by a deficiency of GH, TSH, LH/FSH, PRL and, less frequently, ACTH. This study was undertaken to investigate the molecular defect in a cohort of patients with CPHD. DESIGN, PATIENTS AND MEASUREMENTS: A multicentric study involving 46 cases of CPHD (17 familial cases belonging to seven kindreds and 29 sporadic cases) selected on the basis of clinical and hormonal evidence of GH deficiency, central hypothyroidism and hypogonadotrophic hypogonadism, in the absence of an identified cause of hypopituitarism. Mutations of PROP1 were investigated by DNA sequencing. Clinical, hormonal and neuroradiological data were collected at each centre. RESULTS: PROP1 mutations were identified in all familial cases: five kindreds presented a c. 301-302delAG mutation, one kindred presented a c. 358C --> T (R120C) mutation and one presented a previously unreported initiation codon mutation, c. 2T --> C. Of the 29 sporadic cases, only two (6.9%) presented PROP1 germline mutations (c. 301-302delAG, in both). Phenotypic variability was observed among patients with the same mutations, particularly the presence and age of onset of hypocortisolism, the levels of PRL and the results of pituitary imaging. One patient presented a sellar mass that persisted into adulthood. CONCLUSIONS: This is the first report of a mutation in the initiation codon of the PROP1 gene and this further expands the spectrum of known mutations responsible for CPHD. The low mutation frequency observed in sporadic cases may be due to the involvement of other unidentified acquired or genetic causes.
