Resumo: A alimentação e o estado nutricional são factores determinantes do estado de saúde e sabe-se hoje que os mecanismos da patogénese de várias doenças crónicas não-transmissíveis podem ocorrer no início da idade adulta. A alimentação é influenciada por uma multiplicidade de factores, entre os quais se contam a importância atribuída à alimentação, o peso e imagem corporal e a percepção dos riscos associados à escolha de alimentos. Esta investigação teve como objectivos analisar, em estudantes universitários até aos 30 anos de idade, o estado nutricional, a importância atribuída a alimentação, as percepções do peso e da imagem corporal e a percepção do risco de doença relacionada com a alimentação. Os objectivos foram cumpridos através de dois desenhos de estudo distintos: estudo descritivo transversal e estudo de caso-controlo. Avaliou-se o peso, altura e os perímetros da cintura e da anca e construiu-se um questionário de auto-preenchimento para recolher a restante informação. Encontraram-se valores para a prevalência de obesidade e excesso de peso de, respectivamente, 6,5% e 24,3% e concluiu-se que existe uma percepção incorrecta do peso e da imagem corporal, mais frequente entre os indivíduos obesos. Os obesos também consideram a alimentação menos importante que os indivíduos normoponderais. A análise da percepção dos riscos revela que os inquiridos consideram que factores como a obesidade e a inactividade física são menos prejudiciais para a saúde do que factores como as alterações climáticas ou as radiações de telefones móveis. Verificaram-se também diferenças entre sexos nos parâmetros estudados: relativamente às mulheres, os homens sobrestimam mais frequentemente o peso e a imagem corporal, consideram a alimentação menos importante, julgam-se em menor risco de doença e classificam os factores de risco estudados como menos prejudiciais. Conclui-se que as estratégias de educação alimentar e de promoção da saúde devem considerar as diferenças registadas entre sexos e a importância atribuída à alimentação e as percepções do risco, do peso e da imagem corporal. Abstract: Nutrition and nutritional status are health determinants and it’s accepted that the mechanisms for the pathogenesis of several chronic non-communicable diseases can occur in early adult age. Nutrition is influenced by a large number of factors, including the value placed on food, weight and body image and the risk perception associated with food choice. Consequently, the analysis of the factors that can influence food behaviour and food choice in young adults can be useful for the control and prevention of nutrition related disease. The objectives of this research were to analyse, in college students up to 30 years of age, nutritional status, value placed on nutrition, weight and body image perceptions and the risk perception of nutrition related disease. Two study designs were used: cross-sectional and case-control. Weight, height and waist and hip circumference were measured and a questionnaire was built to collect the remaining information. Prevalences of 6,5% for obesity and 24,3% for excess weight were found, along with the existence of biased weight and body image perceptions, more frequent in obese subjects. Obese subjects also placed less value on nutrition than non-obese. Risk perception analysis shows that risk factors like obesity and physical inactivity are considered less hazardous than risk factors like climate changes and mobile phone radiation. Men, comparatively to women, overestimated more frequently their weight and body image, placed less value in nutrition, considered themselves less disease susceptible and classified the risk factors studied as less hazardous. The conclusions of this study show that nutrition education and health promotion strategies should consider the gender related differences reported and, also, the value placed on nutrition and weigh, body image and risk perceptions.

RESUMO - A segurança dos doentes é, actualmente, um princípio fundamental dos cuidados de saúde. Apesar desta realidade, cada ponto no processo de prestação de cuidados contém um certo grau de insegurança inerente. Os eventos adversos podem resultar de problemas na prática, produtos, processos ou sistemas. A melhoria da segurança do doente implica um esforço de todo um sistema complexo, envolvendo uma ampla gama de acções de melhoria do desempenho, segurança e gestão ambiental de risco, incluindo, entre outras prioridades, a prática de cuidados de saúde em ambiente seguro e com a melhor qualidade. É sabido que todos os anos morrem nos hospitais portugueses, cerca de três mil pessoas, devido a erros cometidos pelos profissionais de saúde. Compreender o erro humano e intervir de forma adequada nas suas causas, implica, muito mais que uma avaliação do facto em si, mas uma avaliação de toda a estrutura onde é passível de ocorrer um erro. São inúmeros os factores que estão relacionados com esta temática, sendo os factores humanos, aqueles que poderão merecer alguma reflexão, pois são ainda algo omitidos nas orientações de gestão e nas considerações de chefias. Assim sendo, pretende-se que, com este projecto de investigação, através de uma revisão da literatura, sejam identificados os vários factores humanos que influenciam o desempenho dos profissionais de saúde, envolvidos na prestação directa de cuidados. Estes factores reflectem-se na segurança dos cuidados de saúde e na melhoria da qualidade prestados nas instituições de saúde. Face aos resultados conclusivos após a revisão de literatura, é verificado que os factores humanos que influenciam o desempenho dos profissionais de saúde são: comunicação e partilha de informação; conhecimento e formação; liderança; trabalho em equipa; cultura de segurança e aprendizagem; carga de trabalho; exigências físicas e ambiente de trabalho. É esperado que o contributo deste trabalho promova o conhecimento em torno desta temática e que se apresente como o início de uma aposta na valorização, cada vez mais importante, mas ainda limitada, destes valores intangíveis da gestão – os factores humanos. ----------------------------------- ABSTRACT - Patient safety is, nowadays, a fundamental principle of health care. Despite this fact, each point in the process of care contains an inherent degree of uncertainty. Adverse events may result from problems in practice, products, processes or systems. Improving patient safety requires an effort of a whole complex system, involving a wide range of measures to improve performance, safety and environmental risk management, including, among other priorities, the practice of health care in a safe environment and the best quality. It is known that every year die, in Portuguese hospitals, around three thousand people, due to mistakes made by health professionals. To understand human error and intervene as appropriate in their causes, it is required much more than an assessment of the fact itself, but an assessment of the whole structure where it is likely to occur. There are countless factors that are related to this issue, but human factors, those who may deserve some consideration, are still left out in management guidelines and supervisors considerations. Therefore, this research project seeks to determine, through a literature review, the identification of various human factors that influence the performance of healthcare providers involved in direct provision of care. The aim is also to know which factors are reflected in safety healthcare and how they are, in fact, crucial in the performance of professionals and in the promotion of quality and safety of the healthcare provided in healthcare institutions. Given the conclusive results after a review of literature, it is found that the human factors that influence the performance of health professionals are: communicating and sharing information, knowledge and training, leadership, teamwork, safety culture and learning; workload, physical demands and work environment. It is expected that this project may contribute to advance of the knowledge regarding this problem and be an essential tool to the beginning of a reliance on the development, increasingly important but still limited, of these intangibles management values – the human factors.

Pollution in coastal ecosystems is a serious threat to the biota and human populations there residing. Anthropogenic activities in these ecosystems are the main cause of contamination by endocrine disruption compounds (EDCs), which can interfere with hormonal regulation and cause adverse effects to growth, stress response and reproduction. Although the chemical nature of many EDCs is unknown, it is believed that most are organic contaminants. Under an environmental risk assessment for a contaminated estuary (the Sado, SW Portugal), the present work intended to detect endocrine disruption in a flatsfish, Solea senegalensis Kaup, 1858, and its potential relationship to organic toxicants. Animals were collected from two areas in the estuary with distinct influences (industrial and rural) and from an external reference area. To evaluate endocrine disruption, hepatic vitellogenin (VTG) concentrations in males and gonad histology were analysed. As biomarkers of exposure to organic contaminants, cytochrome P450 (CYP1A) induction and the ethoxyresorufin-O-deethylase (EROD) activity were determined. The results were contrasted to sediment contamination levels, which are overall considered low, although the area presents a complex mixture of toxicants. Either males or females were found sexually immature and showed no significant evidence of degenerative pathologies. However, hepatic VTG concentrations in males from the industrial area in estuary were superior than those from the Reference, even reaching levels comparable to those in females, which may indicate an oestrogenic effect resulting from the complex contaminant mixture. These individuals also presented higher levels of CYP1A induction and EROD activity, which is consistent with contamination by organic substances. The combination of the results suggest that the exposure of flatfish to an environment contaminated by mixed toxicants, even at low levels, may cause endocrine disruption, therefore affecting populations, which implies the need for further research in identification of potential EDCs, their sources and risks at ecosystem scale.

RESUMO - O quadro legislativo de um país, no que concerne aos resíduos hospitalares (RH), contém a sua designação, definição e classificação. É essa a matriz de referência para a separação efectuada na origem e todo o circuito que, a partir desse momento, um determinado resíduo toma até ao seu tratamento. Assim, faz-se o estudo comparativo das definições e tipos de classificação de RH em quatro países da União Europeia: Alemanha, Reino Unido, Espanha (Região Autónoma da Catalunha) e Portugal. Reconhecem-se as diferentes designações deste tipo de resíduos e discute-se o seu significado e as suas implicações na percepção de risco por parte dos profissionais e do público. Identificam-se duas estratégias subjacentes à elaboração das definições: a contaminação de materiais com microrganismos patogénicos bem definidos, as suas fontes e as actividades que os produzem. Apresentam-se as classificações de RH propostas pelos organismos internacionais de referência e analisa-se comparativamente a evolução do enquadramento legal português e da Região Autónoma da Catalunha, evidenciando-se a variabilidade temporal e justificando-se a necessidade de se efectuar o estudo da variabilidade geográfica. Utilizam-se três critérios para a análise das classificações consideradas: a concordância definição-classificação, o número e tipo de grupos das classificações e os tipos de resíduos por grupos. Identificam-se os denominadores comuns às classificações analisadas, assim como as suas principais diferenças. Conclui-se que a definição de RH adoptada por cada país condiciona o tipo de classificação de RH nesse mesmo país. Verifica-se ainda que a inexistência de critérios claros de avaliação da contaminação pode dificultar a tarefa da triagem dos RH por parte dos profissionais de saúde.

Phosphorus is a macronutrient essential to life which comes from phosphate rock, a non-renewable resource. Sewage sludge from wastewater treatment plants (WWTP) is a secondary resource rich in phosphorus that can be valorized. However, organic compounds are detected in sewage sludge, due to its non-polar and hydrophobic character, being considered an environmental risk. The present dissertation aims to study the efficiency of the electrodialytic process (ED) when applied to sewage sludge aiming phosphorus recovery and organic contaminants removal. Four organic compounds were analyzed: 17α-ethynylestradiol (EE2), bisphenol A (BPA), caffeine (Caf) and oxybenzone (MBPh). The experiments took place in an ED cell with two compartments and an anion exchange membrane, with the sludge in the cathode compartment. The experiments were carried out for three days with spiked sewage sludge (six assays). One control experiment was done without current, three experiments were carried out applying a constant current of 50, 75, and 100 mA and two experiments were carried out applying sequential currents: 50 mA, 75 mA and 100 mA and the opposite (100-75-50 mA). A qualitative and quantitative analysis of microorganisms existing in the samples was also done. At the end, the pH increased in the sewage sludge favoring phosphorus recovery. In terms of phosphorus, the highest recovery was achieved in the experiment run with 100 mA, where 70.3±2.0% of total phosphorus was recovered in the electrolyte. Generally, compounds degradation was favored by the current. Caf and MBPh achieved degradation percentages of 96.2±0.2% and 84.8±1.3%, respectively, in 100 mA assay. EE2 (83.1±1.7%) and BPA (91.8±4.6%) degradations were favored by 50 mA current. A total of 35 taxa from four different groups were identified, totalizing between 81,600-273,000 individuals per gram of initial sludges. After ED, microbial community population decreased between 47-98%. Arcella gibbosa represented 61% of the total observed organisms and revealed to be more tolerant to medium changes.

Estuaries are perhaps the most threatened environments in the coastal fringe; the coincidence of high natural value and attractiveness for human use has led to conflicts between conservation and development. These conflicts occur in the Sado Estuary since its location is near the industrialised zone of Peninsula of Setúbal and at the same time, a great part of the Estuary is classified as a Natural Reserve due to its high biodiversity. These facts led us to the need of implementing a model of environmental management and quality assessment, based on methodologies that enable the assessment of the Sado Estuary quality and evaluation of the human pressures in the estuary. These methodologies are based on indicators that can better depict the state of the environment and not necessarily all that could be measured or analysed. Sediments have always been considered as an important temporary source of some compounds or a sink for other type of materials or an interface where a great diversity of biogeochemical transformations occur. For all this they are of great importance in the formulation of coastal management system. Many authors have been using sediments to monitor aquatic contamination, showing great advantages when compared to the sampling of the traditional water column. The main objective of this thesis was to develop an estuary environmental management framework applied to Sado Estuary using the DPSIR Model (EMMSado), including data collection, data processing and data analysis. The support infrastructure of EMMSado were a set of spatially contiguous and homogeneous regions of sediment structure (management units). The environmental quality of the estuary was assessed through the sediment quality assessment and integrated in a preliminary stage with the human pressure for development. Besides the earlier explained advantages, studying the quality of the estuary mainly based on the indicators and indexes of the sediment compartment also turns this methodology easier, faster and human and financial resource saving. These are essential factors to an efficient environmental management of coastal areas. Data management, visualization, processing and analysis was obtained through the combined use of indicators and indices, sampling optimization techniques, Geographical Information Systems, remote sensing, statistics for spatial data, Global Positioning Systems and best expert judgments. As a global conclusion, from the nineteen management units delineated and analyzed three showed no ecological risk (18.5 % of the study area). The areas of more concern (5.6 % of the study area) are located in the North Channel and are under strong human pressure mainly due to industrial activities. These areas have also low hydrodynamics and are, thus associated with high levels of deposition. In particular the areas near Lisnave and Eurominas industries can also accumulate the contamination coming from Águas de Moura Channel, since particles coming from that channel can settle down in that area due to residual flow. In these areas the contaminants of concern, from those analyzed, are the heavy metals and metalloids (Cd, Cu, Zn and As exceeded the PEL guidelines) and the pesticides BHC isomers, heptachlor, isodrin, DDT and metabolits, endosulfan and endrin. In the remain management units (76 % of the study area) there is a moderate impact potential of occurrence of adverse ecological effects and in some of these areas no stress agents could be identified. This emphasizes the need for further research, since unmeasured chemicals may be causing or contributing to these adverse effects. Special attention must be taken to the units with moderate impact potential of occurrence of adverse ecological effects, located inside the natural reserve. Non-point source pollution coming from agriculture and aquaculture activities also seem to contribute with important pollution load into the estuary entering from Águas de Moura Channel. This pressure is expressed in a moderate impact potential for ecological risk existent in the areas near the entrance of this Channel. Pressures may also came from Alcácer Channel although they were not quantified in this study. The management framework presented here, including all the methodological tools may be applied and tested in other estuarine ecosystems, which will also allow a comparison between estuarine ecosystems in other parts of the globe.

Estuaries and other transitional waters are complex ecosystems critically important as nursery and shelter areas for organisms. Also, humans depend on estuaries for multiple socio-economical activities such as urbanism, tourism, heavy industry, (taking advantage of shipping), fisheries and aquaculture, the development of which led to strong historical pressures, with emphasis on pollution. The degradation of estuarine environmental quality implies ecologic, economic and social prejudice, hence the importance of evaluating environmental quality through the identification of stressors and impacts. The Sado Estuary (SW Portugal) holds the characteristics of industrialized estuaries, which results in multiple adverse impacts. Still, it has recently been considered moderately contaminated. In fact, many studies were conducted in the past few years, albeit scattered due to the absence of true biomonitoring programmes. As such, there is a need to integrate the information, in order to obtain a holistic perspective of the area able to assist management and decision-making. As such, a geographical information system (GIS) was created based on sediment contamination and biomarker data collected from a decade-long time-series of publications. Four impacted and a reference areas were identified, characterized by distinct sediment contamination patterns related to different hot spots and diffuse sources of toxicants. The potential risk of sediment-bound toxicants was determined by contrasting the levels of pollutants with available sediment quality guidelines, followed by their integration through the Sediment Quality guideline Quotient (SQG-Q). The SQG-Q estimates per toxicant or class was then subjected to georreferencing and statistical analyses between the five distinct areas and seasons. Biomarker responses were integrated through the Biomarkers Consistency Indice and georreferenced as well through GIS. Overall, in spite of the multiple biological traits surveyed, the biomarker data (from several organisms) are accordant with sediment contamination. The most impacted areas were the shipyard area and adjacent industrial belt, followed by urban and agricultural grounds. It is evident that the estuary, although globally moderately impacted, is very heterogeneous and affected by a cocktail of contaminants, especially metals and polycyclic aromatic hydrocarbon. Although elements (like copper, zinc and even arsenic) may originate from the geology of the hydrographic basin of the Sado River, the majority of the remaining contaminants results from human activities. The present work revealed that the estuary should be divided into distinct biogeographic units, in order to implement effective measures to safeguard environmental quality.
