In the present work, we studied a common outbreaking Lepidoptera species in Portuguese pine stands – Thaumetopoea pityocampa (Den. & Schiff.) - and one of its potential predators – Parus major (L.). The population dynamics of the immature stages of the Lepidoptera was studied in several types of Pinus pinaster (Aiton) plantations in three different areas: Setúbal Peninsula, Abrantes and National Pine Forest of Leiria. Location and plantation structure was the most important factors determining population density of T. pityocampa. Setubal and Abrantes was highly susceptible to attacks by the Lepidoptera, whereas Leiria had lower densities. Young and homogeneous pine stands was more susceptible to attacks than older and more heterogeneous pines stands. However, a desynchronized population of T. pityocampa, in which the larvae develops during summer instead of during winter, reached high densities also in Leiria. The impact of several mortality factors and climatic conditions on the immature stages of the insect (eggs and larvae), in normal and desynchronized populations are discussed, as well as possible evolutionary implications of the sudden appearance of the new version of T. pityocampa. The break of the pupa diapause and adult emergence times the annual life cycle of this insect. Adults from the desynchronized population emerged earlier than adults from the normal population, which in turn determined the change in the larvae development period. Different factors, potentially affecting the timing of adult emergence in both normal and abnormal populations are also discussed. To study P. Major, nest-boxes were placed in the areas of Setúbal and Leiria and they were monitored during three seasons. The nest-boxes increased the density of breeding and wintering birds in the studied pine plantations, indicating that a lack of natural holes are in fact a limiting factor for this populations. The earliest breeding start for this species was recorded in my study area, indicating that Portuguese coastal pines provide good breeding conditions earlier than in other areas of Europe and North Africa. This leads to an overlap between the end of the larvae stage of T. pityocampa and the beginning of the breeding season of P. major. The aim of this article is to characterize musculoskeletal disorders (MSDs) in EU working population according to available Eurostat data, to identify relevant risk factors and to refer existing legislation and standards to prevent MSDs. The following questions will be answered: How often do MSDs occur in EU? What are the risk factors? Do legislation and standards exist to prevent these disorders?

During the recent years human society evolved from the “industrial society age” and transitioned into the “knowledge society age”. This means that knowledge media support migrated from “pen and paper” to computer-based Information Systems. Due to this fact Ergonomics has assumed an increasing importance, as a science/technology that deals with the problem of adapting the work to the man, namely in terms of Usability. This paper presents some relevant Ergonomics, Usability and User-centred design concepts regarding Information Systems.

Resumo: A decisão da terapêutica hormonal no tratamento do cancro da mama baseiase na determinação do receptor de estrogénio alfa por imunohistoquímica (IHC). Contudo, a presença deste receptor não prediz a resposta em todas as situações, em parte devido a limitações do método IHC. Investigámos se a expressão dos genes ESR1 e ESR2, bem como a metilação dos respectivos promotores, pode estar relacionada com a evolução desfavorável de uma proporção de doentes tratados com tamoxifeno assim como com a perda dos receptores de estrogénio alfa (ERα) e beta (ERß). Amostras de 211 doentes com cancro da mama diagnosticado entre 1988 e 2004, fixadas em formalina e preservadas em parafina, foram utilizadas para a determinação por IHC da presença dos receptores ERα e ERß. O mRNA total do gene ESR1 e os níveis específicos do transcrito derivado do promotor C (ESR1_C), bem como dos transcritos ESR2_ß1, ESR2_ß2/cx, and ESR2_ß5 foram avaliados por Real-time PCR. Os promotores A e C do gene ESR1 e os promotores 0K e 0N do gene ESR2 foram investigados por análise de metilação dos dinucleotidos CpG usando bisulfite-PCR para análise com enzimas de restrição, ou para methylation specific PCR. Atendendo aos resultados promissores relacionados com a metilação do promotor do gene ESR1, complementamos o estudo com um método quantitativo por matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) suportado pelo software Epityper para a medição da metilação nos promotores A e C. Fez-se a avaliação da estabilidade do mRNA nas linhas celulares de cancro da mama MCF-7 e MDA-MB-231 tratadas com actinomicina D. Baixos níveis do transcrito ESR1_C associaram-se a uma melhor sobrevivência global (p = 0.017). Níveis elevados do transcrito ESR1_C associaram-se a uma resposta inferior ao tamoxifeno (HR = 2.48; CI 95% 1.24-4.99), um efeito mais pronunciado em doentes com tumores de fenótipo ERα/PgR duplamente positivo (HR = 3.41; CI 95% 1.45-8.04). A isoforma ESR1_C mostrou ter uma semi-vida prolongada, bem como uma estrutura secundária da região 5’UTR muito mais relaxada em comparação com a isoforma ESR1_A. A análise por Western-blot mostrou que ao nível da 21 proteína, a selectividade de promotores é indistinguivel. Não se detectou qualquer correlação entre os níveis das isoformas do gene ESR2 ou entre a metilação dos promotores do gene ESR2, e a detecção da proteína ERß. A metilação do promotor C do gene ESR1, e não do promotor A, foi responsável pela perda do receptor ERα. Estes resultados sugerem que os níveis do transcrito ESR1_C sejam usados como um novo potencial marcador para o prognóstico e predição de resposta ao tratamento com tamoxifeno em doentes com cancro da mama. Abstract: The decision of endocrine breast cancer treatment relies on ERα IHC-based assessment. However, ER positivity does not predict response in all cases in part due to IHC methodological limitations. We investigated whether ESR1 and ESR2 gene expression and respective promoter methylation may be related to non-favorable outcome of a proportion of tamoxifen treated patients as well as to ERα and ERß loss. Formalin-fixed paraffin-embedded breast cancer samples from 211 patients diagnosed between 1988 and 2004 were submitted to IHC-based ERα and ERß protein determination. ESR1 whole mRNA and promoter C specific transcript levels, as well as ESR2_ß1, ESR2_ß2/cx, and ESR2_ß5 transcripts were assessed by real-time PCR. ESR1 promoters A and C, and ESR2 promoters 0N and 0K were investigated by CpG methylation analysis using bisulfite-PCR for restriction analysis, or methylation specific PCR. Due to the promising results related to ESR1 promoter methylation, we have used a quantification method by matrixassisted laser desorption/ionization time-of-flight mass spectrometry (MALDITOF MS) together with Epityper software to measure methylation at promoters A and C. mRNA stability was assessed in actinomycin D treated MCF-7 and MDA-MB-231 cells. ERα protein was quantified using transiently transfected breast cancer cells. Low ESR1_C transcript levels were associated with better overall survival (p = 0.017). High levels of ESR1_C transcript were associated with non-favorable response in tamoxifen treated patients (HR = 2.48; CI 95% 1.24-4.99), an effect that was more pronounced in patients with ERα/PgR double-positive tumors (HR = 3.41; CI 95% 1.45-8.04). The ESR1_C isoform had a prolonged mRNA half-life and a more relaxed 5’UTR structure compared to ESR1_A isoform. Western-blot analysis showed that at protein level, the promoter selectivity is undistinguishable. There was no correlation between levels of ESR2 isoforms or ESR2 promoter methylation and ERß protein staining. ESR1 promoter C CpG methylation and not promoter A was responsible for ERα loss. We propose ESR1_C levels as a putative novel marker for breast cancer prognosis and prediction of tamoxifen response.

RESUMO - Enquadramento/objectivos: Apesar do elevado nível de comprometimento em estratégias eficazes para o controlo da tuberculose, em todo o mundo, esta constitui ainda um sério problema de Saúde Pública, com uma estimativa global de 9,4milhões de casos novos em 2008 e 1,8milhões de mortes/ano. O reduzido conhecimento das barreiras e facilitadores para o sucesso terapêutico constitui um importante obstáculo na procura de soluções eficazes de melhoramento da qualidade dos programas de controlo da tuberculose. Este estudo procura contribuir para a identificação atempada de doentes com perfis preditivos de insucesso terapêutico, através da identificação inicial de potenciais determinantes do resultado, com base num modelo epidemiológico e estatístico. Métodos: Foi desenvolvido um estudo de caso-controlo para a população de casos notificados ao Programa Nacional de Controlo da Tuberculose (n=24491), entre 2000-2007. Os factores preditivos de insucesso terapêutico foram identificados na análise bivariada e multivariada, com um nível de significância de 5%; a regressão logística foi utilizada para estimar a odds ratio de insucesso terapêutico, em comparação com o sucesso terapêutico, para diversos factores identificados na literatura, e para os quais os dados se encontravam disponíveis. Resultados: A dependência alcoólica (OR=2,889), o país de origem (OR=3,910), a situação sem-abrigo (OR=3.919), a co-infecção pelo VIH (OR=5,173), a interrupção (OR=60.615) ou falha terapêutica no tratamento anterior (OR=67.345) e a duração do tratamento inferior a 165 dias (OR=1930,133) foram identificados como factores preditivos de insucesso terapêutico. A duração do tratamento inferior a 165 dias provou ser o mais importante determinante do resultado terapêutico. Conclusões: Os resultados sugerem que um doente imigrante, em situação de sem-abrigo, dependente alcoólico, com tratamentos anteriores para a tuberculose e co-infectado pelo VIH apresenta uma elevada probabilidade de insucesso terapêutico. Assim, deverão ser definidas estratégias específicas, centradas no doente por forma a impedir este resultado. A base de dados (SVIG-TB), provou ser uma ferramenta de qualidade para a investigação sobre diversos aspectos do controlo da tuberculose. ------------------------------- ABSTRACT - Background/Objective: Despite the high commitment in good strategies for tuberculosis control worldwide, this is still a serious Public Health problem, with global estimates of 9,4million new cases in 2008 and 1,8million deaths/year. The poor understanding of the barriers and facilitators to treatment success is a major obstacle to find effective solutions to improve the quality of tuberculosis programs. This study tries to contribute to the timely identification of patients with predictive profiles of unsuccessful treatment outcomes, through the initial identification of characteristics probably affecting treatment outcome, found on the basis of an epidemiological and statistical model. Methods: A case-control study was conducted for the population of cases notified to the National Program for Tuberculosis Control (n=24 491), between 2000-2007. Predictive factors for unsuccessful outcome were assessed in a bivariate and multivariate analysis, using a significance level of 5%; a logistic regression was used to estimate the odds-ratio of unsuccessful, as compared to successful outcome, for several factors identified in the literature and to which data was available. Results: Alcohol abuse (OR=2,889), patient´s foreign origin (OR=3,910), homelessness (OR=3,919), HIV co-infection (OR=5,173), interruption (OR=60,615) or unsuccessful outcome in the previous treatment (OR=67,345) and treatment duration below 165 days (OR=1930,133) were identified as predictive of unsuccessful outcomes. A low treatment duration proved to be the most powerful factor affecting treatment outcome. Conclusions: Results suggest that a foreign-born patient, alcohol abuser, who has had a previous treatment for tuberculosis and is co-infected with HIV is very likely to have an unsuccessful outcome. Therefore, specific, patient-centered strategies should be taken to prevent an unsuccessful outcome. The database (SVIG-TB), has proved to be a quality tool on research of various aspects of tuberculosis control.
