This paper incorporates egocentric comparisons into a human capital accumulation model and studies the evolution of positive self image over time. The paper shows that the process of human capital accumulation together with egocentric comparisons imply that positive self image of a cohort is first increasing and then decreasing over time. Additionally, the paper finds that positive self image: (1) peaks earlier in activities where skill depreciation is higher, (2) is smaller in activities where the distribution of income is more dispersed, (3) is not a stable characteristic of an individual, and (4) is higher for more patient individuals.

There has been an increase in the use of telephone-based services and internet throughout the years and, therefore, the Saúde 24 Hotline has become an important service in Portugal. This service aims to screen, counsel and refer the patient in order to avoid unnecessary visits to health institutions and also to indicate the most appropriate resource according to the illness. This work has two different questions: the first one examines the determinants of satisfaction that have more influence on the overall satisfaction of the Saúde 24 Hotline users. The second one aims to analyze if the confidence level of the users is increasing over time, measured by following the recommendation. The first study was conducted on a random sample collected from June to October 2014, which was taken from the User Satisfaction Survey. The second approach includes data from January 2008 to December 2014 from the Clinical Data Base of all users who have called the Hotline. Findings suggest that the majority of users are very satisfied with the service and the variables with more impact on the overall satisfaction are commitment and availability from the nurse, adequacy of call duration and quick identification of the problem. The survey indicates that 94% of respondents follow the recommendation and on average people have called the hotline 3 times in the previous year. The results from the Clinical Database show that people who were recommended to go to the emergency room are more likely to follow the advice than the people who were recommended to book routine appointments

RESUMO: Enthesitis is the hallmark of spondyloarthritis (SpA), and is observed in all subtypes. Wide information on SpA abnormalities, including synovitis, tendinitis and enthesitis, can be efficiently perceived by Doppler ultrasound. Furthermore, several studies on imaging of enthesis showed that imaging techniques are better than clinical examination to detect enthesis alterations; and vascularized enthesitis detected by Doppler ultrasound appears to be a valuable diagnostic tool to confirm SpA diagnosis. However, data published until now concerning entheseal elementary alterations that characterize SpA enthesitis (enthesis inflammatory activity) or enthesopathy (permanent structural changes) reflect rather the authors’ empiric opinion than a methodological validation process. In this sense it seems crucial to identify elementary entheseal lesions associated with activity or damage, in order to improve monitoring and treatment response in SpA patients. The development of better assessment tools is today a challenge and a need in SpA. The first study of this thesis focused on the analysis of the reliability of inter-lector and inter-ultrasonography equipment of Madrid sonography enthesitis index (MASEI). Fundamental data for the remaining unrolling project validity. In the second and third studies we concerned about two entheseal elemental lesions: erosions and bursa. In literature erosions represent a permanent structural damage, being useful for monitoring joint injury, disease activity and therapeutic response in many rheumatic diseases; and to date, this concept has been mostly applied in rheumatoid arthritis (RA). Unquestionably, erosion is a tissue-related damage and a structural change. However, the hypothesis that we decided to test was if erosions represent a permanent structural change that can only grow and worsen over time, as occurs in RA, or a transitory alteration. A longitudinal study of early SpA patients was undertaken, and the Achilles enthesis was used as a model. Our results strongly suggested that previously detected erosions could disappear during the course of the disease, being consistent with the dynamic behavior of erosion over time. Based on these striking results it seems reasonable to suggest that the new-bone formation process in SpA could be associated with the resolution of cortical entheseal erosion over time. These results could also be in agreement with the apparent failure of anti-tumor necrosis factor (TNF) therapies to control bone proliferation in SpA; and with the relation of TNF-α, Dickkopf-related protein 1 (Dkk-1) and the regulatory molecule of the Wnt signaling pathway in the bone proliferation in SpA. In the same model, we then proceeded to study the enthesis bursa. Interestingly, the Outcome Measures in Rheumatology Clinical Trials (OMERACT) enthesopathy definition does not include bursa as an elementary entheseal lesion. Nonetheless, bursa was included in 46% of the enthesis studies in a recently systematic literature review, being in agreement with the concept of “synovio-entheseal complex” that includes the link between enthesitis and osteitis in SpA. It has been clarified in recent data that there is not only a close functional integration of the enthesis with the neighboring bone, but also a connection between enthesitis and synovitis. Therefore, we tried to assess the prevalence and relevance of the bursa-synovial lesion in SpA. Our findings showed a significant increase of Achilles bursa presence and thickness in SpA patients compared to controls (healthy/mechanical controls and RA controls). These results raise awareness to the need to improve the enthesopathy ultrasonographic definition. In the final work of this thesis, we have explored new perspectives, not previously reported, about construct validity of enthesis ultrasound as a possible activity outcome in SpA. We performed a longitudinal Achilles enthesis ultrasound study in patients with early SpA. Achilles ultrasound examinations were performed at baseline, six- and twelve-month time periods and compared with clinical outcome measures collected at basal visit. Our results showed that basal erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are higher in patients with Doppler signal in enthesis, and even that higher basal ESR, CRP and Ankylosing Spondylitis Disease Activity Score (ASDAS) predicted a higher Doppler signal (an ultrasound alteration accepted as representative of inflammation) six months later. Patients with very high disease activity assessed by ASDAS (>3.5) at baseline had significantly higher Achilles total ultrasound score verified at the same time; and ASDAS <1.3 predicted no Doppler signal at six and twelve months. This seems to represent a connection between classical biomarkers and clinical outcomes associated with SpA activity and Doppler signal, not only at the same time, but also for the following months. Remarkably, patients with inactive disease (ASDAS < 1.3) at baseline had no Doppler signal at six and twelve months. These findings reinforce the potential use of ultrasound related techniques for disease progression assessment and prognosis purposes. Intriguingly, Ankylosing Spondylitis Disease Activity Index (BASDAI) didn’t show significant differences between different cut-offs concerning ultrasound lesions or Doppler signal, while verified with ASDAS. These results seem to indicate that ASDAS reflects better than BASDAI what happens in the enthesis. The work herein discussed clearly shows the potential utility of ultrasound in enthesis assessment in SpA patients, and can be important for the development of ultrasound activity and structural damage scores for diagnosis and monitoring purposes. Therefore, local promotion of this technique constitutes a medical intervention that is worth being tested in SpA patients for diagnosis, monitoring and prognosis purposes.

Resumo: A cárie dentária, um problema que tem atingido populações em grande parte do mundo, é a doença de maior prevalência da cavidade oral, gerando graves consequências económicas e sociais. Esta doença tem sido estudada ao longo do tempo em diferentes países com o emprego de diversos índices, geralmente para o estudo da sua prevalência, a avaliação de medidas preventivas e o adequado planeamento das acções e serviços de saúde oral. O objectivo deste Projecto foi determinar se a escovagem quando realizada na escola com pasta fluoretada, e supervisionada pelos professores, duas vezes por dia, seria ou não eficaz na diminuição das populações microbianas de Streptococcus mutans e Lactobacillus e na consequente diminuição da incidência de cárie dentária. Material e métodos: Foram seleccionadas todas as crianças (universo = 178), com idades compreendidas entre os 5, 6 e 7 anos, residentes no Concelho de Aljustrel e a frequentar o pré-escolar e o primeiro ciclo do ensino básico oficial. Foi realizada a escovagem bi-diária com pasta fluoretada a 500 ppm F-, na escola, segundo o método de Bass modificado, supervisionado pelos professores titulares de turma, que tiveram formação da técnica de escovagem utilizada. Durante os 3 anos de estudo foram realizadas 6 observações dentárias e recolhas salivares para contagem de Streptococcus mutans e Lactobacillus e avaliação da capacidade tampão da saliva. Resultados: O grupo de estudo no início da intervenção apresentava valores dos índices de cárie dentária mais elevados do que os do grupo de controlo (mais 0,109 no CPO-S, 0,0749 no CPO-D, 1,505 no cpo-s e 0,831 no cpo-d), porém sem diferenças de significância estatística. A análise estatística dos resultados não veio confirmar este pressuposto uma vez que o grupo de estudo apresentou um aumento percentual ligeiramente maior do índice CPO-D (12,5%) do que o grupo de controlo (11,6%). Para além deste aspecto, ao contrário do que seria de esperar, não foi possível detectar nenhuma diferença estatisticamente significativa em nenhum dos índices de cárie dentária (cpo-s, cpo-d, CPO-S e CPO-D) entre o grupo de estudo e grupo de controlo entre a 1ª e última observação.Ainda que os resultados do estudo aqui apresentado tenham ficado aquém do esperado, deveria ser efectuada a escovagem diária na escola, uma vez por dia, com pasta fluoretada a 1000 ppm F-, atendendo a que esta medida contribui para a promoção da saúde e prevenção da doença e é facilitadora da construção de estilos de vida saudáveis.-------ABSTRACT: Dental caries, a problem that has affected populations worldwide, is one of the most prevalent diseases of the oral cavity, causing severe economic and social consequences. This disease has been studied over time in different countries with the use of various indices, usually for the knowledge of its prevalence, evaluation of preventive measures and appropriate planning of actions and oral services. The aim of this study was to determine whether toothbrushing when performed in schools, with fluoride toothpaste, and supervised by teachers twice a day, was effective in reducing microbial populations of Streptococcus mutans and Lactobacillus with consequent reduction in the incidence of dental caries. Material and Methods: All children aged 5, 6, and 7 years, from Aljustrel County, attending official pre-school and first cycle of basic educatio were selected. Toothbrushing was performed twice a day with toothpaste with 500 ppm F-,in the school, according to the modified Bass method, supervised by professors in the class, who were trained in the brushing technique used. During the study period were performed 6 observations of the dental status, and were also collected saliva for the count of Streptococcus mutans and Lactobacillus, and assessment of buffering capacity of saliva. Results: The study group at the beginning of the intervention had higher values of dental caries than the control group (more than 0,109 in DMF-S, 0, 0749 in DMF, dmf-s 1,505 and 0,831 in dmf-t) although without statistical significance. The expected results were not confirmed, since the study group had a slightly higher percentage increase of the DMF-T (12,5%) than the control group (11,6%). Apart from that, contrary to what one would expect, we could not detect any statistical significant difference in any of the indices of dental caries (dmf-s, dmf-t and DMF-S, DMF-T) between the study and the control group in all study periods. Although the study results were not has expected, toothbrushing should be performed daily at school, once a day with fluoride toothpaste with 1000 ppm F-, since this measure contributes to health promotion and disease prevention and encourages healthy lifestyles.

ABSTRACT - It is the purpose of the present thesis to emphasize, through a series of examples, the need and value of appropriate pre-analysis of the impact of health care regulation. Specifically, the thesis presents three papers on the theme of regulation in different aspects of health care provision and financing. The first two consist of economic analyses of the impact of health care regulation and the third comprises the creation of an instrument for supporting economic analysis of health care regulation, namely in the field of evaluation of health care programs. The first paper develops a model of health plan competition and pricing in order to understand the dynamics of health plan entry and exit in the presence of switching costs and alternative health premium payment systems. We build an explicit model of death spirals, in which profitmaximizing competing health plans find it optimal to adopt a pattern of increasing relative prices culminating in health plan exit. We find the steady-state numerical solution for the price sequence and the plan’s optimal length of life through simulation and do some comparative statics. This allows us to show that using risk adjusted premiums and imposing price floors are effective at reducing death spirals and switching costs, while having employees pay a fixed share of the premium enhances death spirals and increases switching costs. Price regulation of pharmaceuticals is one of the cost control measures adopted by the Portuguese government, as in many European countries. When such regulation decreases the products’ real price over time, it may create an incentive for product turnover. Using panel data for the period of 1997 through 2003 on drug packages sold in Portuguese pharmacies, the second paper addresses the question of whether price control policies create an incentive for product withdrawal. Our work builds the product survival literature by accounting for unobservable product characteristics and heterogeneity among consumers when constructing quality, price control and competition indexes. These indexes are then used as covariates in a Cox proportional hazard model. We find that, indeed, price control measures increase the probability of exit, and that such effect is not verified in OTC market where no such price regulation measures exist. We also find quality to have a significant positive impact on product survival. In the third paper, we develop a microsimulation discrete events model (MSDEM) for costeffectiveness analysis of Human Immunodeficiency Virus treatment, simulating individual paths from antiretroviral therapy (ART) initiation to death. Four driving forces determine the course of events: CD4+ cell count, viral load resistance and adherence. A novel feature of the model with respect to the previous MSDEMs is that distributions of time to event depend on individuals’ characteristics and past history. Time to event was modeled using parametric survival analysis. Events modeled include: viral suppression, regimen switch due virological failure, regimen switch due to other reasons, resistance development, hospitalization, AIDS events, and death. Disease progression is structured according to therapy lines and the model is parameterized with cohort Portuguese observational data. An application of the model is presented comparing the cost-effectiveness ART initiation with two nucleoside analogue reverse transcriptase inhibitors (NRTI) plus one non-nucleoside reverse transcriptase inhibitor(NNRTI) to two NRTI plus boosted protease inhibitor (PI/r) in HIV- 1 infected individuals. We find 2NRTI+NNRTI to be a dominant strategy. Results predicted by the model reproduce those of the data used for parameterization and are in line with those published in the literature.

RESUMO: A confiabilidade e qualidade dos resultados clínicos de um laboratório, estão dependentes da qualidade do equipamento onde estes são processados. A qualidade de um equipamento consiste em apresentar resultados com uma confiabilidade metrológica de acordo com os processos de medição realizados. Desta forma, os equipamentos de medição devem conter características metrológicas de acordo com os requisitos dos processos que estão inseridos, tais como: erro máximo, incerteza, resolução… Todos os equipamentos por muito bons que sejam, apresentam perdas das suas características metrológicas ao longo do tempo, o que se torna importante a determinação periódica das suas calibrações, manutenções e o seu respectivo ajuste. A periodicidade das calibrações, devem ser baseadas, em critérios bem estabelecidos de modo a evitar a ocorrência da não conformidade, devido às medições com os equipamentos fora das suas especificações, garantindo-se assim, a confiabilidade metrológica e o aumento da produtividade devido ao controlo dos processos, evitando-se desta forma qualquer tipo de avaria por parte destes. Os normativos de Qualidade e Certificação estabelecem como requisito a calibração dos equipamentos de medição em intervalos adequados, mas não definem claramente qual o intervalo adequado. O importante é gerir as etapas de calibração e manutenção de acordo com os equipamentos de modo a garantir que os resultados emitidos por estes sejam mantidos em condições confiáveis, dentro do período entre calibrações e manutenções, de modo a que nãoocorra uma condição de falha, e por conseguinte, medições com grande quantidade de erros.------------ ABSTRACT: The reliability and quality of the results of a clinical laboratory, are dependent on the quality of the equipment where they are processed. The quality of a product is to present results with a metrological reliability according to the measuring processes carried out. Thus, measuring equipment must contain metrological characteristics according to the requirements of the processes that are included, such as: maximum error, uncertainty, resolution ... All equipment for very good they are, show losses of its metrological characteristics over time, it becomes important to determine its periodic calibrations, maintenance and its adjustment. The frequency of calibrations should be based on well-established criteria in order to avoid the occurrence of non-compliance due to the measurements with the equipment beyond its specifications, thus ensuring the metrological reliability and increased productivity due to the control of processes, thus avoiding any kind of damage on the part thereof. The normative and Quality Certification as a requirement to establish calibration of measuring equipment at appropriate intervals, but not clearly stipulate the proper range. It is important to manage the steps of calibration and maintenance equipment according to ensure that the results emitted by these conditions are maintained in trusted within the period between calibrations and maintenance, sothat there occurs a fault condition, and therefore, large amount of measurements with errors.
