RESUMO - O objetivo deste estudo foi conhecer o comportamento face ao consumo de tabaco reportado pelos adolescentes escolarizados da ilha Terceira e caracterizar alguns fatores associados. O estudo realizado assumiu uma metodologia de investigação observacional, descritiva, quantitativa e de carácter transversal. Os dados foram recolhidos no ano letivo 2012/2013, nos alunos do 9.º ano de escolaridade, através da aplicação de um questionário de autopreenchimento, anónimo e voluntário, em contexto de sala de aula. A amostra foi constituída por 323 adolescentes, 142 do sexo masculino e 181 do sexo feminino, com idade média de 14,71 anos. Verificou-se que cerca de 16,9% dos adolescentes consome tabaco regularmente, 6,6% ocasionalmente e 76,6% não fuma. Mais de metade dos adolescentes já experimentou fumar tabaco (56%), sendo a idade média de experimentação de 12,49 anos. A maioria dos adolescentes que experimentaram fumar referiram tê-lo feito por curiosidade (84,7%) e, sobretudo, na companhia dos amigos (83,1%). As variáveis associadas ao comportamento tabágico foram: o desempenho escolar, os hábitos tabágicos dos amigos, a permissividade dos pais para se fumar em casa, a frequência à exposição ao tabaco em casa, os conhecimentos da “impotência sexual” como consequência do consumo de tabaco, conhecimento dos danos na saúde em frequentar ambientes com fumo e a crença de que os jovens fumadores são mais atraentes. Os resultados mostram a elevada necessidade de serem desenvolvidas estratégias de prevenção e controlo do tabagismo entre os adolescentes da ilha Terceira.

VII - CONSIDERAÇÕES FINAIS Este estudo desenvolveu-se devido facto de tanto quanto nos é dado a conhecer não terem sido determinados cutoff points de diferenciação entre fumadores e não fumadores em Portugal. Segundo vários autores é importante que estes valores sejam determinados em cada país devido às suas particularidades de prevalência do tabagismo e exposição involuntária ao fumo do tabaco. Foram também estudadas as variáveis que estão associadas e que influenciam os níveis do COex, da COHb, da cotinina plasmática e da cotinina urinária. De forma a cumprir os objectivos definidos, utilizou-se uma amostra de 102 indivíduos (53 fumadores e 49 não fumadores), tendo sido o tipo de amostragem não probabilística de conveniência. A dimensão da amostra e o tipo de amostragem são duas das limitações do estudo, uma vez que esta amostra não é representativa da população fumadora e não fumadora portuguesa, o que impede de generalizar os resultados nela obtidos à totalidade da população. Outra limitação identificada é o facto de não se terem encontrado estudos nacionais de características semelhantes a esta investigação, o que faz com que não seja possível comparar os resultados obtidos com estudos que tivessem utilizado amostras idênticas. Por essa razão a discussão de resultados foi efectuada considerando estudos internacionais cujas suas amostras para além de serem de maior dimensão os seus indivíduos possuem características particulares (prevalência do tabagismo e exposição involuntária ao fumo do tabaco) que dificultam a comparação dos resultados. Nos estudos internacionais muitas vezes não são estudadas as variáveis caracterizadoras dos hábitos tabágicos, o que leva mais uma vez à impossibilidade de relacionar correctamente os nossos resultados com outros obtidos por diferentes autores. Devido ao interesse dos resultados obtidos, seria importante que este estudo, tivesse continuidade, mas com algumas modificações que foram impossíveis de realizar no contexto em que se desenvolveu esta investigação. De forma a generalizar os resultados seria ideal utilizar uma amostra representativa da população fumadora e não fumadora nacional, o que permitiria ficar a conhecer mais fidedignamente os cutoff points de diferenciação entre fumadores e não fumadores em Portugal. Esta informação seria de grande utilidade para caracterizar correctamente os hábitos tabágicos dos indivíduos, permitindo reconhecer em que casos é necessário intervir para cessar os hábitos tabágicos e ainda nas consultas de cessação tabágica para um apropriado follow-up dos sujeitos. Outro aspecto em que esta investigação poderia ser complementada, diz respeito às variáveis caracterizadoras dos hábitos tabágicos, isto porque de acordo com a análise de regressão linear multivariada, através do modelo proposto, uma grande percentagem da variabilidade dos marcadores biológicos de tabagismo ficou por explicar, assim sendo seria importante estudar também outras variáveis, e construir outro modelo que explicasse de forma ainda mais expressiva a variabilidade dos biomarcadores. Sugerem-se como outras variáveis a ser estudas aquelas que estão relacionadas com as características inalatórias de cada indivíduo (número, frequência, profundidade, duração e percentagem de cigarros consumido e desperdiçado). Também a exposição involuntária ao fumo do tabaco deveria ser melhor caracterizada, acrescentando às variáveis analisadas neste estudo, outras que caracterizassem de forma mais precisa o tipo de exposição a que os indivíduos podem estar sujeitos no seu dia a dia (domiciliária, laboral e ambiental). Idealmente deveriam conhecer-se o número de cigarros que são fumados na proximidade dos indivíduos, assim como saber quais as características dos espaços fechados sobretudo no que respeita às condições de ventilação. É importante que surjam trabalhos a nível nacional relacionados com os marcadores de tabagismo, porque o tabagismo representa um problema de saúde pública sobre o qual é necessário intervir. É importante caracterizar correctamente os biomarcadores de tabagismo de forma a determinar a real prevalência do tabagismo e avaliar medidas preventivas e políticas de saúde pública. Actualmente decorridos 3 anos da aprovação da Lei 32/2007 em que foram aprovadas as normas de protecção dos cidadãos da exposição involuntária ao fumo do tabaco, a avaliação dos marcadores biológicos pode desempenhar um papel preponderante para caracterizar o impacto da implicação da Lei, uma vez que estes reflectem a exposição activa e passiva ao fumo do tabaco.

ABSTRACT – Background: According to the Report on Carcinogens, formaldehyde ranks 25th in the overall U.S. chemical production, with more than 5 million tons produced each year. Given its economic importance and widespread use, many people are exposed to formaldehyde environmentally and/or occupationally. Presently, the International Agency for Research on Cancer classifies formaldehyde as carcinogenic to humans (Group 1), based on sufficient evidence in humans and in experimental animals. Manyfold in vitro studies clearly indicated that formaldehyde can induce genotoxic effects in proliferating cultured mammalian cells. Furthermore, some in vivo studies have found changes in epithelial cells and in peripheral blood lymphocytes related to formaldehyde exposure. Methods: A study was carried out in Portugal, using 80 workers occupationally exposed to formaldehyde vapours: 30 workers from formaldehyde and formaldehyde-based resins production factory and 50 from 10 pathology and anatomy laboratories. A control group of 85 non-exposed subjects was considered. Exposure assessment was performed by applying simultaneously two techniques of air monitoring: NIOSH Method 2541 and Photo Ionization Detection equipment with simultaneously video recording. Evaluation of genotoxic effects was performed by application of micronucleus test in exfoliated epithelial cells from buccal mucosa and peripheral blood lymphocytes. Results: Time-weighted average concentrations not exceeded the reference value (0.75 ppm) in the two occupational settings studied. Ceiling concentrations, on the other hand, were higher than reference value (0.3 ppm) in both. The frequency of micronucleus in peripheral blood lymphocytes and in epithelial cells was significantly higher in both exposed groups than in the control group (p < 0.001). Moreover, the frequency of micronucleus in peripheral blood lymphocytes was significantly higher in the laboratories group than in the factory workers (p < 0.05). A moderate positive correlation was found between duration of occupational exposure to formaldehyde (years of exposure) and micronucleus frequency in peripheral blood lymphocytes (r = 0.401; p < 0.001) and in epithelial cells (r = 0.209; p < 0.01). Conclusions: The population studied is exposed to high peak concentrations of formaldehyde with a long-term exposure. These two aspects, cumulatively, can be the cause of the observed genotoxic endpoint effects. The association of these cytogenetic effects with formaldehyde exposure gives important information to risk assessment process and may also be used to assess health risks for exposed worker

RESUMO - Introdução: Actualmente 11,3 milhões de indivíduos estão co-infectados pela Tuberculose/Vírus Imunodeficiência Humana (TB/VIH), uma das principais causas de incapacidade e morte no mundo. É determinada pela exposição dos indivíduos aos factores de risco e condições/determinantes sociais de saúde. Várias são as medidas criadas a nível nacional e internacional na luta contra TB e a infecção VIH. Objectivo: Caracterizar e comparar os casos de TB entre os indivíduos não infectados com VIH e os infectados com VIH, considerando as características sócio-demográficas, o tratamento, patologias associadas e factores de risco. Método: Estudo descritivo, quantitativo e observacional. A informação foi obtida a partir da base de dados do Sistema Nacional de Vigilância Epidemiológica da Tuberculose dos casos de TB notificados entre 1 de Janeiro de 2008 a 31 de Dezembro de 2009. Para tratamento e análise estatística (descritiva e inferencial) o programa usado foi o SPSS versão 18,0. Resultados: 12,8% dos indivíduos estavam co-infectados com TB/VIH e 87,2% não estavam co-infectados. A presença de VIH nos casos de tuberculose apresenta evidência de relação com quase todas variáveis em estudo (p<0,00) excepto a presença de insuficiência renal (p<0,307). Apresentam maior probalidade de risco da co-infecção TB/VIH os homens, a faixa etária [35;44[, os estrangeiros, os desempregados, estar em retratamento e fumar. Os indivíduos com Doença Hepática (OR= 5,238; IC95%: 3,706;7,403; ORA = 3,104; IC95%: 2,164;4,454), patologias associadas (OR=13,199; IC95%: 11,246; 15,491; ORA=21,348; IC95%:17,569; 25,940) e factores de risco (OR=3,237; IC95%: 2,968; 3,531; ORA=2,644; IC95%: 2,414; 2,985) tem maior probalidade da co-infecção TB/VIH. O ajustamento para o sexo e a idade interferiu em todas variáveis em estudo. Conclusão: Os homens, da faixa etária [35;44 [, desempregados, estrangeiros, em retratamento, fumadores apresentam maior probalidade de risco de estar co-infectado com TB/VIH.

ABSTRACT - The problem of how to support “intentions to make behavioural changes” (IBC) and “behaviour changes” (BC) in smoking cessation when there is a scarcity of resources is a pressing issue in public health terms. The present research focuses on the use of information and communications technologies and their role in smoking cessation. It is developed in Portugal after the ratification of WHO Framework Convention on Tobacco Control (on 8 November 2005). The prevalence of smokers over fifteen years of age within the population stood at 20.9% (30.9% for men and 11.8% for women). While the strategy of helping people to quit smoking has been emphasised at National Health Service (NHS) level, the uptake of cessation assistance has exceeded the capacity of the service. This induced the search of new theoretical and practical venues to offer alternative options to people willing to stop smoking. Among these, the National Health Plan (NHP) of Portugal (2004-2010), identifies the use of information technologies in smoking cessation. eHealth and the importance of health literacy as a means of empowering people to make behavioural changes is recurrently considered an option worth investigating. The overall objective of this research is to understand, in the Portuguese context, the use of the Internet to help people to stop smoking. Research questions consider factors that may contribute to “intentions to make behavioural changes” (IBC) and “behavioural changes” (BC) while using a Web-Assisted Tobacco Intervention Probe (WATIP). Also consideration is given to the trade-off on the use of the Web as a tool for smoking cessation: can it reach a vast number of people for a small cost (efficiency) demonstrating to work in the domain of smoking cessation (efficacy)”? In addition to the introduction, there is a second chapter in which the use of tobacco is discussed as a public health menace. The health gains achieved by stopping smoking and the means of quitting are also examined, as is the use of the Internet in smoking cessation. Then, several research issues are introduced. These include background theory and the theoretical framework for the Sense of Coherence. The research model is also discussed. A presentation of the methods, materials and of the Web-Assisted Tobacco Intervention Probe (WATIP) follows. In chapter four the results of the use of the Web-Assisted Tobacco Intervention Probe (WATIP) are presented. This study is divided into two sections. The first describes results related to quality control in relation to the Web-Assisted Tobacco Intervention Probe (WATIP) and gives an overview of its users. Of these, 3,150 answered initial eligibility questions. In the end, 1,463 met all eligibility requirements, completed intake, decided on a day to quit smoking (Dday) and declared their “intentions to make behavioural changes” (IBC) while a second targeted group of 650 did not decide on a Dday. With two quit attempts made before joining the platform, most of the participants had experienced past failures while wanting to stop. The smoking rate averaged 21 cigarettes per day. With a mean age of 35, of the participants 55% were males. Among several other considerations, gender and the Sense of Coherence (SOC) influenced the success of participants in their IBC and endeavour to set quit dates. The results of comparing males and females showed that, for current smokers, establishing a Dday was related to gender differences, not favouring males (OR=0.76, p<0.005). Belonging to higher Socio-economic strata (SES) was associated with the intention to consider IBC (when compared to lower SES condition) (OR=1.57, p<0.001) and higher number of school years (OR=0.70, p<0.005) favoured the decision to smoking cessation. Those who demonstrated higher confidence in their likelihood of success in stopping in the shortest time had a higher rate of setting a Dday (OR=0.51, p<0.001). There were differences between groups in IBC reflecting the high and low levels of the SOC score (OR=1.43, p=0.006), as those who considered setting a Dday had higher levels of SOC. After adjusting for all variables, stages of readiness to change and SOC were kept in the model. This is the first Arm of this research where the focus is a discussion of the system’s implications for the participants’ “intentions to make behavioural changes” (IBC). Moreover, a second section of this study (second Arm) offers input collected from 77 in-depth interviews with the Web-Assisted Tobacco Intervention Probe (WATIP) users. Here, “Behaviour Change” (BC) and the usability of the platform are explored a year after IBC was declared. A percentage of 32.9% of self-reported, 12-month quitters in continuous abstinence from smoking from Dday to the 12-month follow- up point of the use of the Web-Assisted Tobacco Intervention Probe (WATIP) has been assessed. Comparing the Sense of Coherence (SOC) scores of participants by their respective means, according to the two groups, there was a significant difference in these scores of non smokers (BC) (M=144,66, SD=22,52) and Sense of Coherence (SOC) of smokers (noBC) (M=131,51, SD=21,43) p=0.014. This WATIP strategy and its contents benefit from the strengthening of the smoker’s sense of coherence (SOC), so that the person’s progress towards a life without tobacco may be experienced as comprehensible, manageable and meaningful. In this sample the sense of coherence (SOC) effect is moderate although it is associated with the day to quit smoking (Dday). Some of the limitations of this research have to do with self-selection bias, sample size (power) and self-reporting (no biochemical validation). The enrolment of participants was therefore not representative of the smoking population. It is not possible to verify the Web-Assisted Tobacco Intervention Probe (WATIP) evaluation of external validity; consequently, the results obtained cannot be applied generalized. No participation bias is provided. Another limitation of this study is the associated limitations of interviews. Interviewees’ perception that fabricating answers could benefit them more than telling the simple truth in response to questions is a risk that is not evaluated (with no external validation like measuring participants’ carbon monoxide levels). What emerges in this analysis is the relevance of the process that leads to the establishment of the quit day (Dday) to stop using tobacco. In addition, technological issues, when tailoring is the focus, are key elements for scrutiny. The high number of dropouts of users of the web platform mandates future research that should concentrate on the matters of the user-centred design of portals. The focus on gains in health through patient-centred care needs more research, so that technology usability be considered within the context of best practices in smoking cessation.

The main objective of this thesis was the development of a gold nanoparticle-based methodology for detection of DNA adducts as biomarkers, to try and overcome existing drawbacks in currently employed techniques. For this objective to be achieved, the experimental work was divided in three components: sample preparation, method of detection and development of a model for exposure to acrylamide. Different techniques were employed and combined for de-complexation and purification of DNA samples (including ultrasonic energy, nuclease digestion and chromatography), resulting in a complete protocol for sample treatment, prior to detection. The detection of alkylated nucleotides using gold nanoparticles was performed by two distinct methodologies: mass spectrometry and colorimetric detection. In mass spectrometry, gold nanoparticles were employed for laser desorption/ionisation instead of the organic matrix. Identification of nucleotides was possible by fingerprint, however no specific mass signals were denoted when using gold nanoparticles to analyse biological samples. An alternate method using the colorimetric properties of gold nanoparticles was employed for detection. This method inspired in the non-cross-linking assay allowed the identification of glycidamide-guanine adducts and DNA adducts generated in vitro. For the development of a model of exposure, two different aquatic organisms were studies: a goldfish and a mussel. Organisms were exposed to waterborne acrylamide, after which mortality was recorded and effect concentrations were estimated. In goldfish, both genotoxicity and metabolic alterations were assessed and revealed dose-effect relationships of acrylamide. Histopathological alterations were verified primarily in pancreatic cells, but also in hepatocytes. Mussels showed higher effect concentrations than goldfish. Biomarkers of oxidative stress, biotransformation and neurotoxicity were analysed after prolonged exposure, showing mild oxidative stress in mussel cells, and induction of enzymes involved in detoxification of oxygen radicals. A qualitative histopathological screening revealed gonadotoxicity in female mussels, which may present some risk to population equilibrium.

This paper studies operating profitability drivers in the Four Main Tobacco Manufacturers for the period 2004-2014. The operating profitability is analyzed as return on assets (ROA) based on the DuPont Extended Model breakdown in degree of operational risk, gross sales margin and assets turnover. The sources of ROA are market share and price strategies appraised through the drivers: firm-size, global value and strategic choices. Using consolidated data, results suggest that firm-size and global value holds a positive relationship with ROA. Also innovation through less harmful tobacco products can lead to better ROA despite no correlation between R&D and ROA.
