Estuaries are perhaps the most threatened environments in the coastal fringe; the coincidence of high natural value and attractiveness for human use has led to conflicts between conservation and development. These conflicts occur in the Sado Estuary since its location is near the industrialised zone of Peninsula of Setúbal and at the same time, a great part of the Estuary is classified as a Natural Reserve due to its high biodiversity. These facts led us to the need of implementing a model of environmental management and quality assessment, based on methodologies that enable the assessment of the Sado Estuary quality and evaluation of the human pressures in the estuary. These methodologies are based on indicators that can better depict the state of the environment and not necessarily all that could be measured or analysed. Sediments have always been considered as an important temporary source of some compounds or a sink for other type of materials or an interface where a great diversity of biogeochemical transformations occur. For all this they are of great importance in the formulation of coastal management system. Many authors have been using sediments to monitor aquatic contamination, showing great advantages when compared to the sampling of the traditional water column. The main objective of this thesis was to develop an estuary environmental management framework applied to Sado Estuary using the DPSIR Model (EMMSado), including data collection, data processing and data analysis. The support infrastructure of EMMSado were a set of spatially contiguous and homogeneous regions of sediment structure (management units). The environmental quality of the estuary was assessed through the sediment quality assessment and integrated in a preliminary stage with the human pressure for development. Besides the earlier explained advantages, studying the quality of the estuary mainly based on the indicators and indexes of the sediment compartment also turns this methodology easier, faster and human and financial resource saving. These are essential factors to an efficient environmental management of coastal areas. Data management, visualization, processing and analysis was obtained through the combined use of indicators and indices, sampling optimization techniques, Geographical Information Systems, remote sensing, statistics for spatial data, Global Positioning Systems and best expert judgments. As a global conclusion, from the nineteen management units delineated and analyzed three showed no ecological risk (18.5 % of the study area). The areas of more concern (5.6 % of the study area) are located in the North Channel and are under strong human pressure mainly due to industrial activities. These areas have also low hydrodynamics and are, thus associated with high levels of deposition. In particular the areas near Lisnave and Eurominas industries can also accumulate the contamination coming from Águas de Moura Channel, since particles coming from that channel can settle down in that area due to residual flow. In these areas the contaminants of concern, from those analyzed, are the heavy metals and metalloids (Cd, Cu, Zn and As exceeded the PEL guidelines) and the pesticides BHC isomers, heptachlor, isodrin, DDT and metabolits, endosulfan and endrin. In the remain management units (76 % of the study area) there is a moderate impact potential of occurrence of adverse ecological effects and in some of these areas no stress agents could be identified. This emphasizes the need for further research, since unmeasured chemicals may be causing or contributing to these adverse effects. Special attention must be taken to the units with moderate impact potential of occurrence of adverse ecological effects, located inside the natural reserve. Non-point source pollution coming from agriculture and aquaculture activities also seem to contribute with important pollution load into the estuary entering from Águas de Moura Channel. This pressure is expressed in a moderate impact potential for ecological risk existent in the areas near the entrance of this Channel. Pressures may also came from Alcácer Channel although they were not quantified in this study. The management framework presented here, including all the methodological tools may be applied and tested in other estuarine ecosystems, which will also allow a comparison between estuarine ecosystems in other parts of the globe.

Portugal had only very few foresight exercises on the automobile sector, and the most recent one was a survey held in a project on work organisation systems in the automobile industry, its recent historical paths and the special strategies of location of companies (the WorTiS project). This involved several teams with different disciplinary backgrounds and from two Portuguese universities. The provisional main results of the first round of a Delphi survey held in Portugal on the automotive sector were already published, but a further analysis was not yet done. This foresight survey was done under the WorTiS project, developed in 2004 by IET – Research Centre on Enterprise and Work Innovation (at FCT-UNL), and financed by the Portuguese Ministry of Science and Technology. Some of this experience on foresight analysis is also been transferred to other projects, namely the WORKS project on work organisation restructuring in the knowledge society that received the support from EC and still is running. The majority of experts considered having an average of less knowledge in almost all the scenario topics presented. This means that information on the automotive industry is not spread enough among academics or experts in related fields (regional scientists, innovation economists, engineers, sociologists). Some have a good knowledge but in very specialised fields. Others have expertise on foresight, or macroeconomics, or management sciences, but feel insecure on issues related with futures of automobile sector. Nevertheless, we considered specially the topics where the experts considered themselves to have some knowledge. There were no “irrelevant” topics considered as such by the expert panel. There are also no topics that are not considered a need for co-operation. The lack of technological infrastructures was not considered as a hindered factor for the accomplishment of any scenario. The experts’ panel considered no other international competence besides US, Japan or Germany in these topics. Special focus will be made in this paper on the topic 2. Public policy and automobile industries, and more specifically on the technological and/or research policies issues, where one can specify the automobile’s role in transport policies with further implications like environment, safety, energy, mobility.

RESUMO - Introdução - Com o presente projecto de investigação pretendeu-se estudar o financiamento por capitação ajustado pelo risco em contexto de integração vertical de cuidados de saúde, recorrendo particularmente a informação sobre o consumo de medicamentos em ambulatório como proxy da carga de doença. No nosso país, factores como a expansão de estruturas de oferta verticalmente integradas, inadequação histórica da sua forma de pagamento e a recente possibilidade de dispor de informação sobre o consumo de medicamentos de ambulatório em bases de dados informatizadas são três fortes motivos para o desenvolvimento de conhecimento associado a esta temática. Metodologia - Este trabalho compreende duas fases principais: i) a adaptação e aplicação de um modelo de consumo de medicamentos que permite estimar a carga de doença em ambulatório (designado de PRx). Nesta fase foi necessário realizar um trabalho de selecção, estruturação e classificação do modelo. A sua aplicação envolveu a utilização de bases de dados informatizadas de consumos com medicamentos nos anos de 2007 e 2008 para a região de Saúde do Alentejo; ii) na segunda fase foram simulados três modelos de financiamento alternativos que foram propostos para financiar as ULS em Portugal. Particularmente foram analisadas as dimensões e variáveis de ajustamento pelo risco (índices de mortalidade, morbilidade e custos per capita), sua ponderação relativa e consequente impacto financeiro. Resultados - Com o desenvolvimento do modelo PRx estima-se que 36% dos residentes na região Alentejo têm pelo menos uma doença crónica, sendo a capacidade de estimação do modelo no que respeita aos consumos de medicamentos na ordem dos 0,45 (R2). Este modelo revelou constituir uma alternativa a fontes de informação tradicionais como são os casos de outros estudos internacionais ou o Inquérito Nacional de Saúde. A consideração dos valores do PRx para efeitos de financiamento per capita introduz alterações face a outros modelos propostos neste âmbito. Após a análise dos montantes de financiamento entre os cenários alternativos, obtendo os modelos 1 e 2 níveis de concordância por percentil mais próximos entre si comparativamente ao modelo 3, seleccionou-se o modelo 1 como o mais adequado para a nossa realidade. Conclusão - A aplicação do modelo PRx numa região de saúde permitiu concluir em função dos resultados alcançados, que já existe a possibilidade de estruturação e operacionalização de um modelo que permite estimar a carga de doença em ambulatório a partir de informação relativa ao seu perfil de consumo de medicamentos dos utentes. A utilização desta informação para efeitos de financiamento de organizações de saúde verticalmente integradas provoca uma variação no seu actual nível de financiamento. Entendendo este estudo como um ponto de partida onde apenas uma parte da presente temática ficará definida, outras questões estruturantes do actual sistema de financiamento não deverão também ser olvidadas neste contexto. ------- ABSTRACT - Introduction - The main goal of this study was the development of a risk adjustment model for financing integrated delivery systems (IDS) in Portugal. The recent improvement of patient records, mainly at primary care level, the historical inadequacy of payment models and the increasing number of IDS were three important factors that drove us to develop new approaches for risk adjustment in our country. Methods - The work was divided in two steps: the development of a pharmacy-based model in Portugal and the proposal of a risk adjustment model for financing IDS. In the first step an expert panel was specially formed to classify more than 33.000 codes included in Portuguese pharmacy national codes into 33 chronic conditions. The study included population of Alentejo Region in Portugal (N=441.550 patients) during 2007 and 2008. Using pharmacy data extracted from three databases: prescription, private pharmacies and hospital ambulatory pharmacies we estimated a regression model including Potential Years of Life Lost, Complexity, Severity and PRx information as dependent variables to assess total cost as the independent variable. This healthcare financing model was compared with other two models proposed for IDS. Results - The more prevalent chronic conditions are cardiovascular (34%), psychiatric disorders (10%) and diabetes (10%). These results are also consistent with the National Health Survey. Apparently the model presents some limitations in identifying patients with rheumatic conditions, since it underestimates prevalence and future drug expenditure. We obtained a R2 value of 0,45, which constitutes a good value comparing with the state of the art. After testing three scenarios we propose a model for financing IDS in Portugal. Conclusion - Drug information is a good alternative to diagnosis in determining morbidity level in a population basis through ambulatory care data. This model offers potential benefits to estimate chronic conditions and future drug costs in the Portuguese healthcare system. This information could be important to resource allocation decision process, especially concerning risk adjustment and healthcare financing.

RESUMO - Pretende-se com este projecto fazer uma reflexão sobre a problemática dos modelos de governação na gestão pública de hospitais e sobre a forma de incrementar a capacidade estratégica da gestão, contextualizando a governação dos hospitais num quadro mais amplo e comunitário, integrando os interesses e expectativas dos diversos interessados no funcionamento dos hospitais públicos. Sinteticamente, a questão fundamental de investigação é: Como conseguirá o sistema de governação de um hospital público incorporar os interesses de todos os seus stakeholders e shareholders? A resposta a esta questão integra duas fases, uma primeira dedicada ao estudo de um novo modelo de configuração para o órgão de governação de topo do hospital público e uma segunda fase dedicada à construção de um questionário para discussão e validação do modelo proposto através da técnica Delphi. A metodologia utilizada passou pela condução prévia de entrevistas exploratórias a informadores privilegiados e pela divisão do projecto em duas partes – uma primeira parte de enquadramento que segue a metodologia clássica da revisão bibliográfica e uma segunda parte prática que segue a metodologia Delphi, precedida de um levantamento das questões críticas para integrar no questionário que servirá de base à discussão dos peritos. Como resultado, o actual modelo pareceu-nos dificultar a consideração das expectativas e dos interesses dos proprietários/accionistas e dos stakeholders em geral do hospital público, concluindo-se pela necessidade de um novo modelo que estabeleça, inequivocamente, os papéis e as funções inerentes ao órgão de governação, garantindo a internalização da perspectiva de todos os interessados. -------------------------------ABSTRACT - The objective of this project is to reflect on the problematic of models of governance in the public management of hospitals and on the means for incrementing strategic management capacity. It does so by contextualizing the governance of public hospitals on a more ample community–wide scale, integrating the interests and expectations of different parties. The main research question is: how can the system of governance of a public hospital incorporate the interests of all its stakeholders and shareholders? The answer to this question involves two phases, one dedicated to the study of the configuration of the top-level governing body of the public hospital and, a second, dedicated to the construction of a questionnaire for discussion and validation of the proposed model by means of a Delphi technique. The methodology involved, first of all, exploratory interviews with key- informants and by the structuring of the project in two parts – the first, dedicated to contextualization by means of a standard literature review and the second, essentially practical by means of the Delphi technique, preceded by the raising of critical questions that integrate the questionnaire that will form the basis of expert discussion. The present model of public hospital governance appears to limit the possibility of integrating the expectations and interests of stakeholders and owner/shareholders. ABSTRACT: The aim of this analysis was to analyze and describe the steps that have been taken in the development of the mental health policy in Suriname after the WHO AIMS. The objectives are: 1.To review the steps to be taken in developing a mental health policy and plan for a country 2.To gather information and data concerning mental health policy and plan development in Suriname 3.To draw conclusion from the experience gained that can be applied to other countries. In general, the information that was gathered from the four countries Guyana, Barbados, Trinidad & Tobago and Suriname, was compared with the WHO steps for developing a mental health policy and plan. Were these steps taken into consideration, when developing their mental health policy and plan? If not, what were the reasons why it did not happen? The checklist for evaluating a mental health plan was used in Suriname. This checklist assisted to see if the results of the recommendations given by the WHO AIMS to develop a effective and balanced mental health plan were taken into consideration. The mayor findings of the analysis are that Suriname as well as Guyana used the steps in developing their mental health policy and plan. Barbados and Trinidad & Tobago did not develop a mental health policy and plan. Suriname and Guyana have a mental health coordinating body at the Ministry of Health. Trinidad & Tobago as well as Barbados have a mental health focal person at the Ministry of Health of the respective countries. It can be concluded that successfully improving of health systems and services for mental health is combining theoretical concepts, expert knowledge and cooperation of many stakeholders. The appointment of a mental health coordinating unit at the Ministry of Health is crucial for the development of mental health in a country. Furthermore, mental health is everyone’s business and responsibility. Implementing the steps to be taken when developing a mental health policy and plan as recommended by WHO may be a slow process requiring the mobilization of political will. That’s why it is crucial that persons responsible for this process work close with all stakeholders in relevant sectors, taking their needs into consideration and try to translate that in clear objectives. It is common knowledge that improving the quality of mental health must be accompanied by the availability of financial and human resources. Finally, a mental health policy and plan should be one document tackling all aspects of mental health of a community.
