RESUMO - Os sistemas de saúde deparam-se, actualmente, com novos paradigmas, ao nível da qualidade da prestação de cuidados de saúde, bem como no que se refere à necessidade de controlo dos custos com o sector da saúde, obrigando as organizações a adaptarem-se por forma a fornecerem a resposta mais adequada às crescentes necessidades dos indivíduos. O reconhecimento desta realidade tem levado os governos de muitos países a definir políticas orientadas para problemas de saúde específicos e a adoptar estratégias de intervenção que privilegiam uma abordagem integrada com o objectivo de melhorarem progressivamente o nível de saúde das populações, a qualidade dos cuidados prestados e a eficiência na utilização de recursos. Em Portugal, a aplicação dos princípios que estiveram na base dos modelos de gestão da doença, deu origem ao modelo experimental de Gestão Integrada da Doença, que incorpora a gestão clínica da doença, centrada no doente, com especial enfoque na sua autogestão e na clarificação das melhores práticas profissionais, visando a sua uniformização; a reorganização do modelo de prestação de cuidados, com a criação de Centros de Elevada Diferenciação e Centros de Tratamento, com especiais preocupações no que concerne à orientação do doente no sistema para que os cuidados lhe seja ministrados no nível mais adequado; um modelo de financiamento específico, indexado aos resultados, que reflicta a adopção das melhores práticas; um sistema de informação que permita a monitorização e avaliação constante deste processo. O desenvolvimento deste modelo organizacional tem-se revelado como uma estratégia inovadora e como uma ferramenta de elevado potencial para a melhoria da prestação de cuidados de saúde e para a promoção de uma maior efectividade e eficiência, tal como poderá, ainda, constituirse como um veículo, importante e permanente, de informação de apoio à decisão em Saúde. Este modelo visa, no fundo, promover uma acção concertada no sentido da obtenção de uma intervenção precisa, através da mobilização de recursos adequados, que permitam uma melhoria do estado de saúde, da qualidade de vida e do bem-estar global dos doentes. Esta abordagem passa pela colaboração e coordenação dos diferentes níveis de prestação de cuidados, no sentido de oferecerem cuidados integrados de saúde, com níveis de qualidade elevados em termos de prevenção, diagnóstico, tratamento, reabilitação e acompanhamento. ------------------------ --ABSTRACT – The health systems are faced with new paradigms, on one hand in the healthcare services delivered to the populations, and on the other hand, in the need to control costs in the health sector, forcing organizations to adapt and provide the most appropriate response to the individuals growing needs. The magnitude of this problem, in terms of public health, requires the adoption of a directed, targeted, planned and integrated action, based on clear and well defined strategies in order to obtain health gains, improving the quality of care and streamlining the costs. In Portugal, the application of those principles forming the basis of the disease management models, led to the Integrated Disease Management model which, apart from the clinical management of the disease, also incorporates the healthcare delivery structure reorganization, a specific financing model based on an information system that allows the process monitoring and evaluation. The development of Integrated Disease Management models is a central strategy and a tool for improving healthcare delivery, more effectively and efficiently, and can even be an important and permanent vehicle of information for health decision support. Therefore, it is important to promote a concerted action towards achieving a precise intervention, mobilizing the resources, improving the health status, quality of life and the overall patients’ wellbeing. RESUMO - Visa-se explicitar a origem, a razão de ser, a natureza e o que se perspectiva da relação entre a Epidemiologia e a Saúde Pública, através de uma leitura histórica. As duas entidades foram-se definindo e fazendo sentido em conjunto, com sucessos e, também, muita polémica, desde há milénios e até meados do século XIX. Nesta época, uma combinação de circunstâncias proporcionou-lhes uma explosão de crescimento e de definição, de par com várias outras áreas disciplinares. Desde o antigo relato bíblico de como boa alimentação explica o bom estado de saúde, até à valorização científica das condicionantes sociais e económicas da saúde por Marmot e Rose, passando por «miasmas» causando doença e pela deslocação do conceito de risco individual de saúde para o de risco populacional — com as implicações inerentes a essa importante inovação —, este percurso permite identificar as fundações de tão notável simbiose, explicar o estado presente, vê-la evoluir e achar nela o significado do património hoje disponível, e o que ele promete. Algumas discrepâncias quanto à designação dos seus métodos, bem como a contínua discussão quanto à sua verdadeira natureza e orientação futura, atestam a juventude da Epidemiologia como disciplina científica. Entretanto, a Saúde Pública esforça-se por manter a sua essência integradora, à medida que outras disciplinas contribuem mais para que concretize os seus objectivos; é desafiada pela exposição das populações, em larga escala, a factores de doença, por vezes de intensidade mínima, e pelo surgimento de novas doenças ou a ampliação do volume de outras na população, muitas vezes não respeitando fronteiras. A história dessa simbiose mostra bem que conhecer o modo como uma doença se origina permite controlá-la na população, ou mesmo evitá-la, e que é grande o número de problemas que, em sinergia, as duas disciplinas podem clarificar e resolver. Assim, a Epidemiologia oferece à Saúde Pública explicações (olhos, inteligência e linguagem) para os problemas de saúde das populações — o que permite à segunda saber sobre o quê agir —, cenários de possível evolução dos problemas — o que permite aos decisores optarem em função de diferentes pressupostos, sobre como agir — e capacidade de juízo sobre os resultados das acções empreendidas, em simultâneo com a elevação do nível de consciência, de compreensão e de intervenção quanto ao que se está a passar, tanto pelos profissionais, como pela população — transferência do conhecimento. Facilmente se antecipa que a relação entre as duas disciplinas irá evoluir para maior complexidade e, também, solicitação e exigência da Saúde Pública sobre a Epidemiologia, que terá que corresponder em utilidade. E esta, continuando a subespecializar-se e a sofisticar-se tanto nos métodos, como nos enfoques sobre categorias específicas de factores, precisará de progredir muito na gestão da sua consistência enquanto corpo de conhecimento integrado e com peculiaridades metodológicas, à semelhança da Saúde Pública.O modo como evoluirá a relação entre ambas depende ainda da evolução dos próprios problemas, conceitos, teorias e soluções relacionados com a saúde das populações, e ainda do desenvolvimento das demais disciplinas chamadas à integração por ambas, para enfrentarem esses desafios. Nomeadamente, a Epidemiologia terá que gerir com perícia dificuldades já identificadas, como: incorporar métodos qualitativos de investigação na sua fortíssima tradição e cultura quantitativa; operacionalizar satisfatoriamente o conceito de «risco atribuível na população», ao serviço da definição de prioridades de acção dirigida às necessidades de saúde; aperfeiçoar modelos de interpretação causal que respeitem a multicausalidade; aproveitar as técnicas estatísticas de análise multivariada, sem se perder na abstracção dos seus modelos; desenvolver a investigação nas dimensões positivas de saúde, além da doença, para contribuir melhor para a realização da Saúde Pública, sua principal cliente e fornecedora de oportunidades.--------------------------ABSTRACT - The aim of the author is to explicit the origin, the rationale, the nature and the prospects of the relationship between Epidemiology and Public health, through an historic approach. The two entities have been defining and making sense together, by achieving successes, but also with much controversy, since millennia ago, until mid XIX century. A combination of circumstances provided them the opportunity for an explosion of growth and definition, then, alongside several other disciplines. From the ancient biblical report on how good food explains good health, up to the scientific appreciation of both social and economical constraints to health by Marmot and Rose, passing through «miasma» causing disease and through displacing from individual health risk to population risk — with the inherent implications of that important innovation —, this route allows the identification of the foundations of such remarkable symbiosis, the explanation of current status, to see its evolution and find in it the meaning of today’s heritage and what it promises. Some discrepancies on the name of its methods, as well as the continuing discussion about its true nature and future orientation, attest Epidemiology’s youth as a scientific discipline. Meanwhile, Public Health strives to keep its integrating essence, while other disciplines increasingly contribute so that it achieves its objectives; it is challenged by large scale population exposure to disease factors, sometimes with a minimum intensity, and by new diseases emerging in the population or by old ones getting amplified, often not respecting regions boundaries. The history of such a symbiosis shows that knowing the way a disease is generated allows to control it in the population, or even to avoid it, and that the number of problems that the two disciplines are able to clarify and solve together in synergy is considerable. Therefore, Epidemiology offers Public Health explanations (eyes, intelligence and language) for populations’s health problems — allowing that the latter knows on what to act —, scenarios on how problems may tend to evolve — allowing decision-makers to make their choices as a function of different assumptions, on how to act — and judgement capabilities on the results of already undertaken actions, accompanied by the raising of conscience level, understanding and intervention of what is going on by both professionals and the population – knowledge transfer. It is easy to anticipate that the relationship between both disciplines will develop towards increasing complexity and demand from Public Health to Epidemiology, and that this one will have to correspond in usefulness. And the latter, while continuing its subspecialisation and sophistication either in its methods, or in its approaches to specific factor categories, will need to progress in managing its consistency as an integrated body of knowledge having methodological peculiarities, similarly to Public Health. Further, the way the relationship between both will evolve depends on the evolution of the problems themselves, of the concepts, theories and solutions related to the health of populations, and on the development of remaining disciplines called to integration by both, in other to face those problems. Namely, Epidemiology will have to manage with expertise some already known difficulties, as: the inclusion of qualitative research methods in its very strong quantitative tradition and culture; to grant satisfactory operation to the «population attributable risk» concept, in support to the definition of action priorities envisaging health needs; to improve causal interpretation models that comply with multicausality; to take advantage of multivariate statistical techniques, without get

A potentially renewable and sustainable source of energy is the chemical energy associated with solvation of salts. Mixing of two aqueous streams with different saline concentrations is spontaneous and releases energy. The global theoretically obtainable power from salinity gradient energy due to World’s rivers discharge into the oceans has been estimated to be within the range of 1.4-2.6 TW. Reverse electrodialysis (RED) is one of the emerging, membrane-based, technologies for harvesting the salinity gradient energy. A common RED stack is composed by alternately-arranged cation- and anion-exchange membranes, stacked between two electrodes. The compartments between the membranes are alternately fed with concentrated (e.g., sea water) and dilute (e.g., river water) saline solutions. Migration of the respective counter-ions through the membranes leads to ionic current between the electrodes, where an appropriate redox pair converts the chemical salinity gradient energy into electrical energy. Given the importance of the need for new sources of energy for power generation, the present study aims at better understanding and solving current challenges, associated with the RED stack design, fluid dynamics, ionic mass transfer and long-term RED stack performance with natural saline solutions as feedwaters. Chronopotentiometry was used to determinate diffusion boundary layer (DBL) thickness from diffusion relaxation data and the flow entrance effects on mass transfer were found to avail a power generation increase in RED stacks. Increasing the linear flow velocity also leads to a decrease of DBL thickness but on the cost of a higher pressure drop. Pressure drop inside RED stacks was successfully simulated by the developed mathematical model, in which contribution of several pressure drops, that until now have not been considered, was included. The effect of each pressure drop on the RED stack performance was identified and rationalized and guidelines for planning and/or optimization of RED stacks were derived. The design of new profiled membranes, with a chevron corrugation structure, was proposed using computational fluid dynamics (CFD) modeling. The performance of the suggested corrugation geometry was compared with the already existing ones, as well as with the use of conductive and non-conductive spacers. According to the estimations, use of chevron structures grants the highest net power density values, at the best compromise between the mass transfer coefficient and the pressure drop values. Finally, long-term experiments with natural waters were performed, during which fouling was experienced. For the first time, 2D fluorescence spectroscopy was used to monitor RED stack performance, with a dedicated focus on following fouling on ion-exchange membrane surfaces. To extract relevant information from fluorescence spectra, parallel factor analysis (PARAFAC) was performed. Moreover, the information obtained was then used to predict net power density, stack electric resistance and pressure drop by multivariate statistical models based on projection to latent structures (PLS) modeling. The use in such models of 2D fluorescence data, containing hidden, but extractable by PARAFAC, information about fouling on membrane surfaces, considerably improved the models fitting to the experimental data.

This Thesis describes the application of automatic learning methods for a) the classification of organic and metabolic reactions, and b) the mapping of Potential Energy Surfaces(PES). The classification of reactions was approached with two distinct methodologies: a representation of chemical reactions based on NMR data, and a representation of chemical reactions from the reaction equation based on the physico-chemical and topological features of chemical bonds. NMR-based classification of photochemical and enzymatic reactions. Photochemical and metabolic reactions were classified by Kohonen Self-Organizing Maps (Kohonen SOMs) and Random Forests (RFs) taking as input the difference between the 1H NMR spectra of the products and the reactants. The development of such a representation can be applied in automatic analysis of changes in the 1H NMR spectrum of a mixture and their interpretation in terms of the chemical reactions taking place. Examples of possible applications are the monitoring of reaction processes, evaluation of the stability of chemicals, or even the interpretation of metabonomic data. A Kohonen SOM trained with a data set of metabolic reactions catalysed by transferases was able to correctly classify 75% of an independent test set in terms of the EC number subclass. Random Forests improved the correct predictions to 79%. With photochemical reactions classified into 7 groups, an independent test set was classified with 86-93% accuracy. The data set of photochemical reactions was also used to simulate mixtures with two reactions occurring simultaneously. Kohonen SOMs and Feed-Forward Neural Networks (FFNNs) were trained to classify the reactions occurring in a mixture based on the 1H NMR spectra of the products and reactants. Kohonen SOMs allowed the correct assignment of 53-63% of the mixtures (in a test set). Counter-Propagation Neural Networks (CPNNs) gave origin to similar results. The use of supervised learning techniques allowed an improvement in the results. They were improved to 77% of correct assignments when an ensemble of ten FFNNs were used and to 80% when Random Forests were used. This study was performed with NMR data simulated from the molecular structure by the SPINUS program. In the design of one test set, simulated data was combined with experimental data. The results support the proposal of linking databases of chemical reactions to experimental or simulated NMR data for automatic classification of reactions and mixtures of reactions. Genome-scale classification of enzymatic reactions from their reaction equation. The MOLMAP descriptor relies on a Kohonen SOM that defines types of bonds on the basis of their physico-chemical and topological properties. The MOLMAP descriptor of a molecule represents the types of bonds available in that molecule. The MOLMAP descriptor of a reaction is defined as the difference between the MOLMAPs of the products and the reactants, and numerically encodes the pattern of bonds that are broken, changed, and made during a chemical reaction. The automatic perception of chemical similarities between metabolic reactions is required for a variety of applications ranging from the computer validation of classification systems, genome-scale reconstruction (or comparison) of metabolic pathways, to the classification of enzymatic mechanisms. Catalytic functions of proteins are generally described by the EC numbers that are simultaneously employed as identifiers of reactions, enzymes, and enzyme genes, thus linking metabolic and genomic information. Different methods should be available to automatically compare metabolic reactions and for the automatic assignment of EC numbers to reactions still not officially classified. In this study, the genome-scale data set of enzymatic reactions available in the KEGG database was encoded by the MOLMAP descriptors, and was submitted to Kohonen SOMs to compare the resulting map with the official EC number classification, to explore the possibility of predicting EC numbers from the reaction equation, and to assess the internal consistency of the EC classification at the class level. A general agreement with the EC classification was observed, i.e. a relationship between the similarity of MOLMAPs and the similarity of EC numbers. At the same time, MOLMAPs were able to discriminate between EC sub-subclasses. EC numbers could be assigned at the class, subclass, and sub-subclass levels with accuracies up to 92%, 80%, and 70% for independent test sets. The correspondence between chemical similarity of metabolic reactions and their MOLMAP descriptors was applied to the identification of a number of reactions mapped into the same neuron but belonging to different EC classes, which demonstrated the ability of the MOLMAP/SOM approach to verify the internal consistency of classifications in databases of metabolic reactions. RFs were also used to assign the four levels of the EC hierarchy from the reaction equation. EC numbers were correctly assigned in 95%, 90%, 85% and 86% of the cases (for independent test sets) at the class, subclass, sub-subclass and full EC number level,respectively. Experiments for the classification of reactions from the main reactants and products were performed with RFs - EC numbers were assigned at the class, subclass and sub-subclass level with accuracies of 78%, 74% and 63%, respectively. In the course of the experiments with metabolic reactions we suggested that the MOLMAP / SOM concept could be extended to the representation of other levels of metabolic information such as metabolic pathways. Following the MOLMAP idea, the pattern of neurons activated by the reactions of a metabolic pathway is a representation of the reactions involved in that pathway - a descriptor of the metabolic pathway. This reasoning enabled the comparison of different pathways, the automatic classification of pathways, and a classification of organisms based on their biochemical machinery. The three levels of classification (from bonds to metabolic pathways) allowed to map and perceive chemical similarities between metabolic pathways even for pathways of different types of metabolism and pathways that do not share similarities in terms of EC numbers. Mapping of PES by neural networks (NNs). In a first series of experiments, ensembles of Feed-Forward NNs (EnsFFNNs) and Associative Neural Networks (ASNNs) were trained to reproduce PES represented by the Lennard-Jones (LJ) analytical potential function. The accuracy of the method was assessed by comparing the results of molecular dynamics simulations (thermal, structural, and dynamic properties) obtained from the NNs-PES and from the LJ function. The results indicated that for LJ-type potentials, NNs can be trained to generate accurate PES to be used in molecular simulations. EnsFFNNs and ASNNs gave better results than single FFNNs. A remarkable ability of the NNs models to interpolate between distant curves and accurately reproduce potentials to be used in molecular simulations is shown. The purpose of the first study was to systematically analyse the accuracy of different NNs. Our main motivation, however, is reflected in the next study: the mapping of multidimensional PES by NNs to simulate, by Molecular Dynamics or Monte Carlo, the adsorption and self-assembly of solvated organic molecules on noble-metal electrodes. Indeed, for such complex and heterogeneous systems the development of suitable analytical functions that fit quantum mechanical interaction energies is a non-trivial or even impossible task. The data consisted of energy values, from Density Functional Theory (DFT) calculations, at different distances, for several molecular orientations and three electrode adsorption sites. The results indicate that NNs require a data set large enough to cover well the diversity of possible interaction sites, distances, and orientations. NNs trained with such data sets can perform equally well or even better than analytical functions. Therefore, they can be used in molecular simulations, particularly for the ethanol/Au (111) interface which is the case studied in the present Thesis. Once properly trained, the networks are able to produce, as output, any required number of energy points for accurate interpolations.

RESUMO - O consumo de tabaco foi responsável por 100 milhões de mortes no século XX. Apesar dos grandes avanços alcançados no controlo deste problema a nível mundial, sob os auspícios da OMS, no contexto da Convenção-Quadro para o Controlo do Tabaco da OMS, se não forem adoptadas medidas consistentes e efectivas de saúde pública, a morbi-mortalidade que lhe está associada continuará a aumentar durante o presente século. A promoção da cessação tabágica constitui a estratégia populacional que permitirá obter ganhos em saúde a mais curto prazo. Embora a larga maioria dos fumadores faça, ao longo da vida, várias tentativas para parar de fumar sem apoio, apenas uma pequena minoria consegue manter-se abstinente a longo prazo. Os médicos de Medicina Geral e Familiar são, de entre todos os profissionais de saúde, os que podem intervir de modo mais consistente e efectivo neste âmbito e que melhores resultados obtêm na cessação tabágica dos pacientes fumadores, dado o vínculo terapêutico e a interacção frequente e continuada que com eles estabelecem ao longo do seu ciclo de vida. O aconselhamento breve, tendo por base a adopção de um estilo de comunicação motivacional centrado no paciente, adaptado aos estádios de mudança comportamental, tem-se revelado efectivo no apoio à mudança de comportamentos relacionados com a saúde e à resolução da ambivalência que caracteriza este processo. A revisão de literatura evidenciou o facto de os médicos nem sempre intervirem nas áreas preventivas e de promoção da saúde, em particular na área da cessação tabágica, com o investimento e a continuidade desejáveis. Por outro lado, muitos pacientes fumadores referem nunca ter sido aconselhados pelo seu médico a deixar de fumar.. Não são conhecidos estudos de âmbito nacional que permitam conhecer esta realidade, bem como os factores associados às melhores práticas de intervenção ou as barreiras sentidas pelos médicos de MGF à actuação nesta área. O presente trabalho teve como objectivos: (i) avaliar a hipótese de que os médicos que disseram adoptar o método clínico centrado no paciente teriam atitudes mais favoráveis relativamente à cessação tabágica e uma maior probabilidade de aconselhar os seus pacientes a parar de fumar; (ii) estudar a relação entre as atitudes, a percepção de auto-eficácia, a expectativa de efectividade e as práticas de aconselhamento sobre cessação tabágica, auto-referidas pelos médicos; (iii) Identificar as variáveis preditivas da adopção de intervenções breves de aconselhamento adaptadas ao estádio de mudança comportamental dos pacientes fumadores; (iv) identificar as barreiras e os incentivos à adopção de boas práticas de aconselhamento nesta área. A população de estudo foi constituída pelo total de médicos de medicina geral e familiar inscritos na Associação Portuguesa de Médicos de Clínica Geral, residentes em Portugal. Para recolha de informação, foi utilizado um questionário de resposta anónima, de autopreenchimento, aplicado por via postal a 2942 médicos, em duas séries de envio. O questionário integrou perguntas fechadas, semifechadas, escalas de tipo Likert e escalas de tipo visual analógico. Para avaliação da adopção do método clínico centrado no paciente, foi usada a Patient Practitioner Orientation Scale (PPOS). O tratamento estatístico dos dados foi efectuado com o Programa PASW Statistics (ex-SPSS), versão 18. Foram utilizados: o índice de α de Cronbach, diversos testes não paramétricos e a análise de regressão logística binária. Foi obtida uma taxa de resposta de 22,4%. Foram analisadas 639 respostas (67,4% de mulheres e 32,6% de homens). Referiram ser fumadores 23% dos homens e 14% das mulheres. Foi identificada uma grande carência formativa em cessação tabágica, tendo apenas 4% dos médicos afirmado não necessitar de formação nesta área. Responderam necessitar de formação em entrevista motivacional 66%, em prevenção da recaída 59%, de treino numa consulta de apoio intensivo 55%, em intervenção breve 54% e em terapêutica farmacológica 55%. Cerca de 92% dos respondentes consideraram que o aconselhamento para a cessação tabágica é uma tarefa que faz parte das suas atribuições, mas apenas 76% concordaram totalmente com a realização de uma abordagem oportunística deste assunto em todos os contactos com os seus pacientes. Como prática mais frequente, perante um paciente em preparação para parar, 85% dos médicos disseram tomar a iniciativa de aconselhar, 79% avaliar a motivação, 67% avaliar o grau de dependência, 60% marcar o “dia D” e 50% propor terapêutica farmacológica. Apenas 21% assumiram realizar com frequência uma intervenção breve com pacientes em preparação (5 Ás); 13% uma intervenção motivacional com pacientes não motivados para mudar (5 Rs) e 20% uma intervenção segundo os princípios da entrevista motivacional, relativamente a pacientes ambivalentes em relação à mudança. A análise multivariada de regressão logística permitiu concluir que as variáveis com maior influência na decisão de aconselhar os pacientes sobre cessação tabágica foram a percepção de auto-eficácia, o nível de atitudes negativas, a adopção habitual do Programa-tipo de cessação tabágica da DGS, a posse de formação específica nesta área e a não identificação de barreiras ao aconselhamento, em particular organizacionais ou ligadas ao processo de comunicação na consulta. Embora se tenha confirmado a existência de associação entre a adopção do método clínico centrado no paciente e as atitudes face à cessação tabágica, não foi possível confirmar plenamente a associação entre a adopção deste método e as práticas autoreferidas de aconselhamento. Os médicos que manifestaram um nível baixo ou moderado de atitudes negativas, uma percepção elevada de auto-eficácia, que nunca fumaram, que referiram adoptar o Programa-tipo de cessação tabágica e que não identificaram barreiras organizacionais apresentaram uma maior probabilidade de realizar uma intervenção breve (“5 Ás”) de aconselhamento de pacientes fumadores em preparação para parar de fumar. Nunca ter fumado apresentou-se associado a uma probabilidade de realizar uma intervenção breve (“5 Ás”) com frequência, superior à verificada entre os médicos que referiram ser fumadores (Odds-ratio ajustado = 2,6; IC a 95%: 1,1; 5,7). Os médicos com o nível de auto-eficácia no aconselhamento mais elevado apresentaram uma probabilidade superior à encontrada entre os médicos com o menor nível de auto-eficácia de realizar com frequência uma intervenção breve de aconselhamento, integrando as cinco vertentes dos “5 Ás” (Odds ratio ajustado = 2,6; IC a 95%: 1,3; 5,3); de realizar uma intervenção motivacional breve com fumadores renitentes a parar de fumar (Odds ratio ajustado = 3,1; IC a 95%: 1,4; 6,5) ou de realizar com frequência uma intervenção motivacional com pacientes em estádio de ambivalência (Odds ratio = 8,8; IC a 95%: 3,8; 19,9). A falta de tempo, a falta de formação específica e a falta de equipa de apoio foram as barreiras ao aconselhamento mais citadas. Como factores facilitadores de um maior investimento nesta área, cerca de 60% dos médicos referiram a realização de um estágio prático de formação; 57% a possibilidade de dispor do apoio de outros profissionais; cerca de metade a melhoria da sua formação teórica. Cerca de 25% dos médicos investiria mais em cessação tabágica se dispusesse de um incentivo financeiro e 20% se os pacientes demonstrassem maior interesse em discutir o assunto ou existisse uma maior valorização desta área por parte dos colegas e dos órgãos de gestão. As limitações de representatividade da amostra, decorrentes da taxa de resposta obtida, impõem reservas à possibilidade de extrapolação destes resultados para a população de estudo, sendo de admitir que os respondentes possam corresponder aos médicos mais interessados por este tema e que optam por não fumar. Outra importante limitação advém do facto de não ter sido estudada a vertente relativa aos pacientes, no que se refere às suas atitudes, percepções e expectativas quanto à actuação do médico neste campo. Pesem embora estas limitações, os resultados obtidos revelaram uma grande perda de oportunidades de prevenção da doença e de promoção da saúde. Parece ter ficado demonstrada a importante influência que as atitudes, em especial as negativas, e as percepções, em particular a percepção de auto-eficácia, podem exercer sobre as práticas de aconselhamento auto-referidas. Todavia, será necessário aprofundar os resultados agora encontrados com estudos de natureza qualitativa, que permitam compreender melhor, por um lado, as percepções, expectativas e necessidades dos pacientes, por outro, as estratégias de comunicação que deverão ser adoptadas pelo médico, atendendo à complexidade do problema e ao tempo disponível na consulta, tendo em vista aumentar a literacia dos pacientes para uma melhor autogestão da sua saúde. Parece ter ficado igualmente patente a grande carência formativa neste domínio. A adopção do modelo biomédico como paradigma da formação médica pré e pós-graduada, proposto, há precisamente cem anos, por Flexner, tem contribuído para a desvalorização das componentes psicoemocionais e sociais dos fenómenos de saúde e de doença, assim como para criar clivagens entre cuidados curativos e preventivos e entre medicina geral e familiar e saúde pública. Porém, o actual padrão de saúde/doença próprio das sociedades desenvolvidas, caracterizado por “pandemias” de doenças crónicas e incapacitantes, determinadas por factores de natureza sociocultural e comportamental, irá obrigar certamente à revisão daquele paradigma e à necessidade de se (re)adoptarem os grandes princípios Hipocráticos de compreensão dos processos de saúde/doença e do papel da medicina.

RESUMO: As doenças mentais são comuns, universais e associadas a uma significativa sobrecarga pessoal, familiar, social e económica. Os Serviços de Saúde Mental devem abordar de forma adequada as necessidades dos pacientes e familiares tanto ao nível clínico como também ao nível social. O presente estudo foi realizado num período de grande transformação nos sistemas de saúde primário e de saúde mental em Portugal, num Departamento de Psiquiatria desenvolvido com base nos princípios da OMS. Os objectivos incluem a caracterização: 1) das Unidades Funcionais do Departamento; 2) dos pacientes internados pela primeira vez no internamento de agudos; 3) da utilização dos serviços nas equipas comunitárias após a alta; e 4) da avaliação de alguns dos indicadores de qualidade do departamento, com recurso ao modelo de Donabedian sobre a articulação entre a Estrutura-Processo-Resultados. Metodologia: Foi escolhido um estudo de coorte retrospectivo. Todos os pacientes internados pela primeira vez entre 2008 e 2010 foram incluídos no estudo. Os seus processos clínicos e a base de dados do hospital onde são registados todos os contactos que estes tiveram com os profissionais de saúde mental foram revistos de forma a obter dados sociodemográficos e clínicos, durante o período do estudo e após a alta. Os instrumentos utilizados foram o WHO-ICMHC (Classificação Internacional de Cuidados de Saúde Mental), para caracterizar o Departamento, o AIESMP (Avaliação Inicial de Enfermagem em Saúde Mental e Psiquiatria) para recolha dos dados sociodemográficos, e o VSSS (Escala de Satisfação com os Serviços de Verona) de forma a avaliar a satisfação dos pacientes em relação aos cuidados recebidos. A análise estatística incluiu a análise descritiva, quantitativa e qualitativa dos dados. Resultados: As Unidades Funcionais do Departamento revelaram níveis elevados de articulação e consistência com as necessidades de cuidados psiquiátricos e reabilitação psicossocial dos pacientes. Os 543 pacientes admitidos pela primeira vez eram maioritariamente (56.9%) mulheres, caucasianas (81.2%), com diagnóstico de perturbações do humor (66.3%), internadas voluntariamente (59.7%), e uma idade média de 45.1 anos. Estas eram significativamente mais velhas, mais frequentemente empregadas, casadas/coabitar e tinham uma prevalência mais elevada de perturbações do humor, comparativamente aos homens. O internamento compulsivo era mais significativo nos homens (54.7%). A taxa de abandono no pós-alta (4.2%) e a taxa de reinternamentos (2.9%) na quinzena após a alta revelaram-se inferiores aos padrões na literatura internacional. De forma global, a satisfação dos pacientes com os cuidados de saúde mental foi positiva. Conclusões: Os cuidados prestados mostraram-se eficazes, adaptados e baseados nas necessidades e problemas específicos dos pacientes. A continuidade e a abrangência de cuidados foram difundidos e mantidos ao longo do processo de cuidados. Este Departamento pode ser considerado um exemplo de como proporcionar tratamento digno e eficiente, e uma referência para futuros serviços de psiquiatria.-------------- ABSTRACT: Mental health disorders are common, universal, and associated with heavy personal, family, social and economic burden. Mental health services should be aimed at adequately addressing patients’ and families’ needs at clinical and social level. The current study was carried out at a time of great transformation in the health and mental health systems in Portugal, in a Psychiatric Department developed taking in consideration the WHO principles. The objectives included characterizing: 1) the Psychiatric Department’s different units; 2) the patients admitted for the first time to the inpatient unit; 3) their use of community mental health services after discharge; and 4) assessing some of the department’s quality indicators, with resource to Donabedian’s Structure-Process-Outcome model. Methodology: A retrospective cohort design was chosen. All the firstly admitted patients in the period between 2008 and 2010 were included in the study. Their clinical records and the hospital’s database which registers all of the contacts the patients had with the mental health professionals during the study period, were reviewed to retrieve sociodemographic and clinical data and information on follow-up. The instruments used were the WHO International Classification of Mental Health Care (ICMHC) to characterize the department, the Initial Nurses’ Assessment in Mental Health and Psychiatry (AIESMP) for patients’ sociodemographic data, and the Verona Service Satisfaction Scale (VSSS) to assess patients’ satisfaction with care received. Statistical analysis included descriptive, quantitative and qualitative analysis of the data. Results: The Department’s Functional units revealed high levels of articulation, and were consistent with patients’ needs for psychiatric care and psychosocial rehabilitation. The 543 patients firstly admitted were mainly (56.9%) female, Caucasian (81.2%), diagnosed with mood disorders (66.3%), voluntarily admitted (59.7%), and with a mean age of 45.1 years. Female patients were significantly older, more frequently employed, married/cohabiting and had a higher prevalence of mood disorders when compared to males. Involuntary admission was more significant in males (54.7%). Dropout rates during follow-up (4.2%) and readmission rates (2.9%) in the fortnight following discharge were lower than standards in international literature. Overall patients’ satisfaction with mental health care was positive. Conclusions: The care delivered was effective, adapted and based on the patients’ specific needs and problems. Continuity and comprehensiveness of care was endorsed and maintained throughout the care process. This department may be considered an example of both humane and effective treatment, and a reference for future psychiatric care.
