RESUMO: O maraviroc (MVC) é o único anti-retroviral antagonista do co-receptor CCR5 licenciado e interage com as ansas transmembranares de CCR5, induzindo uma alteração da sua conformação e impedindo a interacção com gp120. O MVC é activo apenas contra estirpes R5 de HIV-1, sendo utilizado em terapia de recurso. Neste trabalho, foi estudada a diversidade genética da região C2V3C3 do gene env de estirpes de HIV-1 de oxicodependentes por via endovenosa da Grande Lisboa, pesquisando-se também a presença de polimorfismos genéticos naturais. Foram utilizadas 52 amostras de plasma e para 35 destas foi amplificado por RT-nested PCR um produto de 565 pb. A análise filogenética revelou a seguinte distribuição de genótipos: 23 B (incluindo, provavelmente, 2 CRF14_BG), 8 A, 3 G e 1 F1. Após tradução, e por comparação com a sequência consenso B, verificou-se uma elevada frequência de polimorfismos genéticos, sendo encontradas algumas “assinaturas de aminoácidos” relativas aos subtipos não-B. Realizou-se ainda uma pesquisa de locais de N-glicosilação e a previsão da utilização de co-receptores (abordagem genotípica), com recurso às regras 11/25 e da carga líquida da ansa V3 e aos programas PSSM e geno2pheno[coreceptor]. Observou-se uma conservação genérica do número de locais de N-glicosilação e foram identificadas 5 sequências com tropismo X4 ou duplo. Por fim, com base na literatura, realizou-se uma pesquisa de polimorfismos genéticos associados a resistência ao MVC presentes na ansa V3. Foi observado um número elevado destas mutações. A presença dos padrões 11S+26V e 20F+25D+26V, num total de 3 sequências, é relevante, visto estes estarem inequivocamente associados à resistência in vivo ao MVC. Apesar de não estar ainda definido um perfil de resistência para o MVC, a presença das mutações encontradas, em indivíduos sem contacto prévio com o fármaco, trará implicações relevantes na sua gestão clínica, considerando a introdução do MVC na terapia de recurso.---------- ABSTRACT: Maraviroc (MVC) is the only CCR5 inhibitor licensed today. This drug interacts with the transmembrane helices of CCR5 co-receptor, inducing a conformation change of its extracellular loops and preventing the interaction with gp120. MVC is only active against R5 strains of HIV-1 and is currently used in salvage therapy. The genetic diversity of the env C2V3C3 region of HIV-1 strains from injecting drug users in the Greater Lisbon was studied, along with the presence of natural genetic polymorphisms. 52 plasma samples were used and the amplification by RT-nested PCR of a 565 bp-product was possible in 35 of them. The phylogenetic analysis revealed 23 sequences classified as subtype B (probably including 2 CRF14_BG), 8 A, 3 G and 1 F1. After translation, the presence of natural genetic polymorphisms was studied by comparison to a subtype B consensus. A high frequency of genetic polymorphisms was observed and significant “amino acid signatures” were found in association with non-B subtypes. A full characterization of the N-glycosylation sites was also performed and a coreceptor prediction (genotypic approach) was accomplished using the 11/25 and the V3 net charge rules and the programs PSSM and geno2pheno[coreceptor]. The number of N-glycosylation sites was generically preserved. Five sequences were defined as X4 or dual-tropic. Based on published data, a search for genetic polymorphisms, present in V3loop, associated to MVC resistance was finally undertaken. Several of such mutations were observed, being particularly interesting the presence of the patterns 11S+26V and 20F+25D+26V, in a total of 3 sequences, since these patterns have unequivocally been associated with MVC resistance in vivo. Although a resistance profile for MVC is not yet defined, the presence of these mutations in MVC-naïve populations may have significant impact in their clinical management in the future, especially considering the introduction of this drug in salvage therapy.

RESUMO: O desenvolvimento de serviços locais adequados deve ser baseado numa avaliação sistemática das necessidades e resultados obtidos nos cuidados a uma população de indivíduos identificados como apresentando uma doença mental na área de referenciação do serviço. Neste sentido foram utilizados os seguintes métodos: dados epidemiológicos acerca das necessidades locais e taxas de utilização de serviços a nível nacional e local, este último com base no case-register. Os diagnósticos de maior prevalência em ambulatório são as perturbações de humor e as perturbações neuróticas de stress ou somatoformes, com uma preponderância de doenças mentais comuns (depressão e ansiedade) em serviços de psiquiatria. Constatam-se baixas taxas de abandono da consulta (12%). A idade, a doença e a escolaridade estão correlacionados com o risco de drop-out, mas utilizada a regressão logística, a idade e a escolaridade perdem o seu significado estatístico. Encontram-se taxas reduzidas de drop-out dos indivíduos com psicose ou perturbações bipolares, em virtude da intervenção activa da equipa. Os custos de transporte, a distância ao local de consulta e o tempo de espera para a primeira consulta são barreiras no acesso aos cuidados a nível local. Os cuidadores não se sentem apoiados pela rede de suporte social e queixam-se sobretudo da acessibilidade, mas exibem elevadas taxas de satisfação com os serviços prestados. Decidiu-se apostar numa organização do serviço baseada na comunidade, com intervenções baseadas na evidência, dando prioridade ao doente mental grave e à qualidade dos cuidados.----------- ABSTRACT: The development of appropriate local services should be based on a systematic assessment of the needs and outcomes of the population of individuals identified as mentally ill within the service’s catchment area. A number of methods may be used as proxies in assessing local needs for services, such as service utilization rates found nationally and locally, by case-register. The most prevalent diagnoses in ambulatory care are mood disorders and neurotic, stress and somatoform disorders, with a majority of common mental disorders (depression and anxiety) in psychiatric services. Low dropout rates (12%) are found in ambulatory care. Age, disease and education are correlated with the risk of drop-out, but after using logistic regression, age and education lose their statistical significance. Low drop-out rates are found in individuals with psychosis or bipolar disorders, because the active intervention from the team. The costs of transportation, distance and the waiting time for the first consultation are barriers in access of care locally. Carers do not feel supported by the network of social support and complain primarily of accessibility, but exhibit high levels of satisfaction with the services provided. It was decided to invest in a service organization based in the community with evidence-based interventions, giving priority to severe mental illness and quality of care.

RESUMO: O planeamento da alta hospitalar representa um momento crucial no internamento de pessoas idosas, que muitas vezes regressam ao domicílio com necessidades complexas ao nível dos cuidados. No entanto, não existe um planeamento de alta universal, tornando prioritário o desenvolvimento de estratégias nesse sentido. Após revisão de literatura foi formulado um modelo de análise, com as dimensões e indicadores críticos para a problemática em estudo. Objectivo: Este estudo teve como objectivo analisar as repercussões que um planeamento de alta hospitalar integrado pode ter na satisfação e no bem-estar dos doentes e famílias, tendo impacto ao nível da redução das reincidências hospitalares. Métodos: Foi seleccionada uma amostra no Serviço de Medicina 1B do Centro Hospitalar Lisboa Norte, dividida aleatoriamente em dois grupos. O grupo controlo teve um planeamento de alta de acordo com os padrões habituais, enquanto o grupo experimental foi alvo de um planeamento de alta integrado, com enfoque na informação e clarificação nos cuidados pós alta, e com recurso à entrega de um documento de orientação para a alta. Foi realizado um contacto telefónico de follow-up um mês após a alta, para realização de um questionário telefónico. Resultados: Os participantes do grupo experimental revelaram ter tendência para estar mais bem preparados para a alta hospitalar e regresso ao domicílio, valorizando de forma mais positiva o processo de internamento e alta, bem como a auto-percepção de bem-estar. Conclusões: Apesar da limitação da dimensão da amostra, os resultados apontam no sentido de que os procedimentos do planeamento integrado de alta hospitalar têm um impacto positivo ao nível da satisfação e sentimento de bem-estar global dos idosos no regresso ao domicílio, influenciando na redução das reincidências hospitalares.------------ ABSTRACT: The hospital discharge planning represents a crucial moment in the hospitalization of the elderly, whom very often return home with very complex caring needs. However, there is no universal discharge planning as such, thus becoming mandatory to develop planning strategies. Upon review of reference literature, an investigational model was developed, that features the key indicators and dimensions of the subject matter. Aim: The aim of this study was to analyze how hospital discharge planning promotes the satisfaction and well-being of patients and their families, reducing the number of hospital readmissions. Methods: A sample population was selected from the 1B Internal Medicine Service of Centro Hospitalar Lisboa Norte, which was divided in two random groups. The control group experienced a standard discharge, whereas the experimental group underwent a planned integrated discharge, which focused on the clarification of the post-discharge caring needs. A discharge guidance document was produced and handed to the patients. One month after discharge, a follow-up telephone call was made. Results: The experimental group turned out to be better prepared for hospital discharge and to return home, positively valuing hospitalization and hospital discharge, as well as their perception of their own well-being. Conclusions: The integrated hospital discharge planning had a positive impact on patient satisfaction and global well-being upon their return, and effectively reduced the number of hospital readmissions.

The paper presented herein proposes a reliability-based framework for quantifying the structural robustness considering the occurrence of a major earthquake (mainshock) and subsequent cascading hazard events, such as aftershocks that are triggered by the mainshock. These events can significantly increase the probability of failure of buildings, especially for structures that are damaged during the mainshock. The application of the proposed framework is exemplified through three numerical case studies. The case studies correspond to three SAC steel moment frame buildings of 3-, 9-, and 20- stories, which were designed to pre-Northridge codes and standards. Twodimensional nonlinear finite element models of the buildings are developed using the Open System for Earthquake Engineering Simulation framework (OpenSees), using a finite-length plastic hinge beam model and a bilinear constitutive law with deterioration, and are subjected to multiple mainshock-aftershock seismic sequences. For the three buildings analyzed herein, it is shown that the structural reliability under a single seismic event can be significantly different from that under a sequence of seismic events. The reliability-based robustness indicator used shows that the structural robustness is influenced by the extent by which a structure can distribute damage.

As an introduction to a series of articles focused on the exploration of particular tools and/or methods to bring together digital technology and historical research, the aim of this paper is mainly to highlight and discuss in what measure those methodological approaches can contribute to improve analytical and interpretative capabilities available to historians. In a moment when the digital world present us with an ever-increasing variety of tools to perform extraction, analysis and visualization of large amounts of text, we thought it would be relevant to bring the digital closer to the vast historical academic community. More than repeating an idea of digital revolution introduced in the historical research, something recurring in the literature since the 1980s, the aim was to show the validity and usefulness of using digital tools and methods, as another set of highly relevant tools that the historians should consider. For this several case studies were used, combining the exploration of specific themes of historical knowledge and the development or discussion of digital methodologies, in order to highlight some changes and challenges that, in our opinion, are already affecting the historians' work, such as a greater focus given to interdisciplinarity and collaborative work, and a need for the form of communication of historical knowledge to become more interactive.
