Introdução – O cancro de pulmão/traqueia e brônquios é a principal causa de morte por neoplasia na União Europeia. A técnica de duas aquisições de imagem em tempos diferentes no Positron Emission Tomography/Computed Tomography (PET/CT) tem sido referenciada em alguns artigos como uma mais-valia no diagnóstico do cancro do pulmão. O objectivo deste estudo consiste em avaliar a eficiência diagnóstica do PET/CT com a aquisição das duas imagens em tempos diferentes na caracterização do nódulo solitário pulmonar (NSP), tendo em conta a histologia e o tamanho do nódulo. Metodologia – Foram analisados 115 NSP, num total de 110 pacientes, dos quais 65 nódulos eram malignos. Adquiriram-se duas imagens, a primeira a um tempo médio de 52 minutos e a segunda a um tempo médio de 125 minutos após administração do 18F-2-fluoro-2-deoxi-D-glucose (18F-FDG). Para a análise das imagens obteve-se o standard uptake value máximo (SUVmax) e a percentagem de variação dos SUVmax (%variação). Resultados – A %variação apresenta valores de eficiência diagnóstica superiores à análise dos SUVmax em separado. Existem também diferenças significativas na histologia e no SUVmax, registando-se um aumento do SUVmax2 comparativamente ao SUVmax1 nas patologias malignas. Conclusão – A técnica da aquisição de duas imagens em tempos diferentes mostrou ser mais eficaz na caracterização do NSP do que a análise de apenas uma imagem.

The rapid growth in genetics and molecular biology combined with the development of techniques for genetically engineering small animals has led to increased interest in in vivo small animal imaging. Small animal imaging has been applied frequently to the imaging of small animals (mice and rats), which are ubiquitous in modeling human diseases and testing treatments. The use of PET in small animals allows the use of subjects as their own control, reducing the interanimal variability. This allows performing longitudinal studies on the same animal and improves the accuracy of biological models. However, small animal PET still suffers from several limitations. The amounts of radiotracers needed, limited scanner sensitivity, image resolution and image quantification issues, all could clearly benefit from additional research. Because nuclear medicine imaging deals with radioactive decay, the emission of radiation energy through photons and particles alongside with the detection of these quanta and particles in different materials make Monte Carlo method an important simulation tool in both nuclear medicine research and clinical practice. In order to optimize the quantitative use of PET in clinical practice, data- and image-processing methods are also a field of intense interest and development. The evaluation of such methods often relies on the use of simulated data and images since these offer control of the ground truth. Monte Carlo simulations are widely used for PET simulation since they take into account all the random processes involved in PET imaging, from the emission of the positron to the detection of the photons by the detectors. Simulation techniques have become an importance and indispensable complement to a wide range of problems that could not be addressed by experimental or analytical approaches.

Introdução – O cancro de mama é a principal causa de morte por cancro na população feminina portuguesa. Assim, pretende-se descrever sucintamente o percurso de uma paciente com tumor de mama, evidenciando a interligação entre algumas áreas das tecnologias da saúde. Metodologia – Selecionou-se um caso clínico de uma paciente com cancro de mama e recorreu-se à pesquisa bibliográfica de forma a apoiar ou refutar os resultados obtidos com o referido caso clínico. Resultados e discussão de resultados – Em 1996 foi diagnosticado à paciente um carcinoma ductal da mama esquerda e posteriormente, em 2011, foi diagnosticada uma metástase do carcinoma mamário com padrão mucinoso. Neste caso, bem como em todos os tumores de mama, foi realizada uma abordagem multidisciplinar envolvendo diferentes áreas das tecnologias da saúde. Para deteção e diagnóstico do cancro de mama são utilizadas a mamografia e a ultrassonografia. Para casos de re-estadiamento e monitorização da terapêutica devem ser utilizadas a tomografia por emissão de positrões (TEP) e a ressonância magnética mamária. No que se refere à terapêutica, neste tipo de tumores recorre-se à cirurgia, à radioterapia e à quimioterapia. ABSTRACT - Introduction – The breast cancer is the main cause of death by cancer in the Portuguese female population. We intend to describe briefly the pathway of a patient with breast cancer, highlighting the connection between some of the health technology areas. Methodology – It was selected a clinical case of a patient with breast cancer and it was used a literature research to support or refute the results obtained with the mentioned clinical case. Results and discussion – In 1996, it was diagnosed to the patient a ductal carcinoma of the left breast and in 2011 it was diagnosed a breast metastases. In this case, as well as other breast tumours, it was done a multidisciplinary approach involving different areas of health technologies. For detection and diagnose of breast cancer it’s used the mammography and ultrasound. For cases of re-staging and treatment monitoring it should be used positron emission tomography and magnetic resonance imaging of the breast. Regarding the therapy approach, in this type of tumours, it is used surgery, radiotherapy and chemotherapy.

The formation of amyloid structures is a neuropathological feature that characterizes several neurodegenerative disorders, such as Alzheimer´s and Parkinson´s disease. Up to now, the definitive diagnosis of these diseases can only be accomplished by immunostaining of post mortem brain tissues with dyes such Thioflavin T and congo red. Aiming at early in vivo diagnosis of Alzheimer´s disease (AD), several amyloid-avid radioprobes have been developed for b-amyloid imaging by positron emission tomography (PET) and single-photon emission computed tomography (SPECT). The aim of this paper is to present a perspective of the available amyloid imaging agents, special those that have been selected for clinical trials and are at the different stages of the US Food and Drugs Administration (FDA) approval.

Introdução – O melanoma maligno cutâneo (MMC) é considerado uma das mais letais neoplasias e no seu seguimento recorre-se, para além dos exames clínicos e da análise de marcadores tumorais, a diversos métodos imagiológicos, como é o exame Tomografia por Emissão de Positrões/Tomografia Computorizada (PET/CT, do acrónimo inglês Positron Emission Tomography/Computed Tomography) com 18fluor-fluorodeoxiglucose (18F-FDG). O presente estudo tem como objetivo avaliar a utilidade da PET/CT relativamente à análise da extensão e à suspeita de recidiva do MMC, comparando os achados imagiológicos com os descritos em estudos CT. Metodologia – Estudo retrospetivo de 62 estudos PET/CT realizados em 50 pacientes diagnosticados com MMC. Excluiu-se um estudo cujo resultado era duvidoso (nódulo pulmonar). As informações relativas aos resultados dos estudos anatomopatológicos e dos exames imagiológicos foram obtidas através da história clínica e dos relatórios médicos dos estudos CT e PET/CT. Foi criada uma base de dados com os dados recolhidos através do software Excel e foi efetuada uma análise estatística descritiva. Resultados – Dos estudos PET/CT analisados, 31 foram considerados verdadeiros positivos (VP), 28 verdadeiros negativos (VN), um falso positivo (FP) e um falso negativo (FN). A sensibilidade, especificidade, o valor preditivo positivo (VPP), o valor preditivo negativo (VPN) e a exatidão da PET/CT para o estadiamento e avaliação de suspeita de recidiva no MMC são, respetivamente, 96,9%, 96,6%, 96,9%, 96,6% e 96,7%. Dos resultados da CT considerados na análise estatística, 14 corresponderam a VP, 12 a VN, três a FP e cinco a FN. A sensibilidade, especificidade, o VPP e o VPN e a exatidão da CT para o estadiamento e avaliação de suspeita de recidiva no MMC são, respetivamente, 73,7%, 80,0%, 82,4%, 70,6% e 76,5%. Comparativamente aos resultados CT, a PET/CT permitiu uma mudança na atitude terapêutica em 23% dos estudos. Conclusão – A PET/CT é um exame útil na avaliação do MMC, caracterizando-se por uma maior acuidade diagnóstica no estadiamento e na avaliação de suspeita de recidiva do MMC comparativamente à CT isoladamente.

Since the first in vivo studies of cerebral function with radionuclides by Ingvar and Lassen, nuclear medicine (NM) brain applications have evolved dramatically, with marked improvements in both methods and tracers. Consequently it is now possible to assess not only cerebral blood flow and energy metabolism but also neurotransmission. Planar functional imaging was soon substituted by single-photon emission computed tomography (SPECT) and positron emission tomography (PET); it now has limited application in brain imaging, being reserved for the assessment of brain death.

Myocardial perfusion imaging (MPI) is used on a daily basis to access coronary blood flow in patients that are suspected or have known Coronary Artery Disease (CAD). A Single Photon Emission Computed Tomography (SPECT) or and Positron Emission Tomography (PET) scan are used to access regional blood flow quantification either at rest or stress, the imaging acquisition is connected to an Electrocardiogram (ECG) and it is able to determine and quantify other myocardial parameters like myocardial wall thickness and wall motion. PET is not used so broadly due to its high procedure cost, the proximity with cyclotron, where are produced the majority of radiopharmaceuticals used in PET, due to their shor thalf-life. This work is intended to carry out a review of the tests relating to radiopharmaceuticals that are used in clinical practice in SPECT or PET for assessment of myocardial perfusion, also focusing very promising radiopharmaceuticals that are under investigation or in clinical trials with great potential for conventional nuclear medicine or PET, proceeding to a comparative analysis of both techniques and respective radiopharmaceuticals used.

Introduction: Anxiety is a common problem in primary care and specialty medical settings. Treating an anxious patient takes more time and adds stress to staff. Unrecognized anxiety may lead to exam repetition, and impedance of exam performance. Objective: The aim of the study was to examine the anxiety levels of patients who are to undergo diagnostic exams related to cancer diagnostic: PET/CT and mammography. Methods: Two hundred and thirty two patients who undergo PET/CT and one hundred thirteen women who undergo mammography filled out one questionnaire after the procedure to determine their concerns, expectations and perceptions of anxiety. Results: Our results show that the main causes of anxiety in patients who are having a PET/CT is the fear of the procedure itself and fear of the results. Patients who suffered from greater anxiety were those who were scanned during the initial stage of an oncological disease. On the other hand, the diagnostic is the main cause of anxiety in women who are requiring a mammography. 28% of the women reported having experienced pain or intense pain. Conclusions: The performance of diagnostic exams related to cancer diagnostic like a PET/CT and a mammography are important and statistically generators of anxiety. Patients are often poorly informed and present with a range of anxieties that may ultimately affect examination quality. These results provide expertise that can be used in the development of future training programs to integrate post-graduate courses of health professionals.

Introduction: Standard Uptake Value (SUV) is a measurement of the uptake in a tumour normalized on the basis of a distribution volume and is used to quantify 18F-Fluorodeoxiglucose (FDG) uptake in tumors, such as primary lung tumor. Several sources of error can affect its accuracy. Normalization can be based on body weight, body surface area (BSA) and lean body mass (LBM). The aim of this study is to compare the influence of 3 normalization volumes in the calculation of SUV: body weight (SUVW), BSA (SUVBSA) and LBM (SUVLBM), with and without glucose correction, in patients with known primary lung tumor. The correlation between SUV and weight, height, blood glucose level, injected activity and time between injection and image acquisition is evaluated. Methods: Sample included 30 subjects (8 female and 22 male) with primary lung tumor, with clinical indication for 18F-FDG Positron Emission Tomography (PET). Images were acquired on a Siemens Biography according to the department’s protocol. Maximum pixel SUVW was obtained for abnormal uptake focus through semiautomatic VOI with Quantification 3D isocontour (threshold 2.5). The concentration of radioactivity (kBq/ml) was obtained from SUVW, SUVBSA, SUVLBM and the glucose corrected SUV were mathematically obtained. Results: Statistically significant differences between SUVW, SUVBSA and SUVLBM and between SUVWgluc, SUVBSAgluc and SUVLBMgluc were observed (p=0.000<0.05). The blood glucose level showed significant positive correlations with SUVW (r=0.371; p=0.043) and SUVLBM (r=0.389; p=0.034). SUVBSA showed independence of variations with the blood glucose level. Conclusion: The measurement of a radiopharmaceutical tumor uptake normalized on the basis of different distribution volumes is still variable. Further investigation on this subject is recommended.

The purpose of this study was to investigate the subjective perception of anxiety pre- and post-procedure, and explore the relationship between demographic, clinical variables and cancer patients' anxiety during a positron emission tomography/computed tomography (PET/CT) scan. Two hundred and thirty-two oncological out patients, with clinical indication for performing an (18)F-2-fluoro-2-deoxy-D-glucose ((18)F-FDG) PET/CT scan and attending a nuclear medicine (NM) department, participated in the study. Patients' anxiety and subjective experience of PET/CT were examined using two self-report questionnaires. The pre-procedure questionnaire focused on demographic information, level of knowledge regarding the scan and subjective perception of anxiety before the procedure. The post-procedure questionnaire included the subjective perception anxiety after the procedure, information adequacy and satisfaction with the NM department. The self-reported data indicate that patients were anxious during PET/CT. Furthermore, our data revealed a significant difference between the anxiety pre-procedure and post-procedure (z = -3909, p < 0.05), in which the anxiety pre-procedure has significantly higher values. No significant correlation was found between anxiety and age of the patients, education levels, adequacy of information or satisfaction with the NM Department. Perception of anxiety post-procedure differs between gender (U = 5641, p = 0.033). In conclusion, PET/CT generated anxiety levels in oncological patients, especially before the procedure. Although patients seemed to be satisfied with information delivered by staff and with the NM Department, attention has to be focused on effective interventions strategies that help patients to reduce anxiety.
