Background: Complex medication regimens may adversely affect compliance and treatment outcomes. Complexity can be assessed with the medication regimen complexity index (MRCI), which has proved to be a valid, reliable tool, with potential uses in both practice and research. Objective: To use the MRCI to assess medication regimen complexity in institutionalized elderly people. Setting: Five nursing homes in mainland Portugal. Methods: A descriptive, cross-sectional study of institutionalized elderly people (n = 415) was performed from March to June 2009, including all inpatients aged 65 and over taking at least one medication per day. Main outcome measure: Medication regimen complexity index. Results: The mean age of the sample was 83.9 years (±6.6 years), and 60.2 % were women. The elderly patients were taking a large number of drugs, with 76.6 % taking more than five medications per day. The average medication regimen complexity was 18.2 (±SD = 9.6), and was higher in the females (p < 0.001). The most decisive factors contributing to the complexity were the number of drugs and dosage frequency. In regimens with the same number of medications, schedule was the most relevant factor in the final score (r = 0.922), followed by pharmaceutical forms (r = 0.768) and additional instructions (r = 0.742). Conclusion: Medication regimen complexity proved to be high. There is certainly potential for the pharmacist's intervention to reduce it as part as the medication review routine in all the patients.

Purpose: To evaluate the effects of a six months exercise training program on walking capacity, fatigue and health related quality of life (HRQL). Relevance: Familial amyloidotic polyneuropathy disease (FAP) is an autossomic neurodegenerative disease, related with systemic deposition of amyloidal fibre mainly on peripheral nervous system and mainly produced in the liver. FAP often results in severe functional limitations. Liver transplantation is used as the only therapy so far, that stop the progression of some aspects of this disease. Transplantation requires aggressive medication which impairs muscle metabolism and associated to surgery process and previous possible functional impairments, could lead to serious deconditioning. Reports of fatigue are common feature in transplanted patients. The effect of supervised or home-based exercise training programs in FAP patients after a liver transplant (FAPTX) is currently unknown.

Abstract: Background: Familial amyloidotic polyneuropathy (FAP) is a neurodegenerative disease leading to sensory and motor polyneuropathies, and functional limitations. Liver transplantation is the only treatment for FAP, requiring medication that negatively affects bone and muscle metabolism. The aim of this study was to compare body composition, levels of specific strength, level of physical disability risk, and functional capacity of transplanted FAP patients (FAPTx) with a group of healthy individuals (CON). Methods: A group of patients with 48 FAPTx (28 men, 20 women) was compared with 24 CON individuals (14 men, 10 women). Body composition was assessed by dual-energy X-ray absorptiometry, and total skeletal muscle mass (TBSMM) and skeletal muscle index (SMI) were calculated. Handgrip strength was measured for both hands as was isometric strength of quadriceps. Muscle quality (MQ) was ascertained by the ratio of strength to muscle mass. Functional capacity was assessed by the six-minute walk test. Results: Patients with FAPTx had significantly lower functional capacity, weight, body mass index, total fat mass, TBSMM, SMI, lean mass, muscle strength, MQ, and bone mineral density. Conclusion: Patients with FAPTx appear to be at particularly high risk of functional disability, suggesting an important role for an early and appropriately designed rehabilitation program.

The deposition of amyloid fibers at the peripheral nervous system can induce motor neuropathy in Familial Amiloidotic Polyneuropethy (FAP) patients. This produces progressive reductions in functional capacity. The only treatment for FAP is a liver transplant, followed by aggressive medication that can affect patients' metabolism. To our knowledge, there are no data on body fat distribution or comparison between healthy and FAP subjects, which may be important for clinical assessment and management of this disease. PURPOSE: To analyze body fat content and distribution between FAP patients and healthy subjects. METHODS: Body fat content and distribution were measured through Double Energy X-ray Densitometry (DXA) in two groups. Group 1 consisted of 43 Familial Amyloidotic Polyneuropathy patients (19 males, 32 + 8 Yrs, and 24 females, 37 + 5 yrs), who had liver transplant less than 2 months before. Group 2 consisted of 18 healthy subjects of similar age (8 males, 36 + 7 yrs, and 10 females, 39 + 5 yrs). RESULTS: Healthy subjects showed higher values than FAP patients for: BMI (24,2+2,3kg/m2 vs 22,3+3,8 kg/m2 respectively, p<0,05), % trunk BF (26,21+8,34kg vs 20,78+9,05kg respectively, p<0,05), % visceral BF (24,43+7,97% vs 19,21+9,30% respectively, p<0,05), % abdominal BF (26,63+8,51% vs 20,63+10,35% respectively, p<0,05) abdominal subcutaneous BF (0,533+0,421kg vs 0,353+0,257kg respectively, p=0,05), abdominal BF/BF ratio (0,09+0,02 vs 0,08+0,02 respectively, p<0,05) and abdominal BF/trunk BF ratio (0,19+0,03 vs 0,17+0,03 respectively, p<0,05). CONCLUSIONS: These results showed that FAP patients soon after liver transplantation exhibited a healthier body fat profile compared to controls. However, fat content and distribution varied widely in FAP subjects, suggesting an individualized approach for assessment and intervention rather than general guidelines. Future research is needed to investigate the long term consequences on body fat following liver transplant in this population.

Familial Amiloidotic Polineuropathy (FAP) is a neurodegenerative disease related with systemic deposition of amyloidal fiber mainly on peripheral nervous system, clinically translated by an autonomous sensitive-motor neuropathy with severe functional limitations in some cases. The unique treatment for FAP disease is a liver transplant with a very aggressive medication to muscle metabolism and force production. To our knowledge there are no quantitative characterizations of body composition, strength or functional capacity in this population.

Liver transplantation is used as a only therapy so far, that stop the progression of some aspects of familial amyloidotic polyneuropathy disease (FAP) an autossomic neurodegenerative disease. FAP often results in severe functional limitations. Transplantation requires aggressive medication which impairs bone and muscle metabolism. Malnutrition plus weight loss is already one feature of FAP patients. All this may produce negative consequences on body composition. The effect of exercise training in FAP patients after a liver transplant (FAPTX) is currently unknown. The purpose of this study is to evaluate the effects of a six months exercise training program on body composition in FAPTX patients.

O actual protagonismo que os suplementos alimentares têm vindo a assumir no quadro de novas lógicas de oferta e de consumo de recursos terapêuticos, constitui um facto que é revelador da emergência de novos fenómenos que geram reconfigurações importantes ao nível da dimensão social de novas práticas em torno destes mesmos recursos, com implicações no papel dos profissionais de farmácia. Face a este novo quadro, em que novas realidades profissionais se desenham como resposta a estes fenómenos, o olhar interdisciplinar que aqui se desenvolve procura problematizar os novos papéis dos profissionais ao nível do aconselhamento e da educação para a saúde. Sustenta-se que esse objectivo dificilmente poderá ser mais do que um mero horizonte normativo, se não for capaz de integrar as abordagens de carácter sociológico sobre as importantes reconfigurações das dinâmicas de autonomia e reflexividade leiga que têm vindo a ocorrer em termos de gestão da saúde e de consumos terapêuticos. The current role that dietary supplements have been playing under new logic of supply and consumption of therapeutic resources, is a fact that reveals the emergence of new phenomena that generate major reconfigurations to the social dimension of new practices around these resources, with implications for the professional role of pharmacy practitioners. In this new framework, in which new realities are emerging as a professional response to these phenomena, the interdisciplinary outlook that we develop aims to raise some critical questions concerning the development of new roles in pharmacy practitioners in counseling and health education. We assert that this goal will scarcely be more than just a normative horizon, if unable to integrate the nature of sociological approaches on important reconfiguration of the dynamics of lay autonomy and reflexivity that have been occurring in terms of health management and therapeutic consumptions.

The purpose of this study was to compare levels of specific strength (isometric strength adjusted by lean mass or muscle quality) and functional capacity (meters in 6 minutes walk test) between FAP patients after a liver transplant (4.1±2 months after transplant surgery) (FAPT) and a healthy group (HG).

Introdução – Os benefícios do exercício físico em sobreviventes de cancro da mama têm sido reportados; contudo, a sua prática permanece baixa, tornando importante o conhecimento dos fatores que promovam a motivação e adesão ao exercício nesta população. Objetivos – Identificar as preferências quanto à programação e aconselhamento do exercício físico de uma amostra da população de mulheres portuguesas sobreviventes de cancro da mama e averiguar a influência das variáveis demográficas e médicas nestas preferências. Método – Foi aplicado um questionário a uma amostra não probabilística sequencial de 26 mulheres sobreviventes de cancro da mama. Resultados – A amostra era maioritariamente constituída por mulheres entre os 45 e os 62 anos, casadas ou em união de facto, com ensino básico, empregadas e com Índice de Massa Corporal (IMC) > 24,4. Maioritariamente tinham realizado cirurgia radical há um mês ou mais, apresentavam estadio I do tumor, efetuavam quimioterapia como tratamento adjuvante e algumas realizavam classes de fisioterapia. A maioria das participantes demonstrava interesse em receber aconselhamento, sentia-se apta a participar num programa de exercício, preferia receber aconselhamento face-a-face no hospital e acompanhada por outros doentes oncológicos. O exercício deveria ser supervisionado e com intensidade moderada, sendo as caminhadas o tipo de exercício preferido. Não foi estatisticamente possível realizar a associação entre as variáveis demográficas e médicas e as preferências. Conclusão – Alguns resultados obtidos estão em concordância com estudos prévios; contudo, outros divergem destes. Os resultados obtidos podem fornecer informações importantes para a construção futura de programas de exercício para esta população. ABSTRACT - Introduction – The benefits of physical exercise in cancer survivors have been reported, although it’s practice remains low, becoming important the acknowledgement of the factors that promote the motivation and adhesion of physical exercise in this population. Objectives – To identify the preferences about programming and counseling of physical exercise inside a population-based sample of Portuguese women who have survived breast cancer. We also intend to investigate the influence of demographic and medical variables in those preferences. Method – A questionnaire was applied to a non-probabilistic sequential sample of 26 women that have survived breast cancer. Results – Our sample was mainly composed by women aged between 45 and 62, married or in a cohabitation state, with basic instruction, employed and with a Body Mass Index (BMI)> 24.4. Most of them have had radical mastectomy for at least one month, had the Stage I of the tumor, and had done chemotherapy as an adjuvant treatment and some of them were practicing post-surgery physical therapy. The majority of participants showed interest in receiving counseling, felt able to participate in an exercise program, preferred receiving face-to-face counseling, at the hospital and with other cancer patients. The exercise should be supervised and with a moderate intensity. Walking was their preferred choice of exercise. It was not statistically possible to establish the relationship between demographic and medical variables and those preferences. Conclusion – Some results are in agreement with previous studies; however, others diverge from these. The results obtained can provide important information for future construction of exercise programs for this population.

Neste artigo identificam-se os padrões de consumo terapêutico na população portuguesa, visando dar conta de um novo padrão emergente nas sociedades modernas, aqui designado de Pluralismo Terapêutico, noção com a qual se categoriza o uso conjugado ou alternado de recursos farmacológicos e naturais nas trajetórias terapêuticas dos indivíduos. O respetivo suporte empírico decorre de uma investigação, já concluída, que teve por base uma amostra nacional representativa. Os resultados mostram uma dualização dos consumos terapêuticos que é constituída por um padrão dominante de Farmacologismo – i.e., uso exclusivo de fármacos – coexistente com uma tendência crescente de pluralismo terapêutico. O efeito das fontes de informação terapêutica e dos seus usos leigos, bem como das perceções sociais de risco sobre o natural e o farmacológico, constitui neste estudo uma referência analítica central para a interpretação dos padrões encontrados. - ABSTRACT: In this article we identify patterns of therapeutic consumption, with the purpose of assessing an emerging pattern in modern societies, here designated as Therapeutic Pluralism, referring to the conjugated or alternated use of pharmacological and natural resources in the therapeutic trajectories of individuals. The empirical basis for this analysis stems from a concluded research on the topic, and is focused on a questionnaire administered to a representative sample of the Portuguese population. The results show a duality in therapeutic consumptions, expressed in the coexistence of a dominant pattern of Pharmacologism – that is, the exclusive therapeutic consumption of pharmaceuticals – and a growing trend towards therapeutic pluralism. The effects of information sources on health and its lay uses, as well as of the social perceptions of risk concerning the natural and the pharmacological, constitute key analytical references for this study’s interpretation of the identified patterns.

A tecnologia do código de barras possibilita que a identificação e a história do doente se relacionem com todos os momentos de prestação de cuidados, como sejam: a prescrição médica, a dispensa e administração de medicamentos. Para tal, é necessário dispor de uma ferramenta informática apropriada, que gera os códigos de barras e respectivas ligações e dispositivos de leitura desses mesmos códigos. Segundo a American Society of Hospital Pharmacists, os tipos de erros de medicação podem agrupar-se em categorias, que não podem ser mutuamente exclusivas, devido à natureza multifatorial e multidisciplinar. Estas são as seguintes: prescribing error; omission error; wrong time error time; unauthorized drug error; wrong dosage form error; wrong drug-preparation error; wrong administration-technique error; deteriorated drug error; monitoring error; compliance error; other medication error. Para a correcta utilização do medicamento tem que se garantir que o doente certo toma o medicamento certo, na quantidade certa, na via de administração e hora certa (5RIGHTS). A evidência científica demonstrou que esta tecnologia conduz a uma REDUÇÃO DOS ERROS DE MEDICAÇÃO, nomeadamente na categoria da administração de medicamentos. Objectivo do estudo: demonstrar o impacto da tecnologia do código de barras na redução dos erros de medicação em meio hospitalar.

A distribuição de medicamentos é uma função da farmácia hospitalar que, com metodologia e circuitos próprios, torna disponível o medicamento correto, na quantidade e qualidade certas, para cumprimento da prescrição médica proposta, para cada doente e todos os doentes do hospital. Assim, um sistema de distribuição de medicamentos é o método que abrange os procedimentos que ocorrem entre a prescrição e a administração de um medicamento. Um sistema de distribuição de medicamentos deve ser racional, eficiente, económico, organizado, seguro e estar de acordo com o esquema terapêutico prescrito. No final da década 50, os sistemas tradicionais de distribuição de medicamentos necessitavam de ser repensados, visando melhorar a segurança na distribuição e administração de medicamentos. Nos anos 60, farmacêuticos hospitalares apresentaram um novo sistema: a dose unitária, capaz de diminuir a incidência de erros de medicação. Estes sistemas podem dividir-se então em coletivos (sistema de distribuição tradicional e sistema de reposição de stocks nivelados) ou individuais (sistema de distribuição personalizada e sistema de distribuição por dose unitária). Os seus objetivos são reduzir os erros de medicação, racionalizar a distribuição, aumentar o controlo sobre os medicamentos, reduzir os custos com os medicamentos e aumentar a segurança para os doentes. Há, portanto, necessidade de cada hospital manter um sistema de distribuição de medicamentos adequado às suas necessidades. Sendo o erro de medicação qualquer discrepância entre a medicação prescrita e a administrada, a verificação da qualidade de qualquer sistema de distribuição tem como um dos indicadores a incidência dos erros de medicação. A ASHP, nas Guidelines on Preventing Medication Errors in Hospitals, refere que, para minimizar o erro, devem ser projetadas formas seguras de requisitar, dispensar e administrar os medicamentos. Objectivo do estudo: descrever e comparar os diferentes sistemas de distribuição de medicamentos, utilizados em meio hospitalar, notando a sua evolução e utilização ao longo dos tempos, bem como salientar o seu contributo para a segurança do doente (segurança da medicação).

The phenomenon of aging is nowadays society as acquired the status of a social problem, with growing attention and concern, leading to an increase number of studies dedicated to the elderly. The lack of domestic, familiar or social support often lead elderly to nursing homes. Institutionalization is in many cases the only opportunity to have access to health care and life quality. Aging is also associated with a higher prevalence of chronic diseases that require long term medication sometimes for life. Frequently the onset of multiple pathologies at the same time require different therapies and the phenomenon of polypharmacy (five ou more drugs daily) can occur. Even more, the slow down of physiological and cognitives mechanisms associated with these chronic diseases can interphere, in one hand, with the pharmacocinetic of many medications and, on the other hand, with the facility to accomplish the therapeutical regimen. All of these realities contribute to an increase of pharmacotherapeutical complexity, decreasing the adherence and effectiveness of treatment. The pharmacotherapeutical complexity of an individual is characterized by the conciliator element of different characteristics of their drug therapy, such as: the number of medications used; dosage forms; dosing frequency and additional indications. It can be measured by the Medication Regimen Complexity Index (MRCI), originally validated in English.

Nowadays, the phenomenon of population ageing represents an worldwide problem, which assumes particular significance in Portugal. As they get older, individuals present more comorbidities and consequently consume an increasing number of drugs, which contributes to a growing drug therapy complexity. The institutionalized elders are particularly affected by this occurrence. Drug therapy complexity is defined as the conciliator of several characteristics of the pharmacotherapy and can affect patient’s safety and medication adherence. It can be measured with Medication Regimen Complexity Index (MRCI). This study aims to determine the drug therapy complexity of institutionalized elders in order to assess the need of pharmacotherapeutic follow-up.
