Introdução – A medição da pressão arterial (PA), utilizando dispositivos automáticos, é frequentemente realizada na prática clínica e na automedição, permitindo adquirir informação fiável para o diagnóstico, controlo e tratamento da hipertensão arterial. Porém, muitos dos dispositivos automáticos disponíveis no mercado não estão validados segundo protocolos existentes para o efeito. O objetivo do estudo foi confirmar a validação do dispositivo de medição automática da PA, OMRON® M6 Comfort, segundo o Protocolo Internacional da European Society of Hypertension (ESH), de 2010, para a validação de dispositivos de medição automática da PA em adultos. Metodologia – Foram estudados 33 indivíduos, aos quais foram realizadas 9 medições sequenciais da PA, no braço esquerdo, com um esfignomanómetro aneróide alternando com o dispositivo automático. Seguidamente avaliaram-se as diferenças entre os valores obtidos pelos dispositivos para a pressão arterial sistólica (PAS) e diastólica (PAD), classificando-as em três níveis (≤ 5, ≤ 10 ou ≤ 15 mmHg). O número das diferenças em cada nível foi comparado ao requerido pelo Protocolo (fase 1.1). Para cada sujeito foi ainda determinado o número de diferenças com valores ≤ 5 mmHg. Pelo menos 24 dos 33 indivíduos devem ter 2 ou 3 diferenças com valores ≤ 5 mmHg e no máximo 3 dos 33 indivíduos podem apresentar as 3 diferenças com valores > 5 mmHg (fase 1.2). Resultados – O dispositivo OMRON® M6 Comfort foi aprovado nas fases 1.1 e 1.2 para a PAS e PAD. A média das diferenças entre as medições da PA, determinada pelos dispositivos automático e manual, foi de -0,82 ± 5,62 mmHg para a PAS e 2,14 ± 5,15 mmHg para a PAD. Considerações Finais – O dispositivo OMRON® M6 Comfort é válido para a medição da PA em adultos, de acordo com o Protocolo Internacional da ESH, de 2010. - ABSTRACT - Introduction – The measurement of blood pressure (BP) using automatic devices is often performed in clinical practice and self-measurement allowing the acquisition of reliable information for the diagnosis, monitoring and treatment of hypertension. However not all of the automated devices available in the market are validated in accordance with the existing protocols for this purpose. The purpose of this study was to confirm the validation of the automatic measuring device of the BP, OMRON® M6 Comfort, according to the “European Society of Hypertension International Protocol revision 2010 for the validation of blood pressure measuring devices in adults”. Methodology – The study involved 33 subjects, in each one of them, 9 sequential measurements of BP were performed, in the left arm, with the aneroid sphygmomanometer alternating with the automatic device. Afterwards, the differences on the values obtained by the different devices were evaluated, for systolic blood pressure (SBP) and diastolic (DBP), and these differences were then classified into three levels (≤ 5, ≤ 10 or ≤ 15 mmHg). The number of differences at each level was compared to the number required by the protocol (phase 1.1). For each subject the number of differences with values ≤ 5 mmHg was also determined. At least 24 of the 33 subjects should have 2 or 3 differences with values ≤ 5 mmHg and a maximum of 3 of the 33 subjects may have all differences with values > 5 mmHg (phase 1.2). Results – The device OMRON M6 Comfort ® was approved in phases 1.1 and 1.2 for SBP and DBP. The average difference between measurements of BP, as determined by automatic and manual devices, was -0.82 ± 5.62 mmHg for SBP and 2.14 ± 5.15 mmHg for DBP. Conclusion – The device OMRON M6 Comfort® is valid for measuring BP in adults, according to the ESH International Protocol of 2010.

In order to evaluate the capacity of laser scanning cytometry (LSC) to detect acid-fast bacilli directly on clinical samples, a comparison between Kinyoun-stained smears analyzed under light microscopy and propidium iodide-auramine-stained smears analyzed by LSC was performed. The results were compared with those for culture on BACTEC MGIT 960. LSC is a new, reliable methodology to detect Mycobacteria.

In this work the identification and diagnosis of various stages of chronic liver disease is addressed. The classification results of a support vector machine, a decision tree and a k-nearest neighbor classifier are compared. Ultrasound image intensity and textural features are jointly used with clinical and laboratorial data in the staging process. The classifiers training is performed by using a population of 97 patients at six different stages of chronic liver disease and a leave-one-out cross-validation strategy. The best results are obtained using the support vector machine with a radial-basis kernel, with 73.20% of overall accuracy. The good performance of the method is a promising indicator that it can be used, in a non invasive way, to provide reliable information about the chronic liver disease staging.

In this work liver contour is semi-automatically segmented and quantified in order to help the identification and diagnosis of diffuse liver disease. The features extracted from the liver contour are jointly used with clinical and laboratorial data in the staging process. The classification results of a support vector machine, a Bayesian and a k-nearest neighbor classifier are compared. A population of 88 patients at five different stages of diffuse liver disease and a leave-one-out cross-validation strategy are used in the classification process. The best results are obtained using the k-nearest neighbor classifier, with an overall accuracy of 80.68%. The good performance of the proposed method shows a reliable indicator that can improve the information in the staging of diffuse liver disease.

This project was developed to fully assess the indoor air quality in archives and libraries from a fungal flora point of view. It uses classical methodologies such as traditional culture media – for the viable fungi – and modern molecular biology protocols, especially relevant to assess the non-viable fraction of the biological contaminants. Denaturing high-performance liquid chromatography (DHPLC) has emerged as an alternative to denaturing gradient gel electrophoresis (DGGE) and has already been applied to the study of a few bacterial communities. We propose the application of DHPLC to the study of fungal colonization on paper-based archive materials. This technology allows for the identification of each component of a mixture of fungi based on their genetic variation. In a highly complex mixture of microbial DNA this method can be used simply to study the population dynamics, and it also allows for sample fraction collection, which can, in many cases, be immediately sequenced, circumventing the need for cloning. Some examples of the methodological application are shown. Also applied is fragment length analysis for the study of mixed Candida samples. Both of these methods can later be applied in various fields, such as clinical and sand sample analysis. So far, the environmental analyses have been extremely useful to determine potentially pathogenic/toxinogenic fungi such as Stachybotrys sp., Aspergillus niger, Aspergillus fumigatus, and Fusarium sp. This work will hopefully lead to more accurate evaluation of environmental conditions for both human health and the preservation of documents.

In the literature, concepts of “polyneuropathy”, “peripheral neuropathy” and “neuropathy” are often mistakenly used as synonyms. Polyneuropathy is a specific term that refers to a relatively homogenous process that affects multiple peripheral nerves. Most of these tend to present as symmetric polyneuropathies that first manifest in the distal portions of the affected nerves. Many of these distal symmetric polyneuropathies are due to toxic-metabolic causes such as alcohol abuse and diabetes mellitus. Other distal symmetric polyneuropathies may result from an overproduction of substances that result in nerve pathology such as is observed in anti-MAG neuropathy and monoclonal gammopathy of undetermined significance. Other “overproduction” disorders are hereditary such as noted in the Portuguese type of familial amyloid polyneuropathy (FAP). FAP is a manifestation of a group of hereditary amyloidoses; an autosomal dominant, multisystemic disorder wherein the mutant amyloid precursor, transthyretin, is produced in excess primarily by the liver. The liver accounts for approximately 98% of all transthyretin production. FAP is confirmed by detecting a transthyretin variant with a methionine for valine substitution at position 30 [TTR (Met30)]. Familial Amyloidotic Polyneuropathy (FAP) – Portuguese type was first described by a Portuguese neurologist, Corino de Andrade in 1939 and published in 1951. Most persons with this disorder are descended from Portuguese sailors who sired offspring in various locations, primarily in Sweden, Japan and Mallorca. Their descendants emigrated worldwide such that this disorder has been reported in other countries as well. More than 2000 symptomatic cases have been reported in Portugal. FAP progresses rapidly with an average time course from symptom onset to multi-organ involvement and death between ten and twenty years. Treatments directed at removing this aberrant protein such as plasmapheresis and immunoadsorption proved to be unsuccessful. Liver transplantation has been the only effective solution as evidenced by almost 2000 liver transplants performed worldwide. A therapy for FAP with a novel agent, “Tafamidis” has shown some promise in ongoing phase III clinical trials. It is well recognized that regular physical activity of moderate intensity has a positive effect on physical fitness as gauged by body composition, aerobic capacity, muscular strength and endurance and flexibility. Physical fitness has been reported to result in the reduction of symptoms and lesser impairment when performing activities of daily living. Exercise has been advocated as part of a comprehensive approach to the treatment of chronic diseases. Therefore, this chapter concludes with a discussion of the role of exercise training on FAP.

Objectives - Review available guidance for quality assurance (QA) in mammography and discuss its contribution to harmonise practices worldwide. Methods - Literature search was performed on different sources to identify guidance documents for QA in mammography available worldwide in international bodies, healthcare providers, professional/scientific associations. The guidance documents identified were reviewed and a selection was compared for type of guidance (clinical/technical), technology and proposed QA methodologies focusing on dose and image quality (IQ) performance assessment. Results - Fourteen protocols (targeted at conventional and digital mammography) were reviewed. All included recommendations for testing acquisition, processing and display systems associated with mammographic equipment. All guidance reviewed highlighted the importance of dose assessment and testing the Automatic Exposure Control (AEC) system. Recommended tests for assessment of IQ showed variations in the proposed methodologies. Recommended testing focused on assessment of low-contrast detection, spatial resolution and noise. QC of image display is recommended following the American Association of Physicists in Medicine guidelines. Conclusions - The existing QA guidance for mammography is derived from key documents (American College of Radiology and European Union guidelines) and proposes similar tests despite the variations in detail and methodologies. Studies reported on QA data should provide detail on experimental technique to allow robust data comparison. Countries aiming to implement a mammography/QA program may select/prioritise the tests depending on available technology and resources.

Aim - To identify clinical and/or genetic predictors of response to several therapies in Crohn’s disease (CD) patients. Methods - We included 242 patients with CD (133 females) aged (mean ± standard deviation) 39 ± 12 years and a disease duration of 12 ± 8 years. The single-nucleotide polymorphisms (SNPs) studied were ABCB1 C3435T and G2677T/A, IL23R G1142A, C2370A, and G9T, CASP9 C93T, Fas G670A and LgC844T, and ATG16L1 A898G. Genotyping was performed with real-time PCR with Taqman probes. Results - Older patients responded better to 5-aminosalicylic acid (5-ASA) and to azathioprine (OR 1.07, p = 0.003 and OR 1.03, p = 0.01, respectively) while younger ones responded better to biologicals (OR 0.95, p = 0.06). Previous surgery negatively influenced response to 5-ASA compounds (OR 0.25, p = 0.05), but favoured response to azathioprine (OR 2.1, p = 0.04). In respect to genetic predictors, we observed that heterozygotes for ATGL16L1 SNP had a significantly higher chance of responding to corticosteroids (OR 2.51, p = 0.04), while homozygotes for Casp9 C93T SNP had a lower chance of responding both to corticosteroids and to azathioprine (OR 0.23, p = 0.03 and OR 0.08, p = 0.02,). TT carriers of ABCB1 C3435T SNP had a higher chance of responding to azathioprine (OR 2.38, p = 0.01), while carriers of ABCB1 G2677T/A SNP, as well as responding better to azathioprine (OR 1.89, p = 0.07), had a lower chance of responding to biologicals (OR 0.31, p = 0.07), which became significant after adjusting for gender (OR 0.75, p = 0.005). Conclusions - In the present study, we were able to identify a number of clinical and genetic predictors of response to several therapies which may become of potential utility in clinical practice. These are preliminary results that need to be replicated in future pharmacogenomic studies.

Doenças crónicas são de longa duração, de progressão lenta e induzem alterações na vida das pessoas, que são confrontadas com um conjunto de fatores que exercem um impacto negativo na sua qualidade de vida (QdV). A QdV é um conceito envolvendo componentes essenciais da qualidade humana: físicas, psicológicas, sociais, culturais e espirituais. Após o diagnóstico e com a doença estabilizada, os doentes procuram novas formas de lidar com esta. Este estudo teve como objetivo identificar fatores psicossociais preditivos (otimismo, afeto positivo e negativo, adesão aos tratamentos, suporte social e espiritualidade) da QdV (bem-estar geral, saúde física, saúde mental) e bem-estar subjetivo (BES) em pessoas com doenças crónicas. Amostra constituída por 774 indivíduos [30% diabetes, 27,1% cancro, 17,2% diabetes, 12% epilepsia, 11,5% esclerose múltipla e 2,2% miastenia, 70,5% do sexo feminino, idade M(DP)=42,9(11,6), educação M(DP)=9,6(4,7), anos diagnóstico M(DP)=12,8(9,7), classificação da doença M(DP)=6,6 (2,8)], recrutados nos hospitais centrais portugueses. Aplicando Modelos de Equações Estruturais e ajustando para variáveis sociodemográficas e clínicas, verificou-se que, pessoas mais otimistas, mais ativas e com uma melhor adesão aos tratamentos apresentam um melhor bem-estar geral, uma melhor saúde mental e um melhor bem-estar subjetivo; uma melhor adesão aos tratamentos contribui para uma melhor saúde física; melhor suporte social reflete-se numa melhor saúde mental; pessoas com mais espiritualidade apresentam uma melhor saúde física e uma melhor saúde mental. Estas conclusões contribuem para a definição de uma terapia que pode ajudar a uma melhor adaptação dos protocolos de tratamento para atender às necessidades dos doentes.

Clinical and environmental samples from Portugal were screened for the presence of Aspergillus and the distributions of the species complexes were determined in order to understand how their distributions differ based on their source. Fifty-seven Aspergillus isolates from clinical samples were collected from 10 health institutions. Six species complexes were detected by internal transcribed spacer sequencing; Fumigati, Flavi, and Nigri were found most frequently (50.9%, 21.0%, and 15.8%, respectively). β-tubulin and calmodulin sequencing resulted in seven cryptic species (A. awamorii, A. brasiliensis, A. fructus, A. lentulus, A. sydowii, A. tubingensis, Emericella echinulata) being identified among the 57 isolates. Thirty-nine isolates of Aspergillus were recovered from beach sand and poultry farms, 31 from swine farms, and 80 from hospital environments, for a total 189 isolates. Eleven species complexes were found in these 189 isolates, and those belonging to the Versicolores species complex were found most frequently (23.8%). There was a significant association between the different environmental sources and distribution of the species complexes; the hospital environment had greater variability of species complexes than other environmental locations. A high prevalence of cryptic species within the Circumdati complex was detected in several environments; from the isolates analyzed, at least four cryptic species were identified, most of them growing at 37ºC. Because Aspergillus species complexes have different susceptibilities to antifungals, knowing the species-complex epidemiology for each setting, as well as the identification of cryptic species among the collected clinical isolates, is important. This may allow preventive and corrective measures to be taken, which may result in decreased exposure to those organisms and a better prognosis.

Objective: A new protocol for fixation and slide preservation was evaluated in order to improve the quality of immunocytochemical reactions on cytology slides. Methods: The quality of immunoreactions was evaluated retrospectively on 186 cytology slides (130 direct smears, 56 cytospins) prepared from different cytology samples. Ninety-three of the slides were air dried, stored at -20 °C and fixed in acetone for 10 minutes (Protocol 1), whereas the other 93 were immediately fixed in methanol at -20 °C for at least 30 minutes, subsequently protected with polyethylene glycol (PEG) and stored at room temperature (Protocol 2). Immunocytochemical staining, with eight primary antibodies, was performed on a Ventana BenchMark Ultra instrument using an UltraView Universal DAB Detection Kit. The following parameters were evaluated for each immunoreaction: morphology preservation, intensity of specific staining, background and counterstain. The slides were blinded and independently scored by four observers with marks from 0 to 20. Results: The quality of immunoreactions was better on methanol-fixed slides protected with PEG than on air-dried slides stored in the freezer: X¯ = 14.44 ± 3.58 versus X¯ = 11.02 ± 3.86, respectively (P < 0.001). Conclusion: Immediate fixation of cytology slides in cold methanol with subsequent application of PEG is an easy and straightforward procedure that improves the quality of immunocytochemical reactions and allows the storage of the slides at room temperature.
