The aim of this study is to assess the levels of airborne ultrafine particles emitted in welding processes (tungsten inert gas [TIG], metal active gas [MAG] of carbon steel, and friction stir welding [FSW] of aluminum) in terms of deposited area in pulmonary alveolar tract using a nanoparticle surface area monitor (NSAM) analyzer. The obtained results showed the dependence of process parameters on emitted ultrafine particles and demonstrated the presence of ultrafine particles compared to background levels. Data indicated that the process that resulted in the lowest levels of alveolar deposited surface area (ADSA) was FSW, followed by TIG and MAG. However, all tested processes resulted in significant concentrations of ultrafine particles being deposited in humans lungs of exposed workers.

The purpose of this study is a cross-qualitative and quantitative gait analysis in 3 traumatic unilateral amputees using prosthesis with pin suspension compared to the use of prosthesis with a high vacuum suspension, the Harmony® system. In Portugal, there aren’t many studies made in the field of orthotic and prosthetic and knowledge about the number of amputees in the country. The only know is that the major cause of lower limb amputation is diabetes mellitus, being the most affected population the older age groups. The combination of technological developments with daily needs of the amputees is becoming more and more important for they better quality of life. This work was done during the curricular unit “Investigation in Prosthetics and Orthotics” class, in the 4th year of Health Technology School of Lisbon, in Portugal. This study analyzes if the change of suspension in transtibial prosthesis will influence some physiological response in amputees.

Physical function is limited post-liver transplantation and exercise training can improve exercise capacity following transplantation but previously assessment of functional capacity is necessary. The 6 minute walk test (6MWT) is routinely used for studying patient’s exercise capacity and is less expensive and time consuming than a Cardiopulmonary Exercise Test (CPXT). There have been some studies looking at the relationship between 6MWT and peak VO2 in patients but few presented an adequate equation for predicted peak VO2 from 6MWT to transplanted patients and none for Familial Amyloidotic Polyneuropathy (FAP) liver transplanted patients (FAPTx). The aim of this study was to compare the validity of 6 equations as predictors of VO2 from the 6MWT and compare it with directly measured VO2 in patients FAPTx.

Levels of risk for future disability can be assessed with grip strength. This assessment is of fundamental importance for establishing prevention strategies. It also allows verifying relationships with functional capacity of individuals. Most studies on grip strength use the JAMAR Hydraulic dynamometer that provides the value of isometric force obtained during the performance of grip movement and is considered the “gold standard” for measurement of grip strength. However, there are different dynamometers available commercially, such as portable computerized dynamometer E-Link (Biometrics), which provides the value of maximum force (peak force) in addition to other variables as the rate of fatigue for hand strength, among others. Of our knowledge, there are no studies that allow us to accept or not and compare values obtained with both devices and perhaps use them interchangeably. The aim of this study was to evaluate the absolute agreement between the measurements of grip strength (peak force or maximum force in kg) obtained from two different devices (portable dynamometers): a computerized (E-Link, Biometrics) and one hydraulic (JAMAR).

De acordo com o estudo de Gatta e col, em Janeiro de 2003, 11,6 milhões de europeus apresentaram história clínica de cancro. Destes, uma em cada 73 mulheres tinham história de cancro da mama enquanto que um em cada 160 homens apresentaram história de cancro da próstata. A prevalência em 2003 conduziu a valores estimados de - 13 milhões de europeus afetados por cancro em 2010. O cancro da mama é o tipo de cancro que mais frequentemente é diagnosticado e a segunda causa de morte por cancro nas mulheres a nível mundial e também em Portugal (Plano Nacional de Prevenção e Controlo das Doenças Oncológicas 2007-2010. De acordo com o INE, 12,4/100.000 habitantes das mortes com menos de 65 anos deveram-se a cancro da mama feminino. As opções de tratamento para o cancro podem incluir cirurgia, radioterapia, hormonoterapia, quimioterapia ou imunoterapia, as quais poderão apresentar efeitos colaterais suscetíveis de influenciar a participação no exercício e a resposta ao mesmo. Independentemente da intervenção terapêutica no cancro, a fadiga é um efeito colateral comum. Um cancro é considerado como curado quando as remissões são permanentes ou quando não existe recorrência há mais de 5 anos. Uma das razões frequentemente subestimada como causa de fadiga é a perda de condição física como resultado do acamamento e do incentivo por parte de familiares e/ou prestadores de cuidados de saúde a um maior descanso quando o doente refere fadiga. Esta situação pode limitar ainda mais as atividades da vida diária, conduzindo a um maior descondicionamento e intolerância ao exercício, cujas consequências são mais dramáticas a nível do sistema cardiorrespiratório e serem por si só responsáveis pela perda de cerca de 30 por cento da capacidade funcional do doente. De acordo com estudos efetuados em portadores de cancro da mama, a uma capacidade aeróbia abaixo dos 8 MET’s associa-se um aumento de mortalidade 3 vezes mais elevado quando comparado com mulheres com cancro da mama com capacidade aeróbia superior a 8 MET’s.

Liver transplantation is nowadays the only effective answer to adjourn the outcome of functional limitations associated with familial amyloidotic polyneuropathy (FAP), a neurodegenerative disease characterized by sensory and motor polyneuropathies. Nevertheless, there is a detrimental impact associated with the after-surgery period on the fragile physical condition of these patients. Exercise training has been proven to be effective on reconditioning patients after transplantation. However, the effects of exercise training in liver transplanted FAP patients have not been scrutinized yet.

O reconhecimento académico e o crescente investimento realizado no seio das escolas catapultaram a expansão da rede de oferta de cursos e instituições de ensino, o intercâmbio e cooperação no plano internacional, assim como o reforço do processo da representação socioprofissional. Na transição do milénio e do século, passados 110 anos dos primeiros registos sobre a evolução e desenvolvimento destas profissões, projeta-se um conjunto de realidades que porventura catalisarão o curso da sua história em Portugal. No campo do ensino, a valorização do reconhecimento académico (com os mestrados e doutoramentos) perspetiva-se, um conjunto de novos desafios de sinergias nos domínios científico e tecnológico, dissipando-se cada vez mais as fronteiras destas duas dimensões. A investigação científica apresenta-se como um instrumento de articulação privilegiada entre o universo do ensino superior e o terreno profissional.

The basic function of the human hand is the manipulation and grasping of various objects in all daily activities, including work activities. This is greatly influenced by strength and manual dexterity. However age, gender and other contexts such as work or leisure activities could influence strength. Handgrip strength, a measure of maximum voluntary force of the hand, has proved to be reliable and valid as an objective parameter to evaluate the functional integrity of the hand as part of the musculoskeletal system. It correlates highly with strength in other muscular groups and is therefore considered as a good indicator of overall muscular strength and functional stress and could be used as a predictor of physical disability. Handgrip strength assessment is simple and reliable and used commonly by several investigators and health professionals, in different contexts (medical, nutritional, rehabilitation, professional settings, engineering, etc.) and with different purposes (research, diagnostic, assessment, etc.). In clinical and rehabilitation settings is of vital importance in the determination of effectiveness of several interventions and for monitoring evolution of diseases. Various ways (methods, techniques and equipments) of collecting information on grip strength have been reported. This chapter will review basic concepts on handgrip function, methodologies of assessment, contexts of application and correlates, such as physical activity, health or nutritional status. Several populations and reference values as also the relationships between handgrip and clinical status, aging, risk of disability and diseases, will be discussed.

In the literature, concepts of “polyneuropathy”, “peripheral neuropathy” and “neuropathy” are often mistakenly used as synonyms. Polyneuropathy is a specific term that refers to a relatively homogenous process that affects multiple peripheral nerves. Most of these tend to present as symmetric polyneuropathies that first manifest in the distal portions of the affected nerves. Many of these distal symmetric polyneuropathies are due to toxic-metabolic causes such as alcohol abuse and diabetes mellitus. Other distal symmetric polyneuropathies may result from an overproduction of substances that result in nerve pathology such as is observed in anti-MAG neuropathy and monoclonal gammopathy of undetermined significance. Other “overproduction” disorders are hereditary such as noted in the Portuguese type of familial amyloid polyneuropathy (FAP). FAP is a manifestation of a group of hereditary amyloidoses; an autosomal dominant, multisystemic disorder wherein the mutant amyloid precursor, transthyretin, is produced in excess primarily by the liver. The liver accounts for approximately 98% of all transthyretin production. FAP is confirmed by detecting a transthyretin variant with a methionine for valine substitution at position 30 [TTR (Met30)]. Familial Amyloidotic Polyneuropathy (FAP) – Portuguese type was first described by a Portuguese neurologist, Corino de Andrade in 1939 and published in 1951. Most persons with this disorder are descended from Portuguese sailors who sired offspring in various locations, primarily in Sweden, Japan and Mallorca. Their descendants emigrated worldwide such that this disorder has been reported in other countries as well. More than 2000 symptomatic cases have been reported in Portugal. FAP progresses rapidly with an average time course from symptom onset to multi-organ involvement and death between ten and twenty years. Treatments directed at removing this aberrant protein such as plasmapheresis and immunoadsorption proved to be unsuccessful. Liver transplantation has been the only effective solution as evidenced by almost 2000 liver transplants performed worldwide. A therapy for FAP with a novel agent, “Tafamidis” has shown some promise in ongoing phase III clinical trials. It is well recognized that regular physical activity of moderate intensity has a positive effect on physical fitness as gauged by body composition, aerobic capacity, muscular strength and endurance and flexibility. Physical fitness has been reported to result in the reduction of symptoms and lesser impairment when performing activities of daily living. Exercise has been advocated as part of a comprehensive approach to the treatment of chronic diseases. Therefore, this chapter concludes with a discussion of the role of exercise training on FAP.

Introdução – A avaliação da aptidão física permite a identificação de níveis de incapacidade e risco de perda funcional, pelo que é extremamente útil na estruturação de programas de atividade física específicos, que constituem a primeira intervenção no que toca à intervenção não-farmacológica na Hipertensão Arterial Sistémica (HAS). Objetivo – Avaliar a aptidão física funcional e os níveis de atividade física de idosos hipertensos da Cidade da Praia – Cabo Verde. Metodologia – A flexibilidade, força e resistência muscular, resistência aeróbia e composição corporal foram avaliadas através da bateria de Fullerton e dinamometria isométrica manual. O nível de atividade física foi avaliado pelo questionário Seven Day Physical Activity Recall (7D-PAR). Resultados – Participaram 75 idosos, com idades compreendidas entre os 60 e os 99 anos, 68% dos quais eram mulheres, com índice de massa corporal (IMC) de 25,2±5,4, com diagnóstico clínico de HAS. O tempo despendido em atividades de intensidade moderada encontrava-se dentro dos valores recomendados, embora a atividade intensa se encontrasse muito abaixo do recomendado e não efetuassem qualquer exercício de força e flexibilidade. A resistência aeróbia expressa pelo Teste 6 Minutos de Marcha (T6MM) apresentou valores médios baixos (293,5±112,2m), bem como a flexibilidade e a força de preensão. Esta associação é um indicador de baixa capacidade funcional. Conclusão – Os baixos níveis de aptidão física apresentados, associados a baixos níveis de atividade física indiciam baixa capacidade funcional e justificam a necessidade urgente da inserção de programas de atividade física nesta população.

The hand is one of the most important instruments of the human body, mainly due to the possibility of grip movements. Grip strength has been described as an important predictor of functional capacity. There are several factors that may influence it, such as gender, age and anthropometric characteristics. Functional capacity refers to the ability to perform daily activities which allow the individual to self-care and to live with autonomy. Composite Physical Function (CPF) scale is an evaluation tool for functional capacity that includes daily activities, self-care, sports activities, upper limb function and gait capacity. In 2011, Portugal had 15% of young population (0-14years) and 19% of elderly population (over 65 years). Considering the double-ageing phenomen, it is important to understand the effect of the grip strength in elderly individuals, considering their characteristics, as the need to maintainin dependency as long as possible.

Dado que os níveis de actividade física se encontram abaixo dos valores mínimos recomendados deveria ser encontrada uma estratégia de aumento do tempo dispendido na actividade física leve a moderada idealmente no PRÉ TRANSPLANTE.

Purpose - To verify the results of a diaphragmatic breathing technique (DBT) on diaphragmatic range of motion in healthy subjects. Methods - A total of 51 healthy subjects (10 male; 41 female), mean age 20 years old and a body mass index (BMI) ranging from 15.6 to 34.9 kg/m2, were enrolled in this study. Diaphragmatic range of motion was assessed by M-mode ultrasound imaging. Measurements were made before and after the DBT implementation in a standard protocol, based on 3 seconds of inspiration starting from a maximum expiration. Differences between assessments were analyzed by descriptive statistics and t-test (p < 0.05). Results - Mean value range of motion before DBT was 55.3 ± 13.4 mm and after DBT was 63.8 ± 13.2 mm showing a significant improvement of 8.5 ± 14.7 mm (p < 0.001). A strong correlation between the slope and the range of motion was found (r = 0.71, p < 0.001). Conclusions - Based on ultrasound measurements, it has been proved that DBT really contributes to a higher diaphragmatic range of motion. Future studies are needed in order to understand the influence of protocol parameters (e.g. inspiration time). Clinical implications - In the contest of evidence-based practice in physiotherapy, it has been showed by objective measurements that DBT improves the diaphragm range of motion, translating into a more efficient ventilatory function and thus can be used in clinical setting. To our knowledge this is the first study to assess the effects of DBT on range of motion of diaphragm muscle with ultrasound imaging.

A DPOC é uma das principais causas mundiais de morbilidade e mortalidade, representando um problema de saúde pública devido ao elevado consumo de recursos sanitários e económicos associados. A reabilitação pulmonar é uma recomendação standard nos cuidados a estes pacientes, de forma a controlar os sintomas da doença e optimizar a capacidade funcional destes indivíduos, reduzindo assim os custos de saúde associados às exacerbações e limitação da actividade e participação. Contudo, em pacientes com DPOC severa o exercício físico pode ser de difícil desempenho devido a dispneia extrema, diminuição da força muscular e fadiga, ou inclusive hipoxemia e dispneia durante pequenos esforços e actividades diárias, limitando a sua qualidade de vida. Assim, a VNI tem sido usada em combinação com o exercício, de forma a melhorar a capacidade de exercício nestes doentes, ainda que sem consenso para a sua recomendação. Assim, tivemos como objectivo verificar se a utilização de VNI durante o exercício é mais eficaz que exercício sem VNI, na dispneia, distância percorrida, gasimetria e estado de saúde, em pacientes com DPOC, através de revisão sistemática e meta-análise.
