Objective: To know the impact of the Dynesys system on the functional outcomes in patients with spinal degenerative diseases. Summary of background data: Dynesys system has been proposed as an alternative to vertebral fusion for several spinal degenerative diseases. The fact that it has been used in people with different diagnosis criteria using different tools to measure clinical outcomes makes very difficult unifying the results available nowadays. Methods: The data base of Medlars Online International Literature (MEDLINE) via PubMed©, EMBASE©, and the Cochrane Library Plus were reviewed in search of all the studies published until November 2012 in which an operation with Dynesys in patients with spinal degenerative diseases and an evaluation of the results by an analysis of functional outcomes had taken place. No limits were used to article type, date of publication or language. Results: A total of 134 articles were found, 26 of which fulfilled the inclusion criteria after being assessed by two reviewers. All of them were case series, except for a multicenter randomized clinical trial (RCT) and a prospective case-control study. The selected articles made a total of 1507 cases. The most frequent diagnosis were lumbar spinal canal stenosis (LSCS), degenerative disc disease (DDD), degenerative spondylolisthesis (DS) and lumbar degenerative scoliosis (LDS). In cases of lumbar spinal canal stenosis Dynesys was associated to surgical decompression. Several tools to measure the functional disability and general health status were found. Oswestry Disability Index (ODI), the ODI Korean version (K-Odi), Prolo, Sf-36, Sf-12, Roland-Morris disability questionnaire (RMDQ), and the pain Visual Analogue Scale (VAS) were the most used. They showed positive results in all cases series reviewed. In most studies the ODI decreased about 25% (e.g. from a score of 85% to 60%). Better results when dynamic fusion was combined with nerve root decompression were found. Functional outcomes and leg pain scores with Dynesys were statistically non-inferior to posterolateral spinal fusion using autogenous bone. When Dynesys and decompression was compared with posterior interbody lumbar fixation (PLIF) and decompression, differences in ODI and VAS were not statistically significant. Conclusions: In patients with spinal degenerative diseases due to degenerative disc disorders, spinal canal stenosis and degenerative spondylolisthesis, surgery with Dynesys and decompression improves functional outcomes, decreases disability, and reduces back and leg pain. More studies are needed to conclude that dynamic stabilization is better than posterolateral and posterior interbody lumbar fusion. Studies comparing Dynesys with decompression against decompression alone should be done in order to isolate the effect of the dynamic stabilization.

Determinados comportamientos pueden ser factores protectores y promotores de salud, o actuar como variables de riesgo de mortalidad prematura, de morbilidad y de discapacidad funcional. Frente a las enfermedades ligadas a la cronología, el comportamiento pasaría a constituir una variable crucial de las enfermedades de biografía. En este artículo se presenta una sucinta revisión de trabajos epidemiológicos que han analizado la relación comportamiento y salud en gerontología. Se presenta también una breve síntesis en relación al estado actual de algunos comportamientos de elección personal implicados en la salud del anciano o de interés para un envejecimiento satisfactorio: fumar, alcohol, ejercicio físico, comportamiento alimentario, hábitos de sueño, y sexo seguro. Finalmente, se sugieren algunas implicaciones prácticas de la revisión, destacando la necesidad de incorporar a las personas de edad en los programas de promoción de comportamientos de salud y de prevención de enfermedad, y de valorar la evidencia científica disponible ala hora de aconsejar cambios en determinados comportamientos de riesgo, sobre todo en aquellos ancianos sin problemas de salud para los que ciertos comportamientos se asocian a un hedonismo ponderado y a disfrute de la vida.

El objetivo de este artículo consiste en mostrar, a partir del análisis de las narrativas de mujeres con diversidad funcional, física y sensorial, las barreras arquitectónicas, mentales y de comunicación que condicionan y configuran el devenir de sus vidas, tanto en el ámbito privado como en el público. Analizamos las situaciones vividas por 60 mujeres con diversidad funcional. El método de investigación tiene un carácter mixto, tanto cuantitativo como cualitativo. Sus narrativas son el medio para conocer en profundidad y analizar sus episodios biográficos que, tanto a nivel social como personal, han contribuido a dar forma al estado y a la situación actual de las participantes con un alcance temporal retrospectivo y prospectivo.

The process of creating an atomically defined and robust metallic tip is described and quantified using measurements of contact conductance between gold electrodes and numerical simulations. Our experiments show how the same conductance behavior can be obtained for hundreds of cycles of formation and rupture of the nanocontact by limiting the indentation depth between the two electrodes up to a conductance value of approximately 5G0 in the case of gold. This phenomenon is rationalized using molecular dynamics simulations together with density functional theory transport calculations which show how, after repeated indentations (mechanical annealing), the two metallic electrodes are shaped into tips of reproducible structure. These results provide a crucial insight into fundamental aspects relevant to nanotribology or scanning probe microscopies.

Understanding and predicting plant response to disturbance is of paramount importance in our changing world. Resprouting ability is often considered a simple qualitative trait and used in many ecological studies. Our aim is to show some of the complexities of resprouting while highlighting cautions that need be taken in using resprouting ability to predict vegetation responses across disturbance types and biomes. There are marked differences in resprouting depending on the disturbance type, and fire is often the most severe disturbance because it includes both defoliation and lethal temperatures. In the Mediterranean biome, there are differences in functional strategies to cope with water deficit between resprouters (dehydration avoiders) and nonresprouters (dehydration tolerators); however, there is little research to unambiguously extrapolate these results to other biomes. Furthermore, predictions of vegetation responses to changes in disturbance regimes require consideration not only of resprouting, but also other relevant traits (e.g. seeding, bark thickness) and the different correlations among traits observed in different biomes; models lacking these details would behave poorly at the global scale. Overall, the lessons learned from a given disturbance regime and biome (e.g. crown-fire Mediterranean ecosystems) can guide research in other ecosystems but should not be extrapolated at the global scale.
