The field of linear optical quantum computation (LOQC) will soon need a repertoire of experimental milestones. We make progress in this direction by describing several experiments based on Grover's algorithm. These experiments range from a relatively simple implementation using only a single nonscalable controlled- NOT (CNOT) gate to the most complex, requiring two concatenated scalable CNOT gates, and thus form a useful set of early milestones for LOQC. We also give a complete description of basic LOQC using polarization-encoded qubits, making use of many simplifications to the original scheme of Knill, Laflamme, and Milburn [E. Knill, R. Laflamme, and G. J. Milburn, Nature (London) 409, 46 (2001)].

The resource potential of shallow water tables for cropping systems has been investigated using the Australian sugar industry as a case study. Literature concerning shallow water table contributions to sugarcane crops has been summarised, and an assessment of required irrigation for water tables to depths of 2 m investigated using the SWIMv2.1 soil water balance model for three different soils. The study was undertaken because water availability is a major limitation for sugarcane and other crop production systems in Australia and knowledge on how best to incorporate upflow from water tables in irrigation scheduling is limited. Our results showed that for the three soils studied (representing a range of permeabilities as defined by near-saturated hydraulic conductivities), no irrigation would be required for static water tables within 1 m of the soil surface. Irrigation requirements when static water tables exceeded 1 m depth were dependent on the soil type and rooting characteristics (root depth and density). Our results also show that the near-saturated hydraulic conductivities are a better indicator of the ability of water tables below 1 m to supply sufficient upflow as opposed to soil textural classifications. We conclude that there is potential for reductions in irrigation and hence improvements in irrigation water use efficiency in areas where shallow water tables are a low salinity risk: either fresh, or the local hydrology results in net recharge. We discuss the phenomenon of system tailoring in the context of data from an observational study of anaesthesia. We found that anaesthetists tailor their monitoring equipment so that the auditory alarms are more informative. However, the occurrence of tailoring by anaesthetists in the operating theatre was infrequent, even though the flexibility to tailor exists on many of the patient monitoring systems used in the study. We present an influence diagram to explain how alarm tailoring can increase situation awareness in the operating theatre but why factors inhibiting tailoring prevent widespread use. Extending the influence diagram, we discuss ways that more informative displays could achieve the results sought by anaesthetists when they tailor their alarm systems. In particular, we argue that we should improve our designs rather than simply provide more flexible tailoring systems. because users often find tailoring a complex task. We conclude that properly designed auditory displays may benefit anaesthetists in achieving greater patient situation awareness and that designers should consider carefully how factors promoting and inhibiting tailoring will affect the end-users' likelihood of conducting tailoring. Background: There is limited information available on the requirement for postoperative analgesic drugs in patients submitted to total laparoscopic hysterectomy (TLH) compared with patients undergoing vaginal hysterectomy (VH). Aim: To compare the postoperative analgesic requirements in patients who underwent a TLH with patients who had a VH. Methods: Chart review of 53 patients who had TLH and 47 who had VH and were seen postoperatively by an acute pain management service in order to assess postoperative analgesic requirements. Patient controlled analgesia (PCA) was part of the standard protocol for postoperative pain management. Analgesic requirement was recorded as the mean doses of morphine and number of days that patients used non-steroidal anti-inflammatory drugs (NSAIDs), oxycodone and tramadol. Results: The requirement for total morphine was approximately half the dose in patients who had a TLH (10.8 +/- 12.6 mg) compared with patients who had a VH (19.4 +/- 21.9 mg) (P 0.017). The length of use of NSAIDs was significantly reduced in patients who had undergone a TLH (2.0 +/- 0.95 days) as compared with patients who had a VH (2.85 +/- 1.1 days) (P < 0.0001). Conclusions: Patients submitted to TLH require less postoperative analgesic drugs when compared with patients who had VH. Prospective randomised trials are warranted to compare analgesic requirements between patients submitted to TLH and VH.

Background: jurisdictions are developing public drug insurance systems to improve access to pharmaceuticals, cost-effective prescribing, and patient health and well-being. We compared 2 Jurisdictions with different pharmaceutical policies to determine prescribing patterns for 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (le, statins). Objective: The aim of this work was to investigate the feasibility of using available prescription admimstrative databases to compare the use of statins in Queensland, Australia, and in Nova Scotia, Canada. Methods: Data from the Nova Scotia Pharmacare Program and the Health Insurance Commission in Australia were used to obtain dispensing data. Utilization was compared for the 5-year period from 1997 through 2001, using the World Health Organization anatomic therapeutic chemical/defined daily dose (DDD) system. Results: In the year 2001, there were 177,000 beneficiaries in the public drug plan in Nova Scotia (62% aged &GE; 65 years old) and 960,000 concession beneficiaries (pensioners and social security recipients, 61% aged &GE; 65 years) in Queensland. These 2 groups were comparable. The overall utilization of statin medications increased steadily in both areas over the study period, from 50 to 205 DDD/1000 beneficiaries per day. Comparison of the 2 growth lines showed no statistically significant differences in overall statin use despite differences in brand availabilities and policies about prescribing. In the year 2001, atorvastatin was the most commonly prescribed statin in both areas, comprising 46% of statin use in Nova Scotia and 51% in Queensland. Mean doses of each statin prescribed were slightly above the DDDs. Expenditure on statins per 1000 beneficiaries and per DDD were similar in each jurisdiction, being slightly higher in Nova Scotia. Conclusions: Despite differences in pharmaceutical reimbursement systems, use of the statins was similar in Nova Scotia and Queensland. The feasibility of the methodology was demonstrated. Future studies, including comparisons of drug utilization for other classes of drugs for which drug policies may be divergent (eg, different pricing structures or prior authorization requirements), or for which less evidence for appropriate use is available, may be useful. © 2005 Excerpta Medica, Inc.

We examined factors affecting roost tree selection by the white-striped freetail bat Tadarida australis (Chiroptera: Molossidae), a large insectivorous bat in suburban Brisbane, Australia. We compared biophysical characteristics associated with 34 roost trees and 170 control trees of similar diameter, height and tree senescence characters. Roost trees used by the white-striped freetail bat had significantly higher numbers of hollows in the trunk and branches (P < 0.003) and were more likely to contain a large trunk cavity with an internal diameter of > 30 cm (P < 0.001) than control trees. These trees also accommodated more species of hollow-using fauna (P = 0.005). When comparing roost trees with control trees of similar diameters and heights, roost trees were on average at a later stage of tree senescence (P < 0.001). None of the roost trees were found in the large forest reserves fringing the Brisbane metropolitan area despite these areas being used for foraging by the white-striped freetail bat. Although all tree locations in this study were in modified landscapes, roost trees tended to be surrounded by groups of trees and undergrowth. Roost trees provide important habitat requirements for hollow-using fauna in suburban, rural and forested environments.

We reviewed the outcome following use of recombinant activated factor VII (rVIIa) in patients with major bleeding post cardiothoracic surgery in our unit between January 2002 and July 2004. The unit consists of 16 cardiothoracic intensive care beds in a public metropolitan teaching hospital which serves as a referral centre for heart and lung transplant surgery Patients with refactory bleeding following cardiothoracic surgical procedures who were treated with rVIIa were identified. A total of 12 episodes of rVIIa use were recorded in ten patients, including three episodes with ventricular assist devices, and 5 heart and/or lung transplants. The median dose used was 85 mu g/kg. Chest tube drainage decreased in all patients following administration of rVIIa; median chest tube drainage decreased front 445 ml/h to 171 ml/h (P=0.03). Despite cessation of bleeding, mortality was high, when rVIIa was used after more than 24 hours. In six episodes, despite early rVIIa use (within six hours), continued bleeding necessitated return to theatre, where a surgical source of bleeding was found. In this small retrospective study, rVIIa significantly reduced bleeding that was refractory to standard blood product transfusion. In this series of patients., those that did not respond to rVIla early in the postoperative phase were found to have a surgical source of bleeding.

The optimum bandwidth for shallow, high-resolution seismic reflection differs from that required for conventional petroleum reflection. An understanding of this issue is essential for correct choice of acquisition instrumentation. Numerical modelling of simple Bowen Basin coal structures illustrates that, for high-resolution imaging, it is important to accurately record all frequencies up to the limit imposed by earth scattering. On the contrary, the seismic image is much less dependent on frequencies at the lower end of the spectrum. These quantitative observations support the use of specialised high-frequency geophones for high-resolution seismic imaging. Synthetic seismic inversion trials demonstrate that, irrespective of the bandwidth of the seismic data, additional low-frequency impedance control is essential for accurate inversion. Inversion provides no compelling argument for the use of conventional petroleum geophones in the high-resolution arena.

This paper examines the challenges of water supply in agriculture, with particular emphasis on requirements of field crops, including maize. It places the issue of water supply to agriculture in the context of increasing demands for water from alternatives users, declining quality water, pressure of increasing population, all of which are placing stresses on water availability at local, regional and national levels. The paper also examines existing freshwater resources and the potential impact of climate change on water supply and distribution and consequential impact on water stress incidence in various parts of the globe. It examines competition for water in both industrialized and developing countries, with particular emphasis on the impacts on agriculture and food supplies. The challenge of water use efficiency (WUE) in agriculture is explored with discussion of agronomic, economic and physiological WUE concepts, with specific reference to maize.
