The study of the periodical press has long played an important role in understanding the transmission and reception of ideas in early modero history. This article explores different facets of the formation of one of Spanish America's most important early periodicals, the_Gazeta de Literatura de México, in order to deepen our understanding of the nature of Mexican periodical publications and in particular the work of José Antonio Alzate y Ramírez. It also seeks to place the Gazeta de Literatura firmly within the international republic of letters, linking it to contemporary periodicals in Mexico and Europe.

El fallecimiento de Carlos III en diciembre de 1788 fue conmemorado en la Roma de Pío VI con unas exequias oficiales españolas organizadas por el embajador José Nicolás de Azara y por un funeral ordenado por el Papa. Azara aprovechó la ocasión para materializar en dicha función fúnebre sus radicales principios neoclásicos. Paralelamente, ambas ceremonias efímeras se fijaron mediante una serie de lujosas publicaciones en las que intervino decisivamente el impresor más característico del Neoclasicismo: Gimbattista Bodoni. Las exequias de Carlos III en Roma fueron la obra de un intelectual. Son producto de la Roma cosmopolita de los años ochenta del siglo XVIII, foco de atracción para artistas, aficionados, coleccionistas e inteligentes en las artes.

A versatile approach for the enantioselective synthesis of functionalised beta-hydroxy N-acetylcysteamine thiol esters has been developed which allows the facile incorporation of isotopic labels. It has been shown that a remarkable reversal of selectivity occurs in the titanium mediated aldol reaction of the acyloxazolidone intermediate using either (S)- or (R)-tert-butyldimethylsilyloxybutanal. The aldol products are valuable intermediates in the synthesis of 4-hydroxy-6-substituted gamma-lactones.

Objective
Preliminary assessment of an automated weaning system (SmartCare™/PS) compared to usual management of weaning from mechanical ventilation performed in the absence of formal protocols.


Design and setting
A randomised, controlled pilot study in one Australian intensive care unit.


Patients
A total of 102 patients were equally divided between SmartCare/PS and Control.

Interventions
The automated system titrated pressure support, conducted a spontaneous breathing trial and provided notification of success (“separation potential”).

Measurements and results
The median time from the first identified point of suitability for weaning commencement to the state of “separation potential” using SmartCare/PS was 20 h (interquartile range, IQR, 2–40) compared to 8 h (IQR 2–43) with Control (log-rank P = 0.3). The median time to successful extubation was 43 h (IQR 6–169) using SmartCare/PS and 40 (14–87) with Control (log-rank P = 0.6). Unadjusted, the estimated probability of reaching “separation potential” was 21% lower (95% CI, 48% lower to 20% greater) with SmartCare/PS compared to Control. Adjusted for other covariates (age, gender, APACHE II, SOFAmax, neuromuscular blockade, corticosteroids, coma and elevated blood glucose), these estimates were 31% lower (95% CI, 56% lower to 9% greater) with SmartCare/PS. The study groups showed comparable rates of reintubation, non-invasive ventilation post-extubation, tracheostomy, sedation, neuromuscular blockade and use of corticosteroids.

Conclusions
Substantial reductions in weaning duration previously demonstrated were not confirmed when the SmartCare/PS system was compared to weaning managed by experienced critical care specialty nurses, using a 1:1 nurse-to-patient ratio. The effect of SmartCare/PS may be influenced by the local clinical organisational context.

Azobenzene dyes derived from various anilines and aminothiaheterocycles ate-coupled with commercially important N,N-diethyl-m-toluidine (T series) and iv,N-diethyl-m-acetylaminoaniline (A series) are positively solvatochromic. The visible spectra of 16 pairs of derivatives have been measured in up to 22 solvents, and the transition energies related to Kamlet-Taft solvent polarity parameters. In general, A-series dyes are more bathochromic than their T-series counterparts in nonpolar solvents, consistent with colour chemistry tradition, However, in more dipolar solvents the more bathochromic T-series representatives unexpectedly become more bathochromic than their A-series partners. The relative solvatochromic shifts of the A and T series are related to their respective dipole moments, These in turn are distinguished by the effect of the anilide carbonyl group dipole moment, which is antiparallel to, and thus reduces, the dipole moment of the chromogen.

Lung disease in cystic fibrosis (CF) is typified by the development of chronic airways infection culminating in bronchiectasis and progression to end-stage respiratory disease. Pseudomonas aeruginosa, a ubiquitous gram-negative bacteria, is the archetypical CF pathogen and is associated with an accelerated clinical decline. The development and widespread use of chronic suppressive aerosolized antibacterial therapies, in particular Tobramycin Inhalation Solution (TIS), in CF has contributed to reduced lung function decline and improved survival. However, the requirement for the aerosolization of these agents through nebulizers has been associated with increased treatment burden, reduced quality of life and remain a barrier to broader uptake. Tobramycin Inhalation Powder (TIP™) has been developed by Novartis with the express purpose of delivering the same benefits as TIS in a time-effective manner. Administered via the T-326™ (Novartis) Inhaler in four individual 28-mg capsules, TIP can be administered in a quarter of the time of traditional nebulizers and is inherently portable. In clinical studies, TIP has been shown to be safe, result in equivalent or superior reductions in P. aeruginosa sputum density and produce similar improvements in pulmonary function. TIP offers significant advantages in time saving, portability and convenience over traditional nebulized TIS with comparable clinical outcomes for individuals with CF.
