Oxybutynin, a drug of choice in the treatment of urinary incontinence, has low oral bioavailability due to extensive first-pass metabolism. A toxic metabolite, N-desethyloxybutynin, has been linked to adverse reactions to oral oxybutynin. This study, therefore, reports on the design of an oxybutynin intravaginal ring (IVR) of reservoir design, comprising an oxybutynin silicone elastomer core encased in a non-medicated silicone sheath, manufactured by reaction injection moulding at 50oC. An unusually high initial burst release of oxybutynin (42.7 mg in 24 h) was observed in vitro with a full length core (100 mg drug loading), with subsequent non-zero order drug release. Use of fractional segment cores substantially reduced the burst effect, yielding linear cumulative drug release versus time plots from days 2 to 14. Thus, a 1/8 fractional segment core gave a 24 h burst of 11.28 mg oxybutynin and, thereafter, zero order release at the target dose of 5 mg/day over 14 days. Two oxybutynin cores, each 1/16 of full length, gave a greater release than a single 1/8 core, due to core segment end effects resulting in an increased surface area for release. The burst release was investigated by determining drug solubilities in the propan-1-ol product of elastomer condensation cure (390 mg/ml) and in the elastomer itself (13.9-20.21 mg/ml, by direct extraction and indirect thermal methods). These high oxybutynin solubilities were considered the major contributors to the burst effect. It was concluded that use of a fractional segment core would allow development of a suitable oxybutynin reservoir IVR.

Calibration is essential for interpretation of radiocarbon dates, especially when the 14C dates are compared to historical or climatic records with a different chronological basis. 14C ages of samples from the marine environment, such as shells or fish bones, or samples with a marine component, such as human bone in coastal regions, require an additional consideration because of the reservoir age of the ocean. While the pre-industrial global mean reservoir correction, R(t), is about 400 years, local variations (?R) can be several hundred years or more. ?R compilations on a global scale have been undertaken previously (Stuiver et al. 1986; Stuiver and Braziunas 1993), but have not been updated recently. Here we describe an on-line reservoir correction database accessed via mapping software. Rather than publishing a static ?R compilation, new data will be incorporated when it becomes available. Previous work has suggested that seasonal and inter-annual upwelling of deep, cold, radiocarbon depleted waters from the South Atlantic has caused variations in the reservoir effect (R) through time along the southern coast of Brazil. This work aims to examine the possible upwelling influence on the paleo-reservoir age of Brazilian surficial coastal waters based on paired terrestrial/marine samples obtained from archaeological remains. On the Brazilian coast there are hundreds of shell-middens built up by an ancient culture that lived between 6500 to 1500 years ago, but there are few located on open coast with a known upwelling influence. Three archaeological sites located in a large headland in Arraial do Cabo and Ilha de Cabo Frio, southeastern coast of Brazil with open ocean conditions and a well-known strong and large upwelling of the Malvinas/Falkland current were chosen for this study. The 14C age differences between carbonized seed and marine samples varied from 281 ± 44 to 1083 ± 51 14C yr. There are also significant age differences between carbonized seed samples (977 14C yr) and marine samples (200 and 228 14C yr) from the same archaeological layer that cannot be explained by a reservoir effect or an old-wood effect for charcoal. Therefore the present data from the southeastern Brazilian coast are inconclusive for identifying an upwelling effect on R. To do so it would be necessary to more precisely define the present-pre-bomb R in upwelling regions and to analyze paired marine/terrestrial samples that are contemporaneous beyond doubt.

Vaginal microbicides for the prevention of HIV transmission may be an important option for protecting women from infection. Incorporation of dapivirine, a lead candidate nonnucleoside reverse transcriptase inhibitor, into intravaginal rings (IVRs) for sustained mucosal delivery may increase microbicide product adherence and efficacy compared with conventional vaginal formulations. Twentyfour
healthy HIV-negative women 18–35 years of age were randomly assigned (1:1:1) to dapivirine matrix IVR, dapivirine reservoir IVR, or placebo IVR. Dapivirine concentrations were measured in plasma
and vaginal fluid samples collected at sequential time points over the 33-day study period (28 days of IVR use, 5 days of follow-up). Safety was assessed by pelvic/colposcopic examinations, clinical laboratory tests, and adverse events. Both IVR types were safe and well tolerated with similar adverse events observed in the placebo and dapivirine groups. Dapivirine from both IVR types was successfully distributed throughout the lower genital tract at concentrations over 4 logs greater than the EC50 against wild-type HIV-1 (LAI) in MT4 cells. Maximum concentration (Cmax) and area under the concentration–time curve (AUC) values were significantly higher with the matrix than reservoir IVR. Mean plasma concentrations of dapivirine were ,2 ng/mL. These findings suggest that IVR delivery of microbicides is a viable option meriting further study.

In 1995. an unexpected reservoir effect was reported in sequences of bulk C-14 dates of raised bog peat. In most peat Studies bulk C-14 dates are used for obtaining chronologies. Therefore it is important to confirm and quantify such a C-14 reservoir effect. Five bulk peat samples from the raised bog Engbertsdijksveen were conventionally C-14 dated. The same core had previously been precisely dated by C-14 AMS dates of carefully selected above-ground plant remains. The existence of a reservoir effect in bulk peat C-14 samples could not be confirmed. Other explanations for the reported reservoir effect are discussed. (C) 2004 Elsevier Ltd. All rights reserved.

We present the first marine reservoir age and Delta R determination for the island of St. Helena using marine mollusk radiocarbon dates obtained from an historical context of known age. This represents the first marine reservoir a.-c and Delta R determination in the southern Atlantic Ocean within thousands of kilometers of the island. The depletion of C-14 in the shells indicates a rather larger reservoir age for that portion of the surface Atlantic than models indicate. The implication is that upwelling old water along the Namibian coast is transported for a considerable distance, although it is likely to be variable on a decadal timescale. An artilleryman's button, together with other artifacts found in a midden, demonstrate association of the mollusk shells with a narrow historic period of AD 1815-1835.
