Background: Breast cancer mortality is declining in many Western countries. If mammography screening contributed to decreases in mortality, then decreases in advanced breast cancer incidence should also be noticeable.
Patients and methods: We assessed incidence trends of advanced breast cancer in areas where mammography screening is practiced for at least 7 years with 60% minimum participation and where population-based registration of advanced breast cancer existed. Through a systematic Medline search, we identified relevant published data for Australia, Italy, Norway, Switzerland, The Netherlands, UK and the USA. Data from cancer registries in Northern Ireland, Scotland, the USA (Surveillance, Epidemiology and End Results (SEER), and Connecticut), and Tasmania (Australia) were available for the study. Criterion for advanced cancer was the tumour size, and if not available, spread to regional/distant sites.
Results: Age-adjusted annual percent changes (APCs) were stable or increasing in ten areas (APCs of -0.5% to 1.7%). In four areas (Firenze, the Netherlands, SEER and Connecticut) there were transient downward trends followed by increases back to pre-screening rates.
Conclusions: In areas with widespread sustained mammographic screening, trends in advanced breast cancer incidence do not support a substantial role for screening in the decrease in mortality.

Near-infrared (NIR) imaging was used to observe water vapour flow in a gas-solid fluidized bed reactor. The technique consisted of a broadband light, an optical filter with a bandwidth centred on strong water vapour absorptions, a Vidicon NIR camera, a nozzle from which an optically active mixture of gas and water vapour was trans-illuminated by an NIR beam and collected data of transmittance were normalized to actual optical path. The procedure was applied to a thin fluidized bed reactor with a low aspect ratio of tube to particle diameters (D-1/d(p)) in order to validate the wall effect on flow dynamics and mass transfer during the reduction of ceria-silica by hydrogen. High concentrations of water vapour emerged in the vicinity of the wall when the bed was operated at pseudo-static conditions but disappeared when the bed was run at minimum bubbling conditions. This result shows the capability of optical methods with affordable costs to 2D imaging opaque packed bed by using a spatially resolved probe located at the exit, which is of great benefit for in situ visualization of anisotropic concentrations in packed beds under industrially relevant conditions and thus for elucidation of the underlying reaction mechanism and diffusion interactions. A research element of the European Union (EU) sixth Framework project BioCop focused on the development of a surface plasmon resonance (SPR) biosensor assay for the detection of paralytic shellfish poisoning (PSP) toxins in shellfish as an alternative to the increasingly ethically unacceptable mouse bioassay. A biosensor assay was developed using both a saxitoxin binding protein and chip surface in tandem with a highly efficient simple extraction procedure. The present report describes the single laboratory validation of this immunological screening method, for this complex group of toxins with differing toxicities, according to the European Decision 2002/657/EC in conjunction with IUPAC and AOAC single laboratory validation guidelines. The different performance characteristics (detection capability CC beta, specificity/selectivity, repeatability, reproducibility, stability, and applicability) were determined in relation to the EU regulatory limit of 800 mu g of saxitoxin equivalents (STX eq) per kg of shellfish meat. The detection capability CC beta was calculated to be 120 mu g/kg. Intra-assay repeatability was found to be between 2.5 and 12.3% and interassay reproducibility was between 6.1 and 15.2% for different shellfish matrices. Natural samples were also evaluated and the resultant data displayed overall agreements of 96 and 92% with that of the existing AOAC approved methods of mouse bioassay (MBA) and high performance liquid chromatography (HPLC), respectively.
