Objective: To compare an accelerated intervention incorporating early therapeutic exercise after acute ankle sprains with a standard protection, rest, ice, compression, and elevation intervention.

Design: Randomised controlled trial with blinded outcome assessor.

Setting: Accident and emergency department and university based sports injury clinic.

Participants: 101 patients with an acute grade 1 or 2 ankle sprain.

Interventions: Participants were randomised to an accelerated intervention with early therapeutic exercise (exercise group) or a standard protection, rest, ice, compression, and elevation intervention (standard group).

Main outcome measures: The primary outcome was subjective ankle function (lower extremity functional scale). Secondary outcomes were pain at rest and on activity, swelling, and physical activity at baseline and at one, two, three, and four weeks after injury. Ankle function and rate of reinjury were assessed at 16 weeks.

Results: An overall treatment effect was in favour of the exercise group (P=0.0077); this was significant at both week 1 (baseline adjusted difference in treatment 5.28, 98.75% confidence interval 0.31 to 10.26; P=0.008) and week 2 (4.92, 0.27 to 9.57; P=0.0083). Activity level was significantly higher in the exercise group as measured by time spent walking (1.2 hours, 95% confidence interval 0.9 to 1.4 v 1.6, 1.3 to 1.9), step count (5621 steps, 95% confidence interval 4399 to 6843 v 7886, 6357 to 9416), and time spent in light intensity activity (53 minutes, 95% confidence interval 44 to 60 v 76, 58 to 95). The groups did not differ at any other time point for pain at rest, pain on activity, or swelling. The reinjury rate was 4% (two in each group).

Conclusion: An accelerated exercise protocol during the first week after ankle sprain improved ankle function; the group receiving this intervention was more active during that week than the group receiving standard care.

The possible adverse effects on health of diet-derived advanced glycation endproducts (AGES) and advanced lipoxidation endproducts (ALES) is of current interest. This study had the objective of determining the effects of the addition of AGE/ALE inhibitors and different types of sugar and cooking oil on N-epsilon-(carboxymethyl)lysine (CML) and N-epsilon-(carboxyethyl)lysine (CEL) formation in model foods (sponge cakes). The cake baked using glucose produced the highest level of CML (2.07 +/- 0.24 mmol/mol lysine), whereas the cake baked using fructose produced the highest concentration of CEL (25.1 +/- 0.15 mmol/mol lysine). Metal foil targets were irradiated with 1 mu m wavelength (lambda) laser pulses of 5 ps duration and focused intensities (I) of up to 4x10(19) W cm(-2), giving values of both I lambda(2) and pulse duration comparable to those required for fast ignition inertial fusion. The divergence of the electrons accelerated into the target was determined from spatially resolved measurements of x-ray K-alpha emission and from transverse probing of the plasma formed on the back of the foils. Comparison of the divergence with other published data shows that it increases with I lambda(2) and is independent of pulse duration. Two-dimensional particle-in-cell simulations reproduce these results, indicating that it is a fundamental property of the laser-plasma interaction.

Nitride-strengthened, reduced activation, martensitic steel is anticipated to have higher creep strength because of the remarkable thermal stability of nitrides. Two nitride-strengthened, reduced activation martensitic steels with different carbon contents were prepared to investigate the microstructure and mechanical property changes with decreasing carbon content. It has been found that both steels had the microstructure of full martensite with fine nitrides dispersed homogeneously in the matrix and displayed extremely high strength but poor toughness. Compared with the steel with low carbon content (0.005 pct in wt pct), the steel with high carbon content (0.012 pct in wt pct) had not only the higher strength but also the higher impact toughness and grain coarsening temperature, which was related to the carbon content. On the one hand, carbon reduction led to Ta-rich inclusions; on the other hand, the grain grew larger when normalized at high temperature because of the absence of Ta carbonitrides, which would decrease impact toughness. The complicated Al2O3 inclusions in the two steels have been revealed to be responsible for the initiated cleavage fracture by acting as the critical cracks.
