The aim of this study was to evaluate the effect on oral health, at age 9 years, of daily oral supplementation with the probiotic Lactobacillus reuteri, strain ATCC 55730, to mothers during the last month of gestation and to children through the first year of life. The study was a single-blind, placebo-controlled, multicenter trial involving 113 children: 60 in the probiotic and 53 in the placebo group. The subjects underwent clinical and radiographic examination of the primary dentition and carious lesions, plaque and gingivitis were recorded. Saliva and plaque were sampled for determination of mutans streptococci (MS) and lactobacilli (LB) in saliva and plaque as well as salivary secretory IgA (SIgA). Forty-nine (82%) children in the probiotic group and 31 (58%) in the placebo group were caries-free (p < 0.01). The prevalence of approximal caries lesions was lower in the probiotic group (0.67 ± 1.61 vs. 1.53 ± 2.64; p < 0.05) and there were fewer sites with gingivitis compared to the placebo group (p < 0.05). There were no significant differences between the groups with respect to frequency of toothbrushing, plaque and dietary habits, but to intake of fluoride supplements (p < 0.05). There were no intergroup differences with respect to L. reuteri, MS, LB or SIgA in saliva. Within the limitation of this study it seems that daily supplementation with L. reuteri from birth and during the first year of life is associated with reduced caries prevalence and gingivitis score in the primary dentition at 9 years of age.

Varje år drabbas 250-300 barn under 15 år av cancer i Sverige. Tidigare ansågs barncancer nästintill obotligt, men dagens framsteg inom medicin har gjort att allt fler barn överlever sin cancer och perspektivet inom hälso- och sjukvård har därmed ändrats radikalt. När barn drabbas av cancer påverkas hela familjen. I denna kaotiska situation är ett bra bemötande av största vikt. För att kunna ge ett bra bemötande och god omvårdnad till barnet och familjen krävs att barnsjuksköterskan har kompetens och kommunikativ förmåga. Även ett bra samarbete mellan barnsjuksköterskan och familjen är av stor vikt. Syftet med studien var att beskriva barns, två till fem år, och deras föräldrars upplevelser av barnsjuksköterskans bemötande inom barnonkologisk omvårdnad, 12 månader efter diagnostillfället. Studien är en kvalitativ studie med deduktiv ansats, där intervjuerna analyserats med innehållsanalys utifrån Swansons omvårdnadsteori. 12 barn i åldern två till fem år ingick i studien, och föräldrarna medverkade som komplement till barnens deltagande. Resultatet visar att barnsjuksköterskans bemötande har stor betydelse för barns och föräldrars upplevelser och välbefinnande i omvårdnaden. Viktigast för ett bra bemötande var delaktighet, tillgänglighet och kontinuitet, information samt att ha ett barnperspektiv. För att skapa bästa förutsättningarna för ett bra bemötande krävs att barnsjuksköterskan gör barnet och familjen delaktiga i vården, ser barnet som en individ och har kompetens och kunskap. Mer forskning behövs kring hur barn och familjer upplever barnsjuksköterskans bemötande och hur det påverkar deras upplevelser av vården, inte bara inom barnonkologisk omvårdnad utan även generellt inom pediatrisk omvårdnad.

Background: Although the negative consequences on health of being obese are well known, most adults gain weight across the lifespan. The general increase in body mass index (BMI) is mainly considered to originate from behavioral and environmental changes; however, few studies have evaluated the influence of these factors on change in BMI in the presence of genetic risk. We aimed to study the influence of multifactorial causes of change in BMI, over 65 years. Methods and Findings: Totally, 6130 participants from TwinGene, who had up to five assessments, and 536 from the Swedish Adoption/Twin Study of Aging, who had up to 12 assessments, ranging over 65 years were included. The influence of lifestyle factors, birth cohort, cardiometabolic diseases and an individual obesity genetic risk score (OGRS) based on 32 single nucleotide polymorphisms on change in BMI was evaluated with a growth model. For both sexes, BMI increased from early adulthood to age of 65 years, after which the increase leveled off; BMI declined after age of 80 years. A higher OGRS, birth after 1925 and cardiometabolic diseases were associated with higher average BMI and a steeper increase in BMI prior to 65 years of age. Among men, few factors were identified that influence BMI trajectories in late life, whereas for women type 2 diabetes mellitus and dementia were associated with a steeper decrease in BMI after the age of 65 years. Conclusions: There are two turning points in BMI in late adulthood, one at the age of 65 years and one at the age 80 years. Factors associated with an increase in BMI in midlife were not associated with an increase in BMI after the age of 65 years. These findings indicate that the causes and consequences of change in BMI differ across the lifespan. Current health recommendations need to be adjusted accordingly.
