This paper focuses on the relationship between metropolitan and regional health planning based on the processes of regionalization and the Pact for Health in the Baixada Santista Metropolitan Area, Sao Paulo State, Brazil. The method used was a case study in two stages, namely during initial implementation of the Pact for Health (2007) and the Regional Administration Committees (CGR) and in 2010. Municipal and regional health systems managers and the director of the Metropolitan Agency were interviewed, and records were analyzed from ten years of meetings of the Regional Inter-Administration Committee and the Regional Development Council. Four issues emerged: financing and infrastructure; health services utilization; inefficiency of the Regional Health Administration's instruments and decision-making levels; and the relationship between different levels in the Administration. Metropolitan health management remained as an underlying issue, appearing only incidentally or tangentially to regional management. Despite some limitations, the CGR has been legitimized as a space for regional health management.

O trabalho aborda aspectos da relação entre metropolização e regionalização em saúde, tendo por base os processos de regionalização e pactuação na Região Metropolitana da Baixada Santista, São Paulo, Brasil. Trata-se de estudo de caso, realizado em dois momentos: o primeiro, no início da implementação do Pacto pela Saúde (2007) e dos Colegiados de Gestão Regional (CGR), e o segundo, em 2010. Foram entrevistados gestores municipais e regionais do SUS e a direção da Agência Metropolitana. Também foram analisadas atas de dez anos de reuniões da Comissão Intergestora Regional e do Conselho de Desenvolvimento da Baixada Santista. Emergiram quatro grandes temas: financiamento e infraestrutura; utilização de serviços; ineficiência das instâncias e instrumentos de gestão; relação entre os níveis de gestão. Permaneceu latente a questão da metropolização, que aparece de forma incipiente ou tangencial à gestão regional. Avalia-se que apesar de algumas limitações, o CGR vem se legitimando como espaço de gestão regional.

Assessment of the suitability of anthropogenic landscapes for wildlife species is crucial for setting priorities for biodiversity conservation. This study aimed to analyse the environmental suitability of a highly fragmented region of the Brazilian Atlantic Forest, one of the world's 25 recognized biodiversity hotspots, for forest bird species. Eight forest bird species were selected for the analyses, based on point counts (n = 122) conducted in April-September 2006 and January-March 2009. Six additional variables (landscape diversity, distance from forest and streams, aspect, elevation and slope) were modelled in Maxent for (1) actual and (2) simulated land cover, based on the forest expansion required by existing Brazilian forest legislation. Models were evaluated by bootstrap or jackknife methods and their performance was assessed by AUC, omission error, binomial probability or p value. All predictive models were statistically significant, with high AUC values and low omission errors. A small proportion of the actual landscape (24.41 +/- 6.31%) was suitable for forest bird species. The simulated landscapes lead to an increase of c. 30% in total suitable areas. In average, models predicted a small increase (23.69 +/- 6.95%) in the area of suitable native forest for bird species. Being close to forest increased the environmental suitability of landscapes for all bird species; landscape diversity was also a significant factor for some species. In conclusion, this study demonstrates that species distribution modelling (SDM) successfully predicted bird distribution across a heterogeneous landscape at fine spatial resolution, as all models were biologically relevant and statistically significant. The use of landscape variables as predictors contributed significantly to the results, particularly for species distributions over small extents and at fine scales. This is the first study to evaluate the environmental suitability of the remaining Brazilian Atlantic Forest for bird species in an agricultural landscape, and provides important additional data for regional environmental planning.

Decentralization and regionalization represent constitutional guidelines for the organization of the Unified Health System, which in the last 20 years has required the adoption of mechanisms to coordinate and accommodate federative tensions in Brazil's healthcare sector. This paper analyzes the national implementation of the Health Pact between 2006 and 2010 involving a strategy that reconfigures intergovernmental relations in the sector. The study involved the analysis of documents, official data and interviews with federal, state and municipal managers in the Brazilian states. The content of the national proposal is initially discussed, including its implications for health policy. The different rhythms and degrees of implementation of the Health Pact are then reviewed, with respect to adherence by states and municipalities and the formation of Regional Management Boards. Lastly, the conditioning factors for the multiplicity of experiences observed in the country are identified and the challenges facing progress toward a decentralized and regionalized health system in Brazil are discussed.

We decompose the recent changes in regional inequality in Brazil into its components, highlighting the role of spatially blind social programs. We aggregate personal income micro data to the state level, differentiating nine income sources, and assess the role of these components in the observed changes in regional inequality indicators. The main results indicate that the largest part of the recent reduction in regional inequality is related to the dynamics of the market-related labor income, with manufacturing and services favoring deconcentration. Labor income in agriculture, retirement and pensions, and property rents and other sources favored concentration. The social programs Bolsa Familia and Beneficios de Prestacao Continuada are responsible for more than 24 percent of the reduction in inequality, although they account for less than 1.7 percent of the disposable household income. Such positive impact on regional concentration is impressive, since the goals of the programs are clearly nonspatial.

This paper presents a method for transforming the information of an engineering geological map into useful information for non-specialists involved in land-use planning. The method consists of classifying the engineering geological units in terms of land use capability and identifying the legal and the geologic restrictions that apply in the study area. Both informations are then superimposed over the land use and a conflict areas map is created. The analysis of these data leads to the identification of existing and forthcoming land use conflicts and enables the proposal of planning measures on a regional and local scale. The map for the regional planning was compiled at a 1:50,000 scale and encompasses the whole municipal land area where uses are mainly rural. The map for the local planning was compiled at a 1:10,000 scale and encompasses the urban area. Most of the classification and operations on maps used spatial analyst tools available in the Geographical Information System. The regional studies showed that the greater part of Analandia's territory presents appropriate land uses. The local-scale studies indicate that the majority of the densely occupied urban areas are in suitable land. Although the situation is in general positive, municipal policies should address the identified and expected land use conflicts, so that it can be further improved.

OBJECTIVE: This study aimed to investigate the frequency of positive results for hepatitis B and C, HIV and syphilis in blood donations at the Centro Regional de Hemoterapia de Ribeirão Preto, to describe donors with positive results according to some demographic and socioeconomic variables, to identify risk factors associated to these donors and the reasons that they were not detected during clinical screening. METHODS: A descriptive study was performed between July 1st 2005 and July 31st 2006 by interviewing 106 donors after medical consultations where they were informed of positive results for hepatitis B, hepatitis C, HIV or syphilis. RESULTS: There was a predominance of first-time donors, males, under 50-year olds, married individuals, from Ribeirão Preto, with elementary education, low economic status and of people who donated at the request of friends or relatives. Hepatitis C was the most frequently detected infection (56.6%), followed by hepatitis B (20.7%), HIV (12.3%) and syphilis(10.4%). About 40% of donors had omitted risk factors for different reasons: because they trusted the results of serological tests, did not feel comfortable about talking of risk factors or did not consider them relevant. Other justifications were the duration of the interview, the interviewer was unskilled, embarrassment and doubts about confidentiality. CONCLUSION: The results indicate the need for changes in the approach to clinical screening and a review of methods to attract and guide potential donors.

JUSTIFICATIVA E OBJETIVOS: Durante o processo de parturição, diversos fatores alteram a intensidade da dor, tais como a paridade, a rotura de membranas corioamnióticas, a dilatação cervical, bem como influências culturais e ambientais. Assim, os objetivos deste estudo foram verificar o número de requisições de analgesia regional e o grau de dilatação cervical no momento da solicitação da analgesia pelas parturientes com membranas corioamnióticas íntegras e aquelas com corioamniorrexe. MÉTODO: Trata-se de um estudo descritivo e retrospectivo, com análise de 208 prontuários de parturientes primigestas, 129 com membranas corioamnióticas íntegras e 79 com corioamniorrexe, assistidas no Centro da Saúde da Mulher de Ribeirão Preto, SP, no período de novembro de 2008 a maio de 2009. Para análise estatística dos dados foram utilizados os testes de Mann-Whitney e o Qui-quadrado, com nível de significância p < 0,05 e intervalo de confiança de 95%. RESULTADOS: Foi solicitada analgesia regional por 87,9% das parturientes selecionadas para esta pesquisa. A média da dilatação cervical para as pacientes com membranas íntegras foi de 6,26 ± 1,67 cm e para aquelas com corioamniorrexe foi com dilatação de 6,11 ± 1,75 cm, não havendo diferença significativa entre esses dois grupos de parturientes (p = 0,12). Em relação ao tipo de analgesia, houve predomínio do duplo bloqueio, sem diferenças significativas entre os dois grupos avaliados (p = 0,84). CONCLUSÃO: A maioria das parturientes deste estudo solicitou analgesia regional tipo duplo bloqueio com em média 6 cm, de acordo com a dilatação cervical, não havendo diferença entre primigestas com membranas corioamnióticas rotas e íntegras.

OBJETIVO: Descrever o perfil clínico e epidemiológico e a prevalência da coinfecção tuberculose/HIV na Unidade Regional de Saúde do Tocantins, que envolve 14 municípios no estado do Maranhão. MÉTODOS: Estudo epidemiológico descritivo baseado em dados secundários das fichas individuais de tuberculose do Sistema Nacional de Informação de Agravos de Notificação. Foram incluídos todos os casos notificados de coinfecção tuberculose/HIV, por município de residência, no período entre janeiro de 2001 e dezembro de 2010. RESULTADOS: Foram notificados 1.746 casos de tuberculose no distrito. Dos pacientes testados para HIV, 100 eram coinfectados. equivalendo a uma prevalência de 39%. Dos coinfectados, 79% eram do sexo masculino, 42% eram de cor parda, 64% tinham idade entre 20 e 40 anos, 31% tinham até quatro anos de estudo, e 88% residiam em Imperatriz. A forma clínica predominante foi a pulmonar (87%), e 73% eram casos novos. Dos coinfectados, 27% apresentaram resultados positivos na baciloscopia de escarro e 89% tinham imagem sugestiva de tuberculose na radiografia do tórax. A cultura de escarro foi realizada em apenas 7% dos casos. CONCLUSÕES: Evidenciou-se que a situação clínica e epidemiológica da coinfecção tuberculose/HIV ainda é um grande problema de saúde pública no sudoeste do Maranhão e impõe uma maior articulação entre os programas de controle de tuberculose e de doenças sexualmente transmissíveis/AIDS. Além disso, são necessários o compromisso e o envolvimento político dos gestores e profissionais de saúde no planejamento de ações e serviços de saúde.
