Aim: To use published literature and experts' opinion to investigate the clinical meaning and magnitude of changes in the Quality of Life (QOL) of groups of patients measured with the European Organisation for the Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30). Methods: An innovative method combining systematic review of published studies, expert opinions and meta-analysis was used to estimate large, medium, and small mean changes over time for QLQ-C30 scores. Results: Nine hundred and eleven papers were identified, leading to 118 relevant papers. One thousand two hundred and thirty two mean changes in QOL over time were combined in the meta-analysis, with timescales ranging from four days to five years. Guidelines were produced for trivial, small, and medium size classes, for each subscale and for improving and declining scores separately. Estimates for improvements were smaller than respective estimates for declines. Conclusions: These guidelines can be used to aid sample size calculations and interpretation of mean changes over time from groups of patients. Observed mean changes in the QLQ-C30 scores are generally small in most clinical situations, possibly due to response shift. Careful consideration is needed when planning studies where QOL changes over time are of primary interest; the timing of follow up, sample attrition, direction of QOL changes, and subscales of primary interest are key considerations. Ensaio argumentativo sobre a concepção de escrita fora da perspectiva etnocêntrica que considera o sistema verbal alfabético ocidental como pólo da formulação conceitual e teórica. A hipótese fundamental em estudo encaminha a abordagem sistêmica a partir da qual a escrita é sistema ambiental de constituição semiótico-cognitiva fundado em possibilidades combinatórias e interpretativas de leitura. Examina-se, então, a perspectiva semiocêntrica fundada pela temporalidade da cultura e não apenas pela dominante espacial.

This paper provides a paleoenvironmental reconstruction of a Late Quaternary lagoon system in the Jaguaruna region of Santa Catarina state, southern Brazil. Integrated results of bulk sedimentary organic matter characterization (delta C-13, delta N-15 and C/N), microfossil (pollen and diatom) and grain-size analysis from three shallow cores (similar to 2.5m depth) allowed us to propose an evolving paleogeographic scenario in this coastal region for the last ca. 5500 cal a BP. The lagoonal system in this area was more extensive during the mid-Holocene than today, with a gradual and continuous lagoon-sea disconnection until the present. We add to the debate regarding relative sea-level (RSL) variations for the Brazilian coast during the Holocene and discuss the importance of sedimentary dynamics for interpreting changes in coastal ecosystems. The multi-proxy analysis suggests that changes in coastal ecosystems could be directly related to local sedimentary processes, which are not necessarily linked to RSL fluctuations and/or to climatic variations. Assessing Pathogenicity for Novel Mutation/Sequence Variants: The Value of Healthy Older Individuals

Improvement in DNA technology is increasingly revealing unexpected/unknown mutations in healthy persons and generating anxiety due to their still unknown health consequences. We report a 44-year-old healthy father of a 10-year-old daughter with bilateral coloboma and hearing loss, but without muscle weakness, in whom a whole-genome CGH revealed a deletion of exons 38-44 in the dystrophin gene. This mutation was inherited from her asymptomatic father, who was further clinically and molecularly evaluated for prognosis and genetic counseling (GC). This deletion was never identified by us in 982 Duchenne/Becker patients. To assess whether the present case represents a rare case of non-penetrance, and aiming to obtain more information for prognosis and GC, we suggested that healthy older relatives submit their DNA for analysis, to which several complied. Mutation analysis revealed that his mother, brother, and 56-year-old maternal uncle also carry the 38-44 deletion, suggesting it an unlikely cause of muscle weakness. Genome sequencing will disclose mutations and variants whose health impact are still unknown, raising important problems in interpreting results, defining prognosis, and discussing GC. We suggest that, in addition to family history, keeping the DNA of older relatives could be very informative, in particular for those interested in having their genome sequenced.

We report on measurements of total cross sections for positron scattering from the fundamental organic molecule methane (CH(4)). The energy range of these measurements was 0.1-50 eV, whereas the energy resolution was similar to 100 meV when our Ni moderator was used and similar to 260 meV when the W moderator was employed. To assist us in interpreting these data, Schwinger multichannel calculations were performed at both static and static plus polarization levels of approximation for elastic positron scattering from 0.001 to 10 eV. These calculations are found to be in quite good qualitative agreement with our measured data, and they clearly educe the crucial role played by the target polarization in the low energy positron-CH(4) scattering dynamics.

(The Clinical Case as a Basis for Research in Fundamental Psychopathology) This article discusses aspects that hinder the process of drawing up clinical cases and stresses their importance for research in fundamental psychopathology. The author bases her thinking on several texts by Freud and his followers about the technique and the interpretation of dreams. In these texts, clinical cases are used to express a problem that must be investigated. The grounds for research follow the same logic as that used for interpreting dreams.

Knowledge of anatomical variations of the musculoskeletal system is important for interpreting unusual clinical presentations. We observed the presence of an abnormal extensor indicis muscle in the left hand of an adult male cadaver. In this case, the muscle comes from the ligament and over the scaphoideum and trapezoideum bones and continues after the short muscle belly; it is attached to the dorsal aponeurosis of the indicis. This muscular disposition was described in other studies which demonstrated approximately 1.0% of incidence. Clinically, this anatomical variation may be associated with pain and swelling at the back of the hand. In these cases symptoms tend to increase due to mechanical stress and can be confused with the presence of a dorsal synovial cyst. This report will help clinicians, surgeons, occupational and physical therapists formulate better clinical or surgical decisions when presented with a rare anatomical variation.

To examine whether the widely used Strengths and Difficulties Questionnaire (SDQ) can validly be used to compare the prevalence of child mental health problems cross nationally. We used data on 29,225 5- to 16-year olds in eight population-based studies from seven countries: Bangladesh, Brazil, Britain, India, Norway, Russia and Yemen. Parents completed the SDQ in all eight studies, teachers in seven studies and youth in five studies. We used these SDQ data to calculate three different sorts of "caseness indicators" based on (1) SDQ symptoms, (2) SDQ symptoms plus impact and (3) an overall respondent judgement of 'definite' or 'severe' difficulties. Respondents also completed structured diagnostic interviews including extensive open-ended questions (the Development and Well-Being Assessment, DAWBA). Diagnostic ratings were all carried out or supervised by the DAWBA's creator, working in conjunction with experienced local professionals. As judged by the DAWBA, the prevalence of any mental disorder ranged from 2.2% in India to 17.1% in Russia. The nine SDQ caseness indicators (three indicators times three informants) explained 8-56% of the cross-national variation in disorder prevalence. This was insufficient to make meaningful prevalence estimates since populations with a similar measured prevalence of disorder on the DAWBA showed large variations across the various SDQ caseness indicators. The relationship between SDQ caseness indicators and disorder rates varies substantially between populations: cross-national differences in SDQ indicators do not necessarily reflect comparable differences in disorder rates. More generally, considerable caution is required when interpreting cross-cultural comparisons of mental health, particularly when these rely on brief questionnaires.

We studied the eutrophication history of a tropical shallow reservoir in the So Paulo metropolitan region, southeast Brazil. We analyzed grain size, geochemistry, diatom assemblages, and land-use records in a sediment core from the reservoir to infer its trophic state history during the last similar to 110 years (1894-2005). Eighty diatom species were observed in the core and shifts in the relative abundances of planktonic and benthic taxa indicate major limnological changes associated with complex interactions between hydrologic factors and eutrophication. Discostella stelligera was associated with deforestation and water physical changes whereas Aulacoseira granulata, a species abundant throughout the core, was mostly associated with high flux conditions and erosion events, regardless of trophic state. Eutrophication was triggered by construction of the city zoo (1958) and installation of the So Paulo State Department of Agriculture (1975) within the Gar double dagger as watershed, and increasing loads of untreated sewage from these institutions. The data suggest that deterioration in water quality began after similar to 1975 and markedly accelerated after similar to 1990. The reservoir has been hypereutrophic since 1999. Steady increases in geochemical proxies for trophic state, along with a decrease in C/N ratios, indicated higher nutrient concentrations and the prevalence of autochthonous production towards the core top. Appearance of Achnanthidium catenatum similar to 1993 highlighted the onset of a marked eutrophication phase. The subsequent dominance of Planothidium rostratum and Cyclotella meneghiniana suggested a sharp shift to a hypereutrophic state since 1999. Land-use history proved valuable for validating the chronology and interpreting anthropogenic impacts. Multi-proxy analysis of the sediment record provided an effective tool for tracking ecological shifts in the reservoir ecosystem. This study provides the first reconstruction of lake eutrophication history in Brazil and highlights the importance of hydrological/physical changes as drivers of diatom assemblage shifts in reservoirs, which may confound trophic state inferences based on shifts in the planktonic/benthic diatom ratio.

Trata-se de uma discussão teórica sobre o estatuto da bioética a partir de suas convergências epistemológicas com a saúde coletiva, campos científicos inter-relacionados, surgidos no contexto da segunda ruptura epistemológica, questionadora da crítica ao senso comum própria da ciência moderna. A reaproximação com o senso comum na segunda ruptura significa considerar na metodologia os determinantes do ambiente e da subjetividade. Assim, em meio a esta segunda ruptura, a saúde coletiva e a bioética incluem os determinantes sociais e subjetivos em suas análises. Caracterizam-se por uma visão ampliada e complexa da saúde e das ações humanas envolvendo o ambiente, a vida e a saúde, com enfoque transdisciplinar em suas abordagens. Qual o significado dessas premissas para o estatuto epistemológico da bioética em sua convergência com a saúde coletiva? Enquanto ética, a bioética precisa ser crítica, mas não como na primeira ruptura da filosofia moral. Necessita ser crítica a partir da facticidade dos determinantes sociais que se manifestam nas iniquidades em saúde. Para integrar crítica e facticidade, o caminho é a hermenêutica que interpreta os significados construídos no real e a partir deles torna-se crítica. Esse seria o estatuto epistemológico apropriado para a bioética na interface com a saúde coletiva.

Intense phytoplankton blooms were observed along the Patagonian shelf-break with satellite ocean color data, but few in situ optical observations were made in that region. We examine the variability of phytoplankton absorption and particulate scattering coefficients during such blooms on the basis of field data. The chlorophyll-a concentration, [Chla], ranged from 0.1 to 22.3 mg m−3 in surface waters. The size fractionation of [Chla] showed that 80% of samples were dominated by nanophytoplankton (N-group) and 20% by microphytoplankton (M-group). Chlorophyll-specific phytoplankton absorption coefficients at 440 and 676 nm, a*ph(440) and a*ph(676), and particulate scattering coefficient at 660 nm, b*p(660), ranged from 0.018 to 0.173, 0.009 to 0.046, and 0.031 to 2.37 m2 (mg Chla)−1, respectively. Both a*ph(440) and a*ph(676) were statistically higher for the N-group than M-group and also considerably higher than expected from global trends as a function of [Chla]. This result suggests that size of phytoplankton cells in Patagonian waters tends to be smaller than in other regions at similar [Chla]. The phytoplankton cell size parameter, Sf, derived from phytoplankton absorption spectra, proved to be useful for interpreting the variability in the data around the general inverse dependence of a*ph(440), a*ph(676), and b*p(660) on [Chla]. Sf also showed a pattern along the increasing trend of a*ph(440) and a*ph(676) as a function of the ratios of some accessory pigments to [Chla]. Our results suggest that the variability in phytoplankton absorption and scattering coefficients in Patagonian waters is caused primarily by changes in the dominant phytoplankton cell size accompanied by covariation in the concentrations of accessory pigments.
