A Bayesian nonparametric model for Taguchi's on-line quality monitoring procedure for attributes is introduced. The proposed model may accommodate the original single shift setting to the more realistic situation of gradual quality deterioration and allows the incorporation of an expert's opinion on the production process. Based on the number of inspections to be carried out until a defective item is found, the Bayesian operation for the distribution function that represents the increasing sequence of defective fractions during a cycle considering a mixture of Dirichlet processes as prior distribution is performed. Medulloblastoma, the most common malignant paediatric brain tumour, is currently treated with nonspecific cytotoxic therapies including surgery, whole-brain radiation, and aggressive chemotherapy. As medulloblastoma exhibits marked intertumoural heterogeneity, with at least four distinct molecular variants, previous attempts to identify targets for therapy have been underpowered because of small samples sizes. Here we report somatic copy number aberrations (SCNAs) in 1,087 unique medulloblastomas. SCNAs are common in medulloblastoma, and are predominantly subgroup-enriched. The most common region of focal copy number gain is a tandem duplication of SNCAIP, a gene associated with Parkinson's disease, which is exquisitely restricted to Group 4 alpha. Recurrent translocations of PVT1, including PVT1-MYC and PVT1-NDRG1, that arise through chromothripsis are restricted to Group 3. Numerous targetable SCNAs, including recurrent events targeting TGF-beta signalling in Group 3, and NF-kappa B signalling in Group 4, suggest future avenues for rational, targeted therapy.

Executive dysfunction is reported in juvenile myoclonic epilepsy (JME). However, batteries employed in previous studies included no more than three tests of executive function. In this study, we aimed to assess executive and attentional functions in JME using a comprehensive battery of eight tests (encompassing fifteen subtests). We also evaluated neuropsychological profiles using a clinical criterion of severity and correlated these findings with epilepsy clinical variables and the presence of psychiatric disorders. We prospectively evaluated 42 patients with JME and a matched control group with Digit Span tests (forward and backward), Stroop Color-Word Test, Trail Making Test, Wisconsin Card-Sorting Test, Matching Familiar Figures Test and Word Fluency Test. We estimated IQ with the Matrix Reasoning and Vocabulary subtests of the Wechsler Abbreviated Intelligence Scale. The patients with JME showed specific deficits in working memory, inhibitory control, concept formation, goal maintenance, mental flexibility, and verbal fluency. We observed attentional deficits in processes such as alertness and attention span and those requiring sustained and divided attention. We found that 83.33% of the patients had moderate or severe executive dysfunction. In addition, attentional and executive impairment was correlated with higher frequency of seizures and the presence of psychiatric disorders. Furthermore, executive dysfunction correlated with a longer duration of epilepsy. Our findings indicate the need for comprehensive neuropsychological batteries in patients with JME, in order to provide a more extensive evaluation of attentional and executive functions and to show that some relevant deficits have been overlooked. Objective: To identify neuropsychological predictors of treatment response to cognitive-behavioral therapy (CBT) and fluoxetine in treatment-naive adults with obsessive-compulsive disorder (OCD). Method: Thirty-eight adult outpatients with OCD underwent neuropsychological assessment, including tasks of intellectual function, executive functioning and visual and verbal memory, before randomization to a 12-week clinical trial of either CBT or fluoxetine. Neuropsychological measures were used to identify predictors of treatment response in OCD. Results: Neuropsychological measures that predicted a better treatment response to either CBT or fluoxetine were higher verbal IQ (Wechsler Abbreviated Scale of Intelligence) (p = 0.008); higher verbal memory on the California Verbal Learning Test (p = 0.710); shorter time to complete part D (Dots) (p<0.001), longer time to complete part W (Words) (p = 0.025) and less errors on part C (Colors) (p<0.001) in the Victoria Stroop Test (VST). Fewer perseverations on the California Verbal Learning Test, a measure of mental flexibility, predicted better response to CBT, but worse response to fluoxetine (p = 0.002). Conclusion: In general, OCD patients with better cognitive and executive abilities at baseline were more prone to respond to either CBT or fluoxetine. Our finding that neuropsychological measures of mental flexibility predicted response to treatment in opposite directions for CBT and fluoxetine suggests that OCD patients with different neuropsychological profiles may respond preferentially to one type of treatment versus the other. Further studies with larger samples of OCD patients are necessary to investigate the heuristic value of such findings in a clinical context. (C) 2012 Elsevier Inc. All rights reserved.

Objetivo: Verificar os efeitos da remediação neuropsicológica em um sujeito com Síndrome Velocardiofacial. Método: Participou do estudo um sujeito do sexo masculino, 13 anos de idade, queixa de baixo rendimento escolar, diagnosticado com Síndrome Velocardiofacial (Del22q11.2) e fissura de palato submucosa. Na avaliação pré e pós programa remediativo utilizou-se: BANI-TS; Matrizes Progressivas, Teste Gestáltico Visomotor; Escala de Inteligência Wechsler para Crianças- WISC-III e o Teste Wisconsin de Classificação de Cartas. O programa de remediação neuropsicológica, com ênfase nas habilidades de execução, atenção, organização perceptual utilizou do delineamento comportamental, treino sistemático de ensino-aprendizagem viso-construtivas, e estratégias diferenciadas na tarefa. Resultado: No pré-teste, as habilidades intelectuais mostraram classificação rebaixado à idade. Na prova cognitiva verbal (QIV=91) apresentou classificação média e execução (QI = 71), limítrofe. Os índices atencionais e de processamento da informação indicaram perfis limítrofes e os de organização perceptual, deficientes. Na avaliação gráfico-percepto-motora obteve resultados inferiores a 6 anos. Na pós-testagem, as habilidades intelectuais mantiveram classificação rebaixada, porém, com aumento do número de respostas corretas. Na escala de execução (QIE) os resultados foram na média, com escore elevado em 77%; a organização perceptual elevou-se para nível médio-inferior. Os resultados percepto-motores demonstraram aumento da pontuação, de 06 para 07 anos. Conclusão: Os resultados obtidos no presente estudo de caso demonstraram que programas remediativos, focados em habilidades específicas, pode ser uma opção a mais na reabilitação de sujeitos com fissura de palato, com queixas na aprendizagem.

Foram conduzidos dois experimentos para avaliar fontes orgânicas e inorgânicas de zinco e cobre nas dietas e seus efeitos no desempenho de leitões desmamados aos 21 dias de idade. Em cada experimento, foram utilizados 90 leitões em delineamento experimental de blocos ao acaso, com cinco dietas, seis repetições e três animais por parcela. As dietas utilizadas nos experimentos 1 e 2 continham 120 ppm de zinco e 10 ppm de cobre na forma de sulfato. No experimento 1, foram suplementadas com 0, 300, 600 e 900 ppm de zinco na forma orgânica ou 2.400 ppm na forma de óxido (ZnO) e, no experimento 2, com 0, 50, 100 e 150 ppm de cobre na forma orgânica ou 240 ppm de cobre na forma de sulfato (CuSO4 H2O). No experimento 1, os níveis de zinco de fonte orgânica tiveram efeito linear no consumo de ração e no ganho de peso nos períodos de 0 a 15 dias e de 0 a 21 dias pós-desmame. O ganho de peso nas fases de 0 a 35 dias e de 0 a 42 dias pós-desmame e o consumo de ração dos leitões que receberam a dieta com 900 ppm de zinco de fonte orgânica não diferiram dos valores observados naqueles que receberam a dieta com 2.400 ppm de zinco na forma inorgânica. A suplementação da dieta com zinco na forma orgânica (900 ppm) ou inorgânica (2.400 ppm) aumentou o consumo de ração e o ganho de peso de leitões nas primeiras três semanas após o desmame. A suplementação da dieta com 2.400 ppm de zinco na forma inorgânica reduziu a incidência de diarreia nas primeiras três semanas pós-desmame. No experimento 2, os níveis de cobre de fonte orgânica tiveram efeito quadrático no consumo de ração dos leitões nos períodos de 0 a 31 e de 0 a 40 dias pós-desmame. A suplementação da dieta com cobre, tanto de fonte orgânica (90 ppm) como inorgânica (240 ppm), aumenta o consumo de ração e o ganho de peso de leitões nos primeiros 40 dias pós-desmame
