This work analyses the mental health policy-making activity of the Brazilian National Health Agency (ANS), responsible for controlling health insurance companies. Three points are discussed: a) the framework of an economic and private health assistance regulatory activity, b) the ANS and its regulation activity and c) the rules produced by ANS in the mental health care field. It was concluded that, despite advances like the legal obligation to ensure medical treatment to all the diseases listed in ICD-10, the inclusion of suicidal patient damage and self-inflicted damage care, care provided by a multiprofessional team, the increase in the number of sessions with a psychologist, with an occupational therapist and of psychotherapy sessions, and mental health day hospitals included as part of the services offered, the authors identified specific regulatory gaps in this area. Some issues that ANS has to solve so that it can really play its institutional role of defending the public interest in the private health system are: the regulation of co-participation and franchise mechanisms, the increasing co-participation as a limitation of psychiatric hospitalization, and the limited number of crisis intervention psychotherapy sessions.

Purpose: Understand the difficulties and experienced of individuals suffering from postpartum depression, related to mood disturbances, the mother-child bond and its repercussions in the meanings established for the experience of being a mother. Methods: Forty-one patients were interviewed, with ages ranging from 20 to 49 years, from a total of 106 attended at the Primary Care Unit, in the interior of the state of Parahiba, Brazil. A sample of 21 women was selected, presenting an inclusion profile, propitious to mapping postpartum depression. The eligible patients were referred by two PSF (Health Family) teams (one each from the urban and rural zones), aimed at diagnosing the psychic disturbance of the perperium. They were accompanied by a psychologist and all signed an informed consent form. A field diary supported the information recorded. Beck's Depression Inventory complemented the inclusion and follow-up of the patients. The data were analyzed statistically. Results: We confirmed the findings of the most recent studies that infant abandonment in the postpartum period occurs in situations where multiple and serious factors add up, such as misery (86.7%), little schooling (67%), lack of a support network (36.5%), estrangement of the mother's family relations (12%) and lack of paternal involvement (91.5%). Conclusion: The feeling of psyching pain and suffering, resulting from postpartum depression, is the most emphasized by women (87%) because it triggers the greatest discomfort, due to the difficulty in overcoming it.

Background: Despite the fact that public speaking is a common academic activity and that social phobia has been associated with lower educational achievement and impaired academic performance, little research has examined the prevalence of social phobia in college students. The aim of this study was to evaluate the prevalence of social phobia in a large sample of Brazilian college students and to examine the academic impact of this disorder. Methods: The Social Phobia Inventory (SPIN) and the MINI-SPIN, used as the indicator of social phobia in the screening phase, were applied to 2319 randomly selected students from two Brazilian universities. For the second phase (diagnostic confirmation), four psychiatrists and one clinical psychologist administered the SCID-IV to subjects with MINI-SPIN scores of 6 or higher. Results: The prevalence of social phobia among the university students was 11.6%. Women with social phobia had significantly lower grades than those without the disorder. Fear of public speaking was the most common social fear. Only two of the 237 students with social phobia (0.8%) had previously received a diagnosis of social phobia and were under treatment. Limitations: Social phobia comorbidities were not evaluated in this study. The methods of assessment employed by the universities (written exams) may mask the presence of social phobia. This was not a population-based study, and thus the results are not generalizable to the entire population with social phobia. Conclusion: Preventive strategies are recommended to reduce the under-recognition and the adverse impact of social phobia on academic performance and overall quality of life of university students. Objective: this study investigated the feelings of women regarding end-of-life decision making after ultrasound diagnosis of a lethal fetal malformation. The aim of this study was to present the decision making process of women that chose for pregnancy termination and to present selected speeches of women about their feelings. Design: open psychological interviews conducted by a psychologist immediately after the diagnosis of fetal malformation by ultrasound. Analysis of the results was performed through a content analysis technique. Setting: the study was carried out at a public university hospital in Brazil. Participants: 249 pregnant women who had received the diagnosis of a severe lethal fetal malformation. Findings: fetal anencephaly was the most frequent anomaly detected in 135 cases (54.3%). Termination of pregnancy was decided by 172 (69.1%) patients and legally authorised by the judiciary (66%). The reason for asking for termination was to reduce suffering in all of them. In the 77 women who chose not to terminate pregnancy (30.9%), the reasons were related to feelings of guilt (74%). Key conclusions: the results support the importance of psychological counselling for couples when lethal fetal malformation is diagnosed. The act of reviewing moral and cultural values and elements of the unconscious provides assurance in the decision-making process and mitigates the risk of emotional trauma and guilt that can continue long after the pregnancy is terminated. Este texto objetiva discutir contribuições da Psicologia Histórico-Cultural para a formação e a atuação do psicólogo junto à educação, num contexto de Educação Inclusiva. Destacam-se os fundamentos e princípios da educação para pessoas com deficiência, bem como postulados da teoria de Vigotski referentes à formação e à atuação dos psicólogos. Considera-se que a perspectiva teórica elaborada pelo autor soviético contribui por: destacar a transitoriedade dos eventos e fenômenos; atrelar o mundo das ideias, valores e representações à vida objetiva e à prática social; relacionar propostas educacionais a um dado projeto de sociedade; defender a possibilidade do desvendamento da constituição social do psiquismo e da possível intervenção sobre a mesma; requisitar uma condição de desenvolvimento humano sobre o patamar do homem cultural e livre. Por fim, destaca-se o fato de que a Educação Inclusiva deve se referir ao processo de apropriação e usufruto das produções humanas mais complexas, elaboradas nas diferentes áreas da vida e do conhecimento.

A inserção da Psicologia na Atenção Primária exigiu dos profissionais um repensar de suas práticas, ações e condutas, considerando a realidade da população com que se pretende atuar e os objetivos desse nível de atenção. O objetivo deste estudo é retratar a experiência de psicólogos do Programa de Aprimoramento Profissional em Promoção de Saúde na Comunidade em Núcleos de Saúde da Família em Ribeirão Preto, SP. Para o desenvolvimento das intervenções, inicialmente foram realizadas análises das necessidades, a fim de se verificar os padrões de morbimortalidade da população. A atuação dos psicólogos foi dividida em duas frentes de trabalho: uma voltada para o suporte à equipe e outra para realizar ações com a comunidade. Com os usuários, o psicólogo realizou visitas domiciliares, atendimentos individuais, familiares e coordenação de grupos. Os grupos eram abertos ou fechados, podendo ser classificados em educativos/reflexivos e de socialização. Para auxiliar no desenvolvimento das atividades, foram utilizados recursos audiovisuais, técnicas de dinâmica de grupo, discussões em grupos e trocas de experiências. Notase a importância de as ações do psicólogo na Atenção Básica estarem focadas na interferência nos fatores de risco à saúde da população, visando a alcançar mudanças comportamentais que proporcionem melhor qualidade de vida.
