Serotonin reuptake inhibitors and cognitive-behavior therapy (CBT) are considered first-line treatments for obsessive-compulsive disorder (OCD). However, little is known about their modulatory effects on regional brain morphology in OCD patients. We sought to document structural brain abnormalities in treatment-naive OCD patients and to determine the effects of pharmacological and cognitive-behavioral treatments on regional brain volumes. Treatment-naive patients with OCD (n = 38) underwent structural magnetic resonance imaging scan before and after a 12-week randomized clinical trial with either fluoxetine or group CBT. Matched-healthy controls (n = 36) were also scanned at baseline. Voxel-based morphometry was used to compare regional gray matter (GM) volumes of regions of interest (ROIs) placed in the orbitofrontal, anterior cingulate and temporolimbic cortices, striatum, and thalamus. Treatment-naive OCD patients presented smaller GM volume in the left putamen, bilateral medial orbitofrontal, and left anterior cingulate cortices than did controls (p<0.05, corrected for multiple comparisons). After treatment with either fluoxetine or CBT (n = 26), GM volume abnormalities in the left putamen were no longer detectable relative to controls. ROI-based within-group comparisons revealed that GM volume in the left putamen significantly increased (p<0.012) in fluoxetine-treated patients (n = 13), whereas no significant GM volume changes were observed in CBT-treated patients (n = 13). This study supports the involvement of orbitofronto/cingulo-striatal loops in the pathophysiology of OCD and suggests that fluoxetine and CBT may have distinct neurobiological mechanisms of action. Introduction - The Centro de Orientacao ao Adolescente of Campinas (Southeastern Brazil) maintains a program to qualify economically disadvantaged adolescent students aged 15 to 18 years to enter the labor market. Objective - To understand life projects of teenagers who became mothers while participating in the program, in the period from 2003 to 2008, aiming to find the place of professional life in their life trajectory before and after motherhood. Method - Eight young mothers were interviewed, and a qualitative methodology was applied to the analysis of the interviews. Results - The trajectories of study and work were discontinued or adapted due to motherhood. Four young mothers completed high school and none had entered university. Three did not return to work and the rest had diverse work experiences. The reported difficulties to enter the labor market were: inadequate instruments to support the children's care, low income, lack of work experience, presence of small children and little educational background. Final Considerations - Teenage motherhood did not indicate the exclusion of educational or work projects but indicated adjustments and the need for a family and social support network. It was noted the need for public policies targeted at the inclusion of youths in the labor market and at support services such as nurseries. Also, the need for a change in gender relations was demonstrated, with greater equality of rights as a precondition for the inclusion of women, especially those who are mothers, in the labor market.

Objectives: To identify levels, tendency and recent differentials in fertility in Curitiba, Brazil. Methods: It is a quantitative and temporal series study, in which the birth rates, general and total fertility indicators, for the period of 1995-2007, were calculated and analyzed in Curitiba, Brazil, as well as the proportion of women with high fertility in 2005-2007, compared to the state of Parana. In order to evaluate inner regional differences in the city of Curitiba the same rates were calculated for each one of the administrative districts in the capital. Results: It was noticed a tendency of decline in fertility rates in Curitiba. The total fertility rate in 2007 was 1.49 children per woman and 1.66 in Parana state. The proportion of women with high fertility in the interior of Parana was 1.8 times higher than in the capital. The analysis of fertility rates by districts in Curitiba pointed out important differences: only 10 out of 75 districts had total fertility rates higher than 2.1 children per woman, and 9 districts concentrated 59.6% of women with high fertility. Conclusions: Curitiba showed a quick and sharp reduction in fertility rates. However, the inner regional differences in the city suggests the need to devise actions of reproductive health and social measures, directed to specific groups of population.

Objective: To estimate factors associated with condom use at last sexual intercourse among adolescents. Methods: Cross-sectional study of a representative sample of 368 sexually active adolescents aged 13-17 years from eight public high schools on Santiago Island, Cape Verde, 2007. The level of significance was 5.0% obtained from logistic regression, considering the association between condom use and socio-demographic, sexual and reproductive variables. Results: The prevalence of condom use at last sexual intercourse was 94.9%. Factors associated with condom use at last sexual relationship were: non-Catholic religion (OR=0.68, 95%CI: 0.52; 0.88) and affective-sexual partnership before the interview (OR=5.15, 95%CI: 1.79; 14.80). Conclusions: There was a high prevalence of condom use at last sexual intercourse of adolescents.

Abstract Background Hepatitis C chronic liver disease is a major cause of liver transplant in developed countries. This article reports the first nationwide population-based survey conducted to estimate the seroprevalence of HCV antibodies and associated risk factors in the urban population of Brazil. Methods The cross sectional study was conducted in all Brazilian macro-regions from 2005 to 2009, as a stratified multistage cluster sample of 19,503 inhabitants aged between 10 and 69 years, representing individuals living in all 26 State capitals and the Federal District. Hepatitis C antibodies were detected by a third-generation enzyme immunoassay. Seropositive individuals were retested by Polymerase Chain Reaction and genotyped. Adjusted prevalence was estimated by macro-regions. Potential risk factors associated with HCV infection were assessed by calculating the crude and adjusted odds ratios, 95% confidence intervals (95% CI) and p values. Population attributable risk was estimated for multiple factors using a case–control approach. Results The overall weighted prevalence of hepatitis C antibodies was 1.38% (95% CI: 1.12%–1.64%). Prevalence of infection increased in older groups but was similar for both sexes. The multivariate model showed the following to be predictors of HCV infection: age, injected drug use (OR = 6.65), sniffed drug use (OR = 2.59), hospitalization (OR = 1.90), groups socially deprived by the lack of sewage disposal (OR = 2.53), and injection with glass syringe (OR = 1.52, with a borderline p value). The genotypes 1 (subtypes 1a, 1b), 2b and 3a were identified. The estimated population attributable risk for the ensemble of risk factors was 40%. Approximately 1.3 million individuals would be expected to be anti-HCV-positive in the country. Conclusions The large estimated absolute numbers of infected individuals reveals the burden of the disease in the near future, giving rise to costs for the health care system and society at large. The known risk factors explain less than 50% of the infected cases, limiting the prevention strategies. Our findings regarding risk behaviors associated with HCV infection showed that there is still room for improving strategies for reducing transmission among drug users and nosocomial infection, as well as a need for specific prevention and control strategies targeting individuals living in poverty.

INTRODUÇÃO: O Centro de Orientação ao Adolescente de Campinas mantém um programa para capacitar adolescentes de 15 a 18 anos, desfavorecidos economicamente, estudantes, para a entrada no mercado de trabalho. OBJETIVO: Compreender projetos de vida de adolescentes, que se tornaram mães, enquanto participavam do programa, no período de 2003 a 2008, buscando desvelar o lugar da vida profissional na trajetória de vida, antes e depois da maternidade. MÉTODO: Oito jovens mães foram entrevistadas, sendo aplicada metodologia qualitativa para a análise das entrevistas. RESULTADOS: As trajetórias de estudo e trabalho foram descontinuadas, ou sofreram adaptações, com a maternidade. Quatro jovens finalizaram o segundo grau e nenhuma havia ingressado na faculdade. Três não voltaram a trabalhar e as demais tiveram experiências diversificadas de trabalho. As dificuldades relatadas para a inserção no mercado de trabalho foram: insuficiência de instrumentos de apoio no cuidado das crianças, baixo ganho salarial, falta de experiência de trabalho, filhos pequenos e pouca formação educacional. CONSIDERAÇÕES FINAIS: A maternidade na adolescência não indicou a exclusão dos projetos de formação educacional ou de trabalho, mas adaptações e necessidade de uma rede de apoio familiar e social. Constatou-se a necessidade de políticas públicas para a inserção do jovem no mercado de trabalho e para serviços de apoio, como creches. Demonstra-se a necessidade de uma mudança nas relações de gênero, com maior igualdade de direitos, como pré-condição à inserção da mulher no mercado de trabalho, em especial, daquelas que são mães.

Durante o verão austral, diversas regiões do Brasil são afetadas por precipitação intensa, geralmente associada à Zona de Convergência do Atlântico Sul (ZCAS). O objetivo geral deste trabalho foi investigar a influência da resolução espacial e temporal dos dados de temperatura da superfície do mar (TSM) na simulação da precipitação associada à ZCAS. Foram realizadas simulações com o modelo BRAMS (Brazilian developments on the Regional Atmospheric Modeling System) para dois eventos de ZCAS, ocorridos em 1998 (El Niño) e 1999 (La Niña). A quantidade de precipitação acumulada na parte oceânica da ZCAS foi maior nos experimentos com TSM mais quente. Índices estatísticos foram utilizados para verificação do desempenho do modelo na simulação de precipitação nas regiões que compõem a ZCAS (oceânica, costeira e amazônica), com diferentes dados de TSM. A resolução espaço-temporal dos dados de TSM influencia de forma pouco significativa na representação da ZCAS pelo modelo BRAMS. O modelo é mais eficiente em identificar a ocorrência/não ocorrência de chuva do que em localizar núcleos mais intensos e seu desempenho foi superior (inferior) na região amazônica (oceânica) da ZCAS.
